Market Street Washington C.H., Ohio (740) NEWS HIT SKIP CRASH: Sheriff s deputies responded to 9146 US Route 62 North on a hit skip traffic crash. A cement mixing truck was traveling North on US 62 when some of the cement began falling onto the roadway. No vehicles were damaged, the investigation is ongoing. THEFT: Christy Dulaney of Mount Sterling reported sometime within the past few days persons unknown used her banking information to make an authorized purchase. TRAFFIC CRASH: Charles M. Yahn of Washington CH reported an unknown person struck his motor vehicle while it was parked on the lot of the Heritage Church BURGLARY/THEFT/CRIMINAL TRESPASSING: Sheriff s deputies responded to Buckeye Community Apartments in Union Township in reference to a Burglary complaint. Unknown person(s) entered an apartment while the renter wasn t home taking several items. The investigation is ongoing. BURGLARY; THEFT: Andrew D. Graham of Washington CH reported sometime within the past few days persons unknown entered his residence, removing several items NEIGHBOR DISPUTE: A Jeffersonville resident reported having an ongoing issue with the neighbor about children being bullied FIRE: Jene Baumgardner of Jeffersonville Ohio reported a fire in his grain dryer at ST Rte 729. Deputies, along with members from the Jefferson Twp and Silver Creek Twp Fire Departments arrived on scene to extinguish the fire. DOMESTIC DISPUTE: Sheriff s deputies responded to 8335 Factory Shops Blvd in reference to a domestic dispute. A male and female got into a verbal argument with no one being assaulted.

2 TRAFFIC CRASH: Cindy Meade of Wilmington, Oh was traveling east on US 22 and passed a vehicle on the right. AS Meade was passing the vehicle on the right the vehicle made a right turn into a business striking Meade s vehicle. Meade was transported to a medical facility for treatment. Meade was issued a citation for Improper Passing. TRAFFIC CRASH: Craig Davis of Greenfield. OH was traveling north on Stringtown Rd. Davis travelled off the right side of the roadway for farm machinery striking a culvert, overturning. Davis was treated at the scene by Ems. Davis was issued a citation for Failure to Control. TRAFFIC CRASH/HITSKIP: An unknown person was traveling north on US 62 and lost control of their vehicle. Person unknown traveled off the left side of the roadway striking a guide wire for a utility pole. The driver departed the scene of the crash. GRAND THEFT: Heath Frazier of Jeffersonville Ohio reported person(s) unknown removed a vehicle from 194 Woodsview Dr. DOMESTIC: Sheriff s deputies responded to Dennis Street in reference to a Domestic. A male and female got in to a dispute with the male subject assaulting the female. Jerry Stewart was arrested and transported to the Fayette County Jail. PASSING BAD CHECK/THEFT: Sheriff s deputies responded to Fastenal in reference to a passing bad checks and theft complaint. A known male subject came in to their business and wrote a bad check for 2 items. The investigation is ongoing. THEFT/CRIMINAL DAMAGING: Sheriff s deputies responded to the 1600 Block of US RTE 22 East on a theft and criminal damaging complaint. Uknown person(s) broke out a window on semi and stole items. The investigation is ongoing. THEFT/CRIMINAL TRESPASSING/CRIMINAL DAMAGING: Sheriff s deputies responded to Vermeer on a criminal trespassing, theft and criminal damaging complaint. Unknown person(s) came onto the lot stealing items and damaging property. The investigation is ongoing. PROTECTION ORDER SERVICE: A protection order was served pursuant to Court order. TRAFFIC CRASH: A two-car motor vehicle crash was reported on S R 41 North at Bloomingburg New Holland Road. Haley Davis of Washington CH was cited for Assured Clear Distance Ahead UNRULY: Deputies responded to the Bloomingburg trailer park on a report of an unruly juvenile, declining to go to school. Deputies made contact with the juvenile and transported him/her to school before filing criminal charges. AGG MENACING: A subject reported being threatened by a known male. Investigation pending. THEFT: Sheriff s deputies responded to the 6000 Block on College Street in Milledgeville in reference to a theft complaint. Reports indicate that an unknown person or person stole his license plates from his vehicle. The investigation is ongoing.

3 LITTERING: Sheriff s deputies responded to Harmony Rd and US Rte 35 on a littering complaint. THEFT: Charles W. Crusie of Washington CH reported sometime overnight persons unknown stole his motor vehicle. THEFT: An employee of DSW reported an unknown person had removed merchandise from the business without payment DOMESTIC DISPUTE: Sheriff s deputies responded to N. Railroad Street in Octa in reference to a domestic dispute. A male and female got in to an argument without either party being verbally or physically assaulted TRAFFIC CRASH: A two vehicle traffic crash was reported on US Route 35Westbound near the 9 mile post. Jordan Sagar of Washington C.H., Ohio was traveling Westbound on US Route 35 in the left lane when the vehicle struck a vehicle driven by James Eversole of Enon, Ohio who was traveling Westbound as well in the right lane causing damage. TELEPHONE HARASSMENT: D. Myers of Jeffersonville reported he had received several unwanted messages via Instagram from a known person. Prosecution was declined at the time of the report. DISORDERLY CONDUCT: Rebecca Krupla of Washington CH reported a loud noise complaint near her residence. Rita Shiltz of Washington CH was charged in the offense MISUSE OF CREDIT CARD: A report was made involving misuse of a credit card by a known person. The investigation is ongoing THEFT: Sheriff s deputies responded Tommy Hilfiger in reference to a theft complaint. Two female subjects stole items from the store. Deputies located the females on the lot of Tanger Outlet and were able to get the items back. Shuka Garrison & Kevina Griffin, both from Columbus, Ohio were arrested. HIT SKIP TRAFFIC CRASH: Sheriff s deputies responded to Allen Rd at the intersection of SR 435 on a hit skip traffic crash. Kylie Hughes of Sabina, Ohio was on Allen Rd stopped at the traffic light at the intersection of SR 435 in the middle lane waiting to go straight. Hughes stated a semi was in the left turn lane next to her, and that when the light turned green the semi turned left and that his rear trailer struck her vehicle causing damage. Hughes stated the semi continued traveling Westbound on SR 435. TRAFFIC CRASH: A two-car motor vehicle crash was reported on S R 41 north at the Cruz Thru North filling station. Holly A. Griggs of Washington CH was cited for Failure to Yield. TRAFFIC CRASH: Michael A. Riddell of Sabina reported he had struck a deer on S R 41 North near S R 734. DISORDERLY CONDUCT: Dona Langley of Washington CH reported a loud noise complaint near her residence. Rita Shiltz of Washington CH was charged in the offense.

