2 Welcome to Activ Recreation 2017 Booking Procedure Where our purpose is to support a better life! Activ Recreation offers support to people with intellectual disabilities, 18 years and over, wishing to access community activities for social, recreational and respite benefits. We are confident this brochure has something of interest for everyone. We offer local day trips, weekends away, week-long holidays down south and trips further afield to a variety of destinations including Tasmania, Gold Coast and Sydney. We also offer a number of overseas adventures to places including New Zealand and the Indonesian Adventure Cruise. Whilst our day programs, Weekenders, Special Events, WA Getaways and Summer Camps are coordinated in-house, the Travel Program will remain a joint project with travelkey which continues to enjoy its long association with Activ Recreation. Their continued support and dedication in providing innovative and exciting holidays is very much appreciated and we thank them for their continued commitment. We would also like to thank our hard working team of Volunteers whose enthusiasm and dedication enabled the Recreation Department to offer over 500 clients annually a program. The continued commitment and support of our Volunteers allows Recreation the opportunity to provide an extensive selection of programs and opportunities. These forms are enclosed with the brochure. If you require additional forms, please call us on or collect from Activ Recreation, Churchlands. NB. Please ensure you complete and send both sides of the application form. Incomplete application forms will be returned for completion. Applications will not be processed and positions on programs will not be allocated until completed forms are received. Please note: All applications are to be received by Activ Recreation by the 30 th of November 2016 at the latest. Step 2: Returning Application Forms Once you have completed pages 1 and 2 of the application form, please send to: Post: Activ Recreation -1 Dolomite Court, Churchlands WA activ.recreation@activ.asn.au or Fax: (08) Step 3: Acceptance on programs When we have received your application, we will endeavor to meet your request. If you are successfully placed on a program you will be sent an acceptance letter asking for a deposit for overnight programs, or full payment for day programs. If you are new to our programs, you will be asked to complete a Customer Profile before being accepted. One to one support options may be available upon application. Step 4: Initial payment To confirm your place on the program, Activ Recreation requires the deposit or full payment on the date specified in the acceptance letter. Deposits are non-refundable. Please refer to cancellation conditions for exceptions. NOTE: Payments for the Cruises and Travel Programs are to be made to travelkey Leederville. Payment options will be provided with your confirmation invoice. Step 5: Final confirmation and payment You will receive final confirmation of your placement and details of any outstanding balance owed for the trip once the full group has been assessed and all program arrangements have been finalised. Step 6: Program details You may be sent other correspondence relevant to your chosen program, e.g. itineraries, advice of orientation dates, departure details, and clothing lists. NOTE: For the Cruises and Travel Programs an orientation evening will be held at Activ Recreation approximately one week prior to the holiday departing. This is an opportunity for you to meet your support staff and for the support staff to obtain relevant information about your support requirements. 3 Booking Procedure Calendar of Events 2017 Responsibiliy The provision of all Activ Recreation programs is subject to availability. To attend any Activ Recreation programs, you must have a current Customer Profile which will enable us to assess your support requirements, and an application form for the specific program you wish to attend. Activ Recreation takes no responsibility for the cancellation of programs beyond our reasonable control. This includes weather conditions, volunteer availability and guest cancellations. All policy standards can be accessed from Activ Foundation s website: Cancellation Conditions All cancellations must be forwarded in writing to Activ Recreation. If you cancel: 0-28 days before departure, you will be required to pay 100% of the cost of the program. If a replacement is found your cancellation fees will be reviewed by the Team Manager. Over 28 days before departure, your deposit will be forfeited and the Recreation Officer will attempt to find a replacement participant for the program. If a replacement is found, a reimbursement (less the deposit) will be issued. If a replacement is not found, the Team Manager will review the case and approve a value to the refund. If cancelling due to illness, you will need to provide a doctor s certificate. Activ Recreation can support you to lodge an insurance application. Cancellation conditions on the Travel Programs are program specific and will go through the travelkey cancellation policy. For more information, please see the Cancellation Procedure Recreation. travelkey terms and conditions travel key provide travellers with booking & advisory services that allow you to acquire a travel product from a third party travel provider. We act solely as an agent for the third party travel provider. By acquiring booking & advisory services from us, you agree that you have read and understood both these terms and conditions and the terms and conditions of the third party travel provider relating to the travel product. Please note all suppliers have different cancellation terms and conditions. Upon acceptance to participate on a program and paying a deposit, travellers agree to the terms and conditions applied by the supplier and travel key. travel key provide travellers with booking & advisory services that allow you to acquire a Travel Product from a Third Party Travel Provider. 4 Individualised Holiday Programs 2017 Weekenders Program 2017 Are you dreaming of that perfect holiday or getaway but not sure how to make it a reality? Activ Recreation now has the capability to help you organise your very own dream holiday for yourself, your family, and/or your support personnel. All you need is your imagination! Tap in to the recreation team s expertise and create a personalised travel program which suits your needs and your preferences. We can help you put together a holiday with as much or as little booked in to your itinerary as you wish, to destinations of your choosing, in time-frames you wish to go. With our knowledge of many great places to stay and visit, your opportunities are endless. If you are interested in discussing your individualised travel program, please contact Activ Recreation on (08) You will be matched with an experienced Recreation Officer who will help you with the planning and booking of your perfect getaway. Weekenders offer participants a variety of activities that run over a weekend. Programs depart on Friday and return on Sunday afternoon to Activ Recreation. Activ Recreation continues to offer popular programs from previous years, along with some new programs. Where possible, Activ Recreation offers positions on Weekenders to those with higher support needs. Program prices include transport, accommodation, meals, support personnel and most activities. Spending money will be required for snacks, souvenirs and any other personal purchases made on the program. 5 Special Events 2017 Special Events 2017 Our Special Events program coincides with annual events on the Western Australian calendar, such as sports and music festivals. Most departures will be either a Friday morning or afternoon depending on the event. Special Events run for the duration of a weekend. Cost includes transport, accommodation, meals, event entry fees, support personnel and some activities. Spending money will be required for the purchase of drinks, snacks and souvenirs. Due to the nature of these programs, where large crowds may be present, Activ Recreation will be adhering strictly to support ratios and may require extra criteria to be met in order for the activity to run safely and be enjoyed by all. 7 Summer Camps 2017 WA Getaway Programs 2017 Summer camps are the biggest and most popular annual programs hosted by Activ Recreation. *Applications due by the 30 th of November 2016* Summer Camp 1 Reference code (SUMMER001) Date: Saturday 7 th to Wednesday 11 th January 2017 Pace: Fast Summer Camp 2 Reference code (SUMMER002) Date Saturday 21 st to Wednesday 25 th January 2017 Pace: Slow Each of the two camps, held at Point Peron, run for 5 days in January. The fantastic summer season along with great weather brings exciting, enjoyable, and relaxing times. Join us to discover the wonders of Western Australia Activ Recreation has put together a variety of holidays which visit the best destinations WA has to offer, from the beautiful beach resorts of Broome to the quiet tranquillity of the forests in the South West. Each holiday will showcase the best attractions and activities the area has to offer. Prices will generally include on-program transport, accommodation, meals and support personnel and some activities. Any additional expenses are the responsibility of the traveller (e.g. entry fees, souvenirs, etc.) Customers with a 1:1 support ratio will be offered a position if Activ Recreation can provide the required resources. All deposits and payments for these programs will be specified on your confirmation paperwork. The Summer Camps offer different programs; one being faster paced for the young at heart, and the second being a bit more relaxed and easy-going. We sometimes offer a limited number of places for people who require a 1:2 support ratio, however this depends upon our volunteer availability.
