3 PART A FACTS ABOUT THE EVENTS Date : Throughout 2016 Venue Address : Faculty of Dentistry, Universiti Teknologi MARA : Faculty of Dentistry, UiTM Sg. Buloh Campus, Jalan Hospital, Sungai Buloh, Selangor, Malaysia A series of events have been planned to commemorate the celebration of 10 th Anniversary of the Faculty of Dentistry. Established in 2006, The faculty has emerged competitively with other dental schools in the country and stand tall with other universities in many ways. The achievement throughout the past years witnessed by many others and recognized throughout the fraternity. It is more meaningful that the celebration will be held at our new campus in Sungai Buloh, Selangor, Malaysia. This new facility houses many sophisticated infrastructure. This year, the year of celebration for the faculty and will be translated in numerous interesting events which encompasses every aspect of dentistry and healthcare at large. 12 Terms of Agreement 1. Half payment (50% deposit) is required before sponsorship can be confirmed. Deadline for deposit payment is Jan 24th, Full payment should be made on or before Feb 1 st, The company may send their preferred colored LOGO to the organizer by i ng to nikmohd@salam.uitm.edu.my. The Logo should be in PNG format (in original size with at least resolution of 300 pixels) 4. The dates shown above do not apply for specific event sponsorship. Cancellation Policy 1. Sponsor withdrawing or cancelling an application on or before Jan 24th, 2016will forfeit 25% of the total cost paid. 2. Sponsor cancelling after Feb 1 st, 2016, but on or before Feb 28th t, 2016, will forfeit 50% of the total cost paid. 3. The dates shown above do not apply for specific event sponsorship. 14 Terms of Agreement 1. Date of event: 17-19November Half payment (50% deposit) is required before exhibitor space can be confirmed. Deadline for deposit payment is September 1 st, If payment isn't received by this date it is at the discretion of the organizer to allocate the space to another applicant 3. No sub-leasing of exhibitor space 4. Exhibitors will be notified of their booth location after the confirmation of first payment. 5. The due date for full payment is on 30 th October The exhibitor agrees to the set-up times arranged by ORGANIZER or UiTM Facility Management or appointed official booth supplier and will not dismantle goods and displays until the closing of the conference. 7. Exhibition must be completed, ready and manned by 0900 hrs 17 th November 2016 in time for the registration counter opens. 8. Hours of conference are 9:00 am to 6:00 pm daily 9. The exhibitor agrees to maintain qualified personnel at their booth at all times during the conference hours. 10. Exhibitors are advised to provide proper attire for their employees during the entire Event. This is particularly important during the opening ceremony where we expect the present of VIP to grace the occasion. Cancellation Policy Exhibitors withdrawing or cancelling an application on or before October 15 th 2016 will forfeit 25% of the total cost paid.
