2 Intrductin The purpse f this agreement is t prvide infrmatin regarding perating plicies and prcedures necessary t ensure that peratins at Buckeye Flight Experience, LLC (hereafter COMPANY) cnfrm t Federal Aviatin Regulatins, Insurance Requirements and COMPANY Administrative Plicies and Prcedures. Every student, renter pilt and instructr using COMPANY facilities must be familiar with and abide by the plicies and prcedures f this agreement. Failure t abide by any f the plicies and prcedures may result in the immediate terminatin r suspensin f flying privileges at COMPANY. Instructrs, dispatchers and pilts must be familiar with the cntents f this agreement. It is the intent f COMPANY t ensure that SAFETY is the primary cnsideratin n each and every flight. Cessna Pilt Center Disclsure COMPANY is a Cessna Pilt Center. As such, this PILOT CENTER is an independent business rganizatin. It is nt Cessna's agent, and Cessna is nt respnsible fr PILOT CENTER r PILOT CENTER'S emplyees' representatins, negligent acts, failure t act prperly, and/r missins. Security Prcedures Psitive identificatin is required fr all students and pilts. A valid driver's license, passprt r birth certificate is required in additin t a valid pilt/medical certificates t rent an aircraft. A valid credit card must be n file. N flight instructin will be cnducted until the student pssesses a valid FAA medical certificate. Written Tests & Flight Prcedures Cpies f the written test and flight checkut prcedures are available at dispatch and must be cmpleted befre a pilt will be authrized t fly a COMPANY aircraft. In additin t a prficiency test, a written test is required fr all new pilts renting COMPANY aircraft. All written materials must be reviewed and signed ff by a COMPANY CFI. COMPANY will retain all written tests. A Rental Endrsement Frm will als be reviewed and endrsed by a COMPANY CFI. Rental Agreement

3 Renter Pilt Agreement All renter pilts and instructrs are expected t abide by all applicable Federal Aviatin Regulatins and the Airman's Infrmatin Manual. Additinally, each renter and instructr agrees t the fllwing stipulatins when renting r perating COMPANY aircraft. 1. Aircraft must be scheduled nly fr the time that the renter pilt actually anticipates using the aircraft. Excessive scheduled time vs. actual flight time may result in additinal billing. N-shws may als result in additinal billing fr bth the scheduled instructr and aircraft as applicable. Yur aircraft reservatin may be released if yu are mre than 20 minutes late and have nt called. 2. Flight plans must be filed with the apprpriate Flight Service Statin n all flights utside the lcal flying area (50 nm). A cpy f the flight plan with an estimated return time and cntact number must be left with Dispatch. Exceptin: Flights cnducted within 50 nm s f KPHX d nt require a flight plan. 3. Any aircraft scheduled fr eight hurs (but less than 24 hurs) will be subject t a (3) three-hur minimum rental fee. Thereafter, a minimum (3) three hurs rental fee will be charged fr each additinal 24-hur perid. Aircraft that will be rented vernight require COMPANY management apprval. 4. When landing at airprts ther than Buckeye Municipal Airprt, the renter pilt is respnsible fr tie dwn, hangar, preheating, landing and any ther incidental fees incurred as a result f his/her usage. 5. COMPANY aircraft are nt authrized t be flwn utside the bundaries (3 sm frm the cast) f the Cntinental United States. This includes Catalina Island, Califrnia. An unauthrized flight int Mexic may result in yur arrest by Mexican authrities. Mexican authrities require certified dcumentatin that the aircraft is in Mexic with the wners' permissin. 6. Landing COMPANY aircraft at ther than paved airprts that are published n aernautical charts is prhibited. Landing n nn-paved airprts r ff airprt withut the express written cnsent f COMPANY management may result in immediate suspensin f flying privileges at COMPANY. 7. Renter pilts may nt allw anyne else t pilt r fly COMPANY aircraft. NO dual instructin may be given in COMPANY aircraft by ther than a COMPANY Certified Flight Instructr r a COMPANY Affiliated Instructr. 8. In keeping with FAA regulatins, n COMPANY aircraft may be flwn lwer than 500' AGL except fr takeff, landing r declared emergency. N sl emergency landing practice is allwed. Dual emergency landing practice is limited t 500' AGL unless ver a runway. Minimum cntrllable airspeed, stalls, steep turns and all ther maneuvers nt necessary fr nrmal flight will be terminated abve 1,500' AGL. 9. In the interest f safety, discrepancies nted during the renter's use f the aircraft are t be reprted t Cmpany s management persnnel immediately. If away frm Gdyear Airprt KGYR, call r t receive authrizatin fr required repairs. Yu must call fr authrizatin befre repairs are made. Renters flying mre than 200 NM frm KGYR and are nt n a required crss-cuntry as utlined in the FAR Part 91 are liable fr any maintenance cst abve and beynd the current in huse maintenance cst. If yu are timebuilding and flying mre than 200 NM frm KGYR, this applies t yu. Rental Agreement