4 DOMESTIC DISPUTE: Sheriff s deputies responded to Bogus Rd on a domestic incident. Upon arrival all parties involved were identified and it was determined that no assault or threats of violence had occurred CRIMINAL DAMAGING: Sheriff deputies responded to Lincoln Street in reference to a criminal damaging complaint. Reports indicate that a known female caused damage to his vehicle. The victim declined criminal charges. DISORDERLY CONDUCT: Dona Langley of Washington CH reports excessive loud noise coming from the neighboring property. Deputies made contact with the neighbors and a summons for disorderly conduct was issued. CIVIL DISPUTE: Crystal Terhune of Washington CH reported she was involved in a property dispute with a known person. She was referred to her attorney VIOLATION OF PROTECTION ORDER: Rachel McGraw of Washington CH reports being with a male subject on who she has a protection order against. Justin Smith was arrested in the incident. ILLEGAL CONVEYANCE OF DRUGS/POSSESSING DRUG ABUSE INSTRUMENTS: An inmate was found to have brought in illegal narcotics and drug abuse instruments into the jail after a search. PROTECTION ORDER VIOLATION: A third party reported a protection order violation. ASSAULT: Deputies responded to the Fayette County Jail on an Assault complaint. Two known males got into an altercation however prosecution was declined RECOVERED STOLEN VEHICLE: A sheriff s deputy located a vehicle which had been reported stolen through the Washington Court House Police Department while patrolling U S Rte 35 near the #9 mile post. SUSPICIOUS: Larry Young of Washington CH reported he witnessed a male subject in all dark clothing walk up to an empty building and hide behind the dumpster. BURLARY/THEFT: Keith Vanslyke of Washington CH reported person(s) unknown forcibly entered his residence and removed items from inside DRUG PARAPHERNALIA: Staff at the Miami Trace High School located drug paraphernalia on a juvenile. The student was charged out of the incident. DRUG ABUSE: Sheriff s deputies responded with EMS to Bogus Rd on reports of an unresponsive male. Upon arrival patient was located and transported to a medical facility for a possible drug overdose.

5 TRAFFIC CRASH: Sheriff s Deputies responded to a one vehicle crash on State Route 435 within Jefferson Township. Charles Huff of Columbus Grove, Ohio was traveling westbound when he lost control of his vehicle striking a concrete barrier. The driver was cited for failure to control. CIVIL: Sheriff deputies responded to the 4000 block of US Route 62 South on a Civil incident. Robert Edwards of Washington C.H., Ohio entered in to a verbal agreement with a known male subject and that the male subject has failed to provide the services that were agreed upon. CRIMINAL DAMAGING: Sheriff s deputies responded to 2000 Block of Bogus Rd in reference to a criminal damaging complaint. Unknown person(s) damaged a mailbox. HIT SKIP TRAFFIC CRASH: Sheriff deputies responded to a hit skip crash on State route 435 near West Lancaster Rd within Jefferson Township. Reports indicate that an unknown vehicle struck a metal traffic divider causing damage then departed. The investigation ongoing ABDUCTION: Renee Smith of Chillicothe reported she was held against her will while traveling in a motor vehicle. Prosecution was declined at the time of the report. ILLEGAL CONVEYANCE: A deputy of the Fayette County Jail reported a known person had entered the facility with suspected narcotics hidden on their person. PROPERTY DAMAGE: Sheriff s deputies responded Miami Trace Rd near US Route 22 West in reference to a property damage incident. Gregory Cartee of Washington C.H., was spraying a field and struck a low power line. Damage was caused to the power pole and utility line. DOMESTIC DISPUTE: Sheriff s Deputies responded to Grace St. on a Domestic Dispute. The dispute was between two household members but there were no threats or assault. DRUG ABUSE/POSSESSING DRUG ABUSE INSTRUMENT: Sheriff s Deputies responded to State Route 41 NW on a possible overdose. The female was transported by EMS to a nearby medical facility for further treatment. TRAFFIC CRASH: Sheriff s Deputies responded to a one vehicle crash on Fishback Rd. within Concord Township. Mary Stout of Wilmington, Ohio was traveling westbound when she lost control and went off the roadway striking a fence. The driver was cited for failure to control MISUSE OF CREDIT CARD/THEFT: Anthony Trout of Jeffersonville reported unknown person(s) stole his credit card. BREAKING & ENTERING/THEFT: Dee Rogers of Wilmington, Oh reported person(s) unknown entered a shed on SR 41 and removed items from inside. TRAFFIC CRASH: Sheriff s deputies responded to The TA Truck Stop on a crash. A male subject got out of his truck failing to set the parking brake. The truck then rolled forward unoccupied striking another truck causing damage. No injuries were reported. DISORDERLY CONDUCT: Sheriff s deputies responded to the 800 block of Staunton-Sugar Grove Rd in reference to a noise complaint. While deputies were in the area of the House of Pain,

