3 Intrductin Page 2 Intrductin t the Quarterly Safety Summary Reprt Welcme t the quarterly safety summary reprt fr the spring f 2017 (Oct/Nv/Dec). The purpse f this dcument is t summarise the accidents and serius incidents that ccurred during the spring quarter f There were fur fatalities in this perid and eight serius injuries. The fur fatalities and fur f the serius injuries ccurred in private sprt aircraft accidents. Five f the seven minr injuries in this quarter als ccurred in private sprt aircraft accidents. Many f these accidents ccurred during the landing phase f flight. Five f the six critical incidents in this quarter were Airspace Incidents. Three invlved near miss events between small aerplanes and helicpters, and tw invlved unmanned aircraft (near large aerplanes). There were als a few majr incidents invlving unmanned aircraft, and many cases f cnflicts and reduced separatin between aircraft. Pr safety practices by airprt grund crew were reprted in large aircraft transprt peratins including damage t a prpeller blade frm a glf cart. Agricultural helicpters suffered wire strikes, and agricultural aerplanes (Pacific Aerspace Cresc ) suffered frm jammed cntrls (including elevatr due t cntact with a bush; and ailerns due t a detached wing tip). In the large aerplanes sectr there were reprted landing incidents including buncing and tail strikes. This quarter als saw several freign registered aircraft n private peratins encunter difficulties with Air Traffic Management prcedures int Palmerstn Nrth and Queenstwn. 4 Executive Summary Page 3 Executive Summary - Aviatin Safety t 31 December 2017 There were 30 accidents in the spring f There were three fatal accidents in this quarter, all in private peratins (there were als three fatal accidents in the spring quarter f 2016): an amateur built aerplane (micrlight) with tw peple n bard; a class 2 micrlight (glider) with ne persn bard; a glider with ne persn n bard; fr details see page 5. There were eight serius injuries in accidents: the pilt f a private glider flight during landing; tw passengers f tandem parachute flights during landing; a passenger f a tandem parachute during the flight; the cameraman f a parachute flight during landing; the pilt f a private hang glider flight just after take-ff; tw pilts f private paraglider flights during landing; see pages 5 and 6. There were seven minr injuries in accidents: tw peple n bard a private amateur built aerplane (glider) that crashed; the pilt f a private glider flight during an ut landing; the pilt and passenger f a tandem paraglider flight during landing; the pilt f a private paraglider flight that failed t becme airbrne; a turist pilt f a private paraglider flight gt caught in a tree; see pages 5 and 6. And withut injury there were: ne airline small aerplane accident while landing after a ferry/psitining flight; ne airline helicpter accident during an emergency landing f a ferry/psitining flight; tw cmmercial small aerplane sl training accidents during/after landing; tw micrlight sl training accidents during/after landing; tw agricultural aerplane accidents, ne during spraying and ne during landing; ne private small aerplane accident during landing; ne private helicpter accident after landing; ne private micrlight clipped a fence and crashed int a paddck; tw private amateur built aerplanes, ne during landing and ne after issues with the landing gear; ne unregistered micrlight accident during landing after experiencing prblems with an elevatr cable; fr details see pages 7 and 8.

The biggest increase is within the Private Operatins - Sprt grup.

6 Sectin 1 - Accidents Page 5 Summary f Accidents This sectin describes all accidents that ccurred during the perid 1 Octber t 31 December These accidents are classified accrding t the highest level f injury sustained and the safety target grup. Nt all f these accidents were investigated by the CAA, and sme f the CAA investigatins have nt been cmpleted, s the text may be cndensed frm the riginal accident ntificatin. Fatal Accidents Private Operatins - Sprt Amateur built aerplane (micrlight), Suth f Dargaville: The aircraft with tw peple n bard crashed. The pilt and passenger were killed. The aircraft was substantially damaged. CAA safety investigatin in prgress. (17/8080) Class 2 micrlight (glider), Kaikhe: The aircraft with ne persn n bard crashed while n a lcal flight. The pilt was killed. The aircraft was destryed. CAA safety investigatin in prgress. (17/7177) Glider, Omarama: A glider with ne persn n bard was reprted verdue and the wreckage was later fund n a steep western facing ridge at apprximately 4,000 feet. The pilt was killed. The glider was destryed. CAA safety investigatin in prgress. (17/7309) Injury Accidents Sprt Transprt Tandem paraglider, Treble Cne Ski Area: On apprach, the passenger's ft hit the tp f a deer fence. The passenger als hit her head during the impact with the fence and sustained a cut (minr injury). The pilt sustained minr bruising. (17/7096) Tandem parachute, Jardines: The passenger failed t lift legs n landing, legs flded behind and ft caught at an awkward angle (suspected brken leg, serius injury). (17/8097) Tandem parachute, Kerikeri: The tandem instructr signalled the passenger t release his hands frm the harness. After a few secnds in freefall, the passenger grabbed hld f the instructrs left arm. The instructr made several attempts t release his arm by pulling his arm backwards. The passenger released his grip. Initially the instructr thught the passengers shulder had dislcated, but it was later fund his upper left arm was fractured (serius injury). (17/6864) Tandem parachute, Queenstwn: The passenger brke an ankle n landing (serius injury). (17/7600) Other Cmmercial Operatins - Sprt Parachute, Wanaka: The skydive cameraman brke bth arms n landing n the drp zne at the aerdrme (serius injury). (17/6261)

