The first is a ; hereinafter referred to as NOEL. NOEL is a 32-year-old Caucasian female who was born on May 03, NOEL is about 5-08 in height and about hundred and 25 pounds in weight with light brown hair. At the time this investigation began, NOEL resided at 416 Kaye Street Coppell, TX NOEL shared this residence with the home owner Lorna Bell. NOEL is known to drive a GRY 2006 Cadillac with Alabama license plate 2BS4176. The Cadillac is registered to Rebecca Noel of 316 Azalea Circle West in Mobile, AL NOEL is a student at the Parker College which is part of the Parker College of Chiropractic Medicine located at 2500 Walnut Hill Lane in Dallas, TX The second person involved with this assignment has been identified as herein after referred to as MURPHY. MURPHY is a 59-year-old Caucasian male who was born on December 29, MURPHY is about 5-07 in height with WHT hair. MURPHY resides at 4104 Shorecrest Drive in Dallas, TX MURPHY is known to drive a BLK 2000 Mercedes CL5 with Texas license plate number FCF The Mercedes is registered to Francis X Murphy of 4104 Shorecrest Drive in Dallas, TX MURPHY is a Chiropractor with a private practice located at 6211 West Northwest Highway in Dallas, TX MURPHY is also on the Board of Trustees, an 1 It was reported that Murphy drove a BLK 1998 Lexus 300 GS with Texas license plate number GTS0145.

2 Page 2 of 14 Adjunct Professor and an Alumni for the Parker College of Chiropractic medicine located at 2500 Walnut Hill Lane in Dallas, TX On Friday June 9, 2017 at about 1200 hours this Investigator drove to the Kaye Street address in Coppell Texas to try and locate vehicles associated with NOEL or MURPHY. Upon arrival, this Investigator found the driveway of the home and the street in front of the home void of any vehicles. This Investigator changed locations to the Shorecrest Drive address in Dallas Texas. Upon arrival, this Investigator found the MURPHY Mercedes parked in the driveway of the home and it was unoccupied. This Investigator changed locations to the to private practice address located at 6211 West Northwest Highway in Dallas, TX. Upon arrival; this Investigator checked the parking lot for vehicles associated with NOEL or MURPHY. Found in the parking lot of the business was a RED 2015 Ford Edge with Alabama license plate 63FM357 the Edge is registered to Sharon E Williams of 4906 Lakehurst Drive in Northport, AL Not finding any vehicles associated with NOEL or MURPHY other than the BLK Mercedes this Investigator changed locations to the Shorecrest Drive home to set surveillance. Arriving at about 1600 hours. the Mercedes was still parked in the driveway of the home and in the same position as it had been seen earlier. No vehicles associated with NOEL were at the property. The surveillance was set and at about 2018 hours. a SLV 2017 Infiniti Q50 with Texas license plate number HYL8221 arrived at the home and used the driveway to turn around to go south on Labron Street. The Infiniti is registered to Nissan-Infiniti LT of PO Box in Sacramento, CA The renewal registration is sent to Bryan Michael Pusateri of 4111 Fairmount Street #1016 in Dallas, TX Parked across from the MURPHY home on Labron Street was a BLU 2015 Fiat 5-door with Texas license plate number DYR8434. The Fiat is registered to Robert G Kail of 8621 Wingate Drive in Dallas, TX During the course of the surveillance no lights were visible inside the MURPHY home until about 2242 hours. At that time, the lights in the kitchen and near the front door were seen illuminating through the windows from inside the home. At about 2300 hours and at

