1 WELCOME TO KIDS COUNTRY SUMMER CAMP 2017 We know your chid wi have a memorabe summer here with us! We beieve the foowing guideines wi hep answer some frequenty asked questions: Genera Guideines Our hours of operation depend upon the site and program. TRADITIONAL Boinger, Creekside, Quai Run, Sycamore Vaey, Twin Creeks and Wat Disney - 7:00 am-6:30 pm TRADITIONAL CAMP/THE Rancho Romero - 7:00 am-6:30 pm SPECIALTY Coyote Creek 7:30 am -6:30 pm Montair 7:30 am-6:30 pm X-TREME Green Vaey and Montevideo 7:30 am-6:30 pm SUMMER SCHOOL Boinger, Montair and Quai Run (transportation avaiabe from sites where SRVUSD summer schoo is hed to the isted KC sites) 12:30pm -6:30pm If someone other than you wi be picking up your chid, pease send a written note and ask that person to have identification with him or her. Pease sign your chid in and out on a daiy basis with your fu signature in ink and incude the time even though we charge by the week. LABEL, LABEL, and LABEL. Labe a persona items with your chid's name We cannot be responsibe for ost or damaged toys. Pease eave persona items at home. Pease provide your chid with waterproof sunscreen. Labe the container with his/her name. We encourage a chidren to bring a swimsuit and towe daiy. Fied Trips Pease refer to the Summer Brochure (coming in February) for programs and sites with avaiabe fied trips. Not a sites or programs incude off-site fied trips. Be sure to fi out the attached permission sip competey. Each chid MUST have a signed permission sip on fie to attend any fied trips. T-shirt must be worn on a fied trips (not on swim days). Fied Trip unches shoud be paced in disposabe bags (no Tupperware). Drinks shoud be in disposabe containers (NO GLASS BOTTLES). Lunches Pease send your chidren with a nutritious unch daiy. Pease send either fruit juice or water, no carbonated drinks are aowed. We do not have the storage capacity to put unches in the refrigerator or to microwave food items. Due to the rising number of chidren with severe nut aergies, a our sites are Nut Free Zones.

4 Summer Schoo : $240 per week/$225 sibing discount *If appicabe: $145 per week/$135 sibing discount (Week 5: Juy 5- Juy 7) fees must be paid in fu prior to the start of the month. Canceation fee is $100 for each week, if weeky services are canceed after invoices are issued. Late payment fee is $25. There is a change fee of $25 for requesting a change from a seected week to a different week after invoices are issued. Change requests can ony be honored on a space avaiabe basis. Late pick up fee is $5 per minute. (Note: hours and cosing times vary per ocation.)

5 PHOTO/VIDEO RELEASE: My signature beow indicates that I give permission for my chid to be photographed (sti or video) whie participating in activities at Kids Country. Photographs and/or videos wi be used for the soe purpose of promoting Kids Country programs. These photographs and/or videos may be used for site abums, program brochures, media productions, advertisements, news artices, socia media posts or pubication to our website: Photographs and/or videos wi not be accompanied by the ast name of any chid. Photographs and/or videos wi be taken during reguar chidcare activities such as fied trips, cubs, outdoor activities, and free choice times. In addition, I knowingy wi not seek any form of compensation or remuneration from Kids Country, or its agents, for the use of my chid s presence in a photograph and/or video. 7 KIDS COUNTRY SUMMER CAMP 2017 ACTIVITIES PERMISSION SLIP I do hereby give my permission for my chid to participate in the Kids' Country Summer activities, fied trips and swimming trips. I understand they wi be transported by First Student bus services, commercia bus or by Kids' Country vans. I give my permission for any emergency medica or denta treatment necessary by a icensed physician or dentist.
