1 Lake Bluff Daily Log Incident Type Date / Time Incident Number Address Traffic Stop 07/23/ :14: CENTRAL SKOKIE RD / SMITH AVE Narrative: Officers conducted a traffic stop in the 400 block of Quassey Ave. after observing traffic violations at Route 176 and Skokie Valley Road and also on West Washington Avenue. The driver was ultimately arrested and charged with DUI, DUI over.08, improper lane usage, improper turn, failure to secure child under 8 years old in an appropriate child restraint, illegal transportation of alcohol, use of unsafe tire, possession of drug paraphernalia, and child endangerment. Alarm Business 07/23/ :15: ROCKLAND RD Narrative: Officers responded to the 300 block of Rockland Rd. for an activated burglar alarm. Officers made contact with multiple employees, and verified the activation was accidental. Suspicious Incident 07/23/ :32: ARBOR DR Narrative: Officers responded to the 400 block of Arbor Drive after a receiving a report from a taxi driver who advised dispatch that he had picked up a person at that location who looked like he had been in an altercation with cuts and scratches to his face and neck area. The subject requested to be taken to the hospital in Highland Park, IL. See formal report for further. Found Property 07/23/ :18: E CENTER AVE Narrative: A resident came to the PSB with a wallet, eyeglasses and lipstick she had found in the area of the village green. Officer made contact with the property owner, who drove to the PSB and collected the property. Driving Complaint 07/23/ :31: SKOKIE HWY / ROCKLAND RD Narrative: Dispatch received a driving complaint southbound Rt. 41 from Rt The vehicle left Lake Bluff jurisdiction before officers could arrive on location. The call was passed to Lake Forest PD. Ordinance Violation 07/23/ :36: W WITCHWOOD LN Narrative: Officer responded to the 100 block of West Witchwood Lane for a solicitor complaint. Officer located the subject on Hickory Court and determined she did not have a valid permit. Officer instructed the subject to contact the driver to pick her up and she is not allowed so solicit without a proper permit. The subject cleared LEADS/NCIC and officer had to clear the location due to an emergency call in progress. Ambulance Assist 07/23/ :55: ROCKLAND RD Narrative: Officer responded to the area of Route 176 near a dealership for a report of a silver vehicle in traffic and the driver was not feeling well. Officer located the vehicle stopped in traffic on eastbound Route 176 across from the dealership. Officer met with the driver who stated he had been working in a greenhouse all day and was heading home. The driver did not appear to be intoxicated. Lake Bluff, Knollwood and Lake Forest Fire Departments responded. Lake Forest Fire Department Ambulance A-421 paramedics treated the driver and he was transported to Lake Forest Hospital for further evaluation

2 Assist Fire 07/23/ :59: EVA TER Narrative: Officer responded to Eva Terrace for an activated fire alarm. The Knollwood and Lake Bluff Fire Departments responded to the scene. They determined the alarm activated in error. Alarm Business 07/23/ :27: ROCKLAND RD Narrative: Officers responded to the 900 block of Rockland Rd for an activated burglar alarm. Officers checked the exterior and found it to be secure. Officers observed no irregularities and cleared. Suspicious Veh 07/24/ :46: W WASHINGTON AVE Narrative: Officers responded to the 300 block of W Washington Ave for a suspicious vehicle parked in the parking lot called in by an employee. The caller stated that she never sees cars parked when she comes in in the morning. Officers were able to make contact with the vehicle owner who stated he began working as a personal trainer a couple of weeks ago and runs before starting work. Officers observed no irregularities and cleared. Motorist Assist 07/24/ :08: SKOKIE HWY / ROCKLAND RD Narrative: Officer responded to north bound IL Route 41 north of IL Route 176 for a report of a disabled vehicle. Officer located the vehicle north of the EJ&E Railroad bridge. The driver needed a tire change due to a blown tire. NCPD responded to the scene. Officer remained on scene until the driver was able to change the tire. Public Health and Safety 07/24/ :14: OAK AVE / E SCRANTON AVE Narrative: Officer responded to Oak Ave at E Scranton Ave for a report of a porta-potty in the roadway. Officer observed two porta potties along with a wash station on the east side of the roadway, left over from the bike race. The roadway was passable, and the porta potties created no hazard. Suspicious Incident 07/24/ :51: E WITCHWOOD LN Narrative: Officer located a vehicle parked in the driveway in the 100 block of E Witchwood Ln with its passenger side door standing open. Officer made contact with the vehicle owner, who stated they had left the door open on purpose. Officer reported no irregularities. Ambulance Assist 07/24/ :23: W WASHINGTON AVE Narrative: Officer responded to the 300 block of W Washington Ave for a report of a subject who wasn t feeling well. Lake Bluff, Lake Forest, and Knollwood Fire Departments responded. It was determined that the patient would not be transported and Officer cleared. Accidental Damage to Property 07/24/ :35: SKOKIE HWY / ROCKLAND RD Narrative: A subject came to the PSB to report something, possibly a bullet, struck his windshield while he was traveling south on Route 41 somewhere north of Route 176. Officer examined the vehicle and found no evidence of a bullet strike. The damage was consistent with a rock strike. Officer provided the subject with a case number for insurance purposes.

