1 HOTEL AND TRANSPORTATION INFORMATION Our Transportation Service We provide a transportation service for our patients who are staying in the immediate Old Town Scottsdale area, as a courtesy to our patients who are not familiar with the area. Transportation is offered on a first come, first served basis. Please schedule your ride as soon as you know your flight times. You should also schedule your return trip at this time to maximize your chances of utilizing our service. You may need to wait and share a ride with another patient. We will do our best to accommodate patients during the above hours; however we cannot always guarantee availability. If you are unable to wait or are coming in at a time other than above, you will need to make your own ground transportation arrangements. Scheduling our Transportation There are a number of ways to schedule with our transportation service. Send your transportation request to driver@tmeltzer.com Please note: is not always perfect. 2 Mail For your convenience a Patient Transportation Information and Request form has been included in your information packet. Please complete this form and return it to our office no later than two weeks prior to your arrival. Changes and Delays: It is your responsibility to let our office know of any changes or delays. Airport You will need to fly in to Phoenix Sky Harbor Airport (PHX). Airport Pickup As soon as your plane has landed please call our driver. Please proceed directly to the baggage claim area - even if you have not checked luggage and walk outside to the curb. Call our driver again and they will meet you curbside. If you do not have a cell phone or have special needs, please proceed directly to the baggage claim area - even if you have not checked luggage-and someone will be waiting with a sign with your name on it. To accommodate as many schedules as possible you may be asked to wait. We will try to let you know this when you schedule your ride, so that you may make alternate ground transportation plans should you not desire to wait. Special Note: Please keep in mind, if you are driving here for your surgery, you will need to make sure that someone is able to drive you home. 3 WHERE TO STAY When you come to Scottsdale, you are welcome to stay anywhere you like; however, if you will be using our transportation service we do ask that you stay in the immediate Scottsdale specifically. This is limited to a 5 mile radius from the hospital. Below is a list of hotels in (and nearby) the area. Some offer special rates for patients. You must state that you would like the Scottsdale Healthcare Rate. Popular travel web sites such as Expedia.com can help you research your lodging options. It is important to note that you must be staying in the immediate Scottsdale area to utilize our transportation services. Please feel free to visit our website for updated information, or call us at Our local number is We have updated the following information to reflect proximity to our office. Some offer discounted rates. Please check with the hotel at their direct number to see if they offer the Scottsdale Healthcare Rate or a special rate for Dr. Meltzer. If you have a recommendation, please let us know and we will add it to this list. It has beautiful views with separate casitas built into Camelback Mountain, a wonderful spa and an awesome restaurant. Even if you don t stay here, you may want to dine at their restaurant - Elements East McDonald Drive Paradise Valley, Arizona Hotel: The Hyatt at Gainey Ranch Located in Central Scottsdale. Incredible Pool Area. Five minutes away from our office and about ten minutes away from the hospital E.
