3 Report of the Meeting i 1. Meeting and Registration 1.1 The 10 th Meeting of the Asia Pacific Regional Aviation Safety Team (APRAST/10) was held at the ICAO Asia and Pacific Office in Bangkok, Thailand from 17 to 21 April The APRAST/10 Meeting was attended by 94 participants from 19 States/ Administrations and 8 International Organizations and Industry Partners viz. Australia, Bangladesh, Bhutan, Cambodia, China, Hong Kong China, Macao China, India, Indonesia, Japan, Malaysia, Maldives, Papua New Guinea, the Philippines, Republic of Korea, Singapore, Thailand, the United States, Vietnam, Airports Council International (ACI), Airbus, Boeing, Global Civil Aviation Training Solutions (CAE), Civil Air Navigation Services Organization (CANSO), Flight Safety Foundation (FSF), International Air Transport Association (IATA) and International Federation of Air Line Pilots Associations (IFALPA). The list of participants is at Appendix A. 1.3 Mr. Kong, Cheong Tuck, Flight Safety Expert of the ICAO Asia and Pacific Office acted as Secretary to the Meeting supported by Capt. Pu Fang Hui, Associate Safety Oversight Officer. 2. Opening Session 2.1 Mr. Arun Mishra, Regional Director of ICAO Asia and Pacific Office welcomed participants, introduced the new Deputy Regional Director, Dr. Manjit Singh and provided a brief update on the events and changes for He informed the Meeting that the RASG-APAC/7 would be held on 3-5 July 2017 separately from the DGCA Conference. RASG-APAC/7 would include a half-day workshop conducted by ICAO Headquarters on ICAO Global Aviation Safety Plan. 2.2 Mr. Tay Tiang Guan, APRAST Co-chair (States), Deputy Director General, Civil Aviation Authority of Singapore (CAAS) updated on the progress of APRAST since its inception in He highlighted the need for States Administrations and Industry to step up their involvement in APRAST/ RASG-APAC and take concrete measures to improve overall regional safety performance. APRAST/10 would also prepare for RASG-APAC/7 and work on improving States/Administrations USOAP CMA EI scores, particularly for AGA and AIG areas. 2.3 Mr. Gerardo Hueto, APRAST Co-chair (Industry), Assistant Director, Safety and Flight Operations Asia-Pacific, IATA, thanked the various working bodies for their contributions and looked forward to further cooperation between States/Administrations and Industry to advance aviation safety in the region.

4 Report of the Meeting 1 REPORT ON AGENDA ITEMS 1. AGENDA ITEM 1: PLENARY SESSION ADOPTION OF AGENDA 1.1 Adoption of the Provisional Agenda WP/ The Secretariat presented the Provisional Agenda, which was adopted by the Meeting. 2. AGENDA ITEM 2: WORKSHOP ON SMS ACCEPTANCE AND SEI IMPLEMENTATION 2.1 Workshop on SMS Acceptance and SEI Implementation The Workshop on SMS Acceptance and SEI Implementation was successfully held on April Mr. Charles A. Galea, Civil Aviation Safety Authority (CASA) Australia and Mr. Zainul Abidin Maslan, Department of Civil Aviation (DCA) Malaysia, presented their experience in evaluating Safety Management System (SMS) and in selecting Safety Performance Indicators for the purpose of setting the Acceptable Level of Safety Performance (ALoSP) respectively. Mr. Greg Marshall, FSF, presented the FSF s report on Go-Around Decision Making & Execution Project, relevant to Safety Enhancement Initiative (SEI) RE/1 on promoting pilot adherence to Standard Operating Procedures for approaches which include go-around decision making. Mr. Xavier Lesceu, Airbus, presented the guidance material on Basic Flying Skills and Upset Prevention and Recovery Training, relevant to SEI LOC/6 on Airplane Upset Prevention & Recovery Training Aid (AUPRTA). The summary of discussions of the workshop is at Appendix B. 3. AGENDA ITEM 3: UPDATE OF RASG-APAC/6 DECISIONS AND WORK PROGRAMME 3.1 Update of RASG-APAC/6 Decisions and Conclusions WP/ The Secretariat presented the progress of RASG-APAC/6 Decisions and Conclusions. The Meeting noted the updates and supported the recommendations on the status of the Decisions and Conclusions, as reflected at Appendix C. 3.2 Progress of RASG-APAC 2016/2017 Work Programme WP/ The Secretariat presented the progress of RASG-APAC 2016/2017 Work Programme which was approved at RASG-APAC/6 in August The Meeting noted the updates provided by the Secretariat and the various champions during the course of Meeting and accepted the status of work programme, as shown in Appendix D. 4. AGENDA ITEM 4: UPDATE AND REVIEW OF PROGRESS OF APRAST ACTIVITIES 4.1 Update of APRAST/8 and APRAST/9 Decisions and Conclusions WP/ The Secretariat presented the progress of outstanding Decisions and Conclusions adopted at APRAST/8 and APRAST/9. The Meeting noted the updates provided by the Secretariat and the various champions during the course of Meeting.

