Upon using the services, there are various limitations and regulations. 2 Common notice Mileage accrual Only ANA Mileage Club members can accrue mileage for purchased travel, services and products. It takes from 1 up to 4 months for accrued mileage to be credited to the member s account. You cannot transfer or combine earned miles. However, members registered with ANA Card Family Miles and the ANA Mileage Club Family Account Services programs can combine miles earned by registered family members only when using some awards. Fractions will be rounded down for the accrued mileage according to the amount paid. Partner points that have been transferred and converted into miles cannot be reconverted back to points. Miles registered with ANA Mileage Club cannot be registered with other mileage programs. ANA number The 10-digit ANA number embossed or printed on your card is required to book flights, earn mileage, and redeem awards. Please quote your ANA number when making inquiries. Please note that your ANA number differs from your credit card number. We cannot respond to inquiries regarding your ANA number. Your ANA number will change every time we issue a new card, in the event of a lost or stolen card, name change, or if you switch to another card. Please note that the 16-digit Electronic Money Edy number on the back of the card differs from your ANA number and credit card number. The name on your card must be the same as the name shown on your passport. Check your mileage Please confirm your earned mileage using the following various information services. Your 10-digit ANA number and your password * are required to use the information services described below. * Refer to P.2 for more information about passwords. You can register your address through ANA Website, ANA Mileage Club Members-Only functions. Statements (spanning 3 months) can be sent by postal mail for a 500-mile handling fee per statement. AMC Password (4-digit number) This password is set or entered by the member when completing the application procedures. It is required when contacting the ANA Reservation and Information Center or ANA Mileage Club Service Center in Japan via telephone, and when using Self Service Checkin machine at airports, etc. If you did not specify a password when applying, a password set by ANA will be sent to you by postal mail. Web Password (8 to 16 digits containing one-byte letters [upper case and lower case] and numbers) This password is set by the member when logging in to the official ANA website for the first time. It is required to use ANA Website online services (logging in, various types of procedures, redeeming mileage, etc.) The AMC Password is required to log in for the first time. The password should only be used by the member. Please do not select a password that you use with other companies. 1 2

3 Common notice When applying for a new ANA Card, your password will be sent via postcard. When switching from an ANA Mileage Club Card to an ANA Card, you can continue using the same password if you enter your current ANA Mileage Club Card customer number on the application form. If you want to change your password or have forgotten your password You can do so via ANA Website. If you have forgotten your password, please click on Forgot your Password? and complete the procedures. Mileage validity Accrued mileage is valid until the last day of the 36th month counting from the month a service or product was used to earn miles. Please redeem your awards within the mileage validity period. [Examples] Miles earned on May 23, 2017 expire on May 31, 2020 Miles earned on June 10, 2018 expire on June 30, 2021 Miles earned on July 1, 2019 expire on July 31, 2022 Awards users Eligible member for Awards Awards may be used by the member, the member s spouse and family members within two generations (family members of the relation in the second degree). Some awards * can only be used by the member only. Awards users registration Award User registration must be completed in advance for persons other than the member to use awards. ANA Website Please visit the official ANA website. Up to 10 people [the member s spouse and family members within 2 generations (family members of the relation in the second degree)] can register as award users, excluding the member. If you cannot complete the procedures via ANA Website, please contact the ANA Mileage Club Service Center. Please note that you will not be able to book awards online if the award user is registered twice. Registered information changes and cancellations Please contact ANA Mileage Club Service Center (Operator service hours) to change or cancel award user s registered data. The relevant forms will be sent to you. The fee to change or delete previously registered information is 5,000 miles per person. No fee will be charged to change or delete registered information due to spelling mistakes/corrections or due to family registry changes such as marriage or death. You will be requested to present official documentation as proof of family changes(you may be asked to provide official documentation proving your relationship with the person you wish to register). If the ANA number changes, the award user information registered under the card will not be automatically updated. Please contact ANA Mileage Club Service Center. 