A few things you will need to know are: WHEN: Monday, June 18 th - Friday, June 22 nd, 2018 WHERE: TIME: West Jessamine Middle School Lice checks will begin no later than 7:15 am. We will leave at 8:30 a.m. SHARP on Monday, June 19 th and return around 12:00p.m. (Noon) on Friday, June 23 rd. Please make sure someone is there to pick you up. ALL CAMPERS WILL BE CHECKED FOR LICE BEFORE THEY CAN LOAD THEIR LUGGAGE ONTO THE LUGGAGE TRUCK. If lice or nits are found, camper will not go to camp, and there will be NO REFUND. Please check your child before camp time. THIS PROCESS MAY TAKE LONGER THAN IN PREVIOUS YEARS. NO DOCTOR S NOTES WILL BE ACCEPTED. REMEMBER, YOU WILL NEED TO BRING THE FOLLOWING: 2 sheets, 1 blanket, 1 pillow or a sleeping bag Personal items (towels, wash clothes, soap, toothbrush, comb, etc.) Sweater or jacket Swim suit Tennis shoes or shoes with backs, (no flip flops as the ground is hilly and they are dangerous to wear.) Clothing for 5 days. (Please mark your child s clothing, every year we have lots of lost and found. They are kept at the office for 2 weeks and then given to the Goodwill. You will be responsible for coming by and checking for any lost items.) Only 1 suitcase plus 1 bed roll per camper. You may bring non-perishable snacks, but these must be in your suitcase. No peanuts or nut products allowed. The camp is North Central 4-H Camp near Carlisle, Kentucky. The phone number (859) is for emergency only!! If you need further information, call me. 5 Kentucky 4-H Camping Program Damage Fees The following contains only those items most frequently damaged or taken. Intentional damage to any other camp property will be assessed at the time of damage. Should intentional damage or theft occur, it is the responsibility of the child and his/her parents to reimburse the costs. No charges are made for worn equipment or normal usage. Charges are required for damage resulting from horseplay and malicious or intentional behavior. Graffiti on camp property will not be tolerated. 6 Kentucky 4-H Camping Program Summer Residential Camp Medication Policy A medication form must be completed for any participant residing on camp property bringing prescription medication to camp. This form should be placed inside a clear bag with the medication. On the outside of the bag, the name of the participant, county name, and cabin number written on the bag. All prescription medication MUST be in its original container. This is a state law. s should send only the number of pills the camper will need for the camp session. Medication may only be given to the person whose name is on the prescription medication container. Medications CANNOT be shared by siblings unless both names are on the container. If a participant s prescription has changed and the directions on the medication bottle are different from the correct instructions, the parent must include a note from the physician (on his/her letterhead) with the correct instructions for taking the medication. The medication cannot be given without the physician s note. If a participant must keep an inhaler or epi pen on their person during the camping session, the parent should provide a backpack or other item in which to securely store them while participating in classes. Camp cannot be responsible for lost inhalers or epi pens. A physician s note confirming the camper s meds be kept with them should be included in the medication baggie. For participants who require special medical treatments, IV s, blood sugar tests, insulin, etc. a trained assistant or the camper will be responsible for this care. Our health care provider is not allowed to administer these special treatments. Camp provides a variety of over the counter medications for general use, e.g. cough syrup, Benadryl, sting ease, etc. If a parent wants aspirin given to their child, it must be sent with the child. Camp does not administer aspirin to anyone less than 18 years of age. If the parent wishes to send a specific brand name over the counter medication, they may do so. 9 4-H Summer Camp Policy OM-4, 9 Camper Code of Conduct / Expectations 2018 These rules will be enforced. Each camper needs to read these and he/she and a parent must sign these rules. BRING THIS SHEET WITH YOU ON MONDAY MORNING. IT IS YOUR TICKET TO GET ON THE BUS TO LEAVE FOR CAMP. 1. Campers are NOT permitted to bring cell phones, stereos, MP3 s, or portable game systems to camp. NO FIDGET SPINNERS. 2. Possession or use of alcohol or illegal drugs by any person involved in the 4-H Camp Program is strictly prohibited. 