1 In order to accommodate our Biltmore Friends, we have established The Cabana Club as a membership club through which our leisure and fitness facilities will be available to be enjoyed by persons other than our in-house hotel guests. We take this opportunity to invite you to be part of the Cabana Club, by filling out the attached membership application and submitting to our office.

2 MEMBERSHIP FEES AND CATEGORIES 1. Hotel Guests - Free Of Charge 2. Registration Fee - Yearly $ Individual Rates - For Single Persons Only 1 Month Member - $ Registration Fee 3 Months Member - $ Registration Fee 6 Months Member - $ Registration Fee 12 Months Member - $ Registration Fee 4. Family - 2 Adults And Up To Three Children below 13 years old 1 Month Member - $ Registration Fee 3 Months Member - $ Registration Fee 6 Months Member - $ Registration Fee 12 Months Member - $1, Registration Fee 5. Corporate - For Up To 4 Individuals Who Work For The Same Company. Family Members Cannot Use This Type Of Membership, They Must Be Signed Up As Additional Members 1 Month Member - $ Registration Fee 3 Month Member - $ Registration Fee 6 Month Member - $1, Registration Fee 12month Member - $2, Registration Fee 6. Additional Person - Adult Monthly Rate $70.00 And 13 years below Monthly Rate $25.00 Applicable Taxes Are Included In The Above Membership Fees. Page 2

3 UPON EXPIRATION OF MEMBERSHIP THE MEMBER WILL BE RESTRICTED FROM USING THE CLUB S FACILITIES UNTIL MEMBERSHIP IS RENEWED. UPON CANCELLING OF MEMBERSHIP BEFORE EXPIRATION, MEMBERS REQUESTING A PRO RATED REFUND. WILL BE REQUIRED TO SEND A LETTER OF REQUEST TO THE ADMINISTRATION DEPARTMENT. SUCH REFUND WILL BE ISSUED AT THE DISCRETION OF MANAGEMENT. MEMBERSHIP CONDITIONS Membership fees are subject to change with adequate notice and registration fee is not refundable. In the event of a lapse in membership for a period greater than six months, a new application must be submitted, and new registration fee must be paid. The member agrees not to hold the B.W Plus Belize Biltmore Plaza Hotel liable for any damages or accidents as a result of use of the club s installations Page 3

6 MEMBERSHIP RULES Club Member Each person is required to complete a membership application form. Member will be required to pay the registration fee and the applicable member fee before accessing the club s facilities. Each member will be issued a membership card that remains the property of the Club. The membership card will be required to be presented in order to sign on your monthly charge account and in order to receive the 10% discount. If the card is lost or stolen, the Manager on duty should be notified immediately and the member is responsible for the cost of replacing the card. Towels are the hotel property and should be returned to place of issue. Members agree to abide by the Club s rules and regulations and make certain that all family members and guests do the same. Management reserves the right to approve, suspend or cease membership, if rule(s) of the club are breached. No child 13 years of age or under is allowed to enter the pool area without the supervision of a parent and, no child 13 years of age or under is allowed to enter the gym are. Club rules may be changed as necessary by the hotel management. Fitness Center is open daily from 5:00 am to 11:00 pm. Use of any one machine is limited to 30 minutes if others are waiting. Consult a doctor before starting any exercise regime. The use of the equipment is at your own risk. No trainer is on Duty. Smoking is prohibited. Only tennis shoes are allowed. Eating Is prohibited in the gym. All entrance & exit will be done through the inside of the hotel compound. Exterior door is for emergency exit only. Page 6

7 FITNESS CENTER Pool Pool is open daily from 6:00 am to 10:00 pm. There is no Lifeguard on duty. Swim at your own risk. Diving is not allowed. Running in the pool area is not allowed. Shower before entering the pool. No food or beverage is allowed in the pool. No glass bottles or containers are allowed by the pool. No food or beverage from off premises is allowed on property. No pets are allowed on the premises. Page 7
