4 Intrductin Page 3 Intrductin t the Quarterly Safety Summary Reprt Welcme t the quarterly safety summary reprt fr the summer f 2017 (Jan/Feb/Mar). The purpse f this dcument is t summarise the accidents and serius incidents that ccurred during the summer quarter f Summer is the busiest time fr NZ aviatin, with an increase in scheduled airline activity and cmmercial flying as well as a big increase in sprt and recreatinal flying. Unfrtunately there were 36 accidents in the summer f 2017, f which tw were fatal. In additin t the accidents there were a ttal f 1,626 incidents reprted t CAA. This reprt includes brief details f 71 f the mst significant incidents. The incidents have been gruped by sectr. This is intended t aid the reader and fster an understanding f risk within each sectr. Nnetheless sme incidents reprted in ne sectr culd equally ccur in thers. In the large aircraft sectr the CAA is mnitring the incidents related t aircraft cnfiguratin issues and cntinuing t wrk with the peratr cncerned. They are included here t illustrate hw human factrs can affect all types f aircraft peratin. It is als nted that there were n less than 6 helicpter engine events this summer. These were spread acrss the airline, cmmercial and private helicpter sectrs, bth turbine and pistn engines s are nt likely t have a cmmn cause. What they d have is a cmmn utcme; the need fr an immediate landing n the terrain belw. The flight safety value frm these reprted events is nt s much in what caused them, but what the crews did t manage the event. 5 Executive Summary Page 4 Executive Summary - Aviatin Safety t 31 March 2017 There were 36 accidents in the summer f This is the highest number f accidents in a summer quarter since 2014, (which had 51 accidents). There were tw fatal accidents in this quarter: ne cmmercial helicpter accident with ne fatality; ne private helicpter accident with ne fatality; see page 6. There were nine serius injuries in accidents: ne passenger f an airline helicpter flight (hand injury in the rtr) ne passenger and tw pilts f tandem parachute flights (adventure aviatin peratins) suffering brken bnes; ne cmmercial helicpter pilt; the pilt and ne passenger f a private aerplane flight (the ther tw passengers suffered minr injuries); ne student n a dual training hang glider flight (brken ankle); ne private paraglider pilt; see pages 6 and 7. There were eight minr injuries in accidents: tw minr injuries n tandem parachute flights (adventure aviatin peratins); tw passengers f a private aerplane flight; the pilt and passenger f a private helicpter flight; ne parachutist and ne paraglider pilt n private flights; see pages 6 and 7. And withut injury there were: ne tandem hang glider accident (adventure aviatin peratin); ne cmmercial small aerplane accident (sl training); ne cmmercial helicpter accident (fence line pst laying); fur agricultural aerplane accidents (a cllisin invlving tw agricultural aerplanes is cunted as tw accidents); tw agricultural helicpter accidents; fur private small aerplane accidents and ne private helicpter accident; ne accident during dual training in a class 2 micrlight; seven private sprt accidents (three class 2 micrlights, ne class 1 micrlight, tw amateur built aerplanes and ne glider); fr details see pages 8 t 10.

The biggest increase is within the Agricultural Operatins - Aerplanes grup, while the biggest decrease is within the Private Operatins - Sprt grup.

