(319) Fax (319) Press Releases NOVEMBER 2014 On Saturday, November 01, 2014, Buchanan County Sheriff's Deputies arrested Rush Robert Cleveland, 30, of Quasqueton. Cleveland was arrested and charged with Disorderly Conduct (simple misdemeanor). This arrest was made following a disturbance in the 300 blk South Third Street in Quasqueton. On Sunday, November 02, 2014, Buchanan County Sheriff's Deputies arrested Tristan Blake Gore, 27, of Waterloo. Gore was arrested and charged with first offense Operating While Intoxicated (serious misdemeanor). Gore was also issued a citation for a Speed Violation. This arrest was made following a traffic stop in the 1500 blk Jackson Avenue (Hwy 150) south of Hazleton. On Sunday, November 02, 2014, Buchanan County Sheriff's Deputies arrested Donald James Dilger, 53, of Manchester. Dilger was arrested and charged with second offense Operating While Intoxicated (aggravated misdemeanor). This arrest was made following a traffic stop on Hwy 20 near the 260 mile marker. On Sunday, November 02, 2014, Buchanan County Sheriff's Deputies arrested Jules James Alexie III, 30, of Elkader. Alexie was arrested on an active arrest warrant out of Buchanan County for two counts of Probation Violation. The Clayton County Sheriff s Office assisted in this arrest. On Sunday, November 02, 2014, Buchanan County Sheriff's Deputies arrested Salvatore Matteo Mancini, 17, of Independence. Mancini was arrested and charged with Assault causing Bodily Injury (serious misdemeanor). This arrest was made following an incident in the 2200 blk Iowa Avenue west of Independence. On Monday, November 03, 2014, Buchanan County Sheriff's Deputies arrested Robert Lee Martinez, 28, of Aurora. Martinez was arrested and charged with Driving While License Suspended (simple misdemeanor). Martinez was also issued a citation for Operation without Registration. This arrest was made following a traffic stop in the 400 blk Fifth Street N in Winthrop. On Tuesday, November 04, 2014, Buchanan County Sheriff s Deputies arrested Eric Dwayne Hardin, 50, of Quasqueton. Hardin was arrested and charged with Driving While License Barred (aggravated misdemeanor), Driving While License Suspended (simple misdemeanor), No SR22 Insurance (simple misdemeanor), third offense Possession of Controlled Substance Marijuana (class D felony), third offense Possession of Controlled Substance methamphetamine (class D felony) and Possession of A criminal charge is merely an accusation and the defendant is presumed innocent until and unless proven guilty in a court of law.

2 Drug Paraphernalia (simple misdemeanor). Hardin was also issued a citation for Failure to Carry Proof of Insurance. A passenger, Nancy Ann Baker, 48, of Quasqueton, was issued a citation for Allowing an Unauthorized Person to Drive. This arrest was made following a Deputies observation of Hardin pulling into his driveway in the 300 blk S Third Street in Quasqueton. On Wednesday, November 05, 2014, Buchanan County Sheriff s Deputies arrested Kelsey Renee Hartsoe, 22, of Independence. Hartsoe was arrested and charged with Possession of Drug Paraphernalia (simple misdemeanor). This arrest was made following an incident in the 1800 blk First Street West in Independence. This incident remains under investigation and further charges are pending. On Wednesday, November 05, 2014, Buchanan County Sheriff s Deputies arrested David John Perry, 52, of Hazleton. Perry was arrested and charged with Assault Causing Injury (serious misdemeanor) and fifth degree Criminal Mischief (simple misdemeanor). Perry was also issued citations for Driving on a Flat Tire and No Proof of Insurance. This arrest was made following an incident that occurred in the Littleton area and concluded in the 1800 blk 140 th Street south of Hazleton. On Thursday, November 06, 2014, Buchanan County Sheriff s Deputies arrested William John Gibson, 63, of Hazleton. Gibson was arrested and charged with third offense Possession of Controlled Substance marijuana (class D felony) and Possession of Drug Paraphernalia (simple misdemeanor). Gibson was also cited for Failure to Maintain Control. This arrested was made following a report of a motor vehicle accident in the 2000 blk Jamestown Avenue (IA Hwy 150) north of Independence. On Saturday, November 08, 2014, Buchanan