5 Report of the Meeting As a follow-up to Decision APRAST 9/21, SEI WG Co-chair (States) presented the analysis of AGA EI scores and identified the crucial critical elements (CEs) that may contribute to the low AGA EI scores The Meeting noted that Decision APRAST 8/6 should be kept open until Secretariat includes the process on the use of the online portal in the RASG-APAC Procedural Handbook. Therefore, except for Decision APRAST 8/6, the Meeting accepted the recommendations on the status of the Decisions and Conclusions as reflected at Appendix E. The Meeting agreed to the following Conclusion: Conclusion APRAST 10/1 Inclusion of the online portal procedures in the RASG-APAC Procedural Handbook That, Secretariat update the RASG-APAC Procedural Handbook to include the use of the online portal which monitors the implementation of RASG-APAC endorsed safety tools, with the aim of seeking approval at RASG-APAC/7. Expected impact: Achievement of global and regional aviation safety priorities and targets Enhancement of USOAP Effective Implementation Monitoring and Administration Capacity Building and Sharing of Information Inter-Regional Why: Update the RASG-APAC Procedural Handbook When: July 2017 (RASG-APAC/7) Status: on-going Who: APRAST APAC-AIG SRP WG SEI WG Capacity Building TF APAC States/Administrations ICAO APAC RO ICAO HQ Other: 4.2 Update of the Regional Aviation Safety Priorities and Targets WP/ APRAST Co-chair (States) presented an update of the Regional Aviation Safety Priorities and Targets, which can be found at Appendix F. He noted the progress made in a number of areas but stressed the need for States/Administrations and Industry to continue making efforts to achieve these priorities and targets With insight gained on the rate of progress and potential challenges in implementation, the Meeting agreed for some targets to be revised for clarity and their deadlines extended. Airbus highlighted that IOSA, ISSA and ISAGO registrations were industry standards which should not be confused with States safety oversight and compliance with ICAO standards. It suggested that the Regional Aviation Safety Priority 2 on improvements to safety oversight and compliance be rephrased for clarity. The Meeting therefore agreed on the following Conclusion:

6 Report of the Meeting 3 Conclusion APRAST 10/2 Proposed Revisions to the Regional Aviation Safety Priorities and Targets (WP/5) That, RASG-APAC/7 approve the proposed changes to Regional Aviation Safety Priorities and Targets as shown below: a. Title of Regional Priority 2 - Improvements to safety oversight and compliance/industry safety certification audits ; b. First target under Regional Priority 1 - RASG-APAC to complete the development of current identified priority SEIs by end 2016 end ; c. Fourth to sixth targets under Regional Priority 2 - By end of (1) maintain at least 60% of increase the number of IOSA registered applicable APAC airlines to be IOSA certified by 50% over end-2016 figures; (2) achieve at least 15% of increase the number of ISSA registered applicable APAC airlines to be ISSA certified by 15% over end-2016 figures; (3) pursue at least a 50% increase in ISAGO registrations over end-2016 figures ; and d. Third target under Regional Priority 4 - By 2020, pursue 50% increase in participation in flight data sharing initiatives by of APAC air operators, with aircraft of mass 27,000kg and above, over end-2016 figures participating in flight data sharing initiative by Expected impact: Achievement of global and regional aviation safety priorities and targets Enhancement of USOAP Effective Implementation Monitoring and Administration Capacity Building and Sharing of Information Inter-Regional Why: Update the regional aviation safety priorities and targets When: July 2017 (RASG-APAC/7) Status: on-going Who: APRAST APAC-AIG SRP WG SEI WG Capacity Building TF APAC States/Administrations ICAO APAC RO ICAO HQ Other: RASG- APAC In time for reporting at RASG-APAC/7, the Meeting agreed to the following Conclusion: Conclusion APRAST 10/3 Update of the Regional Aviation Safety Priorities and Targets (WP/5) That, States/Administrations and Industry provide information on the progress of the APAC Regional Aviation Safety Priorities and Targets to the ICAO APAC Regional Office with an aim of updating RASG-APAC/7. 