3 Page 3 of 14 the instruction of the client this Investigator terminated the surveillance and returned to the office and concluded the activity at about 2318 hours. On Saturday June 10, 2017 at about 2230 hours, this Investigator drove to the Shorecrest Drive address to check for vehicles in the driveway of the home. Upon arrival this Investigator found the Mercedes parked in the driveway of the residence and it was unoccupied. No other vehicles were found at the address. This Investigator changed locations to the Kaye Street address in Coppell Texas where upon arrival this Investigator found the driveway the home and the street in front of the house void of vehicles. After no vehicles were found at the Coppell residence this Investigator terminated the surveillance and returned to the office and concluded the activity at about 0024 hours. On Saturday June 17, 2017 at about 0348 hours. this Investigator arrived at the Kaye Street address while on the way back to the office from another assignment. Upon arrival at the Kaye Street address in Coppell Texas this Investigator found a GRY 2013 Volkswagen Jetta 4-door with Texas license plate number FXP1945 parked in the driveway of the home. The car was unoccupied. The Volkswagen is registered to Lorna Kathleen Bell of 416 Kaye Street in Coppell, TX No other vehicles were found parked at the residence. At about 0624 hours, this Investigator returned to the Kaye Street address to set surveillance. Upon arrival at about 0713 hours. this Investigator found the Volkswagen still parked in the driveway of the home and it was unoccupied. Surveillance was set and no activity would be recorded at the home until about 0723 hours. At that time, the GRY Cadillac SUV registered to NOEL arrived at the home. The Cadillac was occupied by an unidentified female 2, who was later identified as NOEL. At this time, the 2 At the time the surveillance was in progress it was not known that the Cadillac belonged to NOEL. The ownership of the vehicle would be established later that morning.

4 Page 4 of 14 unidentified female entered the Kaye Street home and remained inside until about 0750 hours. After returning to the Cadillac, the unidentified female drove from property. This Investigator did not have a positive identification on the driver and as a result the car was not followed and this Investigator remained on surveillance at the Kaye Street residence. At about 0923 hours, this Investigator changed locations to the Parker College of Chiropractic Medicine on Walnut Hill Lane in Dallas Texas. Upon arrival this Investigator found the Cadillac parked in the parking lot of the college. Parked next to the Cadillac was the MURPHY Mercedes. Both vehicles were unoccupied. This Investigator terminated the surveillance at about 1000 hours and returned to the office where upon arrival the activity for the morning was concluded. At about 1600 hours, this Investigator returned to the Parker College to set surveillance. No activity involving MURPHY or NOEL would be recorded until about 1738 hours. At that time MURPHY was seen near his Mercedes. Shortly after MURPHY arrived at his car NOEL and several other students also arrived at the Mercedes. Noted is that NOEL placed unidentified objects into the Mercedes and then walked to the Cadillac and placed more unidentified objects into her vehicle. After placing those objects into the Cadillac NOEL returned to the MURPHY Mercedes where MURPHY was standing partially inside his Mercedes waiting for NOEL to return to him. Upon NOEL S arrival at MURPHY s location NOEL and MURPHY engaged in a PUBLIC DISPLAY OF AFFECTION AS THEY HUGGED. Noted is that the other students had walked away from the couple and were no longer in view of the camera and no other students were hugged by either NOEL or MURPHY. After the public display of affection, NOEL returned to the Cadillac where she drove to the main door about 100 feet from where the Cadillac was originally parked and picked up an unidentified female. Noted is that MURPHY was parked behind the Cadillac waiting for NOEL and the other female to leave the area. However, after a short period MURPHY drove around the Cadillac and off the college property. This Investigator followed NOEL as she drove directly to Fernando s Mexican Cuisine Restaurant located at 4347 West Northwest Highway #100 in Dallas, TX Arriving at this location at about 1759 hours, NOEL and the unidentified female entered the restaurant. This Investigator maintained surveillance in the parking lot and at about 1906 hours, MURPHY and several other unidentified persons arrived at the restaurant in the