Since maintenance delays and weather delays are a nrmal part f the aviatin business, COMPANY will nt be respnsible fr any csts incurred by the renter pilt and his/her passengers as a result f such delays. Such csts may include fd, ldging, grund transprtatin, etc. In additin, if COMPANY is required t recver an aircraft due t negligence f the renter, the cst f recvering that aircraft will be the respnsibility f the renter. This includes, but is nt limited t, grund r air transprtatin, labr csts and mechanical expenses. 11. A detailed receipt must be presented t the dispatcher when seeking reimbursement fr fuel and il purchased fr the aircraft while away frm the Buckeye Municipal Airprt. COMPANY will reimburse full fuel and il expenses. 12. Student pilts must have each and every sl flight authrized by a COMPANY apprved CFI. Insurance requirements mandate that every student sl flight be supervised. 13. Only Instrument rated pilts are authrized t fly night crss-cuntry in COMPANY aircraft. During night hurs, crss-cuntries are restricted t IFR certified aircraft nly. Exceptins: a. Dual instructinal flights. b. Apprved by the Chief Flight Instructr r Directr f Operatins. 14. Student pilts must have their student pilt certificate and lgbk endrsed by a COMPANY apprved CFI befre an aircraft will be dispatched t them. A cpy must als be in the students file. 15. COMPANY Flight Training Weather Minimums Dual VFR Visibility Ceiling Wind Lcal 5 SM 2000 ft AGL Maximum sustained wind: 20 kts Crss-Cuntry 8 SM (actual r frecast) 2500 ft AGL Maximum sustained wind: 25 kts (actual r frecast) Sl VFR Visibility Cluds Wind Lcal 10 SM 2500 ft AGL Maximum wind: 18 kts Crsswind cmpnent nt t exceed 10 kts r limit given by instructr (actual r frecast) Crss-Cuntry 10 SM N muntain bscuratins r Thunderstrms (actual r frecast) 3500 ft AGL Maximum wind: 18 kts Crsswind cmpnent nt t exceed 10 kts r limit given by instructr (actual r frecast) Winds fr lcal r crss-cuntry flights must be less than 10 knts (frecast r actual) unless specifically authrized fr higher n the Student Pilt's custmer card. COMPANY reserves the right t refuse t dispatch an aircraft if a COMPANY Instructr r Dispatcher deems weather cnditins unsafe. 16. A prficiency checkride by a COMPANY apprved CFI is required if the renter pilt has nt made Rental Agreement

5 at least 3 takeffs and landings in the past 90 days. 17. Smking is NOT permitted in the aircraft r n the ramp. This rule applies t passengers as well as pilts. 18. Renter pilts are expected t d everything in their pwer t safely return at the scheduled time. If fr sme reasn beynd the pilt's cntrl, the aircraft will be returned late, call COMPANY Operatins at r if able. The next scheduled pilt will appreciate yur curtesy. 19. Insurance is maintained n the aircraft. The renter will be respnsible fr all csts due t negligence. Renter pilts may purchase additinal insurance thrugh a third party. Rental/Invice Prcedures 1. Aircraft are scheduled by COMPANY dispatch persnnel. Aircraft are nt t be rescheduled r switched withut Dispatch apprval. COMPANY reserves the right t make changes t the scheduled aircraft reservatins prviding that the change will be int the same make and mdel f aircraft. 2. Renter pilts must prduce a pht ID, current pilt certificate, a valid medical certificate and current lgbk when renting an aircraft. Regulatins require the pilt t have his/her current medical and pilt's license "n their persn" while exercising the privileges f his/her certificate. 3. Dispatch persnnel and the renter pilt will verify the Custmer Recrd and Accunt Ledger t ensure all infrmatin is current and accurate. N aircraft will be checked ut unless this dcumentatin is cmplete and accurate. Name, address, phne numbers, checkut endrsements, medical and BFR due dates must be verified. A phtcpy f the renter pilt certificate, medical, and driver's license will be kept n file at COMPANY. 4. The aircraft bk will be given t the renter pilt after the infrmatin in paragraph 3 abve has been verified and the renter pilt has signed the sign-ut sheet and presented his/her sl authrizatin if applicable. This bk cntains a lg fr entering hbbs and tach times as well as a listing f recurring AD's and discrepancies. The hbbs and tach times shuld be verified by the renter pilt befre starting the aircraft. 5. After flying, the renter pilt shuld fill in the ending hbbs and tach times and, if necessary, cmplete a discrepancy frm indicating any inperative equipment r malfunctins. If the hbbs and tach times have begun t rll t the next number, recrd the next number. 6. After cmpleting the flight, the renter pilt is required t prperly secure the aircraft t include installing tie dwn chains, reinstallatin f the sun screens, installing the gust lck, ensuring that the master switch is turned ff and lcking the aircraft. A $25.00 fee may be charged if any f these items are nt cmpleted. The aircraft must be parked in the apprpriately labeled parking spt. Tw bars, purchased with each aircraft are t be used t mve the aircraft int the tie dwn if taxi thrugh is nt achievable. At n time is an aircraft t be "tailed" int the tie dwn psitin. Tw bars are nt t be remved frm the aircraft. Rental Agreement