7 Report of the Meeting 4 Capacity Building and Sharing of Information Inter-Regional Why: Improve the tracking of implementation status of regional aviation safety priorities and targets When: 31 May 2017 Who: Status: on-going APRAST APAC-AIG SRP WG SEI WG Capacity Building TF APAC States/Administrations ICAO APAC RO ICAO HQ Other: IATA, AAPA, Industry 4.3 Presentation on the Safety Tools Implementation Monitoring Mechanism and online submission/update of the Safety Tool Implementation status The Secretariat presented updates on the development of the online portal to monitor the implementation of RASG-APAC endorsed safety tools. Given that only four States/ Administrations had thus far responded on the online portal, the Meeting urged more States/ Administrations to submit their status of implementation through the online portal. The Meeting noted that the online portal can be improved by including an option which allows States/Administrations to indicate 100% implementation for RASG-APAC endorsed safety tools and agreed to the following Decision: Decision APRAST 10/1 - Safety Tools Implementation Monitoring Mechanism That, the online portal for monitoring the implementation of RASG-APAC endorsed safety tools, be enhanced with an option for States/Administrations to indicate 100% implementation. Expected impact: Achievement of global and regional aviation safety priorities and targets Enhancement of USOAP Effective Implementation Monitoring and Administration Capacity Building and Sharing of Information Inter-Regional Why: Monitoring the implementation of SEIs and safety tools When: 31 May 2017 Status: on-going Who: APRAST APAC-AIG SRP WG SEI WG Capacity Building TF APAC States/Administrations ICAO APAC RO ICAO HQ Other: Bangladesh 4.4 Update of APAC-AIG Activities WP/ Mr. Chan Wing Keong, Chairman APAC-AIG, presented updates on the APAC-AIG activities and work plan, as well as the follow-ups to the Decisions made at the APRAST/9. The Meeting noted the following training which aims to assist States/Administrations in achieving better AIG EI scores: i) ICAO Asia Pacific Regional Workshop on Protection of Safety Information in Bangkok on 2-4 May 2017; ii) Airbus training in Toulouse on May 2017; iii) TSIB s Annual Training in Singapore on May 2017; iv) APAC-AIG/5 Meeting in Singapore on 1-2 August 2017;

8 Report of the Meeting 5 v) ICAO Asia Pacific Regional Accident Investigation Workshop in Singapore on 3-4 August 2017; and vi) ISASI Annual Seminar in San Diego, California, USA on August The Meeting agreed to the following Conclusion: Conclusion APRAST 10/4 - Update of APAC-AIG Activities (WP/6) That, States/Administrations be strongly encouraged to actively participate in the training identified by the APAC-AIG, which has the potential to enhance their USOAP AIG EI. Expected impact: Achievement of global and regional aviation safety priorities and targets Enhancement of USOAP Effective Implementation Monitoring and Administration Capacity Building and Sharing of Information Inter-Regional Why: Improve APAC USOAP AIG EI scores When: 31 December 2017 Status: on-going Who: APRAST APAC-AIG SRP WG SEI WG Capacity Building TF APAC States/Administrations ICAO APAC RO ICAO HQ Other: 4.5 Survey on Training Demands for Accident Investigation Topics WP/ Secretariat presented the results of the survey on training demands for AIG topics conducted in December