County Sheriff s Deputies arrested Russell Earl Bailey, Jr., 44, of Oelwein. Bailey was arrested and charged with Possession of Substance marijuana (serious misdemeanor), Possession of Drug Paraphernalia (simple misdemeanor) and No SR22 Insurance (simple misdemeanor). This arrest was made following a traffic stop in the 1600 blk Nathan Bethel Avenue northeast of Independence. On Saturday, November 08, 2014, Buchanan County Sheriff s Deputies arrested Christopher Ryan Williams, 28, of Dunkerton. Williams was arrested and charged with Driving While License Suspended (simple misdemeanor). This arrest was made following a traffic stop in the 2700 blk Racine Avenue north of Quasqueton. On Sunday, November 09, 2014, Buchanan County Sheriff s Deputies arrested Shawnie Lightning Lavoie, 21, of Lamont. Lavoie was arrested and charged with Assault (simple misdemeanor). This arrest was made following an incident that occurred near Church Street and Washington Street in Lamont on 10/26/2014. On Sunday, November 09, 2014, Buchanan County Sheriff s Deputies arrested Mike Earl Miller, 40, of Jesup. Miller was arrested and charged with Driving While License Revoked (simple misdemeanor) and Driving While License Suspended (simple misdemeanor). Miller was also issued a citation for No Proof of Insurance. This arrest was made following a traffic stop in the 400 blk State Street in Littleton. On Monday, November 10, 2014, Buchanan County Sheriff s Deputies arrested Jerred Jeffrey Brockmeyer, 25, of Independence and Austin Lee Fortner, 21, of Rowley. Brockmeyer was arrested Page 2

3 and charged with first offense Possession of Controlled Substance Marijuana (serious misdemeanor) and Possession of Drug Paraphernalia (simple misdemeanor). Fortner was arrested and charged with second offense Possession of Controlled Substance Marijuana (aggravated misdemeanor) and Possession of Drug Paraphernalia (simple misdemeanor). These arrests were made in the 100 blk Rowley Street in Rowley. On Monday, November 10, 2014, Buchanan County Sheriff s Deputies arrested Carrie Nichole Boyce, 17, of Quasqueton. Boyce was arrested and charged with Possession of Drug Paraphernalia (simple misdemeanor) and Possession of Tobacco Under Legal Age (simple misdemeanor). Boyce was later released and will be referred to Juvenile Court Services in this matter. On Wednesday, November 12, 2014, Buchanan County Sheriff s Deputies arrested Seth Francis Dana, 30, of Jesup. Dana was arrested and charged with second degree Burglary (class C felony), fourth degree Theft (serious misdemeanor) Driving While License Barred (aggravated misdemeanor) and Driving While License Suspended (simple misdemeanor). These charges stem from an incident in the 2500 blk Daniel Avenue southeast of Jesup. On Wednesday, November 12, 2014, Buchanan County Sheriff s Deputies arrested Matthew Christian Fish, 29, of Rowley. Fish was arrested and charged with Driving While License Suspended (simple misdemeanor) and Failure to File SR22 Insurance (simple misdemeanor). Fish was also issued a citation for Unlawful Use of Driver s License. This arrest was made following a traffic stop on Hwy 20 near Hwy 150 south of Independence. On Thursday, November 13, 2014, Buchanan County Sheriff s Deputies arrested Sonja R Caldwell, 53, of Waterloo. Caldwell was arrested and charged with second offense Operating While Intoxicated (aggravated misdemeanor). This arrest was made following the receipt of lab tests stemming from a traffic stop that occurred on 09/20/2014. On Thursday, November 13, 2014, Buchanan County Sheriff s Deputies arrested Derrick Deontae Fulton, 24, of Kalona. Fulton was arrested and charged with Domestic Abuse Assault Causing Injury (serious misdemeanor). This arrest was made following an incident in the 2700 blk Nolen Avenue west of Quasqueton. On Saturday, November 15, 2014 at approximately 2:55 pm, Buchanan County Sheriff s Deputies were dispatched to a report of a motor vehicle accident with injuries on I380 near the 54 mile marker. According to the investigation, a 1999 Cadillac Escalade operated by Chrishunda Sisk of Cedar Falls was traveling north on I380 when she lost control of her vehicle. The vehicle entered and crossed the median into the southbound lanes and was struck head-on by a 2003 Ford Explorer operated by