The Tade Show will be held on Thusday, Octobe 9 f om 1:00 p.m. until 4:00 p.m. and is open to hospital puchasing decision-makes. It will showcase poducts, equipment and sevices fo the healthcae industy with an emphasis on acute cae, psychiatic and ehabilitation hospitals. The Statehouse Convention Cente povides plenty of space fo all exhibits and meal sevice to be located in a single oom. LOCATION The Akansas Hospital Association Annual Meeting will be held at the Little Rock Maiott, 3 Statehouse Plaza. 6 AHA EXHIBIT RULES AND REGULATIONS It is the esponsibility of all Exhibitos/Copoate Sponsos to ead and follow the ules and egulations listed below. An attestation is povided fo signatue on the AHA Exhibit Booth Registation and Contact. 1. SPONSORSHIP/PURPOSE: The AHA Annual Meeting and Tade Show is sponso ed by the A kansas Hospital Association, a not-fopofit copoation. The pupose of the Annual Meeting is to povide educationally oiented sevice to the healthcae field, its pesonnel and elated associations; to offe a foum fo the exchange of infomation on health-elated issues, sevices, skills and technology; and to pomote effective two -way communications between healthcae povides and supplies of elated poducts and sevices. The tems AHA o Association used in these egulations shall mean the Akansas Hospital Association, and, as the context equies, its employees acting fo the AHA in the management of the tade show. 2. EXHIBIT DATE AND HOURS: The Tade Show will be opened to attendees on Thusday, Octobe 9, fom 1:00 4:00 p.m. 3. ELIGIBILITY: The technical exhibit aea is opened to exhibito s/sponso s whose p oducts and se vices a e di ectly elated to the healthcae field. Acceptance of exhibitos/sponsoship is at the sole and absolute discetion of the AHA. An application to exhibit is not accepted until the exhibito is notified by the AHA in witing of the acceptance, and any such acceptance is subject to the conditions and equiements contained in these Rules & Regulations and within the exhibito contact. 4. APPLICATION FOR EXHIBIT SPACE: Applications fo space a e submitted on the AHA Exhibit Booth Regist ation and Cont act fo m. EXHIBIT SPACE WILL NOT BE HELD WITHOUT A DEPOSIT. Upon acceptance, confimation will be mailed to the contact peson and seve as a ecod of space assignment. Each fim epesented at the Annual Meeting must have contacted fo space. 5. BOOTH RENTAL RATES AND DEPOSITS: All t ade show booths a e ten feet by ten feet. Space assignment shall not be less than one booth o multiples of that size at a ental ate of: $950 fo pemium booths ($1100 afte 7/1/14) o $850 fo all othe booths ($1000 afte 7/1/14), which includes daping, 6 table (if equested), basic electicity (if equested), 2 chais, capet and company sign. A deposit of 50% of the total payment fo sponsoship and exhibit space ental is equied with the etun of the AHA Exhibit Booth Registation and Contact fom; howeve, the entie space ental fee may be sent with the egistation. The balance in full must be paid on o befoe July 1. Booth assignments will not be guaanteed until full payment is eceived. The entie fee is due on all new applications afte July 1. Exhibitos will not be allowed to occupy space until payment is made in full. Checks should be made payable to the A kansas Hospital Association. MasteCad and Visa ae accepted. 6. ASSIGNMENT OF SPACE: Booth space will be assigned acco ding to the following citeia: p io ity fo companies that exhibited the pio yea; pefeence of the exhibito; numbe of spaces equested; date of eceipt and acceptance of AHA Exhibit Booth Registation and Contact fom; and the dispesion of simila exhibits. Evey effot will be made to assign the booth space equied o the next closest location. The final assignment of booth space is, howeve, at the sole discetion of the AHA. Afte final assignment of booth space, AHA eseves the ight to eaange the floo plan and may elocate any exhibit in its discetion. Howeve, the exhibito will be consulted pio to such elocation. AHA Educational Tables ae eseved fo Akansas membe institutions wishing to display educational mateials only. Copoate Sponsos and AHA Sevices, Inc. endosed companies will be given the oppotunity to display in special aeas, booths 1-27, and 47-51, espectively. 7. REFUNDS AND CANCELLATIONS: If w itten notice of space cancellation is eceived by the Association pio to J uly 1, a full efund less a $100 administative fee will be made. Fo sponsoship cancellation, please see sponso contact. If space is cancelled on o afte July 1, no efunds will be made and the outstanding balance will be due the AHA. If, howeve, the AHA is able to ent the space to anothe exhibito, the ent paid will be efunded, less a $100 administative fee. The AHA is unde no obligation to solicit a eplacement exhibito. 