1 2017 Day Camp Information Girl Scouts of Central Indiana is happy that you will be part of the 2017 Summer Day Camp program at Camp NaWaKwa! We hope your experience will be a valuable one. If you have any questions please feel free to contact Camp Director, Blair Adams, at or Please save this sheet it contains information you need to get ready for camp! Camp Open House Camp Open House for NaWaKwa will be on Sunday, June 11 th from noon to 4 p.m. Parents and girls are invited to visit and get acquainted with camp and the camp staff. This will give you a chance to check out camp and meet the people who work there since there is NO VISITATION during the sessions. This includes opening day, when parents will not be allowed in the units. We hope you plan to join us at open house! Drop-off and Pick-up BUS RIDERS: Bus schedules will be available two weeks prior to camp. Typical pick up times are between 7:15 and 8:30 a.m.; drop off times are between 3:30 and 4:45 p.m. No bus transportation will be available Thursday afternoon or Friday morning due to the overnight. Please be sure to fill out the Authorization for Release form included in this confirmation packet and give to the bus monitor on the first day of camp. CAR RIDERS: Drop-off is between 8:30 and 9 a.m. Pick-up is between 3:30 and 4 p.m. It is very important for your camper to be dropped-off and picked-up during these times. Closer to the start of camp you will receive more information on pick-up procedures. Please be sure to fill out the Authorization for Release form included in this confirmation packet and give to the Camp Director on the first day of camp. What to Pack A comprehensive packing list is included in this confirmation packet. Meals We do not provide lunch Monday, Tuesday, or Wednesday; campers will need to bring a lunch and a drink (if desired) on these days. Campers will receive a snack every afternoon. All campers receive lunch on Thursday and Friday. Those who wish to participate in Thursday s overnight will be served dinner that evening and breakfast Friday morning. We are unable to a ccommoda te food allerg ies or specia l diets at Camp NaWaKwa. If your camper has any food allergens please make sure to include specifics on camper s health history form. Please send a sack lunch/dinner/breakfast/snacks with your camper if there are allergy concerns. High Ropes Participation NOTE: In order for a camper to participate in the high ropes course, they MUST bring with them to camp the Challenge Adventure Program Participation Agreement (included in this confirmation packet) signed by a parent or guardian. Campers must be 9 to climb and zip line.

2 Health Form *****DO NOT MAIL HEALTH FORM***** Please complete the health history form which is enclosed or can be found in the camp information book. This form must be completed in order for your camper to participate in camp activities. Please bring the completed form to camp on the first day. Do not mail this form to camp of the Girl Scout office. Written permission and instructions must accompany all medication brought to camp. Any prescription medication must be labeled and in original bottles. Please complete the medication section of the health form, including dosing instructions. By signing the health form you are authorizing camp staff to treat your daughter. In the event your daughter needs to be taken to an off-sight medical facility the Camp Director will notify you. Camp Telephone The phone number for camp is , but please be aware that there is not always someone in the camp office to answer the phone. Please do not call during inclement weather. Be assured that we will call you immediately if something is wrong. Our policy states that you will not be able to speak to your camper if you call camp; you will be able to speak to the director and/or your camper s counselors who are always happy to help. General Policies Head Lice Policy If head lice are discovered you will be notified and arrangements will need to be made for your child to be picked up. They are not allowed to return for the remainder of the week. Cell Phones Volunteers need to refrain from using cell phones while at camp. If cell phones need to be used please do this away from campers as it distracts them from their camp experience. Campers are not allowed cell phones during camp. Smoking Volunteers cannot smoke on camp property. Camp Overnight Campers not attending the overnight must be picked up no later than 5:30pm. They will return to camp at the normal drop-off time. Campers staying overnight on Thursday are not permitted to leave camp after 4 pm; if there is a conflict and a camper needs to be excused, the parent must contact the camp director at least 48 hours in advance. NO ADULTS ARE PERMITTED TO SPEND THE NIGHT UNLESS THEY ARE VOLUNTEERING FOR THE FULL WEEK OF CAMP. Camp T-shirts, Pictures, and Recognitions A camp T-shirt, patch and picture are included in the day camp fee. All campers will receive their t-shirt on the first day of camp. Campers will receive their patch and picture at the end of their session. Payment Procedures If you applied for a program grant you will receive a separate letter from the Program Services Department regarding the amount of assistance your camper will receive. Please deduct the amount of the grant from the balance due. Submit the difference to our office no later than May 31 st for all camp sessions. Any camper not paid in full will be cancelled from camp. Refund Procedures The fee for camp is not designed to cover the entire cost of the camp stay. The remaining cost is covered by various funding sources that support Girl Scouts. The refund policy is as follows: 1. Refunds are only given if cancellation is made two weeks prior to camp session. 2. $25 of the fee is retained for handling charges. 3. The $10 PAG fee is non-refundable unless your daughter cannot be placed in a unit. 4. Bus fees are non-refundable.

