The following information provides details about the camp in which you are enrolled. For additional information, please check our web site at or contact our office. 2 Welcome to the Cal Youth & Outdoor Programs (CYOP) at Washington Elementary School in Point Richmond. Please read the following information. If you have any addition question please contact our office. We look forward to having your child enrolled in our summer camp. Check-In Check-In/Pick-Up Information Morning (AM) Sports Camp Check-In On the first day of camp, campers should check themselves into the CYOP Morning Camp at 8:45am (9am for Wed. All-Day camp) at the check-in area located at Washington Elementary School (Sports Camp drop off is located on East Richmond Drive on the south side of the school). Participants will then be directed to a meeting area where they will attend an orientation session and to meet their group leaders. Our staff greet the campers, so it is not necessary for parents to accompany their children to the meeting area after their children are checked-in. Please note that participants are not admitted into the program prior to the starting time of the camp for which he or she is registered. All participants arriving after 9:00 am must check in with the Camp Coordinator. If arriving late, parent may be asked to escort child to their instructor and activity area. Afternoon (PM) Sports Camp Check-In Parents do not need to check in afternoon sports participants with CYOP. ATDP instructors escort campers attending morning ATDP classes to the CYOP program area at lunchtime. Counselors direct the campers to a meeting area to attend an orientation session and meet their group leaders. Extended Care Morning Morning Extended Care begins at 7:30am. Campers should check-in at the ATDP Sports Camp general check-in area. After checking in, participants will be directed to the Extended Care room. Children, with morning ATDP classes, are walked to the ATDP meeting area by CYOP staff. Children enrolled in the morning sports program are walked to their group leader. Extended Care - Afternoon Children enrolled in the Extended Care-PM program remain in the care of the CYOP staff from 4:15pm - 5:30pm. Checkout Information Please note that the following checkout information only applies to those campers enrolled in the afternoon sports, Wednesday camps, and afternoon Extended Care programs. Checkout: Self-Checkout Children with a self-checkout option are allowed to leave on their own at 4:15pm (Extended Care participants leave at 5:30pm), and are required to leave the checkout area, as the CYOP staff no longer assume responsibility for them. Self check-out students are released out of the red gate facing East Richmond Drive at 4:15pm or out of the Sports Camp check-in table if attending extended care. Signed Checkout The CYOP instructors supervise participants until 4:15pm (Extended Care participants until 5:30pm). Children are kept in the checkout area until the designated checkout time and until an authorized person arrives to sign the child out of camp.

3 ID Checkout Staff only release campers to the person(s) designated on enrollment form. The authorized pick up person(s) must bring a photo I.D. if this photo option was selected on their form. * To ease the congestion during this time, parents should remain outside of the camp area, until the designated checkout time. Early Check-Out If leaving early, an authorized sign-out person must report to the Camp Site/Office to sign-out the camper or an early release permission note (see below) may be sent and the child may sign him or her self out. An early release permission note needs to include the following information: the camper s name the time and date the camper needs to leave permission for the camper to sign him or her self out parent or legal guardian signature. Late Pick-Up We understand that problems and disruptions sometimes occur during a parent's day and that schedules are thrown off. Nonetheless, after a 10 minute grace period, we must assess a late fee of $1 per every minute child is not picked up by. Payment must be made at the time of pick-up. Special One-Day Extended Care We realize that a one-day special extended care session may be needed. We ask that you notify the camp coordinator 24 hours ahead of time. Fee for one-day extended care is $15 payable to UC Regents. Carpool Lists If you agree to be on this list, your phone number and address are put on the carpool list. The lists are usually available one week prior to the start of camp. You may receive the list via . If you would like to be added to the carpool list, or to receive the list via , contact scrainfo@berkeley.edu or the Enrollment Center at (510) BART The CYOP does not provide escort/transportation service to and from the Bart station. Please contact the ATDP office for information and transportation to