All child care fees must be paid in full by the first Friday of the next school term. I am aware that on excursion days I need to be at the College by the allocated time on the program. I am aware that once I have made a booking there will be no refund. I understand that I am unable to swap days once my booking form has been handed in and a deposit paid. I give permission for my child to travel on private buses for excursions. I consent to photographs / video being taken as part of the children s program and for them to be displayed in the OSHC room, in newsletters and DVD slide projections. I am aware the children need to be wearing a full brimmed hat. Tank tops and thongs are not acceptable attire for excursions. I certify that the information entered upon this form is true to the best of my knowledge and that I undertake to inform the Program Coordinator if any of these details change.