3. Any weapon or article construed to be a weapon, hunting or pocket knife is not allowed at camp at any time. 4. Use of tobacco products is not allowed for campers/teens at 4-H camp. Should a county(s) decide to permit adults (18 years and over) to use them, it may occur only in areas designated by the Camp Manager. Absolutely no tobacco products in cabins, woods or other areas of camp. 5. Boys and girls cabin areas are restricted. A camper of the opposite sex is not, at any time, to enter a restricted area unless approved by the Camp Program Director. 6. Campers are not allowed in the cabins during a class or activity. If a camper is ill, he/she is to stay at the medical center (not in a cabin) until the Health Care Provider (HCP) feels the camper may return to activities. 7. There is to be no visitation to friends in cabins. 8. Campers are to be attentive, responsive and courteous to any staff, adult or teen counselor making a presentation before the group. 9. Absolutely no phone calls are to be made by campers (camp phone or cell phone) without approval of the County Extension Agent. All County Extension Agents should be informed of incoming calls to campers. 10. Campers are not permitted to bring cell phones to camp. 11. Accidents or illnesses, no matter how minor, are to be reported to the HCP and County Agent. 12. Obscene, discriminatory and/or inappropriate language or dress, roughhousing, and insubordination is not acceptable at any time during camp. 13. Fireworks are not to be used by campers at any time during camp. 14. Swimming, boating, or any waterfront activity is not permitted except during designated times and under proper supervision. 15. Shoes with ties or straps are to be worn at all times, including going to and from swimming and boating activities. Flip-flops should not be worn on the campground, except inside the bath houses or pool area. Tennis shoes or sandals with straps are highly recommended for safety. Shirts are to be worn at all times, unless approved by the Camp Program Director. 16. Campers are always to remain with their groups. Individuals are not to be on the trails or near the lakes without an accompanying adult.

10 17. Campers are not permitted to leave the grounds at any time without notifying and receiving approval from the Camp Program Director and their County Extension Agent. 18. All campers are expected to be in their cabins, with lights out, as designated on the camp program. 19. No visitors are allowed at camp. 20. No camper is to be around or on maintenance equipment parked or being used on camp property. 21. Campers who are having personal conflicts with other campers are encouraged to discuss these with their cabin counselor, dean or County Extension Agent. 22. Campers are to work with counselors in carrying out daily assigned jobs to help keep the camp running smoothly. Grounds are to be kept clean at all times. 23. Campers are expected to leave the cabins, facilities and grounds clean and orderly. 24. Campers are to respect camp property. Any malicious or intentional damage to camp property or buses shall be paid for by the camper and/or parent or guardian, including graffiti on any camp property. 25. All medications must be turned in to the designated adult and picked up by the parent/guardian at the bus pick up site. The Health Care Provider will be responsible for securing all medications at camp. 26. Electrical appliances (hair dryers, curling irons, etc.) are to be used in cabins only; not in restrooms. 27. Camp is not responsible for personal property of any camper, volunteer or staff. 28. We care about the safety of your child, incidents of serious misbehavior (i.e. fighting, bullying, causing injury, alcohol/drug incidents, any altercations between adults and/or minors, intentional property damage/vandalism, etc.) will be reported to the Camp Manager and an incident report will be completed. 29. Campers should demonstrate respect toward others. Bullying, hazing or malicious pranks (i.e.: shaving cream, toothpaste in pillow/sleeping bags, defacing property, including inappropriate use of electronics/social media) will not be tolerated and may result in the perpetrator(s) being sent home. 30. The consequences of flagrant violation of the above rules, following consultation of those concerned (i.e. Camp Manager, Camp Program Director, County Extension Agent, and camper), may result in the camper being sent home, restricting future participation in 4-H activities, termination of 4-H membership, or other consequences determined by the county s policy. 31. If a camper must be sent home, it will be the responsibility of the parent/guardian to pick him or her up at camp. There is no refund of the camper fee for an early departure. I HAVE READ AND UNDERSTAND THE 4-H CAMP RULES AND WILL ABIDE. IF I DO NOT OBEY THESE RULES, I UNDERSTAND MY PARENTS WILL BE CONTACTED AND IT MAY RESULT IN MY BEING SENT HOME.