7 Sectin 1 - Accidents Page 6 Summary f Accidents This sectin describes all accidents that ccurred during the perid 1 January t 31 March These accidents are classified accrding t the highest level f injury sustained and the safety target grup. Nt all f these accidents were investigated by the CAA, and sme f the CAA investigatins have nt been cmpleted, s the text may be cndensed frm the riginal accident ntificatin. Fatal Accidents Other Cmmercial Operatins - Helicpters Eurcpter AS 350 BA, Christchurch: Helicpter crashed during a fire fighting exercise. The pilt was killed. The helicpter was destryed. TAIC and CAA safety investigatins in prgress. (17/566) Private Operatins - Helicpters Rbinsn R22, Larrys Creek: Fatal helicpter accident during venisn recvery peratin. The helicpter was destryed. TAIC and CAA safety investigatins in prgress. (17/1543) Injury Accidents Airline Operatins - Helicpters Hughes 369E, Fx Glacier: A passenger was injured embarking int running helicpter. Befre the passenger had cmpleted embarking nt the aircraft, they extended their arm t full length and their fingers made cntact with the main rtr blades causing a serius injury t their right hand. (17/1519) Sprt Transprt Tandem parachute, Taup: During apprach t land, a strng gust f turbulent wind spun the canpy arund 90 degrees t the right. This cmmitted the tandem master t landing crss wind. As he flared the canpy, this had n effect n slwing the descent and a hard landing fllwed with the tandem master injuring his right ft (minr injury). (17/174) Tandem parachute, Whangarei: On final a wind shift resulted in a fast dwnwind landing withut the passenger getting their feet up in time. The tandem master ended up n tp f the passenger, this resulted in grazes t the passenger s leg (minr injuries). (17/227) Tandem parachute, Wanaka: During landing at 10 ft caught in an up-lifting gust, resulting in canpy pressure lss and a hard landing. Tandem master has been diagnsed with a brken fibula and a slight chip t the base f her tibia (serius injuries). (17/276) Tandem parachute, Jardines: A tandem training parachute jump was being cnducted with a new trainee instructr. They suffered a heavy landing which resulted in multiple fractures t bth persns (serius injuries). CAA safety investigatin in prgress. (17/287)

8 Sectin 1 - Accidents Page 7 Other Cmmercial Operatins - Helicpters Hughes 369E, Ohakune: During sling peratins, the strp went thrugh the tail rtr, causing it t cme ff, and resulting in serius injuries t the pilt. CAA safety investigatin in prgress. (17/631) Private Operatins - Aerplanes Cessna A185F, Kekerengu: While landing up slpe n a private airstrip, the aircraft mst prbably encuntered a sudden windshear r tail wind cmpnent. This caused the aircraft t descend rapidly resulting in a very hard landing. The pilt lst directinal cntrl f the aircraft which then departed the airstrip t the left and dwn an embankment. The pilt and frnt seat passenger suffered serius back and internal injuries and were trapped in the aircraft until emergency services arrived t free them. The tw peple seated in the rear suffered minr injuries and were able t free themselves frm the aircraft. The aerplane was destryed. CAA safety investigatin determined it was prbable that the aircraft encuntered windshear r tailwind gust which the pilt did nt anticipate. This resulted in a high rate f descent clse t the grund frm which the pilt culdn't recver. (17/154) Private Operatins - Helicpters Rbinsn R44 II, Raukkre River: Appraching river flat t land, the tail rtr struck the grund and brke ff. Helicpter spun a number f times and impacted the grund n left side, resulting in minr injuries t the pilt and passenger. CAA safety investigatin cmpleted. (17/935) Private Operatins - Sprt Hang Glider, Otag: Take-ff accident during a dual training flight. Failed t becme airbrne, n grund cntact the student suffered a badly brken ankle (serius injury). (17/1331) Parachute, Parakai: Parachutist failed t flare at adequate height resulting in a hard landing, with apprximately 25% flare. Parachutist cmplained f sme back pain n landing and an ambulance was called due t suspected back injury (minr injury). (17/349) Paraglider, Queenstwn: Paraglider cllided with 11 kv pwer lines and sustained serius burns as well as a brken leg (serius injuries). Strng winds were prevailing at the time f the accident. (17/92) Paraglider, Omarama: Paraglider lst cntrl n take-ff. Pilt injured his ankle (minr injury). (17/861)

