Bking Date Under each trip descriptin a bking date is listed. This is the last day LEEP will guarantee acceptance int a vacatin. If yu are interested in traveling n a trip and the bking date has passed, please cntact LEEP. Travel plans will be made after that date has passed and LEEP may be unable t accmmdate thse requesting participatin. Travelers will be ntified after the bking date f any travel plans, price changes, etc. NEW!! Final Payment Due Dates Under each trip descriptin a Final Payment Due date is listed. This is the required due date fr all travel fees included with trip. Withut prir written apprval, all fees nt paid in full will result in remval frm registered trip. Travelers will be respnsible fr all fees assciated with said trip. A late fee f $50 will ccur if payment is nt received in full by this date. Traveler will be unable t participate in LEEP vacatins and/r prgrams, including Fall Elegance, until balance is paid in full. NEW!! Cancellatin Plicy If a participant chses nt t g n the trip riginally requested and cancels befre the final payment due date, travelers will receive their depsit back, less a 20% return fee. Depsits and/r full payment will nt be refunded after the final payment due date has passed. NEW!! Fees Unless therwise nted, LEEP will require full payment fr trips 60 days prir t departure date fr any trips within the 48 cntiguus states. Any trips utside f thse states such as Hawaii, Alaska, Internatinal Destinatins r a Cruise will require full payment fr their trip, 90 days prir t departure. Travelers will nt have a cnfirmed spt n a trip until the apprpriate depsit, paperwrk, and full payment have been received. Please ask LEEP abut ptinal trip insurance fr flying trips. We encurage the purchase f this thrugh a travel agent t prtect yu frm unfreseen medical emergencies. Participants are respnsible fr making timely payments by dates given within the vacatin package. If payments missed r late, a $50 fee will be assessed. Legal Identificatin Required When traveling with LEEP all travelers are required t bring their legal pht identificatin card r fficial passprt. Yu will nt be allwed n the vacatin withut it. If yu frget yur identificatin card r passprt, LEEP will nt be held respnsible. Yu will nt be able t bard the airplane and yu will nt receive a vacatin refund. Each traveler s identificatin will be checked numerus times thrughut the vacatin s be sure t carry it in a safe place r have LEEP staff carry it. All travelers will be required t have an fficial passprt t travel t any destinatin utside f the United States which includes Canada and Mexic. 3

Cpies f the fllwing frms are included in this vacatin package unless therwise nted. If yu need mre frms yu can visit the LEEP website and print anther cpy. Late frms will incur a $50 fee. 1. Medicatin Release Frm This frm must be cmpleted by staff, guardian r individual and turned in at the time the medicatin is handed ver t the LEEP Staff. This frm will be included with the infrmatinal letters that get mailed ut t all travelers. Travelers ARE allwed t administer their medicatins independently if desired and n frm is required. LEEP is nt a primary care giving rganizatin and staff are nt certified medicatin administratrs. LEEP staff can hld medicatins and remind travelers t take medicatins thrughut the trip. Medicatins must be pre-packaged in individual envelpes r tightly sealed daily medicatin cntainers. Tpical medicatin, eye drps, etc. requiring staff administratin must be apprved prir t trip and will require traveler t prvide necessary safety glves, etc. LEEP staff must take physical pssessin f any medicatin they are t assisting with. 2. Current Pht f traveler All travelers are required t prvide LEEP with a current pht f them due 30 days prir t vacatin departure. If yu are unable t print a new pht, a digital cpy can be ed t inf@mankatleep.rg and it will be added t the traveler s file. (Please include traveler s name and vacatin destinatin within the ) Pht release: must be checked yes in rder fr travelers t have their phts taken n a LEEP Vacatin. If n is selected, travelers will nt be allwed t have their phts taken. Travelers will receive a vacatin jurnal withut phts f themselves r their face will be blurred/ blacked ut. 3. Traveler Infrmatin Frm Be as specific as pssible when cmpleting this frm. LEEP staff will use the infrmatin t better serve each traveler. Frm must be cmpleted each time a traveler takes a trip with LEEP t ensure that we have the mst updated infrmatin. This frm must be submitted t LEEP at least 30 days prir t vacatin departure. 4. LEEP Participant Infrmatin Frm If a traveler des nt have a current LEEP Membership Frm n file at LEEP, ne must be cmpleted and returned 30 days prir t trip departure. Occupancy Duble ccupancy is the standard accmmdatin fr travelers. If a traveler is interested in single ccupancy, please nte this request n yur Vacatin Request Frm. Fees will be adjusted t meet these accmmdatins. Rming Requests If tw peple wish t rm tgether, please indicate this at time f registratin n Vacatin Request Frm. LEEP will make every attempt t meet yur requests, hwever cannt guarantee this. Supervisin during the Night Travelers must be able t g unsupervised during the hurs f 10:00pm 8:00am while in their htel rms. LEEP Staff will always be available if they are needed. LEEP Staff d nt share rms with travelers. Thse requiring an awake staff during nrmal hurs f sleep cannt be accmmdated n LEEP Vacatins. Nrmal hurs f sleep are between 10:00 p.m. and 8:00 a.m. These times may be adjusted due t arrival and departure times. Smking is never allwed in the htel rms. Persnal Care Staff LEEP Staff are unable t prvide ne t ne care fr travelers due t the extent f ur vacatins. If a traveler wuld like t have a residential/persnal staff travel with them, they may d s. Bth the traveler and their staff wuld have t pay the fee fr a duble ccupancy rm. 4
