Our current overall standings (as of May 8, 2013) for returned and clean data are: Cases data : 88.3% Cases and : 71.5%* * The number of cases with has decreased slightly since the last report due to the fact that a new round of queries is now being generated by the Data Co-ordinating Centre. We should see that number increase again, as these new queries are answered! 2 CHIPS Data Tutorial: Submitting supporting documents: Before sending any supporting documents to the CHIPS Data Co-ordinating Centre, please remember to block-out all identifying information, such as patient names and addresses, mother s date of birth, doctor names, and hospital numbers. For example: Please also ensure that the following information is clearly visible and translated into English (where applicable), on any supporting documentation submitted: 1) Text that confirms a pregnancy loss reported on the Delivery Form. 2) Admission and discharge date and time to high level neonatal care. 3) Reasons for any admission to high level neonatal care. 4) Text about any other neonatal events reported on the Neonatal Form or the Postpartum Questionnaire. 5) Text about any maternal outcomes or other maternal events reported on the Maternal Form or the Postpartum Questionnaire. Please contact the DCC directly at chipsdata@sunnybrook.ca should you have any questions about sending or completing the CHIPS data. Important Reminders Please remember that the CHIPS Trial will not be considered closed until all data collection and cleaning are complete and we have officially closed the database for the final analysis. The central CHIPS co-ordinating team will inform all collaborators when this happens. Until that time, please be reminded of the following requests from the CHIPS Co-ordinating Centres: 1) Please maintain your local ethics approvals for CHIPS. 2) Please keep your local CHIPS data readily accessible. 3) Please collect and keep the Mother s Diary for each patient.