The Meeting noted that a few of the training topics desired by States/Administrations had already been included in the APAC-AIG work plan. AGENDA ITEM 5: PRESENTATION STATE / INDUSTRY / ICAO 5.1 Coordination between APANPIRG and RASG-APAC Subsidiary Bodies WP/ The Secretariat presented the coordination between APANPIRG and RASG-APAC subsidiary bodies as an update to Decision APRAST 9/12. The Meeting noted the existing efforts within APRAST and existing mechanism for the exchange of information and issues between relevant APANPIRG and RASG-APAC subsidiary bodies via Secretariat. The Meeting also noted the progress of a related Decision APRAST 9/11, where SRP WG would coordinate with IATA to get updates on two issues (large height deviations and English language proficiency) discussed at APANPIRG and its subsidiary bodies, to support the coordination between APANPIRG and RASG-APAC. 5.2 Update of the Global Aviation Safety Plan (GASP) WP/ The Secretariat highlighted the changes in the Edition of the Global Aviation Safety Plan (GASP) which was endorsed at the 39 th Session of the ICAO Assembly (27 September 7 October 2016). The inclusion of a new Global Aviation Safety Roadmap in the GASP provided a structured and common frame of reference for all relevant stakeholders to ensure that safety initiatives deliver the intended benefits associated with the GASP objectives. The Meeting agreed to the following Conclusion: Conclusion APRAST 10/5 - Update of the Global Aviation Safety Plan (GASP) (WP/9) That States be encouraged: a) to develop a national safety plan in line with the GASP objectives and based on their operational safety needs; and b) to provide feedback to ICAO on the new Global Aviation Safety Roadmap and suggestions for the future Edition of the GASP via to: GASP@icao.int. Expected impact: Achievement of global and regional aviation safety priorities and targets Enhancement of USOAP Effective Implementation Monitoring and Administration Capacity Building and Sharing of Information Inter-Regional Why: Implement GASP When: On-going Status: on-going Who: APRAST APAC-AIG SRP WG SEI WG Capacity Building TF APAC States/Administrations ICAO APAC RO ICAO HQ Other: 5.3 Update of ICAO Support for Safety Management Implementation WP/ The Meeting noted Secretariat s presentation on the list of initiatives undertaken by ICAO to assist States in implementing their State Safety Programme. 5.4 Enhancements to Singapore s Aerodrome Certification Framework WP/ Singapore shared the enhancements made to its aerodrome certification framework for significant new aerodrome developments and modifications. Indonesia shared that it had a similar process requiring aerodrome operators to submit designs and to seek approval for completed aerodrome infrastructure. The Meeting agreed to the following Conclusion:

10 Report of the Meeting 7 Conclusion APRAST 10/6 ICAO guidance on aerodrome certification That, ICAO consider developing guidance material in the area of new aerodrome developments and modifications as part of aerodrome certification. Expected impact: Achievement of global and regional aviation safety priorities and targets Enhancement of USOAP Effective Implementation Monitoring and Administration Capacity Building and Sharing of Information Inter-Regional Why: Enhance aerodrome certification When: 31 August 2017 Status: on-going Who: APRAST APAC-AIG SRP WG SEI WG Capacity Building TF APAC States/Administrations ICAO APAC RO ICAO HQ Other: 5.5 Review of Description of Terminologies related to Aeronautical Study, Safety Assessment and Safety Reviews WP/ The Meeting discussed WP/13 prepared by Pakistan, requesting for clarity on 3 terms related to safety management in ICAO documents: safety review, safety assessment and aeronautical study. The Meeting noted that there was a similar request made by Nepal which led to the adoption of Conclusion APRAST 8/7. The Meeting requested the Secretariat to check with ICAO on the status of this matter. 