4 Common notice Awards requests Notification of change of personal information Only members can request awards. Members are requested to confirm their identity when requesting awards. Please have your ANA Card or ANA Mileage Club Card number ready. Members cannot request awards without their 10-digit ANA number. As a rule, we cannot accept award requests from persons who are not members. If an agent or representative requests an award on behalf of a member, we will verify the agent s identification. Members must have sufficient mileage in their account when requesting to redeem awards. We will automatically use the oldest accumulated mileage in your account to redeem awards. Your miles cannot be combined with miles earned under a different name, such as miles from family members or friends. In addition, miles earned under other airline s mileage programs cannot be combined with miles earned with ANA Mileage Club. However, members registered with ANA Card Family Miles and the ANA Mileage Club Family Account Services programs can combine miles among family members. Any requested, but unused awards become invalid. Miles that were redeemed for awards, but were not used cannot be returned to a mileage account or exchanged for other awards. Please note that there is a Refund for Unused Awards for Flight Awards and Upgrade Awards. Please check at P.107 for details. When personal information that is registered on ANA Mileage Club such as address and name has been changed, please contact the following. ANA does not take responsibility for any disadvantages to the members including non-communication of necessary information or non-delivery of awards due to lack of change notification submission. Premium Class mileage accrual Every time you pay to upgrade to Premium Class, you will earn 50% bonus miles from the original purchased fare. [Example]Value21 fare 75% + 50% = 125% of basic mileage You will not earn additional 50% Premium Class miles when you pay to upgrade to Premium Class with a ticket that is not eligible for mileage accrual. Notes Only flight sectors actually flown by a member qualify for mileage accrual. All bonus miles are accrued only if the flight miles for the eligible flight have been registered with ANA Mileage Club. A single passenger occupying two or more seats to accommodate over-sized carry-on baggage such as musical instruments or paintings will only accrue mileage based on one seat. Mileage is not accrued if you did not board your flight for any reason, including flight delays or cancellations. Miles registered with ANA Mileage Club cannot be registered with other mileage programs. Mileage registered under other frequent flyer mileage programs, including mileage registered with a temporary card, cannot be transferred to ANA Mileage Club. In addition, mileage registered with ANA Mileage Club cannot be transferred to other frequent flyer mileage programs. ANA Mileage Club Service Center, Retroactive Registration Desk KAMATA POST OFFICE We can only accept documents sent by regular postal mail. Documents sent by parcel delivery service (takuhai-bin), or other non-postal services are not accepted. Documents and applications forms (Memos) sent to the Retroactive Registration Desk will not be returned. Please send the original documents and retain copies for your personal records. (Once the mileage registration application is completed, documents will be destroyed in an appropriate manner). Registration cannot be completed if any of the confirmation documents are missing. If the information on the application form (Memo) differs from the information registered with ANA Mileage Club, we may return your application and documents and confirm their content. When applying by mail, postage for Retroactive Registration applications is borne by the customer. Postal fees will not be refunded. When you apply by mail to register itineraries of domestic flights only, it may take up to two weeks to process and complete your retroactive registration. (If your itinerary includes flight sectors on ANA International flights or partner airlines, it may take up to 1 month to process your application). Earning Miles 17 18

11 Earn Miles with ANA Japan Domestic Flights Application Period Applications may be submitted from 1 day (p.m.) after your boarding date within 6 months. Applications sent by mail must be received within 6 months of your boarding date. Earning Miles Documents required for retroactive registration Please retain your retroactive registration documents until you confirm that the mileage has been accrued. 1 Boarding pass or Security certificate or Boarding information (original) 2Memo with your 10-digit ANA number and registered name. Please make sure that he name on 1 and 2 is the same. We cannot accept retroactive registration applications after 6 months of your boarding date. Only passengers who have applied to join ANA Mileage Club can apply for retroactive registration. (Applying for retroactive registration does not qualify as membership enrollment.) You can confirm the status of your retroactive registration on ANA Website. We cannot answer to individual inquiries regarding this matter

After Check-in Register at the airport check-in counter. When connecting You must register at the check-in counter in each connecting airport. Retain your comfirmation documents If your change or upgrade your booking at the airport on the day of your flight, your mileage may not be accrued automatically to your account. Always retain the necessary documents [boarding pass (original) and your e-ticket Itinerary & Receipt or the passenger s receipt of your air ticket (copies accepted)] until you have confirmed that mileage has been accrued. Time required for mileage to be accrued Mileage is accrued in 2-3 days after your boarding date. In the case of codeshare flights with ANA flight numbers operated by partner airlines, it takes from 3 days to 1 month after the date of departure for miles to be added to the account. Mileage calculation International flights miles are calculated according to your flight s basic mileage multiplied by the booking class fixed accrual rate. The resulting mileage is then accrued to your account. International flight miles When travelling on code share flights operated by partner airlines, the actual mileage accrual rate may differ, or mileage may be ineligible for accrual. 