1 Mountain Guide Training and Certification Program Box 3010, Kamloops, BC Canada, V2C 5N3 phone: (250) , (250) fax: (250) Application Package About This Application Package This application package assumes you have the Program Information booklet which describes the various programs, courses, and exams offered in the Mountain Guide Training and Certification Program and that you meet or exceed the requirements for the program you have chosen. If you do not have the general information booklet or are uncertain whether you qualify to apply, please contact the University College of the Cariboo for further information or assistance. This application package contains the following: Guidelines for preparing your application (page 2) Preparing your curriculum vitae (page 3) Example format of a curriculum vitae (page 3) Preparing your personal skills resume (page 4) Example a personal skills resume (page 4-5) Payment and cancellation policies (page 6) Medical form (page 7) Registration form (page 8) Waiver (page 9) Dates and fees information for 2001 (page 10) Warning: Read This Incomplete and late applications will be returned immediately. Space on courses and exams may be limited and if your application is returned, you may not get a seat even if you qualify. If you are applying to more than one program simultaneously, you must indicate each course separately. Personal skills resumes must reflect each course applied to. It is essential to type or word process your curriculum vitae and personal skills resume so they are formatted exactly as the examples provided. Poorly organised applications or unformatted curriculum vitaes and resumes jeopardise your chances of acceptance because your background, skills, and qualifications may not be clear and/or may compare unfavourably to other applications. If your application is not organised and formatted according to the guidelines and examples in this package, it may be rejected immediately. It is strongly recommended that you allow yourself adequate time to create a well structured, organised, and complete application that arrives on time. This will greatly enhance your chances of being accepted if you qualify. When you sign the registration form you warrant the information in your application is true and correct. False, exaggerated, or embellished information will result in rejection of your application. If you are accepted and it becomes clear in the field that you are not at the standard indicated on your application, instructors have the right to require you leave the course immediately. Please call for information or advice if you are uncertain about any aspect of applying, have questions, or need advice. July, 2000 Page 1

2 Guidelines for Preparing Your Application Keep in mind that your application is a first impression. As such, it plays a significant role in whether you will be accepted into the program or not. Not all the steps or documents shown below are required for every course or exam. Check the requirements on the applicable page where your course or exam is described in the Program Information booklet to ensure your application contains the correct documentation. To ensure your qualifications and background are presented in as effective a manner as possible, follow the steps below carefully: 1. Check the Dates and Fees information to ensure you have the correct date, application deadline, deposit, and fee information (see page 10 or contact the Mountain Guide Training and Certification Program office). 2. Complete the registration form. 3. Staple a cheque or money order to the front of the registration form. If you are paying by credit card, please call and provide your card number. 4. Find a witness for the waiver. Read and sign the waiver on the back of the registration form. Have the witness sign the waiver. 5. Place two passport photos in an envelope and staple to the back of the registration form (if required). 6. Prepare a curriculum vitae or update (see page 3). 7. Have a physician complete the medical form. NOTE: Applicants to Climbing Gym Instructor courses ARE NOT REQUIRED to submit a medical form. 8. Obtain letters of reference and proof of apprenticeship or work experience if required. 9. Make copies of the following certificates as required. Do not send originals. First aid certificate CPR certificate Avalanche course certificate Prerequisite guide course certificates and/or performance evaluation forms Post-secondary school transcripts and/or diplomas 10. Staple the documents from steps 7, 8, and 9, in that order, to your curriculum vitae. 10. Attach the curriculum vitae to the back of the registration form. 11. Prepare a personal skills resume or update (see page 4-5). Attach the resume to the registration form below the curriculum vitae. 12. Double check all requirements and documentation, place your application in a 9 x 12 envelope, and mail to the Mountain Guide Training and Certification Program. Preparing Your Curriculum Vitae The curriculum vitae provides an overview of your personal information and work history that relates to the program, course, or exam. When first applying, ensure that your original c.v. is complete and that all applicable documents are attached. If sending an update you should carefully review your original and submit any changed or new information, for example, change of address, phone number, etc., new/further education, new references, changes in contact information for references, etc. Keep a copy of your curriculum vitae for future reference. Example Of A Curriculum Vitae Scores of c.v. s are reviewed every year. 3 Preparing Your Personal Skills Resume Your personal skills resume should clearly provide the information identified in the prerequisites describing the program to which you are applying. Keep a copy of your resume for future reference. If you are preparing an update to your personal skills resume, you should carefully review your original and the additional requirements (if any) listed for progressing in the program. Your update should use the same format as the original but should supply only information that is new or changed since you sent your original. For example, new references or changes to reference contact information, new background, pertinent routes or