9 Sectin 1 - Accidents Page 8 Nn-Injury Accidents Sprt Transprt Tandem hang glider, Crnet Peak: Paraglider failed t get airbrne n launch. The paraglider spun back int the hillside. (17/319) Other Cmmercial Operatins - Aerplanes Cessna 152, Dunedin: Minr landing accident. The student pilt was carrying ut their first sl flight but bunced the aircraft several times n landing. The nse wheel cllapsed n the 3rd bunce and the aircraft came t rest beside the runway. (17/280) Other Cmmercial Operatins - Helicpters Rbinsn R44 II, Mahenui: The helicpter was engaged n an peratin laying ut fence line psts when they nticed the lad line slip and witnessed the centre pst spill ut. The pilt lwered cllective and pulled speed back in an attempt t put the lad n the grund and secure it, hwever the remaining psts began t fall away. The pilt elected t jettisn the lad. When he did he felt a sudden bang and realized that tail rtr authrity had been lst when the empty strp flicked up and wrapped arund the tail rtr pitch links. The pilt attempted a run n landing but lst cntrl and cllided with terrain. The helicpter was destryed. CAA safety investigatin in prgress. (17/289) Agricultural Operatins - Aerplanes Air Tractr AT-402B, Kumera: Experienced sink n final with tw peple n bard, hit grund prir t strip. Damaged landing gear and came t rest n strip. CAA safety investigatin in prgress. (17/595) Gippsland GA200C, Kawakawa Bay: Take-ff accident during aerial tpdressing peratins. On lift-ff experienced a sudden wind change which caused the aircraft t sink. The aircraft impacted heavily with the grund in the paddck ahead causing the undercarriage t cllapse. (17/715) Pacific Aerspace Cresc , SE f Te Kuiti: Tw agricultural aircraft cllided - ne was taking ff, the ther landing n the same strip. CAA safety investigatin in prgress. (17/1544 & 17/1934)

10 Sectin 1 - Accidents Page 9 Agricultural Operatins - Helicpters Hughes 369E, Beaumunt: During the first spray run in a new treatment area, the aircraft came int cntact with twin wires at the Eastern end f the spray area, resulting in damage t the machine. The wires, ne f tw sets ver the spray blck, had been nted in the safety briefing befre cmmencing the peratin. The wires blended int the terrain. (17/209) Rbinsn R44 II, Tapanui: During spraying peratins, flying up hill and ran ut f pwer, attempted a precautinary run n landing while jettisning the lad. The helicpter impacted the grund and rlled three times. CAA safety investigatin determined that the aircraft was heavier than the pilt anticipated and started t sink twards the hill. Unable t arrest the sink with cllective. (17/535) Private Operatins - Aerplanes Cessna 180A, Lansbrugh River: On take-ff with tw peple n bard, aircraft caught a large gust f tail wind n a ne way strip and aircraft failed t becme airbrne. Aircraft hit rugh grund and sustained damage t left wing, left undercarriage leg and prp. (17/278) De Havilland DH 82A Tiger Mth, Omaka: Minr landing accident. Left main wheel clipped inbard crner f left runway threshld marker, resulting in initial damage t undercarriage brace. Pilt initiated g-arund and attempted dead stick apprach, but was prevented frm reaching RWY 30 by cnflicting traffic, landing shrt f the runway n bush land. Damage t undercarriage and lwer wings. CAA safety investigatin in prgress. (17/1103) Maule M-5-210C, Ruahine Crner: Minr landing accident. Bunced n landing, during g-arund a gust pushed the aircraft sideways, ne wheel entered the lnger tussck pulling the aircraft ff the strip. Take-ff abrted, mments later the right hand le cllapsed. CAA safety investigatin determined the likely cause was an unanticipated gust f wind fllwing a bunced landing. The bunced landing was prbably a result f the aircrafts higher than expected rate f descent due t the pressure altitude f the strip. (17/1593) Piper PA-28R-200, Nelsn: Minr taxiing accident with tw peple n bard. Fllwing taxiing ver a small drain grate munted in a small cncrete pad which the nse wheel had crssed, aircraft fell int a hle n the trailing side f the cncrete pad causing the nse wheel t fld up. The prpeller struck the grund and the engine stpped. (17/1447) Private Operatins - Helicpters Rbinsn R22 Beta, Glenrae Valley: Tw peple n bard, had finished flare fr landing and levelled helicpter. Applying pwer t enter int hver and btain grund effect when strng gust lifted tail pushing nse frward and dwn. Aft cyclic and full pwer culd nt crrect s attempted run n landing. 