2 SHIPPING BOOTH INFORMATION (FILL OUT FOR ALL ATTENDEES): There will be a shipping booth set up at the show for customers to send literature collected to their servicing Fastenal store. List the Fastenal employee that will be responsible for delivering this to the customer after the expo. Fastenal Employee Responsible for Delivery 5 Digit Store Code OR 4 Digit Hub Code (location of where box should be delivered) Do not send boxes to the District Manager s home store or hub. Must be the servicing store of that customer! TRAVEL INFORMATION (FILL OUT FOR ALL ATTENDEES): How will the Attendee be Arriving? Commercial Flight Car Bus/Van Charter IF FLYING BY COMMERCIAL FLIGHT: Please check the following: Fastenal will book attendee s flight through Carrousel Travel Attendee will personally BOOK and PAY for flight *Flight details below are collected to start the booking process with Carrousel. The Fastenal Rep is still responsible for calling or ing Carrousel to complete the booking process. Attendees that must book/ pay for their own flight are still encouraged to use Carrousel Travel. This will ensure they are included on all shuttle reports.