5.6 Regional Data Collection, Analysis and Information Sharing Status WP/ Flight Safety Foundation and MITRE presented WP/14 on Regional Data Collection, Analysis and Information Sharing for Aviation Safety, which was supported by Japan, Singapore, the Philippines, Thailand, AAPA, Airbus and IATA. FSF updated that preparations are underway for the first Governing Board and Technical Working Group Meetings to be held later in APRAST Cochair (States) highlighted that the finalized Governance Plan provides a set of core principles guiding this project, to ensure non-punitive use of safety information and to preserve data confidentiality. Japan also strongly encouraged States/Administrations and industry to consider participating in the Demonstration Project. The Meeting agreed to this Conclusion: Conclusion APRAST 10/7 - Regional Data Collection, Analysis and Information Sharing Status (WP/14) That, States/Administrations and Industry be strongly encouraged to participate in the Demonstration Project for Regional Data Collection, Analysis and Information Sharing for Aviation Sharing, by submitting Statement of Intent (Appendix B of the Governance Plan) to FSF/MITRE. AGENDA ITEM 6: BREAKOUT SESSIONS 6.1 SEI WG and SRP WG held their coordination Meeting prior to the breakout sessions, and updated each other on work priorities. 6.2 The Capacity building Task Force, SEI WG and SRP WG held breakout sessions which included review of the work items in the RASG-APAC 2016/2017 Work Programme, past and emerging issues. 6.3 Update of Capacity Building Task Force Breakout Session The Task Force developed and presented a Standardized Capacity Building Programme (SCBP) (see WP/7), based on the information collected from the CE-4 gap analysis questionnaire and the istars data on CE-4 compliance by the APAC region. States/Administrations were encouraged to use the SCBP for their capacity building efforts. The Meeting agreed to the following Decision: Decision APRAST 10/3 - Update of Capacity Building Task Force Breakout Session That, APRAST/10 approve the Standardized Capacity Building Programme developed by the Capacity Building Task Force. Why: Capacity Building When: 21 Apr 2017 Expected impact: Achievement of global and regional aviation safety priorities and targets Enhancement of USOAP Effective Implementation Monitoring and Administration Capacity Building and Sharing of Information Inter-Regional Status: Closed Who: APRAST APAC-AIG SRP WG SEI WG Capacity Building TF APAC States/ Administrations ICAO APAC RO ICAO HQ Other: 6.4 Update of SRP WG Breakout Session The Meeting noted that with regard to Decision APRAST 8/21 1, SRP WG had completed and published both the public and member versions of the Annual Safety Report The Meeting noted that with regard to Decision APRAST 9/4 2, the relationship between aircraft types, equipage and accidents related to CFIT had been rigorously studied through existing SEIs that had been developed on CFIT. In reducing CFIT occurrences, Airbus opined that besides aircraft equipage, it is also important for States to ensure the accuracy of terrain/obstacle databases. IATA supported Airbus s views and urged States/Administrations which have yet to comply with ICAO WGS-84 standard to do so. The Meeting noted that this issue on terrain data falls under the ambit of APANPIRG, and hence coordination is needed between APANPIRG and RASG- APAC subsidiary bodies. 1 Decision APRAST 8/21: That, The AP-SRP WG complete the APAC Annual Safety Report 2016 by APRAST/9. 2 Decision APRAST 9/4: That, SRP WG is to study the relationship between aircraft types, equipage and accidents related to CFIT, and provide recommendations to APRAST on further development of SEIs by SEI WG as needed.