7 Sectin 1 - Accidents Page 6 Private Operatins - Sprt Glider, Centennial Park: Landing accident during ut landing. Water in the wings had nt cmpletely discharged befre landing, as such n turning frm right base t final the wings culd nt be levelled resulting with the right wing striking the grund, fllwed by the fuselage landing heavily, the undercarriage cllapsed, the glider grund lped, the pilts head brke the canpy (minr injury). (17/7071) Glider, Mrrinsville: During landing, glider came in cntact with a small rise n the grund. The pilt was seriusly injured. (17/7456) Amateur built aerplane (glider): The aircraft with tw peple n bard crashed, bth received minr injuries. (17/8079) Hang glider, Christchurch: Hang glider appeared t catch a "bad bit f air" immediately after take-ff, banking hard and spiralling befre impacting the hill. The pilt suffered a brken femur (serius injury). (17/6312) Paraglider, Manaia: The paraglider failed t becme airbrne and impacted the beach belw the launch site. The pilt sustained minr injuries. CAA safety investigatin in prgress. (17/6721) Paraglider, Nelsn: Hard landing resulting in a serius injury. (17/7827) Paraglider, Queenstwn: A turist withut the crrect apprvals frm the paragliding club had an unauthrised jump frm Skyline Gndla which resulted in an accident after being caught in a tree at the schl landing site. The turist received minr injuries. (17/7326) Paraglider: The paraglider was affected by wind rtr n landing - the pilt suffered a brken back (serius injury). (17/7754)

8 Sectin 1 - Accidents Page 7 Nn-Injury Accidents Airline Operatins - Small Aerplanes Cessna U206F, Awara: The pilt f a ferry/psitining flight (with three peple n bard) was established n prfile fr landing. As the aircraft crssed the bundary it encuntered windshear and sank nt the runway landing heavily. There is n g-arund ptin frm this pint s the pilt tried t cunter a bunce but the aircraft's nse wheel cllapsed. It als suffered a prpeller strike. (17/7434) Airline Operatins - Helicpters Single turbine engine helicpter, Carrick Range: Lud bang heard during cruise f a ferry/psitining flight (ne persn n bard), fllwed by "Lw Rtr" annunciatin. Pilt entered autrtatin and "Engine ut" annunciatin ccurred twice. Made emergency landing n steeply slped grund, aircraft pitched up and three rtr blades cntacted frnt windscreen and centre pillar supprt. On leaving helicpter it rlled nt its stinger. The aircraft received minr damage. CAA safety investigatin in prgress. (17/8089) Other Cmmercial Operatins - Aerplanes Cessna 172R, Ardmre: During taxi after cmpleting sme sl circuit training, the aircraft's wing cntacted the tail plane f anther aircraft which was als taxiing. Aircraft taxied t maintenance (substantial damage). (17/6837) Cessna A152, Whanganui: During landing after a sl training flight, aircraft landed heavily which resulted in a bunce. When the aircraft cntacted the runway the secnd time, it landed n the nse wheel first causing it t cllapse rearwards. The prpeller and engine munts were als damaged (minr damage). CAA safety investigatin cmpleted. (17/7006) Other Cmmercial Operatins - Sprt (Pilt Training - Sprt) Class 2 micrlight, Thames: Fllwing a dual circuit check, the student pilt had cmpleted three sl circuits. During the landing n cmpletin f the furth circuit, the student pilt lst directinal cntrl after tuch-dwn. The student pilt attempted t g-arund but the aircraft veered ff the runway int lng grass. The prt wing, prpeller and nse undercarriage were damaged (substantial damage). CAA safety investigatin cmpleted. (17/7686) Class 2 micrlight, Whangarei: Nse-wheel cllapsed n landing after a sl training flight. The aircraft was substantially damaged. (17/7370)

