I begin my tenure as a P r e s i d e n t o f o u r prestigious society by thanking everyone for supporting and bestowing responsibilities on me. I would like to begin by welcoming new members to executive council and welcoming back continuing members. President Elect: Dr Ajit Vaze, Honorary Secretary: Dr. Ravindra Sabinis, Honorary Treasurer: Dr Rajeev TP, West Zone Member: Dr. Lalit Shah, North Zone Member: Dr. Sudhir Rawal, East Zone Member: Dr. Muktikant Senapati, South Zone Members: Dr. Nitin Kekre and Dr. Ganesh M Kamath. I look forward to working with all of you with positive inputs. As your new president, I will strive to continue the tradition of accomplishment set by earlier executive council and will further enhance the goal set by our predecessors. My heartfelt condolence to our following belovedmembers of the society.dr. R K Mahanta, Guwahati; Prof. C L Ashok Kumar, Chennai; Dr. Vasant Krishna, Hyderabad; Dr. Snehal Patel, Baroda and Dr. Shyam K Garg, Ghaziabad. Many members expressed their grief and highlighted the contributions made by departed souls. It is very heartening to know that we are a strong contingent of 3602 members and exponentially growing every year. Many International Associations are happy and keen to be associated with us, including SAARC Countries. We have some memorandum of understanding / barter agreement on reciprocal basis but we have to see how best we can utilize these resources of opportunity for our association. As I always say that aspirants are many but opportunities are few, so I encourage all the members especially younger one to actively participate in zonal /Regional Conferences to strengthen the zonal chapter and become a bond of interactions between zonal and central meetings. BOE is doing excellent services under the Chairmanship of Dr. Venkatesh K. for the y o u n g e r o n e s t h r o u g h v a r i o u s educational activities, Webcasts, Mock Exam, Master class, USI AUA Board review courses etc. Breaking news of the activities was UROMET programme on the EUREP model started last year by Dr. Rajeev Sood our immediate past president. I will urge BOE to have archives of all the activities to be available online for members to refer to these programmes at their convenience. It is a matter of pride for us to see the Indian journal of Urology getting increasing no. of high quality Articles, also the financial status is improving Kudos to the editor: - Dr. Rajeev Kumar and his Editorial team. I commend the tremendous work and effort put in by Dr. D Ramesh past Hon. Secretary and Dr. Keshavamurthy Past Hon. Treasurer for the spectacular job. I am fully aware of certain chronic but important issues regarding registration of our society and legal issues with the charities commissioner of Mumbai since n e e d s t o b e r e s o l v e d expeditiously, I will work on this. Congratulations to all the awardees of USI and good luck to the convenors of various subspecialties. You will be happy to know that we are positively working on the uniform policy regarding renewal / introductions of various orations and awards in USI. The USICON 2018 at Jaipur will in fact be the culmination of the Golden Jubilee, so prepare yourself for the great event to enjoy and cherish the scientific programme and hospitality as promised by the organizers under the able leadership of Dr. K K Sharma, Chairman and Dr. Vinay Tomar, Organising Secretary. I would like to conclude by thanking you and those who have made a difference for our organization. Many thanks to retiring executive council members. 2 HON. SECRETARY'S MESSAGE USI NEWSLETTER Dear friends, Since the time I have taken over as secretary, I have communicated to you on 1st February & 1st March. Today, on 1st April, I have great pleasure in releasing first newsletter of this year. Newsletters is a medium where members have opportunity to know what is happening in USI, what are future plans, what are the upcoming programs of USI, who in USI passed away in last 3 months, who got some special awards & also what is happening in various zonal chapters. This newsletter will cover all these aspects. In this newsletter, I have given a space for happening in zone All zonal secretaries & presidents were informed about providing information which they feel appropriate to be put in this space. I am sure over period of time, this space will have lots of information from every zone, which members from other zone can read. I am also intending to start a special column Members area, where any member can contribute some information. I request all members to write to me if they wish to put some information, which will be of interest to all members. It could be some interesting research, news, historic significance etc. I will also be putting some interesting write up mainly of nonacademic nature but of interest to urologists. In this issue, I have put up article Getting started post residency. It will be interesting to read such articles in future as well. Friends, Since USICON 2017, society's work has started well. Slowly, I am gearing up in the role of Secretary USI, trying to fulfill the expectations of all members. I must thank all members for overwhelming response to the survey I conducted through on 1st March. It was just a trial survey, which generated good response. I will conduct more such surveys on various topics. I request members to participate actively in such surveys.. It will be interesting to understand views on various issues, which can guide the council to take certain decision with some authority. Since USICON, we had 2 program from BOEdetails of which you will see from BOE chairman's message. NAUS (Nepal association of urological surgeons) & BAUS (Bangla Desh Association of Urological surgeons) had their annual conferences in month of February. President Dr. Dogra represented USI in both m e e t i n g s. E UA ( E u ro p e a n U ro l o g y Association) conference is going on & by the time