12 Report of the Meeting 9 Conclusion APRAST 10/8 - Coordination between APANPIRG and RASG-APAC subsidiary bodies - terrain/obstacle databases and WGS-84 That, Secretariat, with the assistance of Airbus and IATA, emphasize the importance of accurate terrain/obstacle databases and compliance of ICAO WGS-84 standard by States/Administrations, in support of the use of Enhanced Ground Proximity Warning System (EGPWS), to APANPIRG. Expected impact: Achievement of global and regional aviation safety priorities and targets Enhancement of USOAP Effective Implementation Monitoring and Administration Capacity Building and Sharing of Information Inter-Regional Why: Address CFIT operational risks When: end May 2017 Status: On-going Who: APRAST APAC-AIG SRP WG SEI WG Capacity Building TF APAC States/Administrations ICAO APAC RO ICAO HQ Other: Airbus, IATA The Meeting noted that Decisions APRAST 8/4 3, 8/23 4, 9/11 5 and 9/20 6 commonly required coordination between APANPIRG and RASG-APAC subsidiary bodies, and identified three issues for coordination: large height deviations, English language proficiency and terrain/obstacle databases. IATA is currently reviewing the geographical correlation between large height deviations and TCAS RAs and preparing a report on English language proficiency issues. SRP WG will review the analyses done by IATA when completed, and determine the next course of action. The Meeting agreed to the following Decision: 3 Decision APRAST 8/4: That, with reference to APRAST Decision 6/2, IATA work with AAPA to follow up to define the extent of the issues related to English Language Proficiency and determine the next course of action, and provide an update at APRAST/9. 4 Decision APRAST 8/23: That, AP-SRP WG provide an update at APRAST/9 on the information to be shared with RASMAG of APANPIRG for further analysis (refer to ASBU Block 0 Modules and APAC Seamless ATM items as safety barriers to CFIT, RS and LOC-I occurrences). 5 Decision APRAST 9/11: That, SRP WG coordinate with IATA to get updates on relevant issues discussed at APANPIRG and its subsidiary bodies. 6 Decision APRAST 9/20: That, SEI WG, with the assistance from Secretariat, obtain information from APANPIRG on any work that had been developed in the areas of CFIT, Fatigue Risk Management, Laser Attack and State Safety Programme, for which RASG- APAC is the lead regional group.

13 Report of the Meeting 10 Decision APRAST 10/4 Issues of Coordination between APANPIRG and RASG-APAC subsidiary bodies That, Decisions APRAST 8/4, 8/23, 9/11 and 9/20 be consolidated (and superceded by this Decision), such that APRAST focuses on the following three issues: large height deviations, English language proficiency and terrain/obstacle databases, with respect to coordination between APANPIRG and RASG-APAC. While the two documents would address SEI LOC/6 to a large extent, the Meeting requested that SEI WG consider providing guidance for regulators to implement ICAO UPRT requirements For SEI LOC/2 (Hazard Identification and Risk Management), the Meeting agreed to combine LOC/2 and LOC/4, as both outputs make use of SMS principles and methodology. As the LOC/2 champion (AAPA) was absent from the Meeting, LOC/4 champion (Malaysia) will coordinate with LOC/2 champion prior to RASG-APAC/7. 17 Report of the Meeting The Meeting discussed possible workshop topics at APRAST/11 and APRAST/12. The Meeting noted that the next workshop should include SSP implementation as previously scheduled, as well as a topic on runway safety, given that there were no APAC workshops/ seminars for runway safety this year, required by the Regional Aviation Safety Priorities and Targets. The Meeting also noted suggestions for topics on large height deviations and TCAS-RA. 71 Appendix F to the Report Regional Priority 2: Improvements in Safety Oversight and Compliance Target: Task force (to be formed by APRAST) to develop an action plan on capacity building by June The Capacity Building Task Force was established at APRAST/8 with a focus on developing an action plan, where specific training requirements that could contribute towards raising the Effective Implementation (EI) of ICAO USOAP Critical Element 4 will be recommended, particularly in the areas of flight operations, airworthiness, air navigation services and accident investigation. The Task Force comprises participants from Brunei Darussalam, Malaysia, Papua New Guinea, Singapore (Champion) and the United States, with support from the ICAO APAC regional office. The action plan on capacity building, developed by the Task Force, was approved at RASG- APAC/6 in 2016, effectively achieving this target. Target: States to resolve any SSCs identified by the ICAO USOAP CMA programme promptly within the timeline specified in the corrective action plan and agreed to by ICAO 2.6 Same as the update given at APRAST/9, two States, with SSCs in the area of Air Operator Certification, are in the process of resolving the SSCs. ICAO APAC Regional Office will continue to monitor the situation of resolving SSCs for APAC States.