3 Identity Theft 07/24/ :25: CRESCENT DR Narrative: A resident came to the PSB to report a possible identity theft. The subject applied for electric service, and was told his social security number was assigned to a female subject, based upon information obtained from a credit monitoring agency. There were no indications of criminal activity in the credit report. Officer provided the subject the address and phone number for the Social Security Administration Office so they can check the account. Officer advised the subject to contact this department if there was criminal activity identified with the social security number. Public Health and Safety 07/24/ :47: OAK AVE / E SCRANTON AVE Narrative: Officer responded to the area of Oak Avenue at East Scranton Avenue reference the refused complainant who stated they could not get out of their parking spot due to portable sanitation there. While on location, the sanitation company arrived on location to remove the units... Animal - Carcass 07/24/ :38: SHERIDAN RD / E BLODGETT AVE Narrative: Officer responded to the area of Sheridan Road at East Blodgett Avenue for a report of a deer carcass in the roadway. Upon arrival officer located the deer carcass on the shoulder of northbound Sheridan Road, just north of East Blodgett Avenue. Officer advised dispatch of his findings, dispatch contacted IDOT to remove the deer carcass Fingerprinting 07/24/ :54: W SHERIDAN PL Narrative: Officer responded to the Lake Bluff PSB to perform fingerprints on a resident for employment purposes. Suspicious Person 07/24/ :36: HAMILTON CT / ARMOUR DR Narrative: Officer observed four subjects fishing in a pond in Tangley Oaks; Armour Dr. at Carlyle Cir on private property. Officer spoke with the subjects and advised them they are not permitted to fish in the ponds in Tangley Oaks. The subjects cleared LEADS/NCIC and were given verbal warnings. Motorist Assist 07/24/ :06: S WAUKEGAN RD / NORTH SHORE DR Narrative: Officer responded to the area of Waukegan Rd at North Shore Dr. for a motorist assist. Officer was unable to locate the vehicle and cleared. Child Seat 07/24/ :25: E CENTER AVE Narrative: Subject came in for a child safety seat check. Upon checking the seat, the seat was installed correctly. An Operation Kids Child Safety Seat Checklist was signed. Ordinance Violation 07/24/ :45: W WASHINGTON AVE Narrative: Officers responded to the 200 block of W. Washington Ave for a complaint of a solicitor; a male walking east on the north side of the roadway. Officer met with the subject who was selling newspaper subscriptions. The subject cleared LEADS/NCIC and was issued a warning citation for soliciting without a permit. Suspicious Veh 07/24/ :12: E CENTER AVE Narrative: Officer responded to the 700 block of East Center Avenue for a report of subject sitting in a parked vehicle listening to music. The complainant thought the subject was under the influence. Officer checked the area, the vehicle was gone prior to officer s arrival. Officer cleared and resumed patrol.