6 they were able to hear the noise. A summons was issued to the owner of the House of Pain, Rita Shiltz. TRAFFIC CRASH: Sheriff s Deputies responded to a one vehicle crash on State Route 753 within Wayne Township. Vincent Morris of Bainbridge, Ohio was traveling northbound when his vehicle struck debris in the roadway. BREAKING AND ENTERING/THEFT: Sheriff deputies responded the 1500 block of State Route 41 north in reference to a breaking and entering and theft complaint. A male subject forced his way into the business then stole items. The investigation is ongoing. TRAFFIC CRASH: A one-car motor vehicle crash was reported on S R 41 South near Ghormley Road. Dillon Faulconer of Hillsboro was cited for Failure to Control. Faulconer was additionally charged with OVI and Underage Consumption BREAKING AND ENTERING/THEFT: Sheriff s deputies responded to 3000 Block of State Route 41 North on a B&E, Theft complaint. Unknown person(s) stole several items. BURGLARY/THEFT: Alicia Zimmerman of Washington CH reported during the daytime hours persons unknown forced entry to her residence, removing several items. ASSAULT: Ashley B. Hudson of Wilmington reported she was assaulted by a known person. Prosecution was declined at the time of the report. DOMESTIC VIOLENCE: Mikka Winters of Washington CH reported being assaulted by known family member. Tony Peterman of Washington CH was arrested and charged in the matter. TRAFFIC CRASH: Marshall Winner of Cincinnati, OH was traveling east on US 35 and struck a deer in the roadway. No injuries were reported. TRAFFIC CRASH: Casey Hamner of Chillicothe, OH was traveling west on Old US 35. Hamner attempted to make a left turn onto Palmer Rd failing to yield for a vehicle traveling east on Old US 35 causing a collision. No injuries were reported. Hamner was issued a citation for Failure to Yield THEFT: Sheriff deputies responded to the 4000 Block of Cisco Rd in reference to a Theft complaint. A money order was sent to an unknown subject thought to be a moving company. It was later determined to be a scam and the company did not exist. The investigation is ongoing. BURGLARY/THEFT: Sheriff s Deputies responded to U.S. Route 62 on a Burglary and Theft complaint. Reports indicate that person(s) gained entry into the residence and removed various items. THEFT: Brad Craig of Washington CH reported person(s) unknown removed a cell phone without consent. Deputies later located the cell phone and returned it to owner. TAKING THE IDENTITY OF ANOTHER: Andrew Hardbarger of Washington CH reported person(s) unknown using his identity has items delivered to his residence.

7 THEFT/RECEIVING STOLEN PROPERTY: James Wheeler of Jasper Township reported a motor vehicle theft. The vehicle was recovered and case remains under investigation. THEFT: Colin Campbell of Jeffersonville contacted the Sheriff s Office reference a known male removing his property from storage shed. Investigation pending THEFT: Benjamin C. Everetts of Washington CH reported a known person had removed an item from his residence. The known person was contacted and the item was returned. BURGLARY: Kyle Leisure of Washington CH Ohio reported a male and a female entered an apartment at 812 Delaware St. Deputies made contact with the tenant of the apartment who advised the female had permission to be in the apartment. MENACING: Patricia Coil of Washington CH Ohio reported being threatened by her neighbor, Jason Bice of Washington CH Ohio. Bice was arrested in the incident. RECEIVING STOLEN PROPERTY: Kenneth Kelly of Washington CH reported locating his stolen trailer at Moor-Acre Lake. Deputies made contact and confirmed the trailer was stolen. The trailer was returned to the owner. THEFT: Johnnie W. Carner of Washington CH reported a known person had deprived him of his personal property. Prosecution was declined at the time of the report ASSIST OTHER AGENCY: Sheriff s deputies assisted the Madison County Sheriff s Office with a combative Suspect. Jeffery L. May of Sandusky was taken into custody. PROTECTION ORDER SERVICE: A protection order was served in the Jail on a respondent. TRAFFIC CRASH: Brian Melody of Ames, Iowa reported he had struck a deer on U S rte 62 South near Barger Road. UNRULY: Miami Trace High School reported a known juvenile ran away from school. Deputies responded and charges were filed on the juvenile. DRUG ABUSE/DRUG ABUSE INSTRUMENTS: Sheriff s deputies responded to Pearson Octa Rd on a male subject possibly under the influence. Deputy made contact at which time a hypodermic syringe was located. DOMESTIC DISPUTE: Sheriff s deputies responded to West Lancaster Rd on a domestic incident. Upon arrival all parties involved were identified and it was determined that no assault or threats of violence had occurred INDUCING PANIC: Sheriff s deputies responded to a bomb threat made on the Miami Trace Schools. THEFT/OBSTRUCTING OFFICIAL BUSINESS: Tommy Hilfiger reported person(s) unknown entered the business, obtained property and departed without making payment. Deputies made

8 contact with suspects locating the stolen merchandise. Marquan Green of Columbus, Oh was arrested and charged in the offense. TRAFFIC CRASH: Jackson Glover of Dover was traveling from a parking space at Flying J truck stop striking a parked semi on the lot. No injuries were reported. DRUG ABUSE/DRVING UNDER THE INFLUENCE: Sheriff s deputies responded to SR 41 near Bloomingburg New Holland Rd on a vehicle driving recklessly. Deputies initiated a traffic stop on the vehicle after numerous violations were observed. The driver appeared to be under the influence. During the investigation drugs of abuse were located. John Johnson Jr of Washington Ch was arrested and charged in the matter. BURGLARY;THEFT: Donald Harris of Bloomingburg reported a known person entered his residence several items, and stole his motor vehicle FIRE: Sheriff s deputies responded to Sheley Rd on a house fire. Jefferson township Fire responded along with other assisting agencies to extinguish the fire. CHILD ENDANGERING: Sheriff s Deputies responded to a complaint reference child endangering. Achild was seen outside with no adults around. No criminal charges were filed. MENACING: John J. Scott of Washington CH reported a known person threatened to damage his personal property. Prosecution was declined at the time of the report. THEFT: David Brown of Jeffersonville reported an unknown person removed items from his residence. THEFT/CRIMINAL TRESPASS/CRIMINAL DAMAGING: Garry K. Matthews of Jeffersonville reported sometime overnight persons unknown trespassed on his property, damaged and entered his motor vehicle, removing several items PROTECTION ORDER VIOLATION: Corey A. Jarrell of New Holland was arrested after a sheriff s deputy witnessed a violation of a protection order. TELEPHONE HARASSMEN: Karen L. Murphy of Washington CH reported she had received an unknown unwanted text message from a known person. THEFT: Angela Dalton of Washington CH reported an unknown person taken money from her juvenile child DRUG ABUSE: Sheriff s Deputies responded to the area of Colonial Dr. within the Village of Jeffersonville for a reported overdose. EMS responded and transported the victim to a nearby medical facility for treatment. TRAFFIC CRASH: Sheriff deputies responded to State route 435 near Garringer Edgefield Rd on a 2 vehicle traffic crash. Roberto Mendoza was traveling Eastbound on State Route 435 near Garringer Edgefield Rd when Mendoza missed his turn onto Garringer Edgefield Rd. Mendoza