this newsletter comes to you, this conference would be over. Dr. Dogra along with many Indian delegates are attending this meeting. Taking clue from the feedback about USICON both scientific aspects, & organizational aspects will be modified. According to survey, 96% members suggested that scientific program needs reform. Majority members have opined against live surgery transmission during conference & are inclined towards working lunch. I am putting detail analysis of survey in this newsletter. Lot of members have given nice suggestions by & are worth implementing. Please ensure that all of you register for Jaipur USICON at earliest & to avoid paying unnecessary high charges of registration. I am working hard to make sure that every member will get something to take home from the conference. We are living in era of digital revolution. Social media, has penetrated our life in such a way that nobody can avoid it. Instead of criticizing it, we have to make use of these advances for our betterment. We have started Twitter account; I request all of you to follow USI on twitter to get some information or breaking news. Also respond whenever possible. Twitter is fasted way to communicate with everyone instantaneously. Let's make record followers of USI on twitter. I have always looked secretary post as an opportunity to serve the members of USI. 3 MESSAGE FROM PRESIDENT - ELECT Dear Colleagues, I am indeed, grateful to all my urology community, for having reposing their faith in me, for electing me as a President Elect. I am truly humbled by your spirit camaraderie and bonhomie. I promise, I will not leave any stone unturn to enhance, the academic and interpersonal unity of our Urological society of India. I plan to work in unison with current president and elected council to promote scientific rigor, social enrichment of this esteem organization. In rapidly growing society like ours, I envision a distinct need for developing subject specific dedicated workshops, hands on training seminars for all three tiers (Residents, Junior consultants and senior active consultants), in sub specialties like Mini and Ultra Mini PCNL, RIRS, Female Urology and Andrology. There is a unmet need to develop National Data Base in areas of Urolithiasis, Oncology and incontinence. We also need our own guidelines in these fields, which is a pressing demand for many years. I, In addition, will also try to increase financial health of USI so that adequate funds are always available for Research in Urology and IJU. Finally, I will try to develop Indian armamentarium of scopes, instruments and imaging which may reduce rising cost of present urological surgery DR. AJIT VAZE President - Elect, Urological Society of India FROM THE DESK OF CHAIRMAN, BOARD OF EDUCATION Dear All, A new year has started for the Urology Society of India, as well as the Board of Education. I write this at the beginning of my second year, of a 2 year term. The feedback on the last year's performance so far has been generally good. There have been hiccups, mainly in the conduct of the online exam, and the implementation criteria. This year that too will be stream lined, as we have clarity on the requirements, and a document has been prepared. By the time I write this, we already have finished 3 BOE programs. The paid lectures at the USICON were a runaway hit. The hall was packed to capacity. Despite some misgiving, the general feedback has been that this is a much awaited session every year. We also completed a webcast on Female Urethral Stricture surgery, a subject that has not been covered so far in the BOE sessions. We also have finished the exam review course at Mumbai, with a record number of more than 150 students attending, and wanting more instruction, even after completion of our program!! 8 USICON 2017, MUMBAI - A REPORT The Golden Jubilee annual conference of The Urological Society of India- USICON 2017, conducted from January 2017 was a path breaker in many ways and the USICON conference committee is happy to share the milestones crossed in this mega national event. USICON 2017 was unique as it had several firsts to its credit A land mark of 3470 self registered delegates reflected the synergy between the organizing committee and the general body It marked a first in being a paperless national conference without a single printed brochure. All registration and payment communications were handled through a secured dynamic website. A tech-savvy young group of enterprising Urologists were responsible. The creation of an international class new state of the art exhibition cum convention centre to house more than 3000 delegates during plenary sessions was achieved. It could well have been an auditorium at any well ranked International convention centre ( AUA/ EUA/SIU) hosting an annual conference. Team Rustic our events management planned the same. Plenary and outreach sessions saw more than 12 large LED screens and 24 huge speakers provided by Sound.com,.the best known in the sound industry providing the best sound solutions for all USICON requirements. The Ceremonial inauguration was held at the hands of the Hon. Governor of Maharashtra and Tamil Nadu as the Chief Guest. The live operative sessions with smooth, uninterrupted, HD transmissions with cables from three Mumbai corporate hospitals Hospitals ( Global, Fortis and Kokilaben Ambani ) at peak hours 1pm to 3 pm saw a very good turnout In a first, USICON 2017 created history by installing and creating a first of its kind HISTORY MUSEUM in a well spread out 5000 square feet area. It covered thirteen booths of relevant Urology history.the support from the industry and contributions from many Urologists made it unique. The first time, industry participation headed by the tireless IPCA team helped create this fascinating arena at USICON The PHILATELY BOOTH with information on all stamps dedicated to Urology and all ancient instruments deserves special mention. The