9 Sectin 1 - Accidents Page 8 Agricultural Operatins - Aerplanes De Havilland Canada DHC-2 Beaver Mk 1, Napier/Taup Rd: Landed shrt f the tpdressing strip, pssible dwndraft n final. Damage t undercarriage (cllapsed), left wing, tail, hrizntal stabiliser and prp strike. Aircraft came t rest ff the side f the strip dwn a small bank. (17/6764) Gippsland GA200C, Hurunui District: While spraying in a steep gully n a dwnhill run, an impact was felt and heard. The remaining lad was dumped and an initial visual check by the pilt failed t identify any damage t the aircraft and the flying characteristics were unchanged. The aircraft landed back n the airstrip where a visual inspectin f the aircraft fund damage t the left wing leading edge near the wing tip. The aircraft had struck a cabbage tree which was nt seen by the pilt. The aircraft had minr damage. CAA safety investigatin cmpleted. (17/7778) Private Operatins - Aerplanes Piper PA-18A-150, Wairau River: Aircraft (with ne persn n bard) verturned while landing n a riverbed. The aircraft received minr damage. (17/6632) Private Operatins - Helicpters Guimbal Cabri G2, Ohpe: During shut dwn, the pilt exited the helicpter t clse a gate and a gust f wind caused the helicpter t rll ver nt its side. The aircraft was substantially damaged. (17/7305) Private Operatins - Sprt Class 2 micrlight: Aircraft clipped fence and crashed int paddck, receiving substantial damage. (17/6251) Amateur built aerplane, Wanaka: Aircraft (with ne persn n bard) lst directinal cntrl n landing resulting in a grund lp. (17/6350) Amateur built aerplane, Whenuapai: Aircraft was perating in the circuit n a sl training flight at Nrth Shre when it had issues with the landing gear. Pilt decided t divert t Whenuapai due t the availability f emergency services. The nse gear cllapsed n landing. As a result f the impact with the grund, the fuselage and prpeller were damaged. The aircraft was substantially damaged. (17/7367) Other Rangira: After experiencing prblems with an elevatr cable, the pilt f a private flight (ne persn n bard) elected t land in a paddck adjacent t the aerdrme. The aircraft landed hard causing the undercarriage legs t bend (minr damage, these have since been straightened). Aircraft was taxied back t the airfield. This aircraft (class 2 micrlight) is nt currently n the aircraft register. (17/7334)

10 Sectin 2 - Incidents Page 9 Sectin 2 - Incidents This sectin describes selected incidents frm the perid. In the perid 1 Octber t 31 December 2017 there were a ttal f 1,636 incidents reprted t the CAA, the 121 incidents presented here have been selected n the basis f significance t risk management. Fr brevity the text may be cndensed frm the riginal ccurrence ntificatin. In sme cases the aircraft mdel descriptins have been reduced t a sectr (e.g. large jet). This is fr tw reasns: t maintain the privacy f the reprter, and t fcus n the nature f the incident. In many incidents such as airspace ccurrences, the specific aircraft type is nt relevant t the prblem. By cmparisn fr defect incidents the specific mdel is highly relevant, but the lcatin is nt. The ccurrences are gruped by sectr t enable cnsideratin f specific risks. While this is intended t assist peratrs t identify their sectr relevant risks, there will be sme events ccurring in a given sectr that culd equally ccur in ther sectrs. Critical Incidents Airline Operatins - Large Aerplanes Aircraft Incident Twin turbine engine, en-rute: Tw passengers became intxicated, and abusive and vilent t cabin crew. Were restrained but brke all restraints carried. Eventually calmed dwn and were met by plice n arrival. CAA safety investigatin cmpleted. (17/6383) Airspace Incident Queenstwn, twin turbine engine: Drne bserved t pass apprximately 200 ft belw the aircraft as the aircraft was cnducting the RNAV (RNP) Y 23 apprach at Queenstwn. (17/8163) Other Cmmercial Operatins - Aerplanes Airspace Incident Matamata, Cessna 172R: Aircraft n a dual training flight turned base in frnt f anther small aerplane which was ahead in the circuit. The crew f the ther small aerplane tk aviding actin by turning left and away frm the Cessna 172. The distance between the aircraft was estimated t be apprximately 1.5 wing spans. (17/7514) Other Cmmercial Operatins - Helicpters Airspace Incident Wanaka, Rbinsn R22 Beta: Near cllisin during a dual training flight with an agricultural aircraft, estimated 30 m hrizntal and slightly abve. The agricultural aircraft was sptted by the student wh abruptly lwered cllective as aviding actin. (17/7987)