5 Page 5 of 14 MURPHY Mercedes. While inside the restaurant the group sat at a table and noted is that MURPHY and NOEL sat on opposite sides of the table. At about 1932 hours and at the instruction the client this Investigator terminated the surveillance and returned to the office and concluded the activity at about 2024 hours. On Monday June 19, 2017 at about 1100 hours and at the instruction the client this Investigator drove to the Parker College to see if the Mercedes and the Cadillac were on property. This Investigator found the Cadillac parked and unoccupied at the Parker College. Surveillance was set and at the instruction of the client this Investigator terminated the surveillance at about 1336 hours and returned to the office and concluded the activity at 1400 hours. At about 1630 hours, this Investigator returned to the Parker College to set surveillance. However, upon arrival at about 1700 hours this Investigator found the Cadillac gone from the property. At the instruction of the client this Investigator drove to the Shorecrest Drive address to try and locate the Cadillac. Upon arrival at the Shorecrest Drive address this Investigator found the MURPHY Mercedes parked in the driveway of the home and it was unoccupied. Regardless of the absence of the NOEL Cadillac the client instructed surveillance be set. No activity would be recorded at the MURPHY home until about 1845 hours at which time a BLK 2017 Subaru Outback with Texas license plate number JFV6826 arrived at the home and parked in the driveway. An unidentified male exited the vehicle and entered the MURPHY home. The Subaru is registered to JP Morgan Chase Bank NA of PO Box in Fort Worth, TX The renewal notice is sent to Gerald Oliverie of 3910 IH30 in Mesquite, TX At about 1851 hours the unidentified male exited the home and entered the Outback and drove from the MURPHY property. Surveillance was maintained on the Shorecrest Drive home until about 2200 hours. At that time and at the instruction of the client this Investigator terminated the surveillance and changed locations to the Kaye Street address in Coppell Texas. Arriving at about 2230 hours this Investigator found the NOEL Cadillac parked in the driveway of the home and it was unoccupied. Upon finding the Cadillac parked in the driveway of the Coppell home, this Investigator terminated the surveillance and returned to the office and concluded the activity at about 2230 hours.

6 Page 6 of 14 On Tuesday June 20, 2017 at about 1330 hours, this Investigator drove to the Parker College to try and locate the Cadillac. Upon arrival, this Investigator found the Cadillac parked at the college and it was unoccupied. Surveillance was not set and this Investigator left the College and returned at about 1642 hours. Upon return, this Investigator resumed the surveillance. No activity involving NOEL would be recorded until about 1844 hours. At that time NOEL was observed entering the Cadillac. After NOEL entered the Cadillac she drove from the College and was followed as she drove directly to the Kaye Street address in Coppell Texas. Arriving at the Coppell home at about 902 hours. NOEL parking in the driveway of the house, exited the Cadillac and entered the residence and out of sight of this Investigator. At about 2035 hours, NOEL drove from the Kaye Street address and was followed to the 7-11 store located at 100 West Sandy Lake Road in Coppell, TX Arriving at the 7-11 at about 2039 hours. NOEL exited the Escalade and entered the business. At about 2041 hours NOEL exited the store and walked to the Escalade. Noted is that as NOEL walked from the store she was on her cellular telephone and in her HAND, SHE HELD A PACK OF CIGARETTES. NOEL entered the Escalade and LIT A CIGARETTE AND SMOKED IT AS SHE DROVE from the 7-11 property. NOEL was followed as she drove directly back to the Kaye Street address where she arrived at about 2050 hours. The surveillance was maintained and at about 2106 hours, the owner of the home arrived driving her Volkswagen. Noted is that the owner of the home parked the Volkswagen inside the garage giving this Investigator an opportunity to see how many vehicles could be parked inside the building. Noted is that due to the number of stored items in the garage there is only enough room for one vehicle to park inside the garage. 3 3 Noted is that this Investigator had checked this address that early morning hours on two occasions and the Cadillac was not present at the address.

7 Page 7 of 14 At about 2134 hours this Investigator terminated the surveillance and returned to the office and concluded the activity at about 2212 hours. On Wednesday June 21, 2017 at about 1800 hours, this Investigator drove to the Shorecrest Drive address in Dallas Texas to see if the Cadillac was at the residence. Upon arrival this Investigator found no vehicles parked at the home other than the MURPHY Mercedes which was unoccupied. At that time, this Investigator returned to the office and concluded the activity at about 1900 hours. On Friday June 23, 2017 at about 1224 hours this Investigator drove to the Shorecrest Drive address in Dallas Texas. Upon arrival this Investigator found the NOEL Cadillac parked in the driveway of the home and it was parked next to the MURPHY Mercedes. Both vehicles were unoccupied and the surveillance was set. No activity would be recorded at the home involving any persons until about 1506 hours. At that time, a BLK pickup truck which was operated by a pool maintenance man arrived at the property. The unidentified male allowed himself into the backyard of the residence and conducted maintenance on what appeared to be a swimming pool located in the backyard of the house. At about 1514 hours, the unidentified male entered the pickup truck and drove from the area. Surveillance was maintained and at about 1711 hours NOEL exited the home wearing a BLK pullover shirt and BLU pants. NOEL was talking on her cellular telephone walked to the Cadillac and removed what appeared to be clothing. At about 1714 hours NOEL reentered to the MURPHY residence and entered the home. At about 1734 hours, NOEL exited the MURPHY house and had changed her clothing. NOEL was now wearing a GRN pullover sleeveless shirt and BLU spandex yoga pants. NOEL walked to the Cadillac and removed what appeared to be sandals from the vehicle. NOEL returned to the

