2 Standard Service Charge The following represent the services and costs essential to assist with the disposition of a deceased person. These charges apply to every family we serve, regardless which options or services are selected for the arrangement. These Services are the following: Professional / Staff Services...$900 Consultation with a Funeral Director concerning funeral choices and rituals for the Celebration of Life. Co-ordination clergy, musicians, cemetery and/or crematorium, regarding funeral arrangements. Ordering of caskets, urns, flowers, catering and other items and services Organizing vehicles, staff and outside services. Assistance with the placement of media obituary notices. Availability of a funeral director 24 hours a day by phone, text or . Documentation...$200 Preparation of all documents: Statement of Death, Burial Permit, Coroner s Certificates, cremation application, interment permission Provide 15 Proof of Death Certificates and letters of attendance and clergy record. Administration Vehicle...$100 Vehicle used for registration. Transportation from Place of Death $300 Transfer into our care from place of death (hospital environment) within a 50km radius Additional charge for Transfer from Private Residence, Nursing Home, Hospice $150 Additional charge for mileage beyond 50 km is $2.50/km (one way only). Use of Holding Room...$300 The sheltering of the deceased in a secured and private area until final disposition can take place. Standard Service Charge Total...$1,800

3 Facilities for Visitation and Ceremony All facility prices listed below pertain to the St John's Dixie Cemetery & Crematorium Small Chapel Viewing / Service: An intimate gathering that is limited to 30 Family members and chosen Guests for a maximum of 1 hour. Staff Services for Small Chapel Viewing / Service...$350 Staff to arrange, set-up and oversee the Private Viewing for 1 hour Facilities for Small Chapel Viewing / Service...$275 Use of room for the Private Viewing for 1 hour (@ St. John s Dixie) Visiting: An informal gathering for up to 4 hours prior to the funeral or memorial service where family and guests may visit, pay respects and support each other with or without the casket or urn present. Facilities for Visitation...$1100 Chapel is used for the visitation for a period of up to 4 hours, seating for up to 300 persons. (@ St. John s Dixie) Additional Visitation Time...$550 Up to 2 hours Facilities for Ceremony...$550 Chapel is used for the ceremony or memorial service for up to 2 hours. The fee includes the use of the piano and audio-video services and coffee lounge, seating for up to 300 persons. (@ St. John s Dixie) Staff for Visitation and Ceremony Staff attendance is required for all events within the facilities. Staff costs for Visitation, Services, Memorials or Receptions are as follows: One Day of Visitation...$550 Additional Day...$350 Staff Attendance at place of worship...$550 Cost includes staff attendance, set-up, direction and coordination of service. Fee does not include clergy, church, musician, honorarium or funeral hearse. Staff Attendance at Graveside...$350 Co-ordinate, arrange and direct graveside service.

4 Embalming...$450 Embalming is the disinfection, restoration and temporary preservation of the Deceased which includes bathing, dressing, cosmetics and hairstyling. Alternative preparation.. $400 Bathing, disinfecting, sanitizing, dressing, cosmetics, hairstyling and the setting of facial features, without embalming. Basic Preparation....$200 Dressing or Shrouding, disinfecting, sanitizing without setting of facial features or embalming Staff for Witnessing Cremation...$250 Staff required to attend Witnessing Cremation Expedited Cremation...$220 Cremated remains are available within 2 business days. Pallbearer...$140 Fee to hire a pallbearer to assist in carrying the casket; 6 pallbearers are necessary for any casket. Ritual Services Private services which include family participation in dressing, anointing and prayers. Staff for Ritual Services...$150 Staff to coordinate and oversee the ritual services for maximum 1 hour Facility for Ritual Services.....$250 Designated facility used for Ritual Services for a maximum 1 hour Vehicles Lead Car...$280 This vehicle is to lead the procession, driven by the Funeral Director and carrying the minister and three passengers if required. This cost is for 3 hour use. Additional hour...$150 Coach or Funeral Hearse...$485 Coach or Funeral Hearse for transportation of the casket within 50 km. and 3 hours. Additional hour...$200 Flower Van...$150 Van to transport floral tributes to place of worship, graveside or any other location necessary.

Additional hour...$175 Mileage Mileage fee of $2.50 per/km (one-way), applies to any vehicle beyond a 50 km radius of the establishment Additional Services Removal of Pacemaker or Implant...$150 Removal of a pacemaker or implant with no preparation. Refundable Deposit Amount for Cremated Remains...$350 When human remains are to be cremated, a refundable deposit may be required to be paid. If the cremated remains are not claimed within one year of the cremation, the deposit may be used to inter the cremated remains in common ground. If the cremated remains are claimed before they are interred, the deposit will be refunded. Disbursements These costs are third party expenses that are paid on your behalf and are subject to change.
