1 Ishigaki Port Information: Ishigaki port is located in the southernmost of Japan, and surrounded by the deep emerald ocean. It is called the entrance of south in Japan which has regular line among Naha, Taiwan and other foreign countries. It opens to the north-west, and there are many reefs on the south side of Ishgaki Island. The reefs form a half-moon shaped barrier between Ishigaki Island and Taketomi Island, therefore only local fishing boats can enter the port from the south. Except for an area to the north-west, the port is largely shallow, and there are also separate reefs in the center, so the area suitable for anchoring is limited. Vessels navigating in the Ishigaki areas should prepare the following charts: Chart No. W 1206 Yaeyama Island W 1285 Approaches to Ishigaki W 1286 Ishigaki Harbor Only World Geodetic System(WGS-84)Charts can be used from April 2002 onward. Position for anchorage (by WGS-84 ) Please refer to BA Chart No and Japanese Chart No. W1285 /W1286/W1206 1) Quarantine Anchorage : (based on International Ton) for vessels of its Gr0ss Ton with tons or under Lat N / Long E 2) Nagura Bay Anchorage : for vessels of its Gross Ton over tons Lat N / Long E Lat N / Long E Anchoring Prohibition Area: Do not drop anchor within the area enclosed by the following points: 1

2 N E N E N E N E N E N E N E **** Attention: Around the Quarantine Anchorage there are many coral reef shallows, submarine electricity cables and water pipes, and recently there has an increasein groundings. Please proceed with extreme caution! At most 4 vessels can drop anchor in the quarantine anchorage area at the same time. Other vessels should stand by for quarantine in a safe area, like Nagura Bay. Position Report Report your position to agent via mail, fax or tel two or one hours before your arrival at anchorage and please kindly try to inform exactly arrival time.. Radio Pratique Message ( has changed as followings) Please kindly send the followings as below orders to our office after your departure from the last port within 24 hours before arriving Ishigaki 1) RPM ( Radio Pratique Message ) 2) Ship s Name /Flag 3) Name of Master 4) Port & estimated time of arrival 5) Name of departure port with date and time. 6) Authority & date of issue of the Sanitary Control Exemption or Control Certificate, and requirement on re-inspection.(or deratting cert.) 7) Has ship visited an affected area identified by World Health Organization. 8) Last ports of call from commencement of voyage with dates of within past 30 days 9) Number of crew & passengers. 10) Has any person died onboard during the voyage other than as a result of accident. 2

3 11) Is there on board of has there been any case of disease suspected to be of an infectious nature. 12) Has the total number of ill passengers during the voyage been greater than normal (In a passenger boat, has any case of outbreak occurred on board) 13) Is there any ill person on board now 14) Was a medical practitioner consulted 15) Are there any conditions which may lead to infection or spread of disease within 30 days before arrival. 16) Has any sanitary measure been applied on board. 17) Are there any stowaways on board. 18) Is there a sick animal or pet on board 19) Has any person and (or) good transferred from an other ship within 30 days before arrival 20) Is the ship s doctor on board Note : In case None is applicable for items from 10 to 19 or 10 to 20, you may state as follows, 10 to 19 None.Al or 10 to 20 None. Please kindly advise Q tine officer the followings since H5N1-influenza-infected birds happens in some countries and areas. 1. Is there any person giving fever more than 38 and acute respiratory symptoms on board? 2. Is there any person giving unidentified pneumonia and dyspnea on board? 3. Is there any unidentified dead on board? 4. Is there any person who has had contact with the dead birds or H5N1-influenaz-infected birds or suspected) include cock, duck, turkey, quail etc.) within the past 10 days on board? 5. Is there any person who has had contact with influenza (H5N1) patients (include suspected cases ) within the past 10 days on board? You can just add the answer to RPM as : 1 To 5 NONE 3

( 1 Piece) 6) Sailing condition report CIQ Normal Working Hours: Weekdays :30 18:00 (Local Time) Weekends & Holidays :00 12:00 (Local Time) In order to have quick and smooth clearance procedure, the vessels should better arrive Ishigaki before 17:00LT during weekdays and 11:00LT on weekends / holidays otherwise she will have to stay until next day for formality.