and from BART. Change of ATDP Class or Classroom Assignment Please notify CYOP Camp Coordinator, in writing, of any changes to camper s ATDP class or class meeting. Activities Campers participate in three to four activities each day for three weeks. We offer a choice of activities each hour that include either a traditional sport (basketball, baseball, etc.), a recreational one (kickball, yoga), or fine art activity (dance, art, etc.). Participants will complete an activity request form on day 1 of camp with the assistance of our staff. While we work hard to enroll students into all of their 1 st choice activites, we cannot guarantee this. Our sports and recreation programs may be very active. Attire & Equipment All sport equipment is provided. Clothing, equipment and supplies should have the child's name clearly printed on them. Please wear or bring: Tennis or running shoes (athletic shoes). No open toed or heeled shoes. Athletic apparel (sweatshirt for cold days) Plastic water bottle Hat or visor recommended Sunscreen. Participants should wear and/or bring sunscreen to camp for sunny days. Staff may apply additional sunscreen. If participant is allergic to sunscreen, or you do not want sunscreen applied to him/her, please notify Enrollment Center in writing prior to the first day of camp (see sunscreen section below). Additionally, please notify Camp Coordinator that additional sunscreen should not be applied. Not Allowed: Weapons controlled substances, pets, portable radio/cassette/cd players, trading cards (Pokemoin, baseball, etc,) electronic game devices, and cell phones.

4 Lost and Found: Lost and found is donated to a local charity two weeks after the camp session ends. Please check lost and found on a regular basis as many items find their way to our camp lost and found. Lunch/Snack Snack and lunch breaks are scheduled throughout the day. Participants should bring a hearty lunch, as well as, nutritional snacks (e.g. crackers, fruit) and beverages. Refrigeration is not available. Participants do get hungry from the various activites throughout the day, therefore please provide an extra snack(s) for the camper to have if needed. Medical Information / Illnesses In the event that your child becomes ill or an emergency arises, the camp staff will make every effort to contact you, or your emergency contact, if you are unable to be contacted. In situations where the camp staff deem it necessary, your child may be transported to a nearby hospital for treatment. Medications/Inhalers A parent completed medication form and current prescription bottle must accompany all medication taken by a child. This includes prescription and potentially hazardous, if misused, over-the-counter medications. Inhalers may be kept with the camper but still require a current prescription or Camp Medication Form. To request a Medication Form, phone (510) or scrainfo@berkeley.edu. You may also download the form from our web site. Sunscreen Camps are outdoors and there is limited shade. We encourage the use of multiple sun protection methods for maximum effectiveness. In addition to wearing a broad-spectrum sunscreen with a SPF of 25 of higher, we also recommend wearing protective clothing such as a hat. If your child needs assistance or needs close monitoring, or you do not want sunscreen applied to your child, notify the Enrollment Center in writing prior to the first day of camp. Camp Rules Each camp will have specific rules related to their program. Below is a general list of rules. Please make sure your child is familiar with them. 1. Respect other participants, instructors, facilities, and equipment. 2. Always listen to instructor. Show consideration when teacher and others are speaking. 3. Teasing and put-downs are not tolerated at camp. 4. Use appropriate language - No cursing. 5. Stay with your group. Never go anywhere alone. If you are not sure where to go check-in with the Camp Coordinator. Never go anywhere alone or without permission. 6. Trash is EVERYONE S responsibility. Throw your trash away. If you see trash pick it up. 7. No gameboys, CD players, or cell phones are to be used during camp. 8. No weapons, drugs, tobacco, or alcohol permitted at any time. 9. No pets may be brought to camp, or to camp check-in or checkout. Behavior Guidelines and Consequences In order to provide a pleasant environment and to ensure the safety of all our staff and participants, destruction of equipment and property and disruptive and/or dangerous behavior by campers are not allowed. If a camper should engage in such behavior our camp staff usually follow the disciplinary steps outlined below. If the behavior threatens his or her safety or the safety of others, the camper will be removed from the program. In some cases, a child may be suspended or dismissed immediately from the program. When a child is suspended or dismissed from camp, a refund is not given. Level One - Staff Intervention: The camp staff handles minor rule infractions and misbehavior immediately. Consequences are specific to the infraction. Level Two - Parent Intervention: If a camper s behavior does not respond to staff interventions, parents are contacted to discuss ways to correct the child's behavior. Level Three - Sent Home: If the behavior occurs again, or there are additional incidents, the camper will be sent home for the remainder of the day. At this point the parents are notified that one more occurrence will result in