8 Page 8 of 14 MURPHY house with her shoes and re-entered the residence. At about 1747 hours NOEL again returned to the Cadillac. NOEL entered the vehicle and drove from the area. Noted is that NOEL left her other clothes inside the MURPHY home when she drove from the residence indicating that NOEL may have resident privileges at this house. This Investigator followed NOEL as she drove directly to the Valero gas station and River Liquor store located at 1632 Storey Lane in Dallas, TX NOEL entered the store and remained inside until about 1758 hours. At that time NOEL returned to the Cadillac entered the vehicle and drove from the property. NOEL was able to drive through a changing RED light at the intersection of the Valero gas station property and Storey Lane. As a result, this Investigator lost the visual advantage on the Cadillac. This Investigator attempted to overcome the disadvantage and drove to the Kaye Street address in Coppell Texas in an attempt to relocate the Cadillac. However, upon arrival at the Kaye Street address this Investigator found the home void of the Cadillac. This Investigator returned to the Shorecrest Drive address and upon arrival found that the Cadillac was not at this address either. As a result; this Investigator terminated the surveillance and returned to the office and concluded the activity at about 2042 hours. On Saturday June 24, 2017 at about 1830 hours, this Investigator drove to the Shorecrest Drive home address in Dallas to see if the NOEL Cadillac was at the residence. Upon arrival, no vehicles were found at the residence other than the Mercedes owned by MURPHY. The Mercedes was unoccupied. This Investigator changed locations and drove to the Kaye Street address in Coppell Texas. Upon arrival, this Investigator found the Cadillac parked in the driveway of the home and it was unoccupied. At that time this Investigator returned to the office and concluded the activity at about 2030 hours. On Wednesday June 28, hours this Investigator drove to the Shorecrest address to see if the NOEL Cadillac was at the property. Upon arrival; this Investigator did not

9 Page 9 of 14 find the Cadillac and as a result this Investigator terminated surveillance and return the office concluded activity at 2300 hours. On Thursday June 29, 2017 at about 1730 hours this Investigator drove to the Parker College to set surveillance. Upon arrival, this Investigator found the Cadillac parked in the parking lot of the college and it was unoccupied. Surveillance was set and no activity involving NOEL would be recorded until about 1830 hours. At that time NOEL drove from the property and was followed to an intersection at Walnut Hill Lane. This Investigator observed NOEL as she drove towards Interstate 35E and at the intersection of Walnut Hill and 35E this Investigator notice that instead of going north on the interstate NOEL turned and drove south towards Northwest Highway. This Investigator immediately drove to the Shorecrest Drive home to set surveillance. Upon arrival at about 1847 hours this Investigator was able to catch NOEL on video as she parked the Cadillac next to the MURPHY Mercedes and enter the home. Surveillance was set and at about 1848 hours an unidentified male driving a BLK 2003 Lexus RX 300 with Texas license plate number GPC9841 arrived at the property. The Lexus is registered to Victor Diblasi of 3828 Shorecrest Drive in Dallas TX The unidentified male entered the MURPHY home and returned to the Lexus shortly with NOEL. MURPHY arrived a few minutes later and entered the Lexus. NOEL sat in the right rear passenger seat and MURPHY set in the front right passenger seat. At about 1853 hours, the unidentified male drove MURPHY and NOEL from the Shorecrest Drive address. This Investigator followed the group to the Shops of Highland Park located at 4212 Oak Lawn Avenue in Dallas, TX At this location, the group entered an unidentified restaurant. This Investigator searched the restaurants in the area for the group however a sighting the trio did not occur.