4 Found Property 07/24/ :23: E CENTER AVE Narrative: While checking the area for incident , officer located a glass jar in the middle of the roadway filled with change in the 700 block of East Center Avenue. The glass jar had a cork stopper on the top to secure the contents from coming out. Officer brought the glass jar and contents back to the Public Safety Building where it was logged into found property. Lost/Mislaid Property 07/24/ :51: ROCKLAND RD Narrative: Officer responded to the Lake Bluff PSB to meet with a subject reference a lost or stolen wallet. Citizen Assist 07/24/ :59: E SCRANTON AVE / MAPLE AVE Narrative: Officer observed a tractor trailer in the 600 block of E Scranton Ave. Officer met with the driver who stated he made a wrong turn and could not turn around due to the narrow roads. Officer assisted the driver with getting the truck turned around and the driver was sent on his way. Suspicious Veh 07/25/ :52: MAWMAN AVE Narrative: Officers responded to the area of 700 block of Mawman Ave for a report of two vehicles circling the area. Officer located one of the vehicles and spoke with the driver, who was an Uber driver and made several pick-ups and drop offs in the 600 block of Mawman Ave. Officers were unable to locate any other vehicles in the area and cleared. Animal - Carcass 07/25/ :36: SHERIDAN RD / E BLODGETT AVE Narrative: Officer responded to Sheridan Rd north of E Blodgett Ave for a deer carcass along the roadway. Afternoon shift on 07/24 had notified IDOT to have the animal removed. Dispatch made a second notification to IDOT. Animal Found 07/25/ :19: GREEN BAY RD / W WASHINGTON AVE Narrative: A subject came to the PSB with a male Corgi she had found along Green Bay Rd north of W Washington Ave. Officer transported the dog to Knollwood Animal Hospital. A voic was left for the dog's owner. Assist Fire 07/25/ :54: E BLODGETT AVE / BIRCH RD Narrative: Officer responded to E Blodgett Ave and Birch Rd for a report of a blown transformer. Upon arrival officer observed a downed tree limb and power line blocking E Blodgett Ave east of Birch Rd. Lake Bluff Fire Department personnel responded. Officer notified the utility company and Public Works. Public Works removed the tree limb and officer stood by on location until the utility company arrived. Animal Complaint 07/25/ :41: SKOKIE HWY Narrative: Officer responded to northbound Rt. 41 at a dealership for a report of an injured duck in the roadway. Officer located the bird south of Gage Ln. Upon officer's arrival, the bird left the roadway. Alarm Residential 07/25/ :54: MAPLE AVE Narrative: Officer responded to the 600 block of Maple Ave for a burglar alarm. Upon arrival, Officer met with the homeowner, who stated he was unaware he had alarmed his home, and did not turn it off upon returning. No irregularities were noted and Officer cleared.

5 Suspicious Person 07/25/ :32: W SANCTUARY LN Narrative: Officers responded to the block of Sanctuary Lane for a report of an unknown subject who entered the garage and went into the residence. The complainant lived across the street and did not recognize the subject who went into the residence and thought it was suspicious. Officers made contact with the subject who was a family member visiting from out of town and confirmed it with the homeowner.. Parking Problem 07/25/ :12: ARBOR DR Narrative: Officer responded to the 700 block of Arbor Dr. for a report of a suspicious vehicle parked on the complainant's property. While speaking to the complainant, the complainant remembered that a tree removal service had sent workers out to her home to remove/cut trees on her property. No further assistance was needed and Officer cleared. Ambulance Assist 07/25/ :54: ROCKLAND RD Narrative: Officer responded to the 900 block of Rockland Road for a report of a subject who was not feeling well. Upon arrival, officer met with Lake Bluff, Lake Forest and Knollwood Fire Department personnel who were treating the patient. Lake Forest Fire Department Ambulance A-421 transported the subject to Lake Forest Hospital for further evaluation. Check Well Being 07/25/ :02: SUNRISE AVE / MOUNTAIN RD Narrative: Officer responded to Sunrise Ave and Mountain Ave for a well-being check on a male who was sleeping on a bench. Officer met with the subject who stated he was taking a rest after biking. No further assistance was needed and Officer cleared. Alarm Business 07/26/ :40: SHORE ACRES DR Narrative: Officers responded to the 1600 block of Shore Acres Road for an activated burglar alarm. Officers verified with the on duty manager that the alarm was set off in error and no other irregularities were observed. Alarm Bank 07/26/ :35: SHERIDAN RD Narrative: Officers responded to an activated ATM alarm located in the 600 block of Sheridan Road and observed no irregularities. The alarm company was notified by dispatch that a bank armored vehicle was observed in the area prior to the alarm. Lock Out 07/26/ :32: NORWICH CT Narrative: Officer responded to the 200 block of Norwich Court for a vehicle lockout. The owner completed a waiver form. Entry to the vehicle could not be made. The driver will make arrangements to unlock the vehicle and have the vehicle removed due to his license status. Assist Fire 07/26/ :09: EVANSTON AVE Narrative: Officer responded to the 700 block of Evanston Avenue with the Lake Bluff Fire Department to assist with a CO detector activation. Fire Department did not locate a hazard. No other police service was requested.

