CARRASCO 12/09/ :03 AM Specific Report Synopsis : UIWPD was dispatched to Mc Combs 4th floor in regards to what smelled like cigarette smoke coming from a dorm room. Smoke source was located and the individual was advised of the no smoking rules on campus and in buildings. WORD : HILLSIDE 3 Specific FIFTH FLOOR DISTURBANCE D. LOPEZ Report Synopsis : UIWPD was dispatched to the fifth floor of Skyview for a report of odor of marijuana coming from one of the rooms. THOMAS 12/04/ :05 AM Specific UIWPD Report Synopsis : UIWPD was dispatched to the University of the Incarnate Word Police Department for a UIW Faculty member requesting a case report in reference to a UIW laptop stolen from his vehicle on the 14 November 2018 at Colonial Squares Apartment off campus. LOPEZ Report Synopsis : UIWPD was flagged down by UIW student for an individual off campus telling him he needed help someone was chasing him. SAPD was contacted SAPD transported individual to a requested location. OFF CAMPUS LOCATION Specific LA MADELEINE RESTAURANT (4820 BROADWAY ST, SAN ANTONIO, TX 78209) ASSIST OTHER AGENCY P. SCOTT Report Synopsis : UIWPD received a call from the Alamo Heights Police Department regarding a University of Incarnate Word student who was urinating in front of La Madeline Restaurant. Northside ISD bus scraped an unoccupied vehicle as it was backing out. WORD : HILLSIDE 3 : GARAGE LOWER LEVEL Specific SKYVIEW ENTRANCE TO LOWER GARAGE CRASH REPORT D. LOPEZ Report Synopsis : UIW PD was flagged down in front of the Skyview dorms for a report of an accident that occurred off campus. Both parties involved were UIW students. One driver failed to yield at the sign going onto the 281 N access road and hit the other driver on the rear passenger side. No injuries were sustained. WORD : Nursing Building West Lot Specific CRASH REPORT R.LADSON 12/06/ :27 PM 12/12/2018 5:05 PM Report Synopsis : UIWPD was dispatched to the west side of the nursing building for a minor vehicle accident between a non UIW member and a UIW shuttle bus. Both vehicles were still drivable and EMS was not needed. WORD : ADMINISTRATION/COLBERT HALL Specific PAID PARKING SPOTS CRASH REPORT D. STODDARD Report Synopsis : UIWPD was dispatched to the paid parking spots in front of the Administration building for a crash report between a UIW Student and a UIW Students parked car. Report Synopsis : UIW Police was dispatched to the Upper Hillside gate entrance about a vehicle colliding into a tree. EMS was not needed. Vehicle was not towed and was drivable. INCARNATE WORD HIGH SCHOOL Specific MT. ERIN CIRCLE LOT DAMAGED PROPERTY C. SMALL Report Synopsis : UIWPD called for service to Incarnate Word High School regarding a damaged to property report. A faculty member's car front driver side window was busted out by an unknown object. No injuries were sustained in the matter. FEIK SCHOOL OF PHARMACY : Pharmacy : Sub-Level 1 Specific G1 EXIT DAMAGED PROPERTY 12/04/2018 7:30 AM 12/03/2018 1:45 PM Report Synopsis : UIWPD dispatched to UIWPD for damaged property report. OFF CAMPUS LOCATION Specific UIW VEHICLE SERVICES DAMAGED PROPERTY L. THOMAS 12/10/2018 1:00 PM Report Synopsis : UIWPD was dispatched to 3301 Broadway San Antonio Tx University of the Incarnate Word Vehicle Services for Damaged Property to the right side back door window one of the white Ford E-350 passenger van #55. EMS was not needed and no one was injured. WORD : GORMAN BUILDING : 1ST FLOOR Specific ROOM DAMAGED PROPERTY Report Synopsis : UIW Police was dispatched to the Gorman BLDG about a UIW student that had injuries to his hands from a battery that exploded. LOPEZ 12/12/ :37 PM Specific OUTSIDE SCREEN FACING NORTH LOT Report Synopsis : UIW PD was dispatched to the kiosk because a librarian had reported a loose screen on the back of the Dubois building facing Brack Villa. When I arrived the librarian was gone, I did check the screen and saw that it was not secure properly. I secured the screen % # of Reports: 6 DAMAGED PROPERTY WORD : Clement Hall : Floor /06/ :00 PM Specific Report Synopsis : UIW Police was dispatched to Clement Hall about a UIW Student who wanted to generate an information report regarding her mother being concerned about a check that may have been fraudulent and that was deposited into her (joint account) account. WORD : STUDENT ENGAGEMENT CTR : MAIL ROOM Specific LUIS GONZALEZ 12/12/2018 9:53 AM Report Synopsis : UIWPD was dispatched to headquarters for a UIW student reporting a possible theft of her property from the UIW post office. After further investigation the property was found at the post office. ST. ANTHONYS HIGH SCHOOL : WEST LOT Specific WEST PARKING LOT NEAR THE MAIN ENTRANCE L. THOMAS Report Synopsis : UIWPD was called out to St. Anthony High School for an unknown individual sleeping in the west parking lot at 3200 McCollugh. WORD : AGNESE/SOSA : Level 1 Specific LUIS GONZALEZ 12/22/2018 6:05 PM Report Synopsis : UIWPD was dispatched to 1st floor Agnese Parking Garage in reference to a unknown individual