4 Welcome Dear Participants & Families, Since 1993 Sport & Recreation Services has become an important part of the disability community here in the Illawarra. Our service provides a variety of programs to children, teenagers and adults with disabilities wanting to improve their health and well-being whilst developing valuable sporting and social skills. Our programs provide more than just the opportunity to attend weekly programs. They enable you to meet and make new friends and develop some lifelong friendships. Over the years we have taken on the feedback provided by participants and families to develop some new exciting programs including annual camps, discos and recreation programs including the Finally It s Friday and Sunday Funday social groups. This has ensured participants can mix with their friends in a relaxed safe environment with the support of qualified staff from The Disability Trust. We have some exciting activities planned across all regions and programs in Our dedicated staff including Amy, Susan, April, Megan, Maddie, Melissa, Kelly and Tara have been working hard to ensure we will continue to provide safe programs of the highest possible standards. We are also pleased to have many of our program staff returning in 2018 along with a few new additions to our team. 5 underestimated as they deliver tailored programs each week across many regions. With the support provided by The Disability Trust we are able to make sure the programs provided meet the needs of all participants. These programs ensure participants are working towards achieving their goals in a supported environment. In 2018 we will also be partnering with local organisations to assist participants to have access to quality inclusive opportunities. These organisations include the St George Illawarra Dragons (Dragons Community), Football South Coast, Illawarra Academy of Sport, Merrigong Theatre Company, Special Olympics Illawarra and Illawarra Netball Association. We are also proud to have some great ambassadors who support our participants and programs whilst sharing the same values as The Disability Trust. We encourage you to read their messages of support to all participants. As we head towards the NDIS we will be working with participants and families to help you prepare for the changes ahead. The Disability Trust has staff members who are able to meet with you to help your prepare for the changes ahead and answer any questions you may have in relation to the NDIS. 6 Ambassadors Messages Ben Creagh The Disability Trust does a lot of great work for people with disabilities in the Illawarra and Shoalhaven regions. I especially like the wide range of sporting and recreational programs they offer children and young adults. The programs give the participants an opportunity to learn about teamwork, mateship and having a go which I know is not only enjoyable but an important part of life. Sport has always been a great medium to help break down the barriers between people in our community and it s great to see people with disabilities accessing these opportunities. 8 Rebecca Bulley I am delighted to be an ambassador for The Disability Trust and in particular their Netball program - The Eagles. I believe that netball and sport in general, is just as important in the life of a person with a disability, as it is in mine. Through sport, these participants have the opportunity to develop their social skills, independence and confidence, whilst also benefiting their health and well-being. Being involved with the Trust has shown me it is all about the individual s ability and what they can achieve and not the disability. 10 Sport & Recreation Services Sport & Recreation Services of The Disability Trust provides programs for children, teenagers and adults with a disability in the Illawarra, Shoalhaven, ACT, Southern Highlands, Southern Sydney and South West Sydney. The service provides opportunities for people with a disability in a variety of ways. 11 Recreation Illawarra Recreation Illawarra is one of Australia s leading sporting and recreational programs for people with disabilities. Since 1993 we have been working closely with our participants, families and carers to develop and implement programs that meet the needs of all participants. We have also developed strong relationships with local sporting and recreational organisations to enhance the opportunities we are able to provide ensuring our programs promote inclusion in our local community. 12 2018 Program Enrolments In 2018 we have changed the process in which you register for your programs with our service. This year all programs will register via mail for your preferred program. You would have received your program enrolment form or pack for Just tick the programs you wish to attend this year and we will add your name to the program attendance sheet and ensure your participant profile sheet is in the program folder. This new process will save participants and families time. If your information has changed over the past 12 months, please fill out any new details on the Profile sheet provided ie change of address, phones numbers, medications. NOTE: Participants with NDIS funding who wish to change programs in 2018 must let us know ASAP as we have to make sure your funding matches the changes you wish to make PROGRAM ENROLMENTS

14 2018 Program Fees Program Fees Program fees will be invoiced in the following ways in NDIS Participants - you will now be invoiced monthly for the program you attend. Recreation Illawarra Funded Participants - you will continue to be invoiced at the end of each term. All participants can continue to pay for services in the following ways - Online B Pay Phone - call TDT Head Office for Credit Card payments In Person at The Disability Trust Head Office Jardine Street Fairy Meadow (eftpos facilities available) or the Warrawong office 210 Shellharbour Road Warrawong (cash/cheque only) Program Cancellations All participants who are accessing programs under the NDIS must contact the office by 5.00pm the day before a program to cancel. You can cancel your attendance at any program the following ways - Phone refer to page 9 for our addresses SMS - reply to any message we have sent you NOTE - when cancelling please state the program and date you want PROGRAM ENROLMENTS

NDIS The Disability Trust is a registered NDIS provider and has already assisted many families with the transition to new services. Over 18 Sunday Funday: First Sunday of each month Under 18 Sunday Funday: Third Sunday of each month Participants will only pay the cost of the activity and any transport costs for each outing. Staffing costs will be covered by your NDIS packages. This is a great night that allows you to dress up and get together with your friends at the Novotel North Beach for a fun night of awards and dancing. 46 2018 Recreation Illawarra Programs BlueScopeWIN Sports Ready Program This is a sporting skill development program targeting children with a disability in primary and special schools. The program aims to teach students the major skills involved in sport including sport specific skills, rules, teamwork and sport Illawarra Academy of Sport / The Disability Trust AWD Program PRE-ELITE TRAINING PROGRAM Program Type: Individualised etiquette by having them participate in modified games, programmed to the specific needs of the students. Lessons are also linked to the NSW BOSTES PDHPE Syllabus. 64 The Disability Trust / TIGS & Sony Foundation Camp 2018 Since 2010 The Disability Trust in conjunction with TIGS (The Illawarra Grammar School) and The Sony Foundation have run a respite camp for families in the Illawarra and Shoalhaven regions. In 2018 we will once again be running the camp in early November at Berry Sport & Recreation Centre. 66 Friends of The Disability Trust Friends of The Disability Trust has been formed by local Illawarra businesses with the support of The Disability Trust Board to raise much needed funds for one of the Illawarra s most respectable and reputable charities, providing care to over 4000 clients. If you were to ask an individual in the Illawarra region about The Disability Trust, it is more than likely they will know someone who has been a part of one of the programs or services provided, but the lesser known fact is The Disability Trust is a charitable organisation reliant on not only government grants, but donations from the community and business sector. Throughout the year, with the support of Illawarra businesses and volunteers, Friends of The Disability Trust will hold four key events to raise funds that will go towards the support of much needed services and equipment to the wide range of people The Disability Trust supports. The Friends of The Disability Trust committee is managed by a group of like minded individuals who volunteer their time to help support The Disability Trust. This committee features individuals from a variety of local Illawarra businesses keen to increase the awareness of the services and programs provided by The Disability Trust and to increase the charitable donations to the organisation.