5 dismissal from the program. Level Four - Dismissal: If the camper s behavior occurs for a third time, or there are additional incidents, the camper will be dismissed from the program. While most inappropriate behavior consequences progress through the above stages, some behavior may result in immediate dismissal from camp. Examples include, but are not limited to: Anything that endangers the health and safety of others. Possession of or use of illegal drugs, alcohol, cigarettes. Possession of firearms, weapons, explosives, etc. Damage or destruction of property. Theft. Fighting. Leaving camp boundaries or property without permission. Damage or Loss of Property Child s parent is financially responsible for all damages caused by child. Parent will be billed for any damage to facilities, equipment, or for items missing from program. Emergencies & Evacuation Plans Should an emergency or natural disaster occur at or near camp that results in the evacuation of campers, each program has an initial evacuation and gathering area (usually the blacktop area west of Washington Elementary School). This site may change depending upon the location and nature of the disaster. We will do everything reasonable to ensure the physical and emotional safety of your child. Please refer to the phone numbers and web sites below for pick-up information. In the event of an emergency resulting in evacuation, the campers will be supervised until the end of their program day. At the end of the camp day, campers who are on the Leave-On-Own Plan will be allowed to leave if the staff is confident the camper has a safe place to go. In an emergency, staff may have to decide whether or not to send your child home with someone (such as a neighbor) who is not on your pick-up list. For this reason it is important to submit names of people NEVER allowed to pick-up your child. Please take into account persons who would most likely pick up your child (i.e. person closest in proximity to camp) in an emergency and add them to your child's authorized pick-up list. Discuss with your child, in advance, what would happen in the event a natural disaster occurred during their camp day. Recommended Family Planning for an Emergency We highly recommend that parents develop a family emergency plan for when your family is not at home. The American Red Cross has information on Family Emergency Plans at Please take into account persons who would most likely pick-up your child (i.e. person closest in proximity to camp) in an emergency and add them to your child's authorized pick-up list. Discuss with your child, in advance, what would happen in the event a natural disaster occurred during their camp day. Who would pick

6 them up? Where would they go? Keep the emergency phone numbers inside your child s backpack and keep the camp s emergency information listed above with you. Cancellations/Refunds/Transfers Questions about enrollment, receipts, dependent care letters, refunds, etc., are handled by the Enrollment Center at (510) or scrainfo@berkeley.edu. A written request for a transfer, credit, or cancellation must be received in the Strawberry Canyon Enrollment Office no less than 30 days prior to the first day of camp to qualify for a refund, transfer, or credit for those who paid the full amount for camp. Those who have ONLY paid the 50% non-refundable deposit and need to cancel their enrollment/s are not eligible for any refund. Request for transfers, credits, or cancellations received prior to 30 days before the first day of camp are assessed a fee of $25 per child per camp. No refunds, credits, or transfers are issued for requests received within one month of the start of camp. Requests for transfer or credit in cases of illness or injury are considered with a doctor's note and letter requesting the exception. No medical requests will be considered after the first day of camp. Camps are not prorated nor session dates individualized. Participant substitutions are not allowed. In the case of a serious illness, or injury, a request for a transfer, credit or partial refund will be considered if the request is received by the first day of camp and includes: Note from a physician that states the participant is unable to participate in camp, date, physician s address, phone number and signature.