11 Sectin 1 - Accidents Page 10 Private Operatins - Sprt Class 2 micrlight, Wanaka: Minr take-ff accident during dual training peratins. On the take-ff rll with the student fllwing thrugh n the cntrls, a gust f wind caused the aircraft t becme airbrne sner than preferable with a higher than cmfrtable nse attitude. The aircraft weather ccked due t crsswind, crrective actin taken, but the student frze n the rudder pedals with the aircraft cntinuing t yaw right, with insufficient airspeed t cntinue flight the aircraft settled heavily nt the grund damaging the left hand undercarriage. (17/82) Amateur built aerplane, Ashburtn: Minr landing accident. Landing in gusty cnditins a gust created a swing t the left, unable t stp r shutdwn the prpeller came int cntact with anther amateur built aerplane, shattering the prpeller tips and damaging the rear fuselage f the ther aircraft. (17/437) Amateur built aerplane, Mtueka: Inadvertently landed with the gear up, ne persn n bard. (17/884) Class 1 micrlight, Kerikeri: Minr landing accident. Immediately after lift-ff engine pwer initially reduced a little then recvered. Pilt decided t land back n ample runway available. A premature tuchdwn was initially n ne wheel and a swing t the left was induced, this develped int an uncntrllable grund lp despite full applicatin f brake and rudder. Aircraft came t rest n the starbard wing tip with starbard undercarriage partially detached. (17/520) Class 2 micrlight, Raglan: Aircraft with tw peple n bard landed dwnwind and failed t stp in time t avid fence at end f the runway. The prpeller wrapped arund the fence wire. CAA safety investigatin cmpleted. (17/51) Class 2 micrlight, Patumahe: Minr landing accident with tw peple n bard. Pilt reprted n cming in t land he was caught in turbulence and made a heavy landing and the aircraft suffered a bent nse leg. (17/121) Class 2 micrlight, Mtueka: Minr abrted take-ff accident. Fllwing tuch and g, when pwer applied there was a lag and nly partial pwer available. Take-ff abrted, thrttle clsed, max braking applied. Due t wet grass and speed unable t stp befre running ut f landing area. Went thrugh fence int field at end f the runway. Damage t nse and right undercarriage legs, end f right wing. (17/300) Glider, near Cartertn: Minr landing accident. Landing gear cllapsed after utlanding in muntain wave cnditins. One persn n bard. (17/411)

12 Sectin 2 - Incidents Page 11 Sectin 2 - Incidents This sectin describes selected incidents frm the perid which had a high ptential risk. In the perid 1 January t 31 March 2017 there were a ttal f 1,626 incidents reprted t the CAA, the 71 incidents presented here have been selected n the basis f ptential risk. Fr brevity the text may be cndensed frm the riginal ccurrence ntificatin. In sme cases the aircraft mdel descriptins have been reduced t a sectr (e.g. large jet). This is dne fr tw reasns: t maintain the privacy f the reprter, and t fcus n the nature f the incident. In many incidents such as airspace ccurrences, the specific aircraft type is nt relevant t the prblem. By cmparisn fr defect incidents the specific mdel is highly relevant, but the lcatin is nt. The ccurrences are gruped by sectr t enable cnsideratin f specific risks. While this is intended t assist peratrs t identify their sectr relevant risks, there will be sme events ccurring in a given sectr that culd equally ccur in ther sectrs. Selected Critical and Majr Incidents Airline Operatins - Large Aerplanes Turbprp, Wellingtn: As aircraft taxied t stand, the aircraft's wingtip passed ver anther turbprp s wind screen. (17/1293) Large jet, Shanghai: Aircraft was cleared t taxi t R5 and then R6, t hld shrt f RWY 16R, but cntinued past the hlding pint. Aircraft crssed RWY 16R. Crew mistk the hlding pint lights f RWY 16L as thse f RWY 16R. (17/671) Large jet, En-rute: Unlcked ULD. ULD in psitin 11 able t shift between tw psitins. Crew heard a bang frm underflr with pitch change while in descent. Grund crew reprted can in Statin 11 nt lcked in psitin. (17/783) Large jet, En-rute: On descent F/A nted rattling and banging sunds frm flr abve aft carg hld. A VMC cntrllability check was carried ut and the apprach and landing cntinued. Upn inspecting the rear carg hld the lck between psitin 41 and 42 was nt engaged. CAA safety investigatin in prgress. (17/1144) Large jet, Queenstwn: Aircraft taxied frward with grund handler still attached. The cntrller nticed this and instructed aircraft t stp immediately. CAA safety investigatin in prgress. (17/1491) Large jet: Arrived in Auckland with the ULD lck between hld 41 and 42 still dwn. CAA safety investigatin in prgress. (17/1146) Large jet: On arrival in Auckland, lcks were dwn between hld 32 and 41. ULD had slid frm 32 t 41. Lcks were up n hld 42 but a cuple f bags flew ut f the can int hld 42. CAA safety investigatin in prgress. (17/1522) Turbprp, En-rute: Climbing thrugh transitin level, "EXCESS CAB ALT" displayed. Levelling ut at FL150, crew nticed the cab altitude reading 12,000 ft. Carrying ut QRH, aircraft descended t 8,000 ft under radar cntrl. Emergency declared fr pririty handling landing. (17/647) Turbprp: During a visual apprach, at apprximately 1,700 ft, flap 30 was requested but flap 0 selected instead. The mistake was nticed immediately and flap 15 reselected. Due t gusty cnditins, prps were put up t 100 and then flap 30 selected. N ver speed detected. (17/612)