11 Sectin 2 - Incidents Page 10 Agricultural Operatins - Helicpters Airspace Incident Maihiihi, single turbine engine (Hughes 369E): While climbing ut frm the lading site during agricultural peratins, a fixed wing aircraft flew in frnt f the helicpter at apprximately 20 t 30 metres, frm right t left. The aircraft was flying at apprximately 100 ft. CAA safety investigatin cmpleted. (17/6722) Other Airspace Incident Wellingtn, Unmanned aircraft: RPAS bserved 50 t 100 m t right f and at similar height t airliner n apprach. (17/8081)

(17/8274) Twin turbine engine, Nrflk Island: Heavy landing n RWY 11 after aircraft was t slw t arrest sink rate. Wind was 165/19G30. Tuchdwn was hard and with bunce. Lad 15 reprt generated and 1.9G recrded at landing. Ntified engineer f need fr heavy landing check. CAA safety investigatin in prgress. (17/7088) Twin turbprp engine, Auckland: Baggage re-weigh required due t a discrepancy f apprximately 130 kg. This was apparently the furth re-weigh f the day in Auckland. CAA safety investigatin cmpleted. (17/6470) Twin turbprp engine, Auckland: On the gate, prpellers still spinning, beacn n, a member f the lading staff apprached the aircraft and cnnected the GPU. CAA safety investigatin cmpleted. (17/6789) Twin turbprp engine, Auckland: Prir t engine shutdwn with the left #1 prpeller spinning, crew nted a lader pushing the GPU past this prpeller. The lader was between the left wingtip and the nse f the aircraft. CAA safety investigatin cmpleted. (17/7211) Twin turbprp engine, Christchurch: During a training flight, aircraft bunced n landing resulting in a tail scrape. G-arund cnducted fllwed by anther apprach and landing. (17/7757) Twin turbprp engine, Dunedin: G-arund due bunced landing. (17/7051) Twin turbprp engine, Hamiltn: Tail strike due high nse attitude n landing. CAA safety investigatin cmpleted. (17/6547) Twin turbprp engine, Hamiltn: Heavy landing. Tail strike n landing (1.44g). (17/7472) Twin turbprp engine, Napier: Hard landing resulting in a bunce fllwed a tail strike with the aircraft veering hard left. CAA safety investigatin cmpleted. (17/7045) Twin turbprp engine, Napier: Glf Cart Impacted Prpeller Blade. While the grund staff member was mving freight up t the aircraft a bx fell frm the seat f the cart, nt the acceleratr. The cart, unattended, drve frward and impacted the lwest prpeller blade n the N.2 engine. The blade tip was substantially damaged. CAA safety investigatin cmpleted. (17/6695) Twin turbprp engine, Taup: Flaps retracted belw acceleratin altitude. Flap was retracted frm 10 t 0 fllwing the gear up call. Bth crew nticed when the Flap 0 was called fr at the acceleratin altitude (1,800 ft). (17/7812)