6 Accident Property Damage 07/26/ :24: E CENTER AVE Narrative: Officer responded to the area of Sheridan Road near E. Witchwood Lane to assist a victim vehicle who was hit by another vehicle when parked on E. Center Avenue near 40 E. Center Avenue and the offending vehicle had left the area. The complainant was able to stop the offending vehicle prior to officer's arrival and information was exchanged. No Illinois Traffic Crash Report was requested due to the minor damage. Citizen Assist 07/26/ :40: SKOKIE HWY Narrative: Officer responded a dealership for a VIN verification on a vehicle being sold to Florida. Service was provided and Officer cleared. Suspicious Person 07/26/ :07: ROCKLAND RD Narrative: Officer responded to the area of Route 176 near the 300 block of Rockland Rd. for a report of a male subject, wearing a reflective vest who was throwing up his arms. Officer responded to the area and located the subject near West Scranton Avenue near Garfield Avenue. Officer has previously spoken with the subject. The subject confirmed that he was not in need of assistance and Officer cleared for another call. Suspicious Incident 07/26/ :08: NORWICH CT Narrative: Officer responded to the 100 block of Norwich Court for a report of increased traffic in the area. Officer responded to the area and observed multiple residences where services were being done, landscaping, painting, etc. The complainant spoke with the reporting Officer reference the concern. Officer advised the complainant that it appeared to be contractors in the area. Motorist Assist 07/26/ :01: ROCKLAND RD / S WAUKEGAN RD Narrative: Officer responded to Rt 176 west of Rt 43 for a vehicle that had broken down in traffic. A Towing arrived on scene and towed the vehicle from the location. No further assistance was needed and Officer cleared. Malkov #134 Ambulance Assist 07/26/ :20: S WAUKEGAN RD Narrative: Officer responded to the 200 block of S. Waukegan Rd for a female employee who was not feeling well. Lake Bluff EMTs responded and Lake Forest Ambulance 421 transported the subject to Lake Forest Hospital. Ambulance Assist 07/26/ :13: E CENTER AVE Narrative: While I was in the rear of the police lot, I was approached by a subject who was not feeling well. Officer contacted rescue and Lake Bluff EMTs responded. The subject was transported by Lake Forest ambulance 421 to Lake Forest Hospital. Animal Found 07/27/ :50: FOSS CT Narrative: Officer responded to the 300 block of Foss Court for a reported found Golden Retriever. The owner was contacted via phone. The dog was brought to the Public Safety Building where the dog was turned over to its owner. A verbal warning was issued for dog at large. Motorist Assist 07/27/ :48: SKOKIE HWY / ROCKLAND RD Narrative: Officer was dispatched to check on a semi-truck somewhere along S/B Rt. 41 possibly causing a hazard. Officer checked the area and no vehicle was located. No other police service was requested.