13 Sectin 2 - Incidents Page 12 Turbprp, Christchurch: Aircraft tipped with 300 kg in hld 4 and passengers barding at rear f aircraft. Ramp agent estimated the nse rse 2 ft ff the grund. (17/1121) Turbprp: During taxi-in, GEN 2 did nt cme back n line. The captain did nt ntice this but n appraching the gate, when he applied the brakes, they were nt there. The park brake was used t stp the aircraft. (17/332) Large jet, En-rute: Brief stick shaker activatin. Turning utbund in the hld F/O nted green trend arrw shwing a rapid decrease in indicated airspeed, mmentary stick shaker activatin. Captain discnnected aut pilt, aut thrttle and increased thrust. (17/1364) Turbprp, En-rute: Pilt Incapacitatin. Declared a PAN PAN and requested t return t aerdrme due pilt incapacitatin. (17/1639) Turbprp, Kerikeri: Aircraft rejected take-ff as nse wheel failed t lift-ff the runway n departure. PA made t the passengers and aircraft taxied back t the aprn. CAA safety investigatin cmpleted. (17/639) Turbprp: Bleed air switches nt selected n after take-ff. Thrugh 10,000 ft received a cabin alt warning. (17/1440) Turbprp: After take-ff the F/O inadvertently raised the flap instead f the landing gear. (17/551) Turbprp: Flaps retracted instead f gear. Departing RWY 34, flaps 5, 17,800 kg. Just after take-ff, when asked fr gear up, PM (F.O.) selected flap 0 instead. CAA safety investigatin cmpleted. (17/1470) Turbprp, Wdburne: Three EGPWS activatins n a visual apprach. The aircraft was apprximately in its lwest psitin at 800 ft AGL nt in cmpliance with SOPs, as the aircraft was belw 1,000 ft AGL un-cnfigured. Aut pilt was still engaged when first warning was received. After the aut-pilt was disengaged, tw mre warnings were received. The warnings were als received in the cabin. CAA safety investigatin cmpleted. (17/725) Large Jet: Freight nly flight. Crew nted #2 engine vibratin and surging, N1 rlled back t 50%. N thrust lever cntrl available ver #2, engine shutdwn, returned t aerdrme. CAA safety investigatin in prgress. (17/1132) Turbprp: Freight nly flight. RPM unstable and left hand engine shut dwn. Aircraft returned t land. CAA safety investigatin in prgress. (17/1321) Airline Operatins - Small Aerplanes Ardmre: Ferry/psitining flight. Aircraft came int clse prximity with anther while perating near Ardmre. Aircraft flew past each ther at the same altitude. CAA safety investigatin cmpleted. (17/962) Wdburne: Lss Of Separatin. 14 Sectin 2 - Incidents Page 13 Airline Operatins - Helicpters Eurcpter AS 350: While in the cruise a slight unusual shudder was felt, immediately the left rear passenger advised the external carg pd had departed the aircraft. CAA safety investigatin in prgress. (17/153) Cascade Valley: Helicpter was tracking up the right hand side f the Cascade at 1,800 ft making regular radi calls n t lk up and find a dark blue small cub type plane directly ahead estimated 70 m away, helicpter turned right immediately t avid. (17/1510) Franz Jsef Helipads: Passenger transprt A t A flight. Aviding actin required t avid cllisin due secnd helicpter lifting withut ding a clearing turn t all ther pads behind. (17/1085) Queenstwn: Helicpter was cleared t the Remarkable Peak and als t crss all runways. Helicpter lifted ff abeam f RWY 05 threshld and passed very clse t a Rescue Fire vehicle which was parked n the aprn. CAA safety investigatin cmpleted. (17/373) Queenstwn: Traffic cnflictin in the vicinity. Extensive helicpter peratins during the afternn, with multiple helicpters perating in frmatin, resulting in ne helicpter having t take aviding actin. CAA safety investigatin in prgress. (17/557) Eurcpter AS 355 F2: Engine Failure. Initially reprted a chip warning light illuminatin, 2 minutes later advised ne f tw engines had failed. CAA safety investigatin in prgress. (17/189) Eurcpter EC 130 T2: At end f flight, the passenger attempted t und seat-belt by rtating release mechanism f the 4 pint harness. Upn rtating, the whle frnt piece separated frm the buckle husing, causing the passenger t be stuck in the seat belt. The pilt had t use pliers t rtate the nut n the mechanism t release the seatbelt. CAA safety investigatin in prgress. (17/257) Hughes 369E: During cruise the pilt nted a rapid increase in Rtr RPM alng with a lud whining nise frm the engine. A precautinary landing carried ut. CAA safety investigatin in prgress. (17/1608) Sprt Transprt Pacific Aerspace 750XL: Parachuting peratin. The lad f skydivers included a grup f 11 skydivers wh wished t exit tgether.