Refunds will not be given for no-shows or campers leaving early due to personal reasons, including homesickness, sports, family vacations, etc. Cancellation For cancellations please contact the Camping Services Department at or We look forward to seeing you at camp! 6 What to Bring to Camp NaWaKwa Clearly mark your camper s name on all articles. Camp clothes will most likely get dirty and damp, and there are not any laundry facilities available. Please be mindful of this and send a trash bag or other plastic bag for dirty clothes. It is the responsibility of your child to care for their own belongings. Please ensure that your child comes to camp wearing comfortable clothing that allows them to move, including tennis shoes or sandals with backs as seen in the example below. (Any brand is acceptable as long as they have a back strap or full back.) DAY CAMPERS should plan to bring a day pack with the following items: Sack lunch Monday - Wednesday OVERNIGHT CAMPERS should plan to bring these additional items: Additional clothing for the following day Water bottle Pajamas One white item to tie-dye (i.e. t-shirt or pillowcase) Shower shoes Raincoat or poncho (just in case) Hat or bandana Swimsuit and towel Sunscreen Comb and/or brush Extra pair of underwear Extra socks (for use at pool, if required) Shampoo and soap Deodorant (if typically used) Toothbrush & toothpaste Towel & washcloth Sleeping bag Twin-size fitted sheet + flat sheet Blanket Pillow with pillowcase ONE stuffed animal (if desired) Flashlight & extra batteries PLEASE DO NOT SEND CANDY, SNACKS, CELL PHONES, OR ELECTRONICS OF ANY KIND TO CAMP. We are not responsible for personal belongings and cannot guarantee their condition upon return home.

7 Directions to Camp NaWaKwa 7865 E. County Road 300 N, Poland, IN Please do not rely on your GPS system to find camp! Most GPS systems take you to the wrong address. Follow these directions for the most accurate routes to camp. If you are lost or need help finding camp, please call Traveling from the west on I-70: Exit at State Road 59 (Exit #23). Proceed south ¼1/4 mile to State Road 42 and turn east (left). Drive 10 miles to Poland. In town, turn south (right) on the blacktop road at the corner of the hardware store and bank. Go south one mile to the brick church on the right. Turn west (right) on the road before passing the church. The road is between the parking lot and the church. Go ½ 1/2 mile west. The camp entrance is on the right. Traveling from the east on I-70: Exit at State Road 231 (Exit #41). Proceed south through Cloverdale approximately 3 miles to State Road 42. Turn west (right) and drive miles to Poland. In town, turn south (left) on the blacktop road at the corner of the hardware store and bank. Go south 1 mile to the brick church on the right. Turn west (right) on the road before passing the church. The road is between the parking lot and the church. Go 1/2 mile west. The camp entrance is on the right. Traveling from the south on U.S. 41 to Terre Haute: In Terre Haute, turn east on I-70 and follow directions above for Traveling from the west on I-70. Traveling from the north on State Road 59: Drive south through Brazil to the intersection of State Road 42 at the Brazil Truck Stop. Turn east (left) and drive 10 miles to Poland. In town, turn south (right) on the blacktop road at the corner of the hardware store and bank. Go south 1 mile to the brick church on the right. Turn west (right) on the road before passing the church. The road is between the parking lot and the church. Go ½ ½1/2 mile west. The camp entrance is on the right. Traveling from the north on State Road 59: At the intersection of State Road 42 at the Brazil Truck Stop, turn east (right) and drive 10 miles to Poland. In town, turn south (right) on the blacktop road at the corner of the hardware store and bank. Go south 1 mile to the brick church on the right. Turn west (right) on the road before passing the church. The road is between the parking lot and the church. Go ½ ½1/2 mile west. The camp entrance is on the right.

9 Camp NaWaKwa Challenge Adventure Program Participation Agreement Participant name: Age: Instructions: Please read this form carefully. Each participant and their parent must sign this agreement before the program begins. Without all appropriate signatures, the individual may not be permitted to participate in the program. I understand that my participation in programs offered by the Challenge Adventure Program at Girl Scouts of Central Indiana is based on the Participation is Voluntary philosophy. I recognize that the program is designed to use experiential, engaging, teaching techniques, but that my participation is purely voluntary. At all times I will choose my level of participation in any activity. I understand the facilitators of Girl Scouts of Central Indiana have received extensive training, and will work to protect the emotional and physical safety of myself and/or my child. I understand that climbing, high ropes courses, ground initiatives, and other activities in the Challenge Adventure Program for which I and/or my child have enrolled, entails certain risks. I elect to participate in spite of these risks. Therefore, for myself/my child, I knowingly and voluntarily assume all risks involved in my participation, and do hereby release Girl Scouts of Central Indiana and its members, trustees, officers, employees, independent contractors and agents from any and all liability, damages, costs and expenses arising out of or relating to bodily or psychological injury, loss of life or personal property that may occur as a result of participating in this program. I have read and understand and accept the terms and conditions stated herein and acknowledge that this agreement shall be effective and binding upon the parties during the entire period of participation in the said program. I grant the Girl Scouts of Central Indiana and persons acting through them, the right to use, reproduce, assign, and/or distribute photographs, films, videotapes, and sound recordings of myself or my child for use in materials they may create.
