6 1.1 Aims and Objectives 1 SECTION 1 - Incident/Accident Reporting The Airport Company has overall responsibility for Airside and Landside Safety Management, Environmental Management and matters relating to Aviation Security. As part of the Safety Management Systems in place for the airport site, this chapter details the processes by which the Airport Company will categorise the various types of incidents. The categorisation of incidents by the Airport Company does not detract from any legal obligation on the stakeholder to report the incident or accident. (E.g. HSE/CAA/Police etc). Nor does it substitute the requirements of the Airport Emergency Orders The Safety Management Manual details the Airport Company s approach to incident categorisation and investigation, along with an outline of the Safety Incident Reporting (SIR) system. All stakeholders should have similar policies in place or have adopted this policy as part of its own Safety Management System. The methodology and processes of LBA safety reporting are clearly defined, with forms, in the Safety Management Manual. 1.2 Safety Incident Report (SIR) Following any incident/accident or in light of any safety concern, a Safety Incident Report is to be raised (at one of the eight Chroma terminals with SIR access or via AOU) to ensure LBA is notified of the incident. Details of immediate corrective action taken must be provided. Following the submission of an SIR, the SIR is subsequently catagorised for trend analysis. The categories chosen have mirrored the severity classification table in the Safety Management Manual (section 4), with the addition of a no risk category for those SIR s which pose no real safety issue. SIR s can be logged as any of the below categories. Copies of which can be found in APPENDIX 1 & Mandatory Occurrence Reporting (MOR) To ensure compliance with ANO Article 226 the provisions of this instruction must applied at all times. Page 6 of 13
7 Mandatory Occurrence Reporting policy is subject to the Airport Company s internal audit system. Any accident notified to the Chief Inspector of Air Accidents in pursuance of regulations made under sections 75 of the Civil Aviation Act 1982 shall not constitute a reportable occurrence for the purposes of Article 226. The filing of a MOR (which must be within 72 hours of the incident) does not supersede the need to file a Safety Incident Report, which will provide visibility to LBA Management of the issue. Notification that a MOR has been filed should be included in the detail of the SIR Description of Task To define how the provisions of Article 226 are applied. (Further information guidance can be found in CAP 382 The Mandatory Occurrence Reporting Scheme. It is available in pdf format on the CAA web site Qualification and Responsibility The following operating staff employed by Leeds Bradford Airport are required to make a report in accordance with the provisions of Article 226. Airside Operations Unit staff Air Traffic Control officers Air Traffic Engineering staff Note: The above have to report occurrences that meet the definition as specified in the ANO. Others may in fact report should they consider it necessary. The person completing a report is responsible for ensuring that it is dispatched to the CAA at the address indicated on the form, without delay. Where an alleged infringement of legislation has occurred, a CA939 report may be completed and submitted with the occurrence report. The following forms should be used for the reporting of occurrences: CA1601 to be used for all types of occurrence except as noted below; CA1602 is for use solely by Air Traffic Controllers when reporting ATC occurrences; CA1603 is for use solely by Air Traffic Engineers for all occurrences associated with Air Traffic Service Ground Equipment. Copies of the aforementioned forms can be obtained from the CAA website at The occurrences set out below shall be considered as reportable, but any other occurrences judged by those involved, to meet the criteria should be reported; Air Traffic Control Services by ATCOs or Airside Ops Staff Provision of significantly incorrect, inadequate or misleading information from any ground sources, e.g. ATC, ATIS, Meteorological Services, maps, charts, manuals, etc. Page 7 of 13
8 Provision of less than prescribed terrain clearance. Provision of incorrect altimeter setting. Misidentification of aircraft by an ATCO or radar operator. Incorrect transmission, receipt or interpretation of significant messages. Separation between aircraft less than that prescribed for the situation. Non-compliance with prescribed letdown or departure procedures, or any ATC instruction. Declaration of an emergency ( Mayday or Pan ) by an aircraft. Unauthorised infringement of any form of regulated airspace. Unauthorised or illegal RTF transmissions Navigation and Communications Equipment failures, malfunctions or defects by ATCO or ATS Maintenance Staff Total failure of navigation system or subsystem being used by an aircraft. Total failure of communications system. Total failure of radar system or subsystem Significant malfunction or deterioration of Service. Significant deficiency in maintenance. Repetitive events of a specific type of occurrence which in isolation may not be considered reportable (e.g. excessive monitor alarms). Provision of erroneous information in the absence of any alarms. Airfields and Airfield Facilities by Airside Ops Staff or ATCOs Failure or significant malfunction of airfield lighting. Major failure or significant deterioration of surfaces of runways or aircraft manoeuvring areas. Runways or aircraft manoeuvring areas obstructed by aircraft, vehicles or foreign objects, resulting in a hazardous situation. Errors or inadequacies in marking of obstructions or hazards on runway or aircraft manoeuvring areas. Collision between a moving aircraft and any other aircraft, vehicle or other ground object. Aircraft departing from a paved surface which results in, or could have resulted in, a significant hazard. Jet or prop blast incidents resulting in significant damage or serious injury. Significant spillage of fuel on airfield ramps. Passengers/Baggage/Cargo by Airside Ops Staff or Ground Support Staff. Difficulty in controlling intoxicated, violent or armed passengers. Incorrect loading of passengers, baggage or cargo, likely to have a significant effect on aircraft weight and balance. Incorrect stowage of baggage or cargo likely in any way to hazard the aircraft, its equipment or occupants or to impeded emergency evacuation (includes hand baggage). Inadequate storing of cargo containers or substantial items of cargo Significant contamination of aircraft structure, systems or equipment arising from the carriage of baggage or cargo. A Stowaway(s). Aircraft Ground Handling/Servicing by ATS Maintenance Staff or Ground Support Staff Loading of incorrect fuel quantities likely to have a significant effect on aircraft endurance, performance, balance or structural strength. Loading of contaminated or incorrect type of fuel or other essential aircraft fluids (includes oxygen and potable water). Significant spillage of fuel. Page 8 of 13