David Tink of Waterloo. Tink and a passenger were transported to Covenant Medical Center in Waterloo by Wheaten Franciscan Paramedics with what were considered non-life threatening injuries. Two other passengers in the Tink vehicle were uninjured. Sisk and a young child were transported to Covenant Medical Center by North Benton Ambulance with what were considered non-life threatening injuries. Road conditions were a factor in this incident. The Buchanan County Sheriff s Office was assisted at the scene by the Iowa State Patrol, LaPorte City Police Department, LaPorte City Fire and Ambulance, North Benton Ambulance and the Brandon Fire Department. On Saturday, November 15, 2014 at approximately 2:55 pm, Buchanan County Sheriff s Deputies were Page 3

4 dispatched to a report of a motor vehicle accident on I380 near the 49 mile marker. According to the investigation, a 2007 Chevrolet Trailblazer operated by Patricia Newcomb of Shannon, MS, was traveling northbound on I380 when she lost control of her vehicle. The vehicle entered and crossed the median into the southbound lanes where it was struck by a 2013 Freightliner semi operated by James Klingborg of Cherokee, IA. After the Klingborg semi struck the Newcomb vehicle, a second semi, a 2010 Freightliner semi, operated by Tonja Barnes of Waterloo, IA, also struck the Newcomb vehicle. No injuries were reported and damage estimates were $5,000 to the Newcomb vehicle, $10,000 to the Klingborg vehicle and $500 to the Barnes vehicle. Road conditions were a factor in this incident. On Saturday, November 15, 2014 at approximately 3:55 pm, while Buchanan County Sheriff s Deputies were investigating a motor vehicle accident on I380 near the 54 mile marker, a 2009 GMC Sierra operated by Malachi Wunderlich of Good Thunder, MN, was traveling northbound on I380 when he lost control of his vehicle. The vehicle entered the median and rolled onto its passenger side. Emergency personnel already on scene for the previous accident assisted Wunderlich out of his vehicle and to safety. No injuries were reported and damage was estimated at $1,500. Road conditions were a factor in this incident. The Buchanan County Sheriff s Office was assisted at the scene by the Iowa State Patrol, LaPorte City Police Department, LaPorte City Fire and Ambulance, North Benton Ambulance and the Brandon Fire Department. On Saturday, November 15, 2014 at approximately 9:30 pm, Buchanan County Sheriff s Deputies were dispatched to a report of a motor vehicle accident on 120 th Street near Central Avenue southeast of Fairbank. According to the investigation, a 2010 Chevrolet Malibu operated by Mitch Penhollow of Hazleton was traveling west on 120 th Street. A 2001 Chrysler 300 operated by Mary Hough of Hazleton was traveling east on 120 th Street when she lost control of her vehicle. The Hough vehicle spun in front of the Penhollow vehicle and both vehicles collided in the roadway. Hough was removed from her vehicle by mechanical means and transported to Allen Hospital in Waterloo by Fairbank Ambulance with serious injuries. Penhollow was able to exit his vehicle and was transported to Mercy Hospital in Oelwein by Oelwein Mercy Ambulance with minor injuries. Both vehicles were considered a total loss. The Buchanan County Sheriff s Office was assisted at the scene by the Fairbank Police Department, Fairbank Fire and Ambulance and Oelwein Mercy Ambulance. On Tuesday, November 18, 2014, Buchanan County Sheriff s Deputies arrested Justin Lee Covault, 18, of Independence. Covault was arrested and charged with fifth degree Criminal Mischief (simple misdemeanor) and Reckless Driving (simple misdemeanor). This arrest was made following an incident that caused property damage on the MHI Campus off of the 1800 blk 232 nd Street west of Independence. On Wednesday, November 19, 2014, Buchanan County Sheriff s Deputies arrested Harley Dwayne Shuck, 31, of Oelwein. Shuck was arrested and charged with Manufacturing a Controlled Substance Methamphetamine (class C felony). This arrest stems from an incident that occurred in the 300 blk Enterprise Drive in Independence on 08/22/2014. On Wednesday, November 19, 2014, Buchanan County Sheriff s Deputies arrested Mason Mitchell Teel, 19, of Walker. Teel was arrested and charged with Driving While License Suspended (simple misdemeanor) and Failure to File SR22 Insurance (simple misdemeanor). Teel was also issued a citation for Unauthorized Use of Driver License. This arrest was made following a traffic stop in the 3300 blk Jamestown Avenue (Hwy 150) east of Brandon. Page 4