7 Motorist Assist 07/27/ :33: SKOKIE HWY Narrative: Officer responded to Rt. 41 for a reported bus on the side of the road with the emergency flashers activated. Officer checked all of Lake Bluff jurisdiction and no disabled vehicle was located. Citizen Assist 07/27/ :48: SKOKIE HWY Narrative: Officer responded to a dealership for a VIN verification for a resident from Kentucky purchasing a vehicle. Service was provided and Officer cleared. Citizen Assist 07/27/ :08: SUNRISE AVE Narrative: Officer spoke with a resident reference a concern over signs he posted on his property related to a Village topic. The complainant stated there was a note left on his property locations on East Blodgett Avenue also related to the same topic. The letter was not threatening in nature. Part of the concern was that on the letter there was a note indicating only one sign is allowed on a single property. After discussing the concern with the complainant, no further action will be taken at this time. Check Well Being 07/27/ :14: W WASHINGTON AVE Narrative: Officer responded to the Lake Bluff Recreational Center Building and met with a manager who requested a no trespass warning be issued to a female who had been parking her black Toyota Avalon near the drop off area for the children's day school for the past two days. Officer checked the vehicle and the occupant had left the area. It appeared that the female had been living in the vehicle. She was last seen on foot and was described as a female white in her late 40's approximately 5'5" and heavy set. Officer advised that if she returns to contact the Lake Bluff Police Department and a no trespass order will be issued. Intoxicated Person 07/27/ :16: E CENTER AVE Narrative: Officers responded to the 10 block of East Center Avenue for a report of a subject who was possibly intoxicated who was now in the driver side of his vehicle. Officers located the vehicle and subject. The subject was ultimately arrested for driving under the influence of alcohol. The subject was processed at the Public Safety Building and released with a pending court date of 08/25/17. The vehicle was removed by a towing company. Theft Under $300 07/27/ :42: ROCKLAND RD Narrative: Officer responded to the 900 block of Rockland Rd for a delayed theft. Report on file. Suspicious Veh 07/27/ :43: W SANCTUARY CT / W SANCTUARY LN Narrative: Officer responded to the area of Sanctuary Court at Sanctuary Lane for a report of a suspicious vehicle that had been parked there all afternoon. Officer located a vehicle matching the description leaving the area. The vehicle cleared LEADS/NCIC and officer followed the vehicle for possible traffic violations. Officer observed no traffic violations and cleared. Suspicious Incident 07/27/ :01: SUNRISE AVE Narrative: Officer responded to Sunrise Beach for a report of subjects using metal detectors at the beach. The complainant stated he believed it to be suspicious behavior. Officer checked the beach and did not locate the subjects. Officer met with Park District employees who stated they do allow people to use metal detectors at the beach. Ambulance Assist 07/27/ :14: GARFIELD AVE Narrative: Officer responded to the 700 block of Garfield Avenue for a report of a male not feeling well. The Lake Bluff, Knollwood, and Lake Forest Fire Departments responded to the scene. The patient was transported to Lake Forest ER by Lake Forest Ambulance 421.

8 Suspicious Person 07/27/ :20: MAPLE AVE / E NORTH AVE Narrative: Officers responded to the area of Maple Ave and North Ave for the report of two subjects walking down after dark. Officers met with two males who were advised of the ordinance. Both subjects were issued ordinance violations and the subjects left without further incident. Found Property 07/27/ :38: PARK PL / SUNRISE AVE Narrative: Officer responded to Park Pl and Sunrise Ave for a bicycle that has been locked to a public sign for several weeks. Officer observed several locks on the bicycle, but couldn't read the serial number. Officer left a 12 hour Tow(Warning) sticker on the bicycle, advising the owner to remove the bicycle within 12 hours, or police will remove it. Officer returned to Park Pl and Sunrise Ave and observed the bicycle still affixed to the post. Officer took the bicycle and lock and logged it into found property. Alarm Business 07/28/ :17: NORTH SHORE DR Narrative: Officers responded to the 900 block of North Shore Dr. for a burglar alarm. Officers checked the exterior and found it secure. There was no keyholder