trade component witnessed a well designed exhibition of International class, constantly abuzz with activity. For the first time flexboards, posters and non-digital branding were not permitted at the venue. Advertisements and branding for trade partners was exclusively digital. Four MASTER CLASSES were organized on the final day to ensure optimal utilization of the venue, time and the invited faculty : Pediatric Urology, Andrology, Reconstructive Urology and Female Urology. It marked the best attended last day of any USICON so far. The concluding program saw maximal attendance on the last day. A new feature MEET THE PROFESSOR sessions were conducted with renowned International faculty: Dr Tony Mundy, Dr Mohd Eissa, Dr Michael Grasso, Dr Mark Soloway, Dr Ramamurthy. A non-acadamic component in the form of unusual Keynote addresses in The Plenary sessions was a first for USICON. Dr V K Mundra spoke on The Game of Marriage and Mr Harsha Bhogle on Twenty Twenty in real life. Both received tremendous appreciation The second day morning, preconference time witnessed the marathon : USIRUN with more than hundred members participating in the run for awareness on Palm beach road. Again a first was the first ever All India Intercollegiate Quiz QUIZACON.The national event culminated in the grand finals at USICON 2017 conducted by guest Quiz Master, Dr Akshay Nair, Consultant Opthalmologist. The proud winners were CMC Vellore Team in the first place ( Prize: Mini Perc Set ) and, PGI Chandigarh team in the second place( Prize : Semirigid Nephroscope ). IPCA organized the interesting, well attended event. USICON 2017 boasted of a very varied motivational, Art and Cultural evening events as follows : The Instrings performance by Sangeeta Shankar, Ragini and Nandini a team of renowned violinists with perfect rhythm support.

9 A scintillating performance by singer, music composer and actor Shri Shankar Mahadevan and his troupe Colonel Lalit Rai's extraordinary talk on The Kargil War The daily Newsletter USICON Times contained news and interviews with leading Faculty members in addition to some interesting non-academic coverage. The first ever USICON to have conducted three Free Operative camps in the year at Rajkot, Puttaparthi and Cuttack, extending the joy of the golden jubilee celebrations to community service Partnering with Radio FM channels for the first time, USICON 2017 spread Public awareness messages on Common Urological problems in fifty cities. Urologist members contribution to this was valuable. 10 Debate : Getting started Post Residency Life Dear Friends, After scoring highest marks, students get admission to MBBS, then those who score well among them, come to surgery & from them, only selected few can get admission to MCh/DNB urology. So, students who come to Urology belong to topmost intelligent creamy layer of the society. When they are in training their objective is to clear exam. Every student thinks that once exam is over, life is easy, simple & enjoyable. However, after passing& after putting so much struggle of several years, they come to a stage, where they are confused, whether to start private practice to remain in academic institutes. Real dilemma starts then. Hence big question in front of them is what to do..??? Last year, NZUSICON had organized a debate attributed to this dilemma. Secretary GETTING STARTED IN POST RESIDENCY LIFE: ACADEMIC UROLOGY In India, training for urology is a 3-year process and has been so since 1965 when the first formal training program began. While the training duration has remained the same, urology has progressed manifolds. What has also increased is the number of trainees at each centre, often without a measurable increase in the infrastructure. The result of these two opposing changes is a widening gap between what needs to be learnt and the opportunities available for doing so. After these 3 years, a fresh graduate in urology faces a choice of how to begin his/her career, continue in academic urology or move to a service-only establishment. To address this question, we began by trying to understand what 'academic urology' means to urologists in India. We invited 530 members of the north-zone chapter of the Urological Society of India to complete an anonymous online survey on the meaning of this term. We received 71 responses from 410 successfully delivered invitations (18%). Considering the number of urologists in India to be around 3500 based on membership of the Urological Society of India, this survey had 89% power at á error of 5%. When you are speaking for 'academic urology', justifying your academic credentials helps! 97% responded that 'academic' urology meant a centre that had trainees (MCh or DNB), 65% believed that it was not a solourologist practice and more than 50% believed that patients at such centres do not 'belong' to a consultant. Further, 70% respondents did not agree that consultants' salary is lower than their market value. Additionally, they believed such centres performed research, published and were ethical. The summary of these findings is that 'academic urology' means a centre that has a training program, shared responsibility, wide spectrum of patients who are not tied to a consultant, performs research, is ethical and the consultants' reimbursements are at par with the market. With these attributes, where is the debate about the best places to work? Academic urology would win hands down! Are 3 years of training really enough in urology?1 Campbell's urology now has 4 volumes, there are 8 recognised subspecialties but 24 residents in one program. Where is the time to learn laparoscopy, microsurgery, robotics and complex reconstructions?2 Surgery requires major decision making and responsibility, something that comes from experience. Academic urology allows ongoing training and the opportunity to seek assistance and second opinions, shared responsibility. It provides infrastructure and support to test your abilities with help being available if required. Jumping into an independent practice immediately after graduation denies these opportunities. Additional benefits include peace of mind from the lack of a financial motive, and consequently, lower distrust among patients. When money is not the driver, it loses its value as a status symbol and there is no pressure to get the bigger car or bigger house. For some it may also mean a loss of drive but then,