5 On Wednesday, November 19, 2014, Buchanan County Sheriff s Deputies arrested Alexander Michael Joseph Carlson, 19, of Independence. Carlson was arrested and charged with second degree Criminal Mischief (class D felony). Carlson was also arrested on two active arrest warrants out of Buchanan County for Failure to Appear. This arrest was made in the 1800 blk First Street W in Independence. On Thursday, November 20, 2014, Buchanan County Sheriff s Deputies arrested William James Gibson, 64, of Hazleton. Gibson was arrested and charged with first offense Operating While Intoxicated (serious misdemeanor). Gibson was also arrested on an active arrest warrant out of Buchanan County for Probation Violation. This arrest was made in the 200 blk Fifth Avenue NE in Independence. This arrest stems from an incident that occurred in the 2000 blk Jamestown Avenue (IA Hwy 150) north of Independence on 11/06/2014. On Thursday, November 20, 2014 at approximately 7:00 pm, Buchanan County Sheriff s Deputies were dispatched to a motor vehicle accident near 285 th Street and Vincent Avenue west of Monti. According to the investigation, a 2005 Dodge Caravan operated by Steve Dale Hiepler, 53, of Coggon, was traveling south on Vincent Avenue when he failed to negotiate a curve and entered the ditch. The vehicle rolled over onto its passenger side. Hiepler and a passenger were uninjured in the incident. Hiepler was arrested and charged with first offense Operating While Intoxicated (serious misdemeanor). Hiepler was also issued citations for Failure to Maintain Control and Failure to Carry Proof of Insurance. The Buchanan County Sheriff s Office was assisted at the scene by Quasqueton Emergency Services and Area Ambulance Service. On Friday, November 21, 2014, Buchanan County Sheriff s Deputies arrested Russell Earl Bailey, Jr., 44, of Oelwein. Bailey was arrest and charged with second offence Operating While Intoxicated (aggravated misdemeanor). This arrest stems from an incident that occurred in the 1600 blk Nathan Bethel Avenue on 11/08/2014. On Saturday, November 22, 2014, Buchanan County Sheriff s Deputies arrested Earnest Laverne Holladay, Jr., 52, of Brandon. Holladay was arrested on an active arrest warrant out of Buchanan County for Probation Violation. This arrest was made in the 600 blk South Street in Brandon. On Saturday, November 22, 2014, Buchanan County Sheriff s Deputies arrested John Alfred Weisbeck, 19, of Cedar Rapids. Weisbeck was arrested and charged with first offense Operating While Intoxicated (serious misdemeanor). Weisbeck was also issued a citation for Failure to Maintain Control. This arrest was made following a motor vehicle accent that occurred on I380 near the 51 mile marker on 09/13/2014. On Saturday, November 22, 2014 at approximately 4:45 pm, the Buchanan County Sheriff s Office received a report of an accident in the 3300 blk Troy Mills Blvd north of Troy Mills. According to the investigation, a 1984 John Deere 4440 operated by Matthew Schlegel of Cedar Rapids was pulling a New Holland 680 manure spreader northbound on Troy Mills Blvd. Schlegel attempted to move over for oncoming traffic and lost control of the tractor, causing the tractor and spreader to jackknife. The right side tire of the tractor dislodged and broke loose. No injuries were reported. The tire was eventually put back on and the tractor was removed from the roadway. Damage was estimated at $200. Page 5