9 attempted to back up but failed to see another vehicle behind him being driven by Aaron Young of Eaton Ohio. Minor damage was reported. No injuries. Mendoza was cited for improper backing UNDERAGE CONSUMPTION: Staff at the Miami Trace High School located a known juvenile consuming alcohol on school property. The student was charged. DRUG PARAPHERNALIA/DRUG ABUSE: Deputies responded to the Miami Trace High School on reports of staff locating an illegal substance and paraphernalia. A 16 year old male was charged out of the incident. TRAFFIC CRASH: Bradley Fessler of Washington C.H. was traveling west bound on Allen Rd when a deer entered the roadway causing a collision. No injuries were reported. FOUND PROPERTY: Sheriff s deputy responded to SR 41 on a found property complaint. Deputy recovered a purse that was located on the property. The purse was placed into property for safekeeping. TELEPHONE HARRASSMENT: A Jeffersonville resident reported receiving unwanted calls and texts from a known subject. CRIMINAL DAMAGING/THEFT: Joyce Hagler of Jeffersonville reported a piece of plastic missing from her vehicle. ASSAULT; CRIMINAL DAMAGING: John Barnett of Washington CH reported a known person assaulted him, and damaged some of his property. Prosecution was declined at the time of the report TRAFFIC CRASH: A single-car motor vehicle crash occurred near 3596 U S Rte 22. Derek J. Myers of Jeffersonville was cited for Failure to Control. TRAFFIC CRASH: A 16 year old female was traveling west bound on Barrett Rd when she traveled off the right side of the roadway. She entered the ditch at which time the vehicle rolled twice and struck a fence. The driver was issued a citation for failure to control. No injuries were reported. TRAFFIC CRASH: A two-vehicle private property crash was reported on the Truck lot of the Flying J. DOMESTIC DISPUTE: A South Solon resident reported she was involved in verbal dispute with a household member. CASUALTY: A deputy of the Fayette County Jail reported a fight had broken out between two inmates. When the deputy intervened in the fight he sustained a minor injury. TRAFFIC CRASH: Hester Hatfield of South Salem reported she was on South Fayette Street near Ohio Avenue when her motor vehicle was splashed with tar by an unknown passing vehicle. FOUND PROPERTY: A sheriff s deputy located a vehicle registration while clearing roadway debris from the interstate.

10 BREAKING & ENTERING/THEFT: David E. Carson of Mount Sterling reported sometime within the past 9 days persons unknown entered an outbuilding, removing several items. ASSAULT: Deputies responded to the Old Jeffersonville School on reports of an altercation. All juvenile parties and guardians were contacted. Case remains under investigation. BREAKING & ENTERING;THEFT : James N. Payton of Washington CH reported sometime overnight persons unknown entered his property, stealing his lawn mower. ASSIST OTHER AGENCY: The Fayette County Sheriff s Office assisted the Washington CH Police Department in a vehicle pursuit. The pursuit ended in the village of Sabina where two men were taken into custody. A Sheriff s patrol cruiser was damaged in the incident. MENACING: Zachary Miqbel of Washington CH reported he was verbal threatened by a known person. Prosecution was declined at the time of the report TRAFFIC CRASH: Adrian Cadena of Campbell, KY was traveling north on State St. Cadena changed lanes from the right lane to the left lane striking a vehicle. No injuries were reported. Cadena was issued a citation for Improper Passing. TRAFFIC CRASH: Amber Self of Bloomingburg was traveling north bound on State Route 41 within Union Township when a tree limb fell from a tree striking her vehicle. No injuries were reported. FIRE: Sheriff s deputies responded to Wentz Rd with Jefferson Township FD on a fire involving vehicle tires. The fire was extinguished and FD will follow up with investigation. TRAFFIC CRASH: Alexis Tyree of Bloomingburg was backing from a parking space at Leading Edge Technologies. Tyree failed to observed a parked vehicle striking the rear of the vehicle. No injuries were reported. THEFT: Sunglass Hut reported person(s) unknown entered the business, obtained property and departed without making payment. THEFT: Paige Pate of Jeffersonville reported person(s) unknown made unauthorized purchases on her credit card. THEFT: Lou Boyle of Columbus, Oh reported after leaving property at a business the property was removed prior to making pickup. TRAFFIC CRASH: A single-car motor vehicle crash was reported on Allen Road near Milledgeville-Octa Road. Thiruvenkadem Sunil Kumar of Scranton, Pennsylvania refused medical treatment on the scene, and was cited for Failure to Yield. TRAFFIC CRASH: A single-car motor vehicle crash was reported on Allen Road near Hamilton Street in Octa. George R. Kingery of Washington CH refused medical treatment on the scene, and was cited for Failure to Yield. Kingery was cited additionally for OVI.