10 Page 10 of 14 At the instruction of the client this Investigator returned to the Shorecrest Drive address to wait for MURPHY and NOEL to return. At about 2129 hours NOEL and MURPHY were dropped off by the unidentified male in the driveway of the Shorecrest Drive home. After the unidentified male drove from the property NOEL and MURPHY entered the residence. NOEL and MURPHY remained inside the home until about 2136 hours. At that time, the couple exited the home and MURPHY walked NOEL to the Cadillac. Standing between the Cadillac and the Mercedes NOEL and MURPHY talked. NOEL held in her hand an unidentified object that she at first was going to place on top of the Cadillac and then appeared to have changed her mind and instead placed it on top of the Mercedes. After placing the object on top of the Mercedes and after about a minute NOEL removed the object from the top the Mercedes and placed the object inside the Cadillac. After NOEL placed the object in the Cadillac, NOEL and MURPHY engaged in a public display of affection as they had HUGGED EACH OTHER AND KISSED. Although the kiss was brief, captured on video is NOEL AFFECTIONALLY RUBBING MURPHY S BACK AND SHOULDERS DURING THE HUG AND THE KISS. At about 2139 hours, NOEL drove from the property and was followed as she drove to the Kaye Street address in Coppell Texas. Arriving at Kaye Street address at about 2200 hours this Investigator terminated the surveillance and returned to the office and concluded the activity at about 2242 hours. On Friday, June 30, 2017 at about 1918 hours this Investigator drove to the Shorecrest Drive address to check to see if the NOEL Cadillac was at the MURPHY property. Upon arrival this Investigator found the MURPHY Mercedes parked in the driveway of the home and no other vehicles were present. No surveillance was set and this Investigator terminated the surveillance at 2130 hours and returned to the office concluded the activity at about 2200 hours.

11 Page 11 of 14 On Wednesday July 5, hours this Investigator drove to the Shorecrest Drive address to try and find the presence of the NOEL Cadillac. However, upon arrival the Cadillac was not present. This Investigator changed locations to the Kaye Street address where upon arrival this Investigator found the Cadillac parked in the driveway of the home and it was unoccupied. Surveillance was set and after no activity was recorded involving NOEL or the Cadillac this Investigator terminated the surveillance at 2300 hours and returned to the office and concluded the activity at about 2318 hours. On Friday, July 7, 2017 at about 2042 hours this Investigator drove to the Shorecrest Drive address in Dallas Texas to check for the presence of the Cadillac. Upon arrival this Investigator did not find the Cadillac at the property however there were several other vehicles parked in the driveway of the home. The first was a GRY 2017 Cadillac with Texas license plate number JBJ1913. The Cadillac is registered to PV holdings Corporation of 3937 S. 26 th Avenue in Dallas, TX The second vehicle was a WHT 2011 Audi with Texas license plate number FNC7741. The Audi is registered to Alexandra Anh-Thu Nguyen of 5504 Ford Normandy Drive in Colleyville, TX After identifying the vehicles this Investigator terminated the surveillance and returned to the office and concluded the activity at about 2224 hours. On Saturday, July 15, 2017 at about 2056 hours while returning from a surveillance in the area of Shorecrest Drive in Dallas Texas this Investigator drove by the MURPHY home to see if the NOEL Cadillac was in the driveway of the home. Upon arrival, this Investigator found a GRY 2015 Toyota 4-Runner with Texas license plate number DWB2136 parked in the driveway of the home next to the MURPHY Mercedes and it was unoccupied. The Toyota is registered to Karen Sue Tarver-Davidson of 9018 Crownpoint Circle in Irving, TX Tarver is a 57-year-old Caucasian female who has a cosmetology license