response. Ordinance Violation 07/28/ :46: OAK AVE Narrative: Officer was dispatched to the 700 block of Oak Avenue for a report of landscaping power equipment being utilized before authorized hours. Officer checked the area and no violators were observed. The complainant did not wish to speak to an officer. Suspicious Person 07/28/ :02: SHERIDAN RD Narrative: Officer was dispatched to Sheridan Road near E. Scranton Avenue for a refused complainant of a male wearing an Army hat attempting to stop vehicles. Officer made contact with the subject who stated he was just waiving cars through when the cars would slow in an attempt to allow him to cross the street. Officer observed no other irregularities. Citizen Assist 07/28/ :30: SKOKIE HWY Narrative: Officer responded to a dealership for a VIN verification for a used car sale to Missouri. The service was provided and Officer cleared. Citizen Assist 07/28/ :42: SHERWOOD TER Narrative: Officer responded to a dealership for a VIN verification for a new vehicle sale to Indiana. The service was provided and Officer cleared. Check Well Being 07/28/ :19: E BLODGETT AVE Narrative: Officer responded for a report of a bus driver possibly needing help in the area of East Blodgett Avenue. It was reported that the bus was now leaving the area. Officer observed the shuttle bus on Evanston/East Center. Officer made contact with the driver who stated he had just stopped for a break. Per the complainant, Officer made contact and advised of the findings. Accident Property Damage 07/28/ :06: W SCRANTON AVE / ROCKLAND RD Narrative: Officer responded to W. Scranton Ave and Lincoln Ave for a two vehicle property damage accident. A formal SR report was completed.

9 Alarm Residential 07/28/ :26: PARK LN Narrative: Officers responded to the 400 block of Park Lane for an activated burglar alarm. Upon arrival officers were notified of an attempt to cancel with proper passcode. Officer met with a family member who activated the alarm in error. Officer noted no irregularities and cleared. Animal Complaint 07/28/ :19: BELLE FORET DR Narrative: Officer responded to the area of Signe Ct for a dog running at large. Officer was unable to locate the dog and cleared. Accident Property Damage 07/28/ :23: ROCKLAND RD / SKOKIE HWY Narrative: Officer responded to the area of Route 176 at Route 41 for a report of a two vehicle accident called in by a third party. Officers were unable to locate the vehicles and cleared. Ambulance Assist 07/28/ :03: RAVINE FOREST DR Narrative: Officers responded to the 200 block of Ravine Forest Drive to assist ambulance with a female not feeling well. Lake Bluff and Knollwood Fire Departments responded. Lake Forest Ambulance 421 transported the subject to Lake Forest Hospital. Alarm Residential 07/28/ :24: E CENTER AVE Narrative: Officers responded to the 500 block of East Center Avenue for an activated basement motion burglar alarm. Officers found the exterior secure and noted no irregularities. No keyholder responded and officers cleared. Alarm Residential 07/29/ :07: GREEN BAY RD Narrative: Officers responded to the 400 block of Green Bay Rd for a burglar alarm. Officers checked the exterior and found the residence secured. There was no keyholder response. Officers reported no irregularities. Alarm Residential 07/29/ :16: E CENTER AVE Narrative: Officers responded to the 400 block of E. Center Avenue for an activated burglar alarm called in by an alarm company. Officers verified that the residence was secured and the alarm company was attempting to locate a key holder. No other police service was requested. Accident Property Damage 07/29/ :56: SKOKIE HWY / ROCKLAND RD Narrative: Officer responded to northbound Rt. 41 north of Rt. 176 for a two vehicle crash. Officer located the vehicles in Lake County jurisdiction and discovered it was a three vehicle crash. LSCO was notified, and officer stood by until their arrival. Ambulance Assist 07/29/ :49: GREEN BAY RD Narrative: Officer responded to the 400 block of Green Bay Rd for a male subject not feeling well. Knollwood and Lake Forest fire departments responded. Ambulance A- 421 transported the subject to Lake Forest Hospital.

10 Suspicious Person 07/29/ :09: PARK PL / SIMPSON AVE Narrative: Officer responded to the area of Park Place and Simpson Avenue for a report of a suspicious male white using a metal detector. Officer located the subject who stated he was doing metal detection on the public parkways and not damaging the ground. Officer observed no irregularities.