6 On Sunday, November 23, 2014, Buchanan County Sheriff s Deputies arrested Henry O. Miller, 21, of Independence. Miller was arrested and charged with three counts of Contributing to Delinquency of Minors (simple misdemeanor) and one count of Supplying Alcohol to Minors (simple misdemeanor). This arrest was made following a report of an underage drinking party in the 1600 blk Fairbank-Amish Blvd. On Tuesday, November 25, 2014, Buchanan County Sheriff s Deputies arrested Michael Charles Hall II, 23, of Quasqueton. Hall was arrested and charged with Domestic Abuse Assault (class D felony) and was also arrested on an active arrest warrant out of Buchanan County for Probation Violation. This arrest was made following a disturbance in the 300 blk S Third Street in Quasqueton. On Tuesday, November 25, 2014, Buchanan County Sheriff s Deputies arrested Tracy Michelle Clinton, 41, of Winthrop. Clinton was arrested on an active arrest warrant out of Buchanan County for Failure to Appear. This arrest was made in the 300 blk Seventh Street South in Winthrop. On Tuesday, November 25, 2014 at approximately 3:30pm, Buchanan County Sheriff s Deputies responded to a motor vehicle accident in the 1300 blk 145th Street south of Fairbank. According to the investigation, a 1995 Jeep Cherokee operated by Jane Berkes of Hawkeye was facing west and stopped along the side of the road on 145th Street delivering mail. A 2006 Dodge Dakota operated by Dustin Fratzke of Hazleton was traveling westbound approaching the Berkes vehicle. When Fratzke attempted to slow down and move over for the Berkes vehicle, the Fratzke vehicle lost traction on ice and slid into the left rear of the Berkes vehicle. No injuries were reported. Damage estimates were $1,500 to the Berkes vehicle and $2,500 to the Fratzke vehicle. On Wednesday, November 26, 2014, Buchanan County Sheriff s Deputies arrested Jason Lee Webster, 35, of Quasqueton. Webster was arrested and charged with third degree Arson (aggravated misdemeanor), third degree Harassment (simple misdemeanor) and Criminal Trespass (simple misdemeanor). This arrest was made in the 2600 blk Quasqueton Diagonal Blvd. On Wednesday, November 26, 2014 at approximately 11:15pm, Buchanan County Sheriff s Deputies responded to a motor vehicle accident in the 1200 blk 120th Street south of Fairbank. According to the investigation, a 2002 Dodge Stratus operated by Angela Pahl of Tripoli was traveling east on 120th Street when she lost control of her vehicle. The vehicle entered the south ditch and rolled over. No injuries were reported and damage was estimated at $6,000. On Friday, November 28, 2014, Buchanan County Sheriff s Deputies arrested Jeffrey Allen Keppler, 45, of Aurora, and Angela Sue Barajas, 42, of Hazleton. Keppler was arrested and charged with Driving While License Suspended (simple misdemeanor) and Failure to Have SR22 Insurance (simple misdemeanor). Keppler was also issued citations for Operating an Unregistered Vehicle, Fraudulent Use of Registration, Unlawful Possession of Driver License and No Proof of Insurance. Barajas was arrested and charged with Possession of Drug Paraphernalia (simple misdemeanor), Interference with Official Acts (simple misdemeanor) and Harassment of a Public Official (simple misdemeanor). These arrests were made following a traffic stop in the 2400 blk 120 th Street east of Hazleton. On Saturday, November 29, 2014, Buchanan County Sheriff s Deputies arrested Austin Lewis Cottrell, 18, of Independence. Cottrell was arrested and charged with third degree Attempted Burglary Page 6

7 (aggravated misdemeanor). This arrest was made following an incident in the 100 blk N Main Street in Hazleton. On Saturday, November 29, 2014, Buchanan County Sheriff s Deputies arrested Jason Michael Jackson, 38, of Independence. Jackson was arrested and charged with Driving While License Revoked (serious misdemeanor) and Failure to Have SR22 Insurance (simple misdemeanor). Jackson was also issued a citation for Failure to Carry Proof of Insurance. This arrest was made following a traffic stop in the 100 blk Third Street NW in Independence. On Saturday, November 29, 2014, Buchanan County Sheriff s Deputies arrested Christopher Alan O Loughlin, 52, of Independence. O Loughlin was arrested and charged with Driving While License Revoked (serious misdemeanor). This arrest was made following a traffic stop in the 200 blk Third Avenue NE in Independence. On Saturday, November 29, 2014, Buchanan County Sheriff s Deputies arrested Gwendolyn Francine Fountain, 36, of Dubuque. Fountain was arrested and charged with Possession of Controlled Substance Marijuana (serious misdemeanor), Possession of Drug Paraphernalia (simple misdemeanor) and Failure to Maintain Control. This arrest was made following the conclusion of an investigation into a motor vehicle accident with injuries that occurred on Hwy 20 at the 245 mile marker on 06/15/2014. Page 7