11 if you are losing your drive within a few years after graduation, you are probably in wrong profession anyway! Academic urology drives you to earn more in the only currency that is allowedrecognition. It provides you greater opportunities to chose a single field and make a name in it, perform research, publish, present and be recognised. Even from a financial view-point, salaries in a public or purely academic centre in the first few years are not much different from the private sector. In fact, at times they are higher. In addition, there are perks such as a house, office, secretarial staff and allowances, some of which may be seen as an ego-trip but then so is the Mercedes of your private practice colleague. An argument against academic urology has been hierarchy, poor infrastructure and lack of jobs. These arguments need to be understood in perspective. Hierarchy exists in medicine at all places, academic or not. It stems from the apprenticeship model of training and also serves a purpose of allowing one the freedom to seek a second opinion and share the blame. It is a process of ongoing learning. The infrastructure of course varies between centres but accreditation for a urology program requires a certain minimum and even the poorest of centres will have to make this available. India today has over 350 medical colleges and 100 urology training programs. Jobs are always available though they may not be in your preferred centre. The final argument against academic urology after graduation is that it takes time to build a practice so one should start early. This is true but so is the concept of 'lead time bias', a concept that urologists are well aware of! The first few years are still a part of the learning curve and what better place could there be to learn than in academic practice? References 1. Gautam G. Academic institutes comprise both Institutes and medical colleges. These are few in number, not fairly distributed across the nation and with inadequate faculty positions and woeful working conditions in most. There are few good teachers; who can inspire hope, ignite the imagination and instill a love of learning. More over these establishments cannot keep pace with the technological developments taking place and lag behind in instrumentation. These huge buildings on prime estate become white elephants. The salary structure though modest is not the tipping point as these jobs provide a whole gamut of perks and incentives. The most important factor is the inability of these establishments to accommodate ambition of the aspiring Urologist who then develops a desire to walk out at the earliest opportunity. All Urologists shall aspire to work in an 'Academic Setup'. What is Academic Urology? It is not working in a so called department and churning out 're-searched' publications and carrying a prefix before your name. It is patient care with full awareness, utilizing documented experience, maintaining meticulous data with periodic analysis, treating patients with data driven decisions rather than hunches and anecdotes. Such an attitude keeps the hunger to explore new frontiers alive. The solution is to form groups of likeminded individuals to share data as well as pool resources. Within the group the way forward is Sub Specialization, which is already upon us as days of general urologist are almost gone. The world is constantly evolving and we need to change ourselves. The need is to stop finding faults and instead look for remedy. If finding likeminded individuals is not happening, signing up with a corporate setup is the way to go. These setups provide the patient clean ambience, courteous staff and professional delivery of service while providing the Urologist with technology and clinical work in an academic environment as most of these are running DNB courses. The work insecurity also keeps the Urologist on his toes and he keeps aspiring to improve all the time. NABH etc also keep a close monitoring of the system What does an Urologist desire Adequate clinical material, decent earnings and reasonable academics. The way to do it is

12 group practice with each taking up a sub division of Urology and sharing each other's opinion, experience and when required a helping hand and yet remaining your own 'Boss'. Pure research is best left to those are trained to do so while Urologists are surgeons trained for performing surgery. Let the so called institutions be staffed with PhDs who are primarily meant to raise the bar regarding scientific content while leaving the Urologist to provide 'standard' care of patients. The young Urologist should not seek protection & anonymity in joining a 'department' to hide his own shortcomings and insecurities by wearing a cloak of intellectual superiority. Instead he should become a yardstick of quality and provide excellence in patient care all the time while retaining his own quality of life as well as a secure a bright future for his children. Professor & HOD, Stanley Hospital, Chennai USI mourns the sad demise of Dr D Chandrashekar who left for his heavenly abode on 27th January He was a popular teacher and active member of the USI. Dr. PVLN Murthy from Hyderabad has been awarded the Dr BC Roy Award by the President of India A thematic workshop on Supine Workshop was conducted on 9th March 2017 at Manipal. Dr Guido Guisti of Milan was the faculty. Dr Arun Chawla organized this well attended ASU supported workshop. Council member Dr Dilip Dhanpal coordinated the event.
