Slauson Blvd #600 Los Angeles, CA CONTACT: Ms. Jane Gutierrez: Phone: jath10-14@hotmail.com THE PROGRAM DESCRIBED IN THIS OFFERING IS BEING OPERATED FOR BY: HOLIDAZE TOURS TERMS AND CONDITIONS This agreement contains the terms and conditions by which Holidaze (a division of Five Star, Inc.) 810 Belmar Plaza, Belmar, NJ, ("Operator") in consideration for a Participant's payment, agrees to provide these travel tours. READ AGREEMENT CAREFULLY BEFORE SENDING ANY MONEY. TOUR PRICES: All rates are based on tariffs in effect as of the date of printing. The airfares used for the tour prices indicated herein are capacity controlled and will be applicable on a "first come, first served" basis and are subject to change without notice. PAYMENT: If payment is made by credit card: (1) a signed, fully completed credit card voucher for such payment, with proper authorization, must be received to secure reservations and (2) Participant(s) waive any charge back rights for services provided but not utilized. In the event of a dispute, requests for refunds must be made under the procedure described below under the Refunds section below. Please note that a 3.5% surcharge will apply for Visa or MasterCard and 4% for AMEX. CANCELLATION CONDITIONS: Certain commitments for these arrangements have been, or are being made on your behalf in connection with the tour described on the reverse. Naturally, along with these commitments comes a certain responsibility on your part that will necessitate certain cancellation penalties in the event you cancel your tour arrangements prior to the departure date. All notices of cancellation must be received in writing at our office in order to be considered valid. The amount of refund, if any, will be determined by the following schedule in accordance with the date such notice is received: Prior to 90-days before departure: NO PENALTY; Between days prior: $250.00* Between days prior: $500.00*; Between days prior: $ *; After 290days prior: Total cost. (*Plus any further charges assessed by hotels, suppliers and airlines.) No shows are non-refundable. Charges indicated are per person. In the event that a canceling passenger is able to provide a substitute for that canceled space there will be an administrative charge of $ to effect that substitution. All substitutions must follow the same itinerary as the passenger being replaced with the exception that there may be occasions where the substitute may wish to purchase additional options. In this event we will make every effort to satisfy that request. PASSPORTS/VISAS: All U. S. citizens are required to have a Passport for International Travel. Obtaining the Passport is the responsibility of the participant. Applications can be downloaded from the Internet at: BAGGAGE: Current international baggage allowance for United is one free checked bag per person. The second checked bag will cost $100. each way. Additionally, you are allowed one carry-on piece that must fit under your seat. Porterage of the carry-on piece is your responsibility. Baggage is at "owner's risk unless insured. Insurance is recommended and offered below. NOT INCLUDED: Tour prices do not include items not specified. Also not included, mineral water, coffee where meals are included, excess baggage, passport fees and items of a purely personal nature. REFUNDS: Since all tours are on the basis of inclusive arrangements, no refunds can be made, considered, or given for any absence from the tour or for any unused services or meals once the tour has commenced and is en route unless arrangements have been made at the time of booking. Any and all claims for refunds are to be made at the booking office, as no agents, representatives, guides or any other persons, are authorized to make or to promise refunds, or to make or promise to make any changes or alterations of the official conditions and regulations. No claims can be considered if not made within 60 days from the completion of the tour. No claim can be considered if it is not accompanied by a completed service adjustment coupon or written authorization from our representatives and/or hotel. Telephone requests for refunds will not be accepted and are not valid. All information and details on resorts, hotels, ski areas, etc. were supplied by the various entities involved. Prices are subject to change, and not all facilities may be in operation at the time of your vacation. If you are in doubt, please ask. RESPONSIBILITY AND LIABILITY: Holidaze Tours, in operating this program is responsible to the Participants for supplying the services and accommodations offered to the Participant as stated in the offering. Holidaze, as tour operator, disclaims all liability for loss or damage or injury sustained in connection with services and accommodations provided by railroads, bus operators, local sightseeing companies, steamship companies, airlines, hotels and lodging facilities, all operating under the laws of the country in which their services are rendered. Holidaze also retains the right to decline to accept any tour participant at any time. Tour participants engaging in sports activities do so at their own risk. Tour participants engaging in these, or any other activities, by the act of joining the tour relinquish any and all rights whatsoever to make claims for damages for injuries that may be sustained while engaging in these activities. The right to alter any land arrangement is reserved. Airlines scheduled to be used are stated herein. Airline tickets, when issued, constitute the sole contract between the carrier and the passenger. The carriers concerned are not, in any event, responsible for any act, omission or event during the time passengers are not on board their plane, vessels or other conveyances. Holidaze is not responsible for typographical errors. HOLIDAZE/FIVE STAR OPTIONAL PASSENGER PROTECTION PROGRAM WILL PROVIDE BENEFITS AS FOLLOWS:: All non-refundable amounts in the event of a covered cancellation or trip interruption up to the total tour price plus: BAGGAGE & TRAVEL DOCU- MENTS: $1500; BAGGAGE DELAY: $750; MEDICAL up to $50,000; EMERGENCY MEDICAL TRANSPORTATION: $250,000.; & TRAVEL GUARDâ ASSISTANCE. 12 How to Sign Up for Urban Winterfest This year's registration process is online at Registrants will enter their information and select their trip and travel options through a series of tabs. If you are seeking registration only, select the Sleep and Spree Package, select Option 7 to remove the lodging, then, select the All Access Party Pass Option. Stepby-step registration instructions are as follows: 1. Select Ski Club/Region 2. Enter Personal information 3. Select Trip Package (discounted three-day lift comes with Sleep Spree and Ski package) 4. Select Lodging Preferences (location, room type, bed, and roommate preferences) 5. Select All Access Pass type (adult, senior, youth, child) 6. Select Rental Equipment Options (performance level and days) 7. Select Lift ticket for options other than three-day with Sleep Spree and Ski package 8. Select Trip Options (free social activities, gondola passes, lessons, ground transportation, t-shirt size) 9.