11 TRAFFIC CRASH: Sheriff s Deputies responded to a deer crash on U.S. Highway 22 within Marion Twp. Cynthia Carle of New Holland, Ohio was traveling eastbound when a deer entered into the path of her vehicle. No injuries reported at time of crash TRAFFIC CRASH: Philip Butler of Washington C.H. was traveling east bound on Old Springfield Rd when he traveled off the right side of the roadway. He overcorrected crossed the center line and ran off road left entering the ditch, traveled thru the ditch and struck a tree and a fence. The Fayette County EMS, Fayette County Sheriff s Response Team, and the Bloomingburg Fire Department all responded to the scene. Butler was issued a citation for failure to control. Butler was transported to an area hospital for minor injuries. THEFT: Ebben Moss of Washington CH reported a bottle of medication was removed from the residence. FAILURE TO CONFINE DOG: Ashley J. Phillips of Bloomingburg reported a dog at large in the village. The dog was later confined by the owner. TRAFFIC CRASH: Sheriff s deputies responded to Allen Rd near SR 435 in reference to a traffic crash. Terry Lutz of Urbana, Ohio was attempting to back up when Lutz failed to see Marcia Collier of Milford, Ohio behind her. Lutz then struck Collier causing damage. No injuries were reported. Lutz was cited for Improper Backing. ENDANGERING CHILDREN/ASSAULT: A report was made regarding a possible endangered child who had been assaulted BREAKING & ENTERING;THEFT: Allen Hale of Mount Sterling reported persons unknown forced entry to his detached garage, removing several items. TRAFFIC CRASH: Sheriff s Deputies responded to a deer crash on Old U.S. Route 35 within Wayne Twp. Marcus Morris of Chillicothe, Ohio was traveling westbound when a deer entered into the path of his vehicle. No injuries reported. FATAL TRAFFIC CRASH: At approximately 1704 hours while on routine patrol a deputy with the Fayette County Sheriff s Office came upon a one vehicle motorcycle crash on Stafford Rd just south of US 62 in Green Township. The deputy located the motorcycle and operator lying off of the right side of the roadway in a beanfield. EMS arrived on scene and determined the motorcycle operator identified as Rodney M. Sherod age 69 of Waynesville Ohio to be deceased. Fayette County Coroner Dr Mesker responded to the scene and requested transport to Montgomery County Coroner s Office for further examination. The Fayette County Sheriff s Office was assisted on scene by Fayette County EMS, Concord-Green FD and the Fayette County Crash Response Team. The crash is currently under investigation by the Fayette County Sheriff s Office Crash Reconstruction Team. THEFT/CRIMINAL DAMAGING: Deputies responded to the Tanger Outlet Mall reference a theft. Upon arrival contact was made with an employee of Tommy Hilfiger as well as the suspect. The employee advised the customer was exiting the store and the door alarm went off indicating he was in possession of security tagged items. Upon speaking with the customer he advised he had concealed items in his bag and attempted to leave the store without paying. It was determined that

12 one item had been damaged during the theft. Joshua E. Galloway age 21 of Columbus Ohio was taken into custody and charged for the offenses. TRAFFIC CRASH: Sheriff s Deputies responded to Factory Shops Blvd near State Route 435 for a 2 vehicle traffic crash. Sandra Scott of Jamestown, Ohio was stopped in the right lane on Factory Shops Blvd waiting for the light to change, when Mitchell Lujan of Concord, California who was in the left lane began backing up in to Scott s lane. Lujan then struck Scott s vehicle with his trailer causing damage to Scott s vehicle. No injuries were reported, Lujan was cited for Improper Backing. TRAFFIC CRASH: Deputies responded to a one vehicle hit skip/traffic crash in the 1200 block of Bogus Rd. Witnesses advise of a grey PT Cruiser traveling northbound on Bogus Rd and when it traveled off the right side of the roadway and struck a mailbox. The witness advises the vehicle continued northbound and failed to stop. The incident is under investigation BREAKING & ENTERING;THEFT: An employee of Country Cakes & Bakes of New Holland reported during the early morning hours persons unknown entered the business, removing merchandise. BURGLARY/THEFT: Sheriff deputies responded to the 8000 Block of US Route 62 North in reference to a Burglary and theft complaint. Unknown person(s) gained entry into a residence and stole several items. THEFT: Deputies responded to a complaint of theft at a residence on St Rt 62N. Upon arrival it was determined sometime over the last 24 hours unknown persons removed prescription medication from his residence. The incident is under investigation. ASSAULT: Billy R. Groves of Washington CH reported his juvenile son was assaulted by another juvenile THEFT: Kathryn Reser of Sabina Ohio reported person(s) unknown removed a scarecrow from her property. TRAFFIC CRASH: Randall A. Childs of Reedsville, West Virginia reported while traveling southbound on I-71 near the # 75 mile post he had struck a deer DRUG ABUSE/NALOXONE USE: Sheriff s deputy responded to an EMS scene. Naloxone was administered & EMS transported the victim for further treatment. ASSAULT: A juvenile reported being assaulted by another known juvenile. CRIMINAL TRESPASS: Randy Wallen of Washington CH reported a known person had trespassed on his property. MENACING: Threats were reported by a resident of Washington CH. She was referred to the City Solicitor