13 Sectin 2 - Incidents Page 12 Airspace Incident Auckland, twin turbine engine: Simultaneus take-ff and g-arund. ATC instructed aircraft t hld psitin while aircraft was in the take-ff rll, passing 100 kts. The instructin frm ATC was ambiguus. Take-ff cntinued with the aircraft behind instructed t g-arund and enter visual circuit. CAA safety investigatin in prgress. (17/7682) Auckland, twin turbprp engine: Separatin reduced belw the prescribed minima between aircraft as they climbed ut f Auckland. CAA safety investigatin in prgress. (17/7258) Auckland, twin turbprp engine: A turbprp was lined up fr immediate take-ff as a secnd turbprp was n a 2 mile final. The first turbprp started rlling but was slw t accelerate. As the secnd turbprp was passing 100 ft with the first turbprp still n the runway, crew asked ATC if they were cleared t land. The secnd turbprp was belw 50 ft when ATC called an incrrect call sign instructing aircraft t g arund. Crew initiated a g arund with the first turbprp climbing underneath. The secnd turbprp crew did nt have the first turbprp in sight. ATC kept calling the secnd turbprp crew t remain in the circuit. Master warning activated in the secnd turbprp after gear retractin as flap 30 was still set. CAA safety investigatin in prgress. (17/7544) Auckland, New Zealand Registered: Lss f separatin as bth aircraft climbed ut f Auckland. Separatin reduced t 2.3 NM with aircraft less than 1,000 ft apart. (17/8074) Brisbane, twin turbine engine: Aircraft descended belw the lwest safe altitude during apprach. CAA and TAIC safety investigatins in prgress. (17/7349) Christchurch, twin turbine engine: Aircraft had a near miss with a drne while n final apprach fr RWY 02. (17/7401) Christchurch, New Zealand Registered: Lss f wake turbulence separatin between a large aerplane and anther aerplane. 5 NM separatin required and separatin reduced t 3.3 NM. (17/8027) Hamiltn, New Zealand Registered: Twin turbprp engine cleared fr take-ff and Cessna 172 climbing n upwind instructed t enter the right hand circuit t facilitate the turbprp's departure. The turbprp's departure SID was ffset 11 degrees t the right. The turbprp crew requested Cessna 172's intentins as the Cessna 172 was mid right crss wind. (17/7690) Napier, New Zealand Registered: Lss Of Separatin. Twin turbprp engine requested a visual apprach fr nn-duty RWY 16. They were cleared visual apprach t 5,000 ft and t track twards final fr RWY 16. After the clearance read back it was realised that this wuld put the aircraft in an area f cnflict with anther aerplane departing n the NR- MOOSE track. Aircraft instructed t track t the VOR. 14 Sectin 2 - Incidents Page 13 New Plymuth, New Zealand registered: Aircraft departed New Plymuth when there was a manning shrtage n the Raglan Cntrl psitin. When Raglan is nt n watch, Auckland Cntrl prvides limited service abve 9,500 ft. The upper limit f the New Plymuth cntrl zne is 6,500 ft, and the airspace between 6,500 ft and 9,500 ft becmes uncntrlled airspace. N NOTAM was issued t this effect, crew were nt advised f the shrtfall in service and n IFR traffic infrmatin was passed t the crew. CAA safety investigatin in prgress. (17/6483) Palmerstn Nrth, twin turbprp engine: On apprach RNAV RWY 25 int NZPM. Between NIKIP (IAS) and UVUSI, CM2 side PF GPS started tracking n IVREP. Nticed discrepancy s levelled ff t 5,000 ft and talked t Radar, gave vectrs fr visual apprach. ATC als picked up the errr and will submit a reprt. Airways Reprt: During the final apprach the pilt flying had different GPS pints set up t the pilt mnitring. One pilt had NIKIP IVREP, the ther NIKIP UVUSI. The aircraft turned differently than the cntrllers were expecting. CAA safety investigatin cmpleted. (17/6661) Paraparaumu, twin turbprp engine: While n the RNAV Apprach and n the PP FIS frequency, aircraft was bserved by the radar cntrller fail t turn left as required n the apprach. Aircraft instead cntinued straight ahead fr mre than 1 NM tracking twards high terrain. Radar requested PP FIS t check if aircraft was in VMC but it was nt. Aircraft instructed, thugh PP FIS t cmmence missed apprach immediately. Aircraft issued radar vectrs fr the secnd apprach and was kept n the radar frequency until established n final apprach. CAA safety investigatin cmpleted. (17/6619) Tauranga, twin turbprp engine: Landed withut establishing cmms. Flew the RNAV 07 apprach and landed withut establishing cmmunicatin with the twer. Attempts were made t establish cmms with the aircraft n tw ccasins befre a blind bradcast was made clearing the aircraft t land. The clearance was als issued by way f a light signal frm the signal lamp. At the time the aircraft was being sequenced with three ther light aircraft in the circuit. CAA safety investigatin in prgress. (17/6346) Wellingtn, twin turbine engine: Aircraft appeared t be t fast n tuchdwn and crew made decisin t g-arund. A preceding departure, a twin turbprp, was n a Damb Departure and entered clud at 2,200 ft. The turbine was instructed t enter the circuit at 1,500 ft r belw but crew advised that they preferred the standard missed apprach and climb t 3,000 ft. Essential traffic infrmatin passed t the turbprp crew. When the turbine crew were requested t advise intentins, they advised that aircraft was climbing t 3,000 ft n the standard missed apprach. Aircraft instructed t immediately cntact Wellingtn Terminal Apprach. CAA safety investigatin in prgress. (17/7626) Wellingtn, twin turbprp engine: RPAS encunter in cntrlled airspace. (17/6472)

15 Sectin 2 - Incidents Page 14 Defect Incident Twin turbine engine, Auckland: During climb, crew had t shut ne f the engines dwn due t high vibratin and EGT. Aircraft made a turn-back t Auckland, dumped fuel and flapless landing cnducted. (17/8171) Tandem paraglider, Queenstwn: While inflating the paraglider, a strng thermal came thrugh causing the paraglider t surge frward aggressively. The thermal increased in strength, further pwering up the paraglider and the left side f the wing, frcing the canpy t the right.