12 Page 12 of 14 in Texas. Tarver has had several marriages and divorces and at the time of this report IS CURRENTLY MARRIED TO DONALD L DAVIDSON A 62-YEAR-OLD MALE. TARVER AND DAVIDSON WERE MARRIED ON JUNE 8, This Investigator surveillance set to wait for instruction from the client and at about 2132 hours after no contact with client, the surveillance was terminated. Notes is that upon departure the Toyota was still parked in the driveway of the home and the lights could be seen illuminating through the blinds which were all closed. On Tuesday July 18, 2017 at 1500 hours this Investigator drove to the Parker College to set surveillance. Upon arrival, this Investigator found the Cadillac in the parking lot and it was unoccupied. Surveillance was temporarily suspended and this Investigator returned to the college at about 1600 hours to resume surveillance. Upon arrival, the Cadillac was found still parked on the property. No activity involving NOEL would be recorded until about 1900 hours at which time NOEL was captured on video as she walked from the clinic buildings and to the Cadillac. NOEL entered the Cadillac and drove from the College. NOEL was followed as she drove directly to the Kaye Street home in Coppell Texas where she arrived at about 1930 hours. This Investigator remained on the surveillance until about 2000 hours at which time the surveillance was terminated and this Investigator returned to the office and concluded the activity at about 2030 hours. On Tuesday, July 19, 2017 at about 1700 hours this Investigator drove to the Parker College to set surveillance. Upon arrival, this Investigator searched the parking lot for the Cadillac however the vehicle was not found at the property. This Investigator changed locations to Kaye Street address in Coppell Texas where upon arrival at about 1847 hours this Investigator found the Cadillac parked in the driveway of the home and it was unoccupied. At the client s instruction this Investigator terminated the surveillance and returned to the office and concluded the activity at about 1912 hours.

13 Page 13 of 14 On Wednesday September 6, 2017 at about 1600 this Investigator drove to the Parker Chiropractic school to follow NOEL from the property to see if she had established a new residence as her car had not been seen at the Kaye Street address for a while. Upon arrival the Cadillac was not found and as a result the surveillance was terminated and this Investigator returned to the office and concluded the activity at about 1830 hours. On Friday September 8, 2017 at about 1200 hours this Investigator drove to the Parker Chiropractic school to set surveillance on NOEL. Upon arrival the Cadillac was found on the property. After leaving the school NOEL was followed to the MURPHY home where upon arrival the MURPHY car was absent. NOEL let herself into the home and began to remove what appeared to be personal items that were contained in cardboard boxes and plastic sacks. NOEL placed those items inside the Cadillac. While loading the Cadillac, MURPHY arrived home driving the Mercedes. MURPHY and NOEL met in the driveway after NOEL had just exited the home and placed a box into the back of the Cadillac. MURPHY and NOEL HUGGED AND KISSED UPON MEETING AND THE WALKED INTO THE HOME TOGETHER 4. A few minutes later MURPY drove from the home and a short time later NOEL also left the home. After NOEL completed loading the property she drove from the home. NOEL was followed to Beltline and McArthur Boulevard in Coppell Texas. At this location NOEL drove south on McArthur Boulevard. At that time this Investigator lost the visual advantage on the Cadillac due to traffic interference. A search of the area did not result relocating the vehicle. 4 As NOEL and MURPHY were kissing in the parking lot this Investigator had to lower the camera as several people were walking past the front of the surveillance unit and the camera had to be lowered momentarily.

14 Page 14 of 14 At that time this Investigator terminated the surveillance and returned to the office and concluded the activity at about 1730 hours. At about 2130 hours this Investigator returned to the McArthur and Beltline area and searched some of the apartments in the area. Not finding the Cadillac this Investigator retuned to the office and concluded the activity at about 2354 hours. On Monday September 11, 2017 at about 2000 this Investigator drove to the Beltline and McArthur Boulevard area in Coppell Texas and began to search the apartment complexes in area for the Cadillac. At about 2200 hours this Investigator found the Cadillac parked near building number 14 at the St. Marin Apartment complex located at 1721 East Belt Line Road in Coppell, TX This Investigator while on foot found NOEL seated on a couch in apartment number The couch appeared to be the same couch that was seen inside the MURPHY home during and advertising of the home for sale on a website by the Zillow.com. After find the car and NOEL this Investigator terminated the surveillance and returned to the office and conceded the activity at about 2336 hours.