13 HIT SKIP TRAFFIC CRASH: Sheriff Deputies responded to a hit skip traffic crash on State Route 435 near the 71 Southbound exit ramp within Jefferson Township. A blue vehicle exited Interstate 71 Southbound then turned in to the construction zone where concert had just been finished causing damage. No injuries were reported. The crash is still under investigation. CRIMINAL DAMAGING: Nathan Spurgeon on Jeffersonville Ohio reported person(s) unknown caused damage to a wooden privacy fence belonging to the Jeffersonville Green Apartments. TRAFFIC CRASH: Thomas Cordray of Ashville Ohio was traveling westbound on US Rte 22 at Old Springfield Rd. Cordray failed to stop with assured clear distance ahead and rear-ended a second vehicle attempting to right onto Old Springfield Rd operated by James Cain of Lancaster Ohio. Cordray reported minor injury and declined transport to the hospital. Cordray was also cited in the incident. DOMESTIC DISPUTE: Stephen O Brien of Greenfield reported he was involved in a verbal dispute with a known person. THEFT: Michelle M. Williamson of Washington CH reported during the month of July, persons unknown had removed numerous items from her residence. ASSAULT: Brandon S. Warner of Washington CH reported while incarcerated at the Fayette County Jail he was assaulted by another inmate. Prosecution was declined at the time of the report PROTECTION ORDER SERVICE: Sheriff s deputies made contact with a respondent in a protection order. TELEPHONE HARASSMENT: Derek J. Meyers of Jeffersonville reported he had received an unwanted telephone call from a known person. DOMESTIC DISPUTE: Sheriff s Deputies responded to W. High St. in the Village of Jeffersonville for a Domestic Dispute compliant. No threats or assault were alleged. UNAUTHORIZED USE OF MOTOR VEHICLE: Sheriff s Deputies responded to White Oak Rd. in Madison Township for a Unauthorized Use of Motor Vehicle. Francisco Cruz of Washington C.H. advised that a known person took his vehicle without his permission. Prosecution was declined at the time of report TRAFFIC CRASH: Blake Rife of Washington CH was traveling west on Ghormley Rd and lost control of his vehicle. Rife traveled off the left side of the roadway striking a guardrail. No injuries were reported. Rife was issued a citation for Failure to Control. ROBBERY: Deputies responded to Fayette Memorial ER reference a Robbery and Assault complaint. The victim was traveling on US 62 when the driver stopped to assist 2 unknown persons. The victim stated the 2 persons pulled her from her vehicle and assaulted her and removed several items from her vehicle before departing. The incident is under investigation DOMESTIC DISPUTE/CRIMINAL DAMAGING: Sheriff deputies responded to West Front Street in New Holland reference to a domestic Dispute. A male and female got in to a verbal argument without either person being verbally or physically assaulted. During the argument the

14 male subject damaged some of the female s clothing. The female declined charges for the damages to her clothing BURGLARY/THEFT: Sheriff deputies responded to Eyman Rd within Wayne Township on a reported burglary. Person(s) entered the residence removing items. The investigation is ongoing. BURGLARY;THEFT: Clayton Lowe of Jeffersonville reported an unknown person had entered his residence, removing several items. DOMESTIC VIOLENCE: Carrie M. Taulbee of Jeffersonville reported she was assaulted by a household member. TRAFFIC CRASH: Sheriff s deputies responded to Lewis Rd near State Route 38 within Paint Township in reference to a single vehicle crash. Reports indicate that Brittany Goen of Washington C.H., Ohio was traveling Westbound on Lewis Road when she was passed by another vehicle. Goen stated that she thought the vehicle was going to hit her so she swerved which caused her to lose control of her vehicle going off the left side of the roadway into the ditch and over turning. Minor injuries were reported, Goen was cited for failure to control TRAFFIC CRASH: Sheriff s deputies responded to the off ramp of Interstate 71 Southbound at the intersection of State Route 41 in reference to a 2 vehicle hit skip traffic crash. Reports indicate that Andrew Magyarics of Mantua, Ohio was stopped at the stop sign in the left lane when a semi pulled up next to him and stopped in the right lane. Magyarics stated that the semi then made a wide left turn striking his vehicle with his trailer. Maryarics stated that the semi continued traveling Eastbound in an unknown direction. No injuries were reported THEFT: Karen Brown of Jeffersonville reported a known subject removed items from her property. TRAFFIC CRASH: Daniel Haynes of London OH was traveling north bound on State Route 38 just north of State Route 734 when he crossed the center line. Haynes over corrected and ran off road right entering the ditch. He continued thru the ditch and crossed a private drive were his vehicle began to roll. The vehicle continued to roll and struck a tree. The vehicle continued and struck a second tree. The three passengers Christopher Tracy of Columbus Oh, Jessica Payne of Bloomingburg Oh, and Dylan Peterman of London Oh were all ejected from the vehicle. MedFlight and Care Flight both responded to the scene and transported all occupants to a Columbus Hospital. The Fayette County Sheriff s Response Team, Fayette County EMS, and the Bloomingburg Fire Department all responded to the scene. TELEPHONE HARASSMENT: James E. Kuebler of Bloomingburg reported he had received an unwanted telephone call from a known person. Prosecution was declined at the time of the report TRAFFIC CRASH: Sheriff deputies responded to US RTE 35 East near Palmer RD in reference to a 2 vehicle traffic crash. Reports indicate that Kevin Mcbee of Washington C.H., Ohio was traveling Eastbound on US RTE 35 when Mcbee went off the right side of the roadway and struck an unoccupied disabled vehicle that was parked on the shoulder. No injuries were reported, Mcbee was cited for failure to control.