wanting to report fraudulent behavior about her truck being sold. The unknown individual left the property before being questioned for further information. WORD : Hillside 1 Dorm : Floor 4 Specific ROOM Report Synopsis : UIW Police was dispatched to Hill Side One about a UIW Student that was behaving Bizarre in the ICC Building. SAFD was dispatched and evaluated the UIW Student and a Non UIW Student. Both were Medically released. WORD : Hillside 1 - Outside Area : MOTORIST ASSIST LUIS GONZALEZ 12/11/2018 5:23 PM Specific Report Synopsis : UIWPD was dispatched to Hillside 1 for a UIW student in need of a jumpstart for her vehicle. WORD : Admin SE Lot Specific : MOTORIST ASSIST P. SCOTT 12/12/2018 1:00 AM Report Synopsis : I was waved down by UIW affiliate in the administration parking lot and advised he needed a jumpstart. Waiver was filled out and jump start was successful. WORD : HILLSIDE 3 : GARAGE UPPER LEVEL Specific : MOTORIST ASSIST D. LOPEZ 12/12/ :03 PM Report Synopsis : UIW PD was dispatched to the second floor of Sky View garage for a report of a UIW student needing a battery boost. Boost successful! The building was cleared and no students were injured. Fire was not call out. The alarm was silenced and acknowledged. LOPEZ 12/05/ :29 AM Specific Report Synopsis : UIW PD was dispatched to the fourth floor of St. Josephs for a report of a 911 hang up call to San Antonio Police Department. The knife was logged into evidence. Non-UIW affiliated patron slipped and fell while attending event over weekend. Event took place at the Rosenberg Sky Room WORD : GORMAN BUILDING : 1ST FLOOR Specific ROOM INJURED PERSON Report Synopsis : UIW Police was dispatched to the Gorman BLDG about a UIW student that had injuries to his hands from a battery that exploded. 15 Specific ON THE SIDE WALK IN FRONT OF SKY VIEW BUILDING Report Synopsis : UIWPD dispatched to the shuttle bus stop area by sky view building in reference to a IWHS student injuring her ankle. EMS was not needed. The student was released to the custody of her uncle to transported to a medical facility. WORD : Natatorium area - South Lot Specific INJURED PERSON L. THOMAS 12/16/ :25 AM Report Synopsis : UIWPD was dispatched to University of the Incarnate Word Natatorium south parking lot for and injured person report. The non UIW student fell off the steps and sustained a cut over her left eye on the southwest side of the building while trying to walk her dog. EMS was refused at that time and was transported to the emergency room by her husband. WORD : Benson Field - North Specific BENSON STADIUM HOME BLEACHERS INJURED PERSON Report Synopsis : UIWPD dispatched to Natatorium for an injured person report. UIW Student cut finger while working, refused EMS. 12/17/ :50 PM 9.09% # of Reports: 5 INJURED PERSON WORD : Ancira Towers C. TAMEZ 12/03/ :00 AM Specific NONE Report Synopsis : UIW dispatched to Ancira Parking Garage for a car horn sounding repeatedly. Nothing found at Ancira, but the (old style alarm) sounding again at the rear of ICC / St. Joe's Hall. Alarm silenced, but not reset dispatched notified Risk & Safety of alarm. The conducted a search of the rooms on the first, seconded, third, floor rooms. There was know visible smoke of fire. The fire panel was silenced and fire and safety was notified. WORD : GORMAN BUILDING : 1ST FLOOR Specific /17/2018 2:55 PM Report Synopsis : UIWPD was dispatched to the Gorman BLDG about a trouble signal coming from the Fire Panel. Fire Panel displayed All Systems Normal. WORD : Joeris Hall Area L. THOMAS 12/25/ :40 AM Specific Report Synopsis : UIWPD was dispatched to Joeris Residence Hall for and trouble alarm. When I arrived on location, the alarm panel was reading normal ( False Alarm). Informed dispatch to send and to Fire and Safety. WORD : HILLSIDE 3 Specific /28/2018 6:25 AM Report Synopsis : UIWPD flagged down by Aramark Staff for trouble alarm at Skyview. Alarm was showing issue with water pump system. Risk and Safety were notified. SAFD did not respond. WORD : AT&T SCIENCE CENTER : 5TH FLOOR Specific /28/2018 9:35 AM Report Synopsis : UIWPD dispatched to AT&T Science Hall for a trouble alarm. All conditions were normal. Alarm Panel was signaling a chemical storage room. SAFD did not respond. Page 16 of 17

Upon arrival fire panel was reading 5th FLR damper RM 525, did a walk through of the building and room 525, nothing was found. We acknowledged and silenced the alarm and Informed dispatch to contact fire and safety about the incident. WORD : STUDENT ENGAGEMENT CTR Specific GENERAL BUILDING DONNIFIER CELESTINE Report Synopsis : UIWPD dispatched to student engagement center building in reference to a fire trouble alarm. Upon investigating there was know visible fire or smoke. The fire panel message read system normal. WORD : ADMINISTRATION/COLBERT HALL : Floor 1 Specific CHAPEL Report Synopsis : UIWPD was dispatched to the Admin BLDG about a trouble alarm that was activated in the Chapel area. The Fire Panel displayed All Systems Normal.