8. SUBLETTING OF SPACE: The subletting, assignment, o appo tionment of the whole o any pa t of the assigned booth space by any exhibito is pohibited. No exhibito may pemit any othe paty to exhibit in thei space any poducts and/o sevices othe than those manufactued o sold by the contacting exhibito within the egula couse of thei business, no pemit the solicitation of business by othes within thei space. If an exhibito has affiliated with anothe business entity to offe one o moe poducts o sevices in the egula couse of its business, the exhibito may exhibit such poduct o sevice within thei booth space and give cedit to the affiliated business entity only with the expess witten pemission of the Association. 9. BOOTH CONSTRUCTION AND ARRANGEMENT: Standad exhibit booths will be supplied and e ected by the AHA, consisting of a daped backgound 8 high and daped side patitions 3 high. The official booth height is 8. The entie exhibit hall, including all booths, is capeted. If an exhibito desies a deviation fom the booth space equiements (10 by 10 by 8 ), pemission must be obtained fom the AHA. All exposed, unfinished pats of an exhibit must be daped to pesent an attactive appeaance. Inspections of the exhibits will be made duing setup time and effot made to advise the exhibito if any deviation fom the exhibit ules is noted. 10. INSTALLATION OF BOOTHS: Installation of exhibits at the Statehouse Convention Cente will be f om 3:30 p.m. on Wednesday, Octobe 8 until 7:30 p.m., and Thusday moning, Octobe 9, fom 7:00 to 10:00 a.m. NO EXCEPTIONS! 11. CONTRACTOR SERVICES AND INFORMATION: The AHA has selected The RoA k G oup to se ve as the official cont acto to p ovide vaious sevices to exhibitos. These sevices include: shipping and dayage, labo fo eecting and dismantling and booth equipment and funitue. All contacto sevices must be povided by The RoAk Goup. Complete infomation, instuctions and schedule of pices egading shipping and dayage, labo fo eecting and dismantling, funitue, etc., will be included in the Exhibito s Sevice Kit to be fowaded by July 15. An Exhibitos Sevice Cente will be maintained on the exhibit floo to facilitate sevice equests fom exhibitos. Payment fo sevices povided to the exhibitos by the official contacto is the esponsibility of the espective exhibito. All sevices not odeed in advance of Septembe 19, must be pocued though the Exhibitos Sevice Cente, which will be maintained in the exhibit aea. 12. EXHIBITOR HOUSING: A block of ooms has been ese ved at the Little Rock Ma iott (which is adjacent to the Statehouse Convention Cente), the Annual Meeting headquates hotel. Hotel oom ates ae $157 single o double occupancy. Each exhibito is esponsible fo making his/he own oom accommodations. The AHA shall contol all taffic into/out of the exhibit aeas to minimize delays. 6

7 14. SHIPPING AND STORAGE: The Statehouse Convention Cente will not accept shipments fo stoage. All a angements must be made though The RoAk Goup. Shipments will be deliveed to the assigned booth space when the displays ae set up. An infomation sheet detailing pocedues fo handling shipments to The RoAk Goup will be included in the Exhibitos Sevice Kit. 15. DISMANTLING OF EXHIBITS: Exhibits must be dismantled between 4:00 and 8:00 p.m., Thusday, Octobe 9. Howeve, exhibito s found dismantling befoe 4:00 p.m. will not eceive an updated attendee list following the Annual Meeting no will they eceive the fist mailing of the exhibito pospectus fo the next yea. All exhibits must be emoved by 8 p.m., Thusday, Octobe 9. The AHA will not be esponsible fo any mateials left afte this time. Exhibitos ae esponsible fo making aangements fo pompt pickup fo all outbound shipments. The official dayage contacto will be available to assist the exhibitos on Wednesday and Thusday. 16. EXHIBITORS REPRESENTATIVES: Each exhibito will name one individual to act as its autho ized ep esentative fo all decisions egading the exhibit thoughout the duation of the exhibito contact. Each exhibito must povide an attendant within his/he exhibit space duing opening hous of the Annual Meeting as defined above unde Exhibit Date and Hous. All attendants must be bona fide employees of the exhibito, o epesentatives who eceive commission, bokeage o salay fom the exhibito, but excluding epesentatives who maintain and own inventoies of mechandise fo esale. False cetification of individuals as exhibitos epesentatives, misuse of exhibitos badges o any othe method o device used to assist unauthoized pesons to gain admission to the exhibit floo will be just cause fo expelling the violato fom the exposition o baing him fom futhe entance on the exhibit floo o emoving his exhibit fom the floo, without obligation on the pat of the Association fo efund of any fees. Upon the execise of this authoity by the Association, the exhibito, fo himself, his employees o his agents, waives any ights o claims fo damage against the Association aising out of enfocement of this paagaph. 