2 WILDERNESS & TRAVEL MEDICINE TRIP GRADE: DEMANDING HIGH ALTITUDE TREKKING This trek requires that participants are in excellent physical condition and participate in a regular cardiovascular exercise program prior to departure from the USA as we will be hiking 4-8 hours on a daily basis. Yaks and Porters will carry all of your gear each day. You need only carry a day pack while hiking. Though not the monsoon season, mountain weather is unpredictable subject to snowfall, storms, ice, rain, wind, and other high mountain hazards. Flexibility and a sense of humor are mandatory. Release of liability and a medical certificate, signed by a physician will be required from all participants. Days 1 & 2 Depart North America: Note: Flying over multiple time zones you lose time as you head east. International Flights: Please let Destination Himalaya know if you require assistance in booking your flights. Trans-Pacific connections are via Hong Kong or Bangkok. Trans-Atlantic via Abu Dhabi or Delhi. Depending on your choice of international carrier you may be required to depart the USA a day earlier. Day 3 Arrive Kathmandu, customs, immigration and meet your Nepalese Guide who will escort you to the Yak & Yeti Hotel. Afternoon orientation, guided walking tour including Dubar Square, Hanuman Dhoka, the ancient palace of the Gorkha Kings and the old bazaar, ending with a welcome dinner, Yak & Yeti Hotel (B,L,D) Day 11 Day 4 Day 5 Day 6 Day 7 Fly to Lukla and Start Trek (9,380 ft.), Mount Kailash Lodge (B,L,D) Trek to Namche Bazaar (11,300 ft.), Khumbu Lodge (B,L,D) A long and challenging day with many ups and downs, towards the end of the day we get our first views of snow-capped summits of Lhotse and Mt. Everest. We ll walk about the bazaar and explore the heart of the Sherpa community. Trek to Kumjung, Amadablam Lodge (B,L,D) An easy day to enjoy a leisurely breakfast and a two hour hike to the village of Kumjung. Trek to Thyangboche (12,700 ft.) Thyangboche Guest House (B,L,D) Hiking today brings us to the monastery of Thyangbouche where we stay in the adjacent Guesthouse. Days 8, 9 & 10 Trek to Lobuche, en route to Everest Base Camp, Lodge (B,L,D) Hiking through the high country surrounded by Everest and all of its satellite peaks. Day 12 Days 13 & 14 Day 15 Day 16 Day 17 Day 18 Day 19 Kala Pathar Excursion (18,200 ft.), Lodge (B,L,D) Relaxed morning and early lunch we start our slow but steady climb reaching the black stone peak of the mountain. Being directly opposite Everest, Kala Pathar offers some of the best panoramic views, the most famous photographs of Everest are taken from this position. Everest Base Camp Excursion (17,500 ft.), Lodge (B,L,D) An early start for a few hours at Base Camp, visiting Everest ER Base Camp Medical Clinic, then returning to Lobuche for overnight stay. Return Trek to Namche (11,300 ft.), Khumbu Lodge (B,L,D) Today we start our return trek to Lukla Airport. First stop is the monastery of Thyangbouche and then to Namche with enough time to do some shopping in the bazaar and a hot shower. Trek to Lukla, farewell dinner with trekking staff, Northface Lodge (B,L,D) Fly to Kathmandu, Yak & Yeti Hotel (B,L,D) Kathmandu, Yak & Yeti Hotel (B,L,D) extra day in case of need. Rose, MD, FACEP Professor Department of Emergency Medicine University of California, Davis Sacramento, California GUEST FACULTY David Breashears Filmmaker, Adventurer, Mountaineer Boston, Massachusetts David Breashears is a world-class filmmaker, adventurer, and mountaineer whose work has taken him to remote locations throughout Tibet, China, Nepal, India, Pakistan, South America, and East Africa. He has worked on such feature films as Seven Years in Tibet and Cliffhanger, as well as the award winning documentary Red Flag over Tibet. In 1983 he transmitted the first live pictures from the summit of Mount Everest and in 1985 became the first American to twice reach its summit. He is the recipient of four Emmy awards for achievement in cinematography. In 1996 he co-directed, photographed, and co-produced the acclaimed IMAX large format film Everest and contributed his still photos from that climb to the best selling book Everest: Mountain Without Mercy. In 1997 he coproduced and photographed Everest: The Death Zone for the PBS science series NOVA, marking his fourth ascent of the world s highest mountain. In 1996 he co-directed, photographed, and co-produced the acclaimed IMAX film Everest and his latest documentary Storm over Everest was released on May 13, David has been traveling to Tibet for 25 years. He will give ongoing talks during the trip on Everest expeditions and lore, including the infamous Mallory/Irvine attempt of 1924, Hillary s successful first summit and the 1996 disaster, with which he is intimately acquainted. He will also educate us on the climatic changes and ecological issues confronting the Everest region, as well as, on the ongoing conservation efforts, of which he is a pioneer. 4 IMPORTANT INFORMATION IMMUNIZATIONS AND HEALTH MATTERS No immunizations are required for entry into Nepal or to return to the USA after visiting Nepal. Several immunization shots are recommended by the Centers for Disease Control and Prevention for travel to Asia. The list will be included in your pre-departure material. TRIP PREPARATION INFORMATION The following material will be sent to all participants upon registration by Destination Himilaya. Immediately after signing up with Destination Himalaya: You will be sent a confirmation letter, medical questionnaire, insurance policy, customized Pre-departure Book containing information on the region, health matters, immunization requirements, Visa instructions, clothing list, baggage, country overview, suggested reading and maps. Three months before departure: Air itinerary (if DH is assisting you with International Air), invoice and a reminder for final payment. One month before departure: Last-minute information on the trip, final itinerary, emergency contact information, air tickets (if DH is issuing your tickets) and rendezvous instructions. Because the nature of adventure travel, it may be necessary to make changes due to weather, road conditions, government restrictions and other variable beyond their control. 5 WILDERNESS & TRAVEL MEDICINE TARGET AUDIENCE This activity is designed for physicians, physician assistants, nurses and nurse practitioners and other health-care providers who seek hands-on training in wilderness and travel medicine. Trek is limited to 35 participants. EDUCATIONAL PURPOSE This course will provide a hands-on trail experience in diagnosing and treating medical conditions and understanding medical care options in remote settings. COURSE OBJECTIVES After attending this activity, participants should be able to: 1. Describe the skills needed for pre-trip planning to remote settings 2. Recognize environmental, physical and biological hazards when traveling to remote settings 3. Properly diagnose and treat commonly expected medical conditions that arise in remote settings 4. Know how to apply current high tech resource and equipment use in remote settings 5. Compare and contrast medical care options in remote settings. 6. Explain the effect of cultural differences in medical care when traveling to remote settings COURSE CONTENT CME will be scheduled for approximately 1.5 hours/day during the trek, primarily in the evening, but may be adjusted based on the daily itinerary. Content will be a combination of didactic lectures and hands-on skills training based on each day s subject. 18 AMA PRA Category 1 Credits Additional to Land Cost is a $300. charge for CME Credit.