15 TRAFFIC CRASH: A two-car motor vehicle crash was reported on the lot of Carr Supply. Sean Peters of Washington CH reported while backing out of the parking lot he had struck a parked vehicle. CRIMINAL DAMAGING/ASSAULT: Shane M. Osborne of Jeffersonville reported a known person caused damages inside his residence, and assaulted him. Prosecution was declined at the time of the report. DOMESTIC DISPUTE: Two household members reported being involved in a verbal dispute TRAFFIC CRASH: Juan Jurado of Miami FL was traveling west bound on the parking of the Flying J attempting to turn left. He failed to turn wide enough and side swiped a parked semi occupied by John Bowers of Nashville TN. Both vehicles suffered minor damage. No injuries were reported THEFT: Connie Loukinas of Washington CH reported she allowed a known subject to borrow an item and now the item has been posted for sale on Facebook. TELEPHONE HARRASSMENT: A Jeffersonville resident reported receiving threats on Facebook. DOMESTIC DISPUTE: Steven Dilley of Washington CH reported he was involved in a verbal dispute with household member TELEPHONE HARASSMENT: Sharon K. Cox of Washington CH reported she was verbally threatened by a known person. Prosecution was declined at the time of the report. FIRE: Sheriff s Deputies responded to Allen Ave. in the Village of New Holland for a reported house fire. Reports indicate while mowing, the lawn mower caught the power line pulling it down, the power line then caught fire starting the house fire. TRAFFIC CRASH: An unknown person operating a tractor-trailer attempted to make a right hand turn from East Walnut Street onto Fawkes Street in Jeffersonville. While making the turn the vehicle struck a stop sign, and a fence. Following the crash the vehicle departed the area. DOMESTIC DISPUTE: Sheriff s deputies responded to Biddle Bl on a domestic incident. Upon arrival all parties involved were identified and it was determined that no assault or threats of violence had occurred DOMESTIC DISPUTE: Sheriff s Deputies responded to Jamison Rd. on a Domestic Dispute complaint. A male and female got into a dispute that occurred with no threats or assault. DISORDERLY CONDUCT: Sheriff s deputies responded to the Sheriff s Office for an intoxicated female in the lobby. Upon contact Annette Burris of Washington Ch was arrested and charged in the offense

16 FOUND PROPERTY: Reportee contacted the Sheriff s office after locating some property. It was recovered and logged it into evidence for safe keeping for the owner. ILLAGEL CULTIATION OF MARIJUANE: Deputies were dispatched to the area of Harmony Rd in reference to marijuana plants being located in a corn field. The plants were taken and logged into evidence. CRIMINAL DAMAGING: A Greenfield resident reported a known subject caused damage to her cell phone. TRAFFIC CRASH: Franklin McCullough of Washington CH was operating an ATV on Barrett Rd and lost control. McCullough traveled off the left side of the roadway overturning. McCullough was transported to a medical facility for treatment. AGGRAVATED MENACING/DISCHARGE FIREMARM: Sheriff s Deputies responded to the area of N. Main St. in Jeffersonville for an Aggravated Menacing and shots fired complaint. A firearm was allegedly discharged in the area. A male advised that he had been recently threatened by a known male. The incident is under further investigation. TRAFFIC CASH: Deputies responded to a 2 vehicle traffic crash on Factory Shops Blvd. near US 435. Upon investigation it was determined that a 16 year old driver was turning left into the entrance of KFC from Factory Shops Blvd when she failed to yield to a 2014 Ford operated by Adam Frisch, age 41 of Wilmington Ohio. The Ford was struck in the driver s side causing functional damage to both vehicles. No injuries. The juvenile driver was issued a citation for Failure to Yield Turning Left TRAFFIC CRASH: William Greene of Portland, Tennessee was attempting to make a right turn when he struck another vehicle parked. No injuries were reported. TRAFFIC CRASH: Randolph Porter of Westerville, Ohio was southbound on State Route 38 in a combine that was over the centerline when Porter struck another vehicle traveling northbound. No injuries. Porter was cited for marked lanes. MENTALLY ILL: Sheriff s deputies responded to a complaint of a mentally ill person. The person had threatened to self-harm. The victim was transported to the Fayette County Memorial hospital for an evaluation. TRAFFIC CRASH: 17 y/o juvenile was traveling north on Harrison Rd and lost control of his vehicle. The vehicel travled off the left side of the roadway into a field. No injuries reported. A citation was issued for Failure to Control. ASSAULT: Deputies responded to the Fayette County Jail in reference to an assault. Upon completion of the investigation Chrystal Shadley of Washington CH was charged with assault. Minor injuries were reported ENDANGERING CHILDREN: Deputies responded to Summer Tree Apartment in Jeffersonville in reference to a endangering children complaint. All parties were contacted and the case was forwarded to the Fayette County Children Services for review.

17 INTERFERENCE WITH CUSTODY: A custody dispute was reported. The juvenile was located & found to be with his mother. She agreed to return custody to the male without further incident. NEIGHBOR DISPUTE: Sheriff deputies responded to Fairview Rd within Wayne Township in reference to a neighbor dispute. Two neighbors got in to a dispute over the property lines. No one was verbally or physically assaulted. DOMESTIC DISPUTE: Sheriff s deputies responded to Rockwell Rd on a domestic incident. Upon arrival all parties involved were identified and it was determined that no assault or threats of violence occurred. ATTEMPTED ROBBERY: Sasha White of Leesburg reported while walking to her car at Tanger Outlets an unknown make subject grabbed her purse and attempted to flee. TRAFFIC CRASH: Edward Swain of Lebanon, Oh was traveling west on SR435. Swain failed to observe a vehicle stopped in traffic striking the vehicle. No injuries were reported. Swain was issued a citation for Assured Clear Distance Ahead. DOMESTIC VIOLENCE: Sheriff s deputies responded to Yorkshire Dr on a domestic violence complaint. Upon arrival deputies were advised that two family members became involved in a physical altercation. Laryssa Williams and Wanda McCoy of Jeffersonville were arrested and charged in the offense. TRAFFIC CRASH: 17 y/o juvenile was traveling north on Stafford Rd and lost control of his vehicle while passing another vehicle. The driver traveled off the left side of the roadway into the ditch. No injuries. The juvenile driver was cited for Failure to Control TRAFFIC CRASH: Todd Wallace of Washington CH Ohio was traveling southbound on ST Rte 41 and failed to stop with assured clear distance ahead and struck a second vehicle operated by a Patti Miller of Washington CH Ohio. No injuries were reported and Wallace was cited in the incident. ASSAULT: Latasha Wheeler of Milledgeville Ohio reported while being on Colonial Dr she was assaulted by a known male. Wheeler did not show sign of assault and declined to pursue criminal charges. DOMESTIC DISPUTE: Sheriff s deputies responded to W. Front St, New Holland on a domestic incident. Upon arrival deputies were advised it was a verbal argument and no assault or threats of violence had occurred. TRAFFIC CRASH: Glenn Crawford of Sabina, Oh was traveling south on SR 729 and struck a dog in the roadway. No injuries were reported. DOMESTIC DISPUTE: A male and female in Jeffersonville were involved in a verbal dispute. DOMESTIC DISPUTE: A male and female were involved in a verbal dispute THEFT: John W. Carner of Washington CH reported within the past seven days a known person had used his bank card without his permission.