13 Earn Miles with ANA International Flights Notes Only flight sectors actually flown by a member qualify for mileage accrual. The accrual rate for child fares is based on ticket class as for adult fares. If a seat is upgraded, including Upgrade Awards, and the difference in fares is not paid by the member, mileage will be accrued according to the original ticket booking class. Even in the case of upgrades that require payments, the number of miles awarded will be calculated according to the seat class of the originally purchased ticket. If a seat is downgraded, mileage is accrued according to the boarding class. All bonus miles are accrued only if the flight miles for the eligible flight have been registered with ANA Mileage Club. Code Share flights Mileage calculation When traveling on code-share flights operated by ANA Mileage Club partner airline aircraft and crew, but booked and boarded under an ANA flight number, excepting some cases mileage is accrued as a flight boarded on the partner airline operating the flight. Code share flights by ANA Mileage Club partner airlines that are not members of Star Alliance and other airlines may not be eligible for mileage accrual. When using codeshare flights operated by partner airlines, the ratio applied in order to calculate the number of miles awarded will be based on the booking class for partner airline flights rather than for ANA flights. Therefore, in some cases, the ratio for calculating the number of miles may differ. In certain cases, no miles may be earned. ANA has no mileage agreement with Shandong Airlines. Even if a passenger pays for an upgrade, the number of miles they receive will depend on the class of the originally purchased ticket. If a seat is downgraded, mileage is accrued according to the boarding class. All bonus miles are accrued only if the flight miles for the eligible flight have been registered with ANA Mileage Club. A single passenger occupying two or more seats to accommodate over-sized carry-on baggage such as musical instruments or paintings will only accrue mileage based on one seat. If there is no day corresponding to the same day of the month 2 months previously, then the period begins on the last day of that month. Example: Passengers who travel on April can apply from February 28 (or February 29 in a leap year) When making a return trip, or flying more than 2 sectors, reservations for all sectors can be made at the time the application period for the first sector begins as long as the subsequent flights are within 2 months and 14 days from the date of the first flight in the itinerary. Ticketing Period Tickets will be issued (and mileage deducted) as soon as the reservation is complete. Applications, reservations and issue of tickets is not possible at airports. Waitlisting There is no waitlisting system for domestic flight awards. Reservations must be confirmed for the entire itinerary. Issuing Open Tickets Open tickets without reservation cannot be issued. Changing the Itinerary after Application After application, the name of the passenger, the name of the individual supplying the miles, and sector cannot be changed. To prevent tickets being sent to the wrong or postal address, the e-ticket Itinerary & Receipt can only be sent to the address or postal address registered by the member. Making Changes to the Application Approval for Changes You can change your boarding day within the validity period of the ticket. Changes are possible until before the departure of your reserved flight and 24 hours prior to the departure of the flight you wish to board.the following changes cannot be made if you do not change or cancel your reservation by the departure of your reserved flight. Reservations cannot be made for departure dates more than 355 days in advance. If a changing the departure date results in a change of season, then adjustments must be made to meet the required number of miles. If the required number of miles increases, then the difference can be made up using further miles. If the mileage account does not contain the required number of extra miles, then the reservation cannot be changed. If the required number of miles decreases, then the difference is returned in miles. However, miles will not be returned if their expiry date has passed. Adjustments can only be made using miles, and not by cash or other means. When changing an unused flight ticket, in some cases additional costs will be incurred due to the difference in fuel surcharges and various taxes. If a difference occurs, you will be informed when you change your reservation. If there are unoccupied seats in a class lower than the reserved class, passengers may switch over to the lower class. However, in such cases, the miles difference between the higher and lower class will not be refunded. For sectors where the ticket was issued using the mileage required for a class higher than the class in which the person who is actually traveling, the passenger can upgrade to the higher class if there are unoccupied seats. Passengers can upgrade to a higher class within the time limit for changing their reservation. Please understand that timetables may change depending on the season, and flights may be cancelled. Disapprove for Changes After application, the name of the passenger, and the name of the individual supplying the miles cannot be changed. The airline, passenger, sector, and route cannot be changed. In certain cases where both ANA and Air Japan operate flights over the same sector, passengers may switch between the 2 airlines. Reserved partner flight awards cannot be changed to ANA international flight awards. Procedures for Making Changes Passengers wishing to change the day of departure or seat class should contact the ANA Mileage Club Service Center or the overseas ANA Mileage Club Service Center. Use of Flight Awards by Infants and Children Infants Infants aged less than 2 years using a flight award can occupy their own