17. EXHIBITOR REGISTRATION: All exhibito s a e to egiste thei pe sonnel in advance. Booths a e limited to fou (4) epesentatives. Any additions o changes in egistation made duing the Annual Meeting must be cetified by the exhibito s authoized epesentative. Admission to the exhibit hall will be by official egistation badge only. The official egistation badge must be won wheneve the exhibito is in the exhibit hall. No badge will be issued until exhibito egistation fee is paid in full. Repesentatives of vendo companies who do not wish to exhibit at the tade show o paticipate as a copoate sponso of the annual meeting, but wish to attend annual meeting events, will be chaged a egistation fee of $950 pe peson. Thee will be no exceptions. 18. PRESENTATION OF PRODUCTS AND SERVICES: The pupose of the Annual Meeting exhibit p og am is to futhe the education of healthcae pofessionals. The consummation of a sale, be it cash o chage, thoughout the duation of the exhibit show is expessly fobidden. Contacts, howeve, may be made fo futue discussion of sale and delivey of poducts and/o sevices. The method and manne of sale shall be pope, and any exhibito shall be subject to eviction, without efund, if his sales pocedue is objectionable. 19. SPECIAL VISUAL AND SOUND EFFECTS: Audio visual and othe sound and attention-getting devices and effects ae pemitted only at such decibel intensity as, in the opinion of the AHA, do not intefee with the activities of neighboing exhibitos. Opeational equipment being demonstated should not opeate at noise levels which ae objectionable to neighboing exhibitos. Moving equipment o machiney compessos o othe appaatus must not ceate objectionable noise o sound. 20. SECURITY AND LIMITATION OF LIABILITY: The AHA will p ovide peiphe al secu ity only in the exhibit hall du ing the exhibition peiod; howeve, the Association shall have no esponsibility fo the potection of the exhibito s booth, mateials, o displays duing the Annual Meeting. Each exhibito must make povisions fo the safeguading of its goods, mateials, equipment and display at all times, and the exhibito heeby eleases the AHA and the Statehouse Convention Cente fom any and all claims, losses, damages, and expenses aising out of any losses to any theeof. The exhibito eleases and waives any claim against the Statehouse Convention Cente, the AHA, and thei espective boad membes, offices, diectos, employees, contactos, and agents (heeinafte efeed to as Indemnitees ) fom any and all claims, losses, and damages aising fom o in connection with any cause whatsoeve, including without limitation, negligence, loss, theft, damage, o destuction of goods, o fo any injuy to itself, its agents, employees and contactos while in o about the exhibition pemises. Exhibito agees to indemnify and to hold hamless and defend Indemnitees fom any losses, claims, liabilities, damages, and expenses (including attoneys fees) aising fom whateve cause whatsoeve, including without limitation popety damage o loss and injuy o ham to pesons, aising out of o caused by exhibito s maintenance, use, set-up, constuction, emoval and opeation of its exhibit and booth, o the acts o failue to act of the exhibito and its offices, diectos, employees, agents, contactos and invitees while in o about the Statehouse Convention Cente o the Little Rock Maiott fo the AHA Annual Meeting and Tade Show and fom any beach of the exhibito contact. The exhibito must confim that dives of any vehicles diven on Statehouse Convention Cente pemises ae insued as equied by Akansas law. 21. IRREGULAR ACTIVITIES: All business activities of the exhibito must be within the exhibito s allocated exhibit space. In keeping with the natue of a healthcae pofessional meeting, undignified pomotional methods and/o mateials consideed to be objectionable by the AHA ae expessly pohibited. To ensue the quality of the meeting any uncommon pomotional techniques should have pio AHA appoval. Distibution of efeshments o othe poducts fo consumption on the pemises, not manufactued by o specifically elated to the sales activities of the exhibito, shall not be pemitted. All foodstuffs and beveages ae contolled by an exclusive ageement with the Statehouse Convention Cente s catee. If samples of these items ae to be pat of the exhibit, appoval fom the AHA is equied. Whee food poducts ae pemitted to be seved, the pepaation of such foods cannot cause odos consideed to be offensive o objectionable to othe exhibitos o egistants. Exhibitos ae liable fo any damage caused by fastening displays o fixtues to the building floos, walls, o to the standad booth equipment. No pat of any exhibit, o any signs elating theeto, shall be posted, nailed, scewed o othewise attached to columns, walls and floos. 22. SOCIAL FUNCTIONS: Any exhibito wishing to sponso a social function duing the Annual Meeting must do so with the app oval of the AHA. No social functions, howeve, will be pemitted duing exhibit hous o duing educational sessions. 23.