trips done since your last application, etc. Example Of A Personal Skills Resume Scores of resumes are reviewed every year. Using the following structure minimises the chance that important information will be missed and maximises your chances of being accepted. The FIRST PAGE of your Personal Skills Resumes should look as follows: Personal Skills Resume for (insert your name) (insert program name, e.g. Alpine Guide, Climbing Gym Instructor, etc.) Program insert date (month, year) References: Include two or more technical references (someone with whom you have been active in the field and can confirm your experience and abilities). Illustrate that you have experience in a variety of locations and/or climates and/or types of terrain. You should include the training regime you used or use to develop and maintain your skills. The SECOND PAGE of your Personal Skills Resume should contain prerequisite route and trip information. Remember that the routes and trips you list on this page should include ONLY those that you have led or co-led. Format as follows: Rock Guide Program: short routes (1-3 pitches in the past year requiring protection placement (minimum 5.10+) sport climbs (fixed protection) flashed in the past year. Minimum Significant redpoints in the past year that indicate your top climbing standard moderate length routes (full day multi-pitch). See page 14 of the general information booklet for examples or more longer routes (very long day/or multi-day). See page 14 of the general information booklet for examples. 6. Alpine Guide Program: moderate length, mixed alpine routes 2. 5 long or multi-day alpine climbs 3. Ski Guide/Alpine Guide/Hiking Guide Program Use this format for skiing, mountaineering, alpine climbing, and hiking trips or routes: Trip Location Duration Date Name and/or description of route Area/Range Number of days (month/year) On the THIRD AND SUBSEQUENT PAGES OF YOUR PERSONAL SKILLS RESUME, you may list additional information pertinent to your application. This could be trips, routes, or courses. It is strongly recommended that you list only those trips or routes that you led or co-led. If you feel that routes or trips where you were a participant and not a leader are pertinent, make sure this is clearly noted and present them separately from led or co-led trips. If presenting courses, write a paragraph or two as an overview and/or use a format similar to the one used for listing education in the c.v. example. Use this format for rock routes (or ice climbing routes for alpine guide candidates): Route Difficulty Location Belayer Date Style (if applicable) Name of Route Route Grade Area/crag Partner s name (month/year) Flash/Redpoint Page 4...this example continues on page 5 Page 5

Waiver ASSOCIATION OF CANADIAN MOUNTAIN GUIDES RELEASE FROM LIABILITY AND WAIVER OF ALL CLAIMS WARNING: By signing this document you give up the right to sue TO: Association of Canadian Mountain Guides and the University College of the Cariboo AND: Her Majesty the Queen in Right of the Province of British Columbia and Alberta (the Provinces ) In consideration of the Association of Canadian Mountain Guides (herein the Guide ) accepting my application and being permitted to go on a (insert name of course or exam here) mountaineering trip ( a Mountaineering Trip ) on lands owned by the Province or elsewhere, I agree to this release of claims, waiver of liability and assumption of risks (collectively this Agreement ). I waive any and all claims I may have against and release from all liability and agree not to sue the Guide and his/her/its employees, mountain guides, agents and representatives (collectively the Guiding Staff ) and the Province for any personal injury, death, property damage or loss sustained by me as a result of my participation in a Mountaineering Trip with the Guide due to any cause whatsoever including, without limitation, negligence on the part of the Guide or the Guiding Staff or the Province. I am aware that mountaineering and participation in a Mountaineering Trip have extreme dangers and risks, some of which include: 1. Mountain Terrain - mountainous areas with steep slopes in their natural state have many obstacles and extreme hazards, may not have been climbed or skied before, are not regularly patrolled or examined and where climbers or skiers may become lost or separated from the Guide or the Guiding Staff or other climbers or skiers and where communication is difficult and rescue and medical treatment may not be available. 2. Avalanches - which may frequently occur in mountainous terrain used for mountaineering and may be caused by natural forces including steepness of the slopes, snow depth, instability of the snow pack or glaciers, by changing weather conditions or by climbers or skiers, the helicopter or failure for any reason of the Guide or the Guiding Staff to predict when or where avalanches may or may not occur. 3. Weather - weather conditions may be extreme and can change rapidly without warning. 4. Aeroplane or Helicopter Travel - additional risks are posed by flying in mountainous areas and in variable weather conditions. 5. Conduct of the Guide - the conduct, including negligence, of the Guide and/or the Guiding Staff and the conduct of other climbers or skiers. I willingly accept all the risks of mountaineering including without limitation the risks described above and the possibility of personal injury, death, property damage or loss resulting therefrom. I acknowledge that the enjoyment and excitement of mountaineering is derived in part from climbing or skiing on steep slopes in mountainous terrain and that the inherent risks of mountaineering contribute to such enjoyment and excitement. In entering into this agreement I am not relying on any oral or written representatives or statements made by the Guide or the Guiding Staff or by the Provinces including those in any brochures to induce me to go on a Mountaineering Trip. I confirm that I am of the Age of Majority (full age of eighteen years in Alberta and the full age of nineteen years in British Columbia) and that I have read and understood this Agreement prior to signing it and agree that this Agreement will be binding upon my heirs, next of kin, executors, administrators and successors.