seat in exchange for the same number of miles as an adult. However, for newborns and infants who are not using a flight award, please purchase a separate infant fare ticket (the infant cannot occupy a seat) or as separate child fare ticket (the infant can occupy a seat) for the same class as the accompanying adult. Ages on the day of boarding of the first sector are targets. On domestic flights, ages for infants and children differ, as do the conditions for using awards. Children When Children aged above 2 but below 12 years travel using a flight awards, they require the same number of miles as an adult. Ages on the day of boarding of the first sector are targets. How to Apply for and Receive Awards Application ANA Website You cannot apply for some airlines on ANA Website. Note that there is no service charge for applications using ANA Website. This does not include additional fees for changing Partner Flight Awards. For service charges in other areas outside Japan, please visit ANA Website. How to Receive Awards Please receive the e-ticket & passenger receipt via , post or from an ANA branch. To prevent tickets being sent to the wrong or postal address, the e-passenger receipt can only be sent to the address or postal address registered by the member. The e-ticket Itinerary & Receipt cannot be received at the airport. Waitlisting Waitlisting is not possible for partner flight awards (including for ANA flights). Passengers must have reserved flight awards for all sectors of their itinerary. Issuing Open Tickets Open tickets cannot be issued for partner flight awards. Making Changes to the Application It is not possible to change the name of the passenger or the person providing the miles after application. After the ticket is issued, the boarding day can be changed within the ticket s validity period. Changes are accepted up to 96 hours before the departure of the flight you wish to board.the following changes cannot be made if you do not change or cancel your reservation by the departure of your reserved flight. The airline, passenger, sector, and route cannot be changed. If seats are available in routes for which tickets were issued using mileage for a class higher than the reserved class, it is possible to change to that higher class within the time limit for change procedures. If there are unoccupied seats in a class lower than the reserved class, passengers may switch over to the lower class. However, in such cases, the mileage difference between the higher and lower class will not be refunded. Using Miles 91 92

48 Flight Awards (Partner Airlines) Passengers may change the date of departure or seat class by contacting the ANA Mileage Club Service Center or the overseas ANA Mileage Club Service Center during business hours prior to the departure of the reserved flight. It is not possible to change to an open ticket or to apply for wait listing. Reservations cannot be accepted more than 355 days prior to the day of departure. When changing an unused flight ticket, in some cases additional payments may be required to cover the difference in fuel surcharge and various taxes. If additional payments are required, you will be informed when you change your reservation. Use of Flight Awards by Infants and Children Infants Infants aged less than 2 years using a flight award can occupy their own seat in exchange for the same mileage as adult. However, for infants who are not using a flight award, please purchase a separate infant fare ticket for the same class as an adult (the infant cannot occupy a seat) or a separate child fare ticket (the infant can occupy a seat). Please understand that timetables may change depending on the season, and flights may be cancelled. (In some cases, flights may not be available even though there are included in the flight award itineraries in the various charts). Flight awards do not result in accrued miles. Award tickets can not be used in conjunction with upgrade awards. Passengers using flight awards will not be provided with alternative modes of transport or accommodation in the event of delayed or cancelled flights. Miles exchanged for flight awards cannot be exchanged for other flight awards or paid back into the mileage accounts even if they have not yet been received or remain unused, except in the circumstances described under "Refunding Unused Awards". Even if your itinerary includes ANA flights, the "Mileage Chart" on P.83 - will apply if the itinerary includes even one flight partner airline. Use of Upgrade Awards by Infants and Children Use of upgrade awards by Infants Infants who have purchased a confirmed child fare ticket or discount ticket for the applicable sector may upgrade are using the same required number of miles as an adult upgrade. Situations Where Infants do not use Awards Please purchase a separate infant fare ticket (the infant cannot occupy a seat) or as separate child fare ticket (the infant can occupy a seat) for the class that will be used by the accompanying adult following their upgrade. Use of Upgrade Awards by Children Children (aged 2 and above but below 12 at the time of boarding the first flight covered by the ticket) require the same mileage as adults when using upgrade awards. Passengers Taxes The taxes listed below vary depending on seat class. When using an award to upgrade from Economy Class to Business Class, the difference in the taxable amount must be paid at the airport. 72 ANA Premium Member Services and Benefits Notes Japan domestic code share flights operated by IBEX Airlines, AIR DO, Solaseed Air, Star Flyer or Oriental Air Bridge are eligible for Premium Points only if the flight is booked and boarded using an ANA flight number. Premium points are calculated by multiplying the basic mileage for the sector by a ratio that depends on the seat class. Various bonus miles from special offers do not count towards Premium points.