9 Failure, malfunction or defect of ground equipment used for test/check out of aircraft systems and equipment if this could have an effect on the safe operation of the aircraft. Non-compliance or significant errors in compliance with required maintenance/servicing procedures. Staff Incapacitation by All Staff Impairment of any member of ground staff (e.g. Aircraft Maintenance Staff, Air Traffic Controllers, Air Traffic Services Maintenance Staff, Airside Ops Staff, Ground Support Staff etc.) when as a result an aircraft was or could have been endangered. Any other occurrence of any type considered to have endangered or which might have endangered the aircraft or its occupants. It is of great importance that reporters keep firmly in mind the concept of endangering or potentially endangering as defined in ANO Article 117 when deciding whether or not to submit a report. The primary objective of occurrence reporting is to monitor, disseminate and record for analysis, critical or potentially critical safety occurrences. It is not intended to collect and monitor the normal flow of day-to-day defects/incidents etc. The latter is an important part of the overall flight safety task but procedures and systems already exist to carry out this function. In the main, these comprise industry responsibilities monitored overall by the CAA. When appropriate such systems also provide the necessary records for statistical purposes. In order to achieve the above objectives for occurrence reporting, the criteria for a reportable occurrence needs to be set above (in terms of the effect on safety) the normal day-to-day defects or minor incidents. Over enthusiastic reporting of such items which fall below this criteria will involve unnecessary duplication and work to both the reporters and the CAA and will also tend (by sheer volume of data generated) to obscure the more significant safety items. A copy of each report must be retained indefinitely. Follow up Action will be determined by the Head of Airfield Services on an individual basis, subject to the nature of the occurrence Notifiable Accidents "Accident" means an occurrence associated with the operation of an aircraft which might take place between the time any person boards the aircraft with the intention of flight and such time as all persons have disembarked, in which: a person suffers a fatal or serious injury. the aircraft sustains damage or structural failure which adversely affects its strength, performance or flight characteristics requiring a major repair or replacement. the aircraft is missing or is completely inaccessible. It does not include engine failure or damage, when the damage is limited to the engine, its cowlings or accessories; or for damage limited to propellers, wing tips, antennas, tyres, brakes, fairings, small dents or puncture holes in the aircraft skin Serious injury An injury which is sustained by a person in a reportable accident and which: requires hospitalisation for more than 48 hours commencing within seven days from the date on which the injury was received; or results in a fracture of any bone (except simple fractures of fingers, toes, or nose); or Page 9 of 13
10 involves lacerations which cause nerve, muscle or tendon damage or severe haemorrhage; or involves injury to any internal organ; or involves second or third degree burns or any burns affecting more than five percent of the body surface; or involves verified exposure to infectious substances or injurious radiation; and seriously injured shall be construed accordingly. Notifiable Accidents are those defined above, whether or not an aircraft accident procedure has been initiated in accordance with the LBA emergency orders. Notifiable Accidents policy is subject to the Airport Company internal reporting and investigation system. Wreckage of an aircraft (involved in an accident as defined above) shall not be removed or interfered with nor shall any person, other than an authorised person, be allowed access to it without the authority of the AAIB. HM Customs and Excise authority will also be required if the aircraft was on a flight subject to Customs controls. An exception may be necessary for the extrication of persons or animals, or any mails, valuables or dangerous goods, or to prevent destruction by fire, or preventing any danger or obstruction to the public, air navigation or to other transport. An Authorised Person is any person authorised by the Department for Transport and includes Police Officers and Officers of HM Customs and Excise. To ensure correct and expeditious completion of post accident action the provisions of the instruction must always be complied with. This instruction specifies supplementary reporting requirements to those required by the Mandatory Occurrence Reporting requirements of ANO Article 142/CAP 382. Description of Task-Post accident action and notification requirements for aircraft involved in Notifiable Accidents. The HAS or in his absence HoATS/AOUS is responsible for completion of the notification action set out at Para 1.3 Until relieved by or in consultation with the HAS, the Fire Operations Incident Commander is responsible for ensuring that the provisions of Para are complied with when the accident has taken place on the airport Reporting Procedures In the event of a Notifiable accident the ATC WM/SCOD shall be responsible for dissemination of initial reports. The notification is required to be passed to the Department for Transport (in effect the AAIB) by the quickest means and giving, as far as possible, the following information: the type, model, nationality and registration marks of the aircraft the names of the owner, operator and hirer (if any) of the aircraft the name of the commander of the aircraft the date and time (UTC) of the accident or serious incident the last point of departure and the next point of intended landing of the aircraft the position of the aircraft in relation to some easily defined geographical location the number of: crew on board and the number killed or seriously injured Page 10 of 13
11 passengers on board and the number killed or seriously injured other persons killed or seriously injured as a result of the accident the nature of the accident or serious incident and the extent of damage as far as is known The person reporting the accident to the AAIB is also required to inform West Yorkshire Police of the accident and the place where it occurred unless the accident has occurred on the airport and Emergency Order procedures have been activated ACCID All reports to the relevant authorities including the AAIB and the ACC s should be completed in accordance with CAP 493; Section 6; Chapter 3. The HAS, HoATS, ATC WM/SCOD shall on termination of the incident, instruct the duty ATE to impound the ATC voice recording tape. All reports made in respect of Notifiable Accidents shall be retained indefinitely. ATC voice recordings shall be retained until the publication of a report by the Chief Inspector Air Accidents, or as otherwise notified by the Chief Inspector. Page 11 of 13
