1 Jindalee School Age Care Program Dec/Jan 2017 / 2018' Vacation Care Program Enriching the lives of Young People Proudly owned by the Jindalee State School P&C Association OPENING HOURS to 6.30am till 6.00pm Contact Details Phone: (07) Mobile: jindaleesacp@gmail.com Tracy Schadendorff Director Clyde McIlroy Coordinator Fees $40 per child per day plus additional charges for incursions / excursions (these charges will be applied to your account). Families may be eligible for Child Care Benefit (CCB) and Child Care Rebate (CCR) to help reduce the cost of fees. Please contact the Family Assistance Office to gain further information. 48 HOURS NOTICE IS REQUIRED FOR CANCELLATIONS TO AVOID BEING CHARGED Excursions Excursion permission forms will be available on the morning of the excursion and need to be completed by a parent or guardian. Please arrive at the service no later than the time specified on the program. This enables us to have the children ready to leave on time. Excursions are subject to change depending on number of children booked and weather conditions. What to bring to Vacation Care An authorised person must drop off children to sign permission forms; Sun Safe Clothing; Wide brim hat; Closed in Shoes; Items Relevant to Excursion Bag to hold Belongings; Sun Safe Clothing Wide Brimmed Hat Water Bottle A healthy lunchbox and enough food for the day Please Do Not Bring! X Toys or items from home X Roller Shoes X Mobile phones X Electronic games As we have children attending our Service that are Anaphylactic to food and animals, we kindly ask that children refrain from bringing food containing nuts. YEAR LEVEL BOOKING Please make sure you book your child/ren into the year level they will be in 2018 (e.g. If your child is going into year 5 next year please book them into the Grade 5 and Up program ) How to Book Please make all bookings as a casual booking using the My Family Lounge App. (Not the web portal) All children are required to have a booking and be signed in / out by a parent or authorised person. As our program has become quite popular, we sometimes have a waitlist in place. To avoid disappointment please ensure you book your child s care in advance. Medication If children require medication to be administered while in our care the medication must be in the original packaging and have a pharmacy label with the child s name and dosage details. A permission form also needs to be completed by you at the service and completed each day the child attends. Please see Tracy or Clyde to complete the required permission form.
