(Nova) provided on-site air monitoring during the removal of asbestos-containing materials located in the Boiler Room and Basement Pipe Chase at the Cleveland Middle School building, 1000 Walsh Street, St. Paul, MN Removal was conducted using glove bag and non-acm enclosure asbestos abatement techniques. All asbestos-related work was performed in accordance with Federal, State, and Local asbestos regulations. Asbestos-related work began on April 10, 2006 and was completed on April 14, TWELL Environmental, Inc., License #AC592, conducted the asbestos abatement. Sample collection and analysis was completed by Holly Berndt, License #AS9752, of Nova Consulting Group, Inc., 1107 Hazeltine Boulevard, Suite #400, Chaska, MN

PCM sample collection and analysis was completed in accordance with NIOSH Method Quality control procedures were followed as required by this method. Air sample analysis results can be found on the air sample summary table located in the appendices.

Waste was transported to the SKB Rosemount Industrial Waste Facility located at Courthouse Blvd., Rosemount, MN. A waste tracking table is located in the appendices.

Nova's project manager arrives on-site to look at the work progress and located the next work area for Friday. 1:00 PM TWELL returns from lunch and resumes carrying out waste. 1:30 PM Nova's project manager leaves site for the day. 2:00 PM TWELL has one employee arrive on-site to load up the equipment. 3:00 PM TWELL has three employees removing equipment and loading the truck. Two TWELL employees continue to remove the waste. 4:00 PM TWELL has three employees loading the truck and two employees are cleaning up the work area. 5:30 PM Nova & TWELL leave site for the day. Nova collects the adjacent air samples and prepare to read them. TWELL begins to pack up equipment and load the truck. 11:00 AM TWELL leaves site for the day. 11:30 AM All samples passed the required clearance criteria, Nova then packs up equipment and leaves site for the day.
