The golf tournament has an additional registration fee. Please make copies of the registration form for each company representative attending (couples may register on the same form). Complete the registration form and return with payment. Cancellation & Refund Policy: Registrations received after August 13 will be charged a $50.00 late fee. Refund requests after August 13 will be subject to a $50.00 processing fee. No refunds after August 31.