6 7. When the aircraft bk is returned t the frnt desk, an invice will be generated fr the flight. Payment fr the full balance is due at check in. Payment may be made by cash, check, prepaid accunt r by an accepted credit card. 8. COMPANY des nt ffer credit and all charges must be paid in full at the end f each flight. Any charges incurred as a result f cllecting a delinquent accunt will be added t the balance wed n that accunt. COMPANY des nt ffer credit accunts. A $35.00 charge will be levied fr all returned checks. A valid credit card must be n file. 9. Refund Plicy: Refund f unused mnies will be returned within 5 business days. If payment was made by credit card, the refund will be credited t the card accunt that was used. Fr payments made thrugh financial institutins, refunds will be returned t said financial institutins, nt the individuals. Fr cash custmers, a check will be written cvering returned amunts. 10. Fuel reimbursement fr ffsite fueling is $6.00 per galln. Minimum Checkut Requirements Single Engine, Fixed Gear, 2 & 4 Place, 200 H.P. r Less: (C-172's) Student Pilt Certificate - Must be under the direct supervisin f a COMPANY CFI wh must authrize each student sl flight. Private r better - checkut by a COMPANY CFI. The abve minimum checkut requirements may be waived at the discretin f either the Chief Pilt r the Directr f Operatins f COMPANY. If such waiver is granted, the renter pilt must successfully pass an ral and flight check given by an FAA apprved Cmpany Check Airman. There may be sme special checkut requirements fr each individual aircraft. Please check with a dispatcher r a flight instructr fr details. Currency Requirements T remain current under COMPANY plicy the fllwing criteria must be met: Flwn the same make and mdel aircraft within the previus 90 days (des nt need t be a COMPANY aircraft, but will need a lgbk entry t prvide prf.) If pilt has nt flwn make and mdel aircraft within previus 90 days, the pilt must d three tuch and ges with an instructr. Aircraft Care The fllwing items must be fllwed t prvide gd care fr ur aircraft: Never leave an aircraft untied and unattended. If it is necessary t leave the aircraft, all three tie dwns must be prperly attached, and gust lck installed. Use chcks when parking n a ramp that des nt have tie dwns. Never put anything n the dash f the aircraft. This is t prtect the windshield. Rental Agreement

7 D nt use the glare shield (dash bard) as a handle when adjusting the pilt seats. Grab the windw r dr ledge instead. When parking, shutdwn the engine while still n the yellw line, perpendicular t the parking spt. Trying t turn the aircraft blws dust and may damage ther aircraft drs by blwing them pen. Never push n the tail t mve the aircraft. Always use tw-bars t push back the aircraft. Tie dwn the aircraft by pulling the chain thrugh the eyelet and lping it back n itself, thereby hking it securely. This keeps the chain frm binding 'shuld the aircraft rll frward r backward in the wind. If chains are brken r therwise unusable, let dispatch knw. Install sunshades with the blue side twards the windw. It is imprtant t install these after every flight thrughut the entire year. Installing sunshades significantly decreases the temperature in the cckpit and prevents sun damage f the aircraft interir. Remve all trash frm the aircraft. Please d yur best t leave the plane cleaner than yu fund it. Clse all windws after every flight. This is nt fr temperature cntrl, it is fr dust cntrl. T increase aircraft security, please lck the plane after each use. DO NOT squawk the aircraft if the lck is nt wrking prperly. Rental Agreement
