2 nnual Health and Medical Record Information and FQs Personal Health and the nnual Health and Medical Record Find the current nnual Health and Medical Record by using this QR code or by visiting HealthandSafety/ahmr.aspx. The Scouting adventure, camping trips, highadventure excursions, and having fun are important to everyone in Scouting and so are your safety and well-being. Completing the nnual Health and Medical Record is the first step in making sure you have a great Scouting experience. So what do you need? ll Scouting vents. ll participants in all Scouting activities complete Part and Part B. Give the completed forms to your unit leader. This applies to all activities, day camps, local tours, and weekend camping trips less than 72 hours. Update at least annually. Part is an informed consent, release agreement, and authorization that needs to be signed by every participant (or a parent and/or legal guardian for all youth under 18). Part B is general information and a health history. Going to Camp? pre-participation physical is needed for resident, tour, or trek camps or for a Scouting event of more than 72 hours, such as Wood Badge and NYLT. The exam needs to be completed by a certified and licensed physician (MD, DO), nurse practitioner, or physician assistant. If your camp has provided you with any supplemental risk information, or if your plans include attending one of the four national high-adventure bases, share the venue s risk advisory with your medical provider when you are having your physical exam. Part C is your pre-participation physical certification. Planning a High-dventure Trip? ach of the four national high-adventure bases has provided a supplemental risk advisory that explains in greater detail some of the risks inherent in that program. ll high-adventure participants must read and share this information with their medical providers during their pre-participation physicals. dditional information regarding highadventure activities may be obtained directly from the venue or your local council. Prescription Medication. Taking prescription medication is the responsibility of the individual taking the medication and/or that individual s parent or guardian. leader, after obtaining all the necessary information, can agree to accept the responsibility of making sure a youth takes the necessary medication at the appropriate time, but the BS does not mandate or necessarily encourage the leader to do so. Standards and policies regarding administration of medication may be in place at BS camps. If state laws are more limiting than camp policies, they must be followed. The HMR also allows for a parent or guardian to authorize the administration of nonprescription medication to a youth by a camp health officer or unit leader, including any noted exceptions. Risk Factors. Scouting activities can be physically and mentally demanding. Listed below are some of the risk factors that have been known to become issues during outdoor adventures. xessive body weight (obesity) Cardiac or cardiovascular disease Hypertension (high blood pressure) Diabetes mellitus Seizures sthma Questions? Why does the BS require all participants to have an nnual Health and Medical Record?. The HMR serves many purposes. Completing a health history promotes health awareness, collects necessary data, and provides medical professionals critical information needed to treat a patient in the event of an illness or injury. It also provides emergency contact information. Poor health and/or lack of awareness of risk factors have led to disabling injuries, illnesses, and even fatalities. Because we care about our participants health and safety, the Boy Scouts of merica has produced and required the use of standardized health and medical information since at least the 1930s. The medical record is used to prepare for high-adventure activities and increased physical activity. In some cases, it is used to review participants readiness for gatherings like the national Scout jamboree and other specialized activities. Because many states regulate the camping industry, this nnual Health and Medical Record also serves as a tool that enables councils to operate day and resident camps and adhere to state and BS requirements. The Boy Scouts of merica nnual Health and Medical Record provides a standardized mechanism that can be used by members in all 50 states. 3 Boy Scouts of merica Western Los ngeles County Council dult Release and Indemnification greement The undersigned represents that he or she is over 18 years of age and wishes to participate, either as a paying customer, volunteer or as a paid staff member, in one or more of the camp programs sponsored or operated by the Western Los ngeles County Council, Boy Scouts of merica, Inc. (the Council ). The undersigned acknowledges that the camp programs contain activities that may pose some risk for personal injury, including, but not limited to, horseback riding, rifle or gun shooting, archery, camping, swimming, snorkeling, scuba diving, boating, sailing, hiking, mountain biking, crafts, use of sharp instruments, including a knife and axe, rock climbing, rappelling and other similar activities (the Participatory ctivities ). The undersigned acknowledges that participating in the Participatory ctivities is at the undersigned s own risk. In addition, each camp has rules and policies that all participants are required to abide by in compliance with the rules and policies of the Council, the Boy Scouts of merica (National) ( Boy Scouts of merica ) and, in the case of Camp merald Bay, the Catalina Conservancy. These rules and policies preclude a Participant from engaging in certain activities (referred to herein as the Prohibited ctivities ). They include, but are not limited to: 1) Participant must not throw rocks. 2) Participant must follow the buddy system such that he must have a buddy for all activities at the Camp and may be asked to return to the camp if found without a buddy. 3) In the case of Camp merald Bay, the Camp has undeveloped and potentially unsafe areas including, but not limited to: Doctor s Point, the Chapel, all cliffs, and all hiking trails. Use of these areas by a Participant is considered to be at his or her own risk. 4) Participant may not swim or otherwise enter the water when the waterfront is closed. 5) Participant may not enter areas designated as off limits or having a similar designation. Off limits areas include, but are not limited to: a) Staff areas such as staff housing, laundry area, maintenance area and the staff lounge, except in case of emergency. b) Program areas when closed. This includes but is not limited to: the field sports ranges, and, in the case of Camp merald Bay, the Pennington Marine Science Center, Scoutcraft area, Nature area, the Ship s Store, and the Camp water tank and helipad. 6) Participant may not smoke. 7) Participant may not feed, handle or in any way interact with animals. This includes, but is not limited to: feral cats, insects, foxes, squirrels, bison, deer and snakes. 8) Participant may not use prohibited items which include: a) lcohol and narcotics (including medicinal marijuana) b) mmunition, firearms, compressed air guns, pellet guns, martial arts weapons, fish spears or spear guns, and bows and arrows (unless participating in an authorized and supervised activity designed for such purpose). c) Bikes d) Fireworks, fuel or propane e) ny other illegal substance or items 1 of 2 3
4 By signing below I agree to abide by the above rules and policies as well as any additional ones that I am informed of by the camp staff. With regard to the Participatory ctivities and the Prohibited ctivities, the undersigned, by signing below, agrees, on behalf of himself or herself and his or her spouse, children and/or family members, that (i) the Council, the Boy Scouts of merica and each of their respective directors, officers, members, activity coordinators or instructors, staff members, participants, employees or volunteers (collectively and individually, the Indemnified Parties ), shall not be liable or responsible for any injury or damage the undersigned may suffer or incur as a result of participating in the Participatory ctivities or the Prohibited ctivities unless solely attributable to the gross negligence or intentional misconduct of the Indemnified Party, and (ii) the undersigned shall defend, hold harmless and indemnify the Indemnified Parties from and against all losses, claims, damages, costs or expenses (including reasonable legal fees and court or similar costs) in connection with any action or claim brought or made (or threatened to be brought or made) for, or on account of, any injuries or damages received or sustained by the undersigned, or in any way related to any action or omission arising, during the course of engaging in said Participatory ctivities or Prohibited ctivities, including, without limitation any action or claim brought or threatened to be brought, by my spouse, child or family member, unless solely attributable to the gross negligence or intentional misconduct of the Indemnified Party.