18 CRIMINAL DAMAGING: Phillip Taylor of Jeffersonville reports person(s) unknown cut the lock off his storage unit. TRAFFIC CRASH: Anthony McIntosh of Cincinnati reported his vehicle was struck by roadway debris while traveling on U S rte 35 near the #8 mile post. TRAFFIC CRASH: Jacob Duffy of Washington CH was riding his bicycle near 8821 Ferguson Road when Duffy struck a stopped motor vehicle in the rear. Duffy was transported by squad to a local area hospital for medical treatment. TRAFFIC CRASH: James Linville of Hillard Ohio was traveling northbound on I-71 at SR 41 and lost control of his vehicle on wet roads. Linville traveled of the road right and struck a drainage ditch causing damage to his vehicle. Linville claimed minor injury, however declined transport to the hospital. Linville was cited in the incident. TRAFFIC CRASH: Mohamed Mohamed of Columbus Ohio was exiting I71 southbound at ST Rte 41. Mohamed stopped for a stop sign, however failed to yield to a vehicle traveling on ST Rte 41 northbound by a Richard Bell of Willeyton NC. Bell stopped to avoid the collision and was rear-ended by a vehicle traveling northbound on ST Rte 41 by an Ivan Adkins of Grove City. Adkins reported injury and was transported to the Fayette County Memorial Hospital with non-life threating injuries. Mohamed was cited in the incident. CRIMINAL DAMAGING: Lowell Lawson of Washington CH Ohio reported, while his semitrailer was parked on Locust St in Staunton person(s) unknown caused damage by writing on it with a marker. CRIMINAL TRESPASS: Kyle Leisure reported a known female was at the apartment complex and has been advised not to return. Deputies made contact and the female departed. PROTECTION ORDER SERVICE: Sheriff s deputies made contact with a respondent serving the protection order. Guidelines of the order and the consequences of any violations were advised. TRAFFIC CRASH: Diane Burton of Harrogate, Tennessee was westbound on Maple Street when her brakes failed to work causing her vehicle to go off the right side of the roadway striking a tree. No injuries. Burton was cited for failure to control. 09/18/17 TRAFFIC CRASH: Karen Hall of Sabina, Oh was traveling west on Milledgeville Octa Rd. Hall traveled left of center striking a vehicle traveling east on Milledgeville Octa Rd. No injuries were reported. Hall was issued a citation for Failure to Drive on Right side of Roadway. CRIMINAL TRESPASS: Charles Fultz of Washington CH reports a known subject came to his residence that he does not want there. Deputies made contact with the male and advised him not to return. TRAFFIC CRASH: Kimberly Williams of Washington CH was traveling east on US 35 and lost control of her vehicle. Williams traveled off the left side of the roadway overcorrected traveling off the right side of the roadway overturning. Williams was transported to a medical facility for treatment. Williams was later issued a citation for Failure to Control.

19 MENACING BY STALKING: A Jeffersonville resident reported a known person followed her to her residence and departed. Contact was made with male & he was advised of the complaint. TELEPHONE HARRASSMENT: A Bloomingburg resident reported receiving unwanted text messages from a known person. THEFT: Randy L. Everhart of Washington CH reported sometime within the past few days persons unknown removed several items from his motor vehicle. DOMESTIC DISPUTE: Sheriff s deputies responded to Bogus Rd on a domestic incident. Upon arrival all parties involved were identified and it was determined that no assault or threats of violence had occurred. DOMESTIC DISPUTE: Sheriff s deputies responded to North Bend Ct on a domestic incident. Upon arrival all parties involved were identified and it was determined that no assault or threats of violence had occurred. TRAFFIC CRASH: David Mullins of New Holland, OH was traveling south on Cook Yankeetown Rd and lost control of his vehicle. Mullins traveled left of center, off the left side of the roadway into a corn field. Mullins refused treatment at the scene. Mullins was issued a citation for Failure to Control. 9/17/17 OBSTRUCTING OFFICIAL BUSINESS; CRIMINAL TRESPASS: Andy M. Pastore of Washington CH reported he observed two juvenile children trespassing on a neighbor s property. The two juveniles were located who had falsely identified themselves. They were taken into custody, and transported to JDC. THEFT: Old Navy reported a subject entered the business and departed with merchandise without making payment. Christine Stevens of West Portsmouth, Oh and Leanna Trischler of Kermit, WV were arrested and charged. CRIMINAL MISCHEIF: James Wary of Washington CH reported someone removed his mail from his mailbox and threw it on the ground. AGGRAVATED MENACING: Raymond Lynch of Bloomingburg reported receiving threats from known male subject. Prosecution was declined in the matter. BURGLARY/THEFT/CRIMINAL DAMAGING: Courtney Clements of Cincinnati, Oh reported unknown person(s) entered her camper and removed items from inside. BURGLARY: Sheriff s Deputies responded to Colonial Dr. in the Village of Jeffersonville on a Burglary complaint. Person(s) gained entry into the residence however nothing was stolen at the time of the report. THEFT: Deputies responded to Old Navy, located at the Tanger Outlet Mall to the report of a theft. Employees identified two female suspects and confirmed a theft occurred. The suspects were located in a vehicle on the lot of McDonalds a short time later. Christine Stevens age 45 of Portsmouth, Ohio and Leanna Trischler age 25 of Kermit, West Virginia were taken into custody and charged for the theft offense.
