Station MUST provide Haas Entertainment with final Trip Winner and Guest names by Thursday, May 1, 2014 at 12:00 Noon Central Standard Time. b. Station MUST completely fill in the attached Trip Winner Information Form and fax or it to Haas Entertainment. c. Any station needing to secure lodging accommodations for their trip winners before Friday, April 17 th or after Sunday, April 20 th at Dallas Renaissance Hotel needs to call Marriott/Renaissance reservations line directly at (888) to arrange the reservations and payment; d. IRS 1099 Forms are to be provided to the Primary Trip Winner by each sponsoring station; e. Station, by acceptance of this trip, agrees to indemnify and hold harmless Haas Entertainment and Academy of Country Music and their owners, employees, trip personnel, representatives, agents, directors, officers, successors and/or assigns from any and all claims or liability including but not limited to such liability arising from any acts or omissions resulting in injury to body or property of contest winners while they are participating in the Trip including while in transit to and from the Dallas Metroplex area. f. Academy of Country Music Awards trip packages cannot be used in conjunction with a syndication company unless approved by the Academy of Country Music in advance. All requests must be in writing. 6 g. There is no market exclusivity for this trip package. The Academy of Country Music welcomes all Country Radio Stations and CBS Television Stations to participate; h. Acceptance of this trip does not necessarily make the station the official ACM station in the market. i. Station Guidelines are subject to change. GROUND PACKAGE GUIDELINES a. Each trip is for two (2) people: one (1) Winner and one (1) Guest; NO EXCEPTIONS. b. At least one (1) trip participant must be twenty-one (21) years of age or older at the time of the trip. Minimum age for 2 nd participant is thirteen (13) years of age. NO EXCEPTIONS. c. A parent or legal guardian must accompany any trip participant under eighteen (18) years of age. d. No parent or legal guardian may leave anyone under eighteen (18) years of age alone (or in the care of anyone else) at any time, event, venue or in the hotel. e. It is illegal for any Trip Participant under twenty-one (21) years of age to consume alcohol at any scheduled events and/or clubs, restaurants and venues where alcohol is served. f. Possession of and/or consumption of alcoholic beverages is not permitted on any ground transportation vehicle. g. No Contest Winner shall be allowed to use trip ground transportation vehicles other than as a passenger. h. The Trip must be taken from Friday, April 17 th through Monday, April 20 th. Trips not taken during and between these dates are subject to forfeiture without rescheduling; i. Lodging accommodations to include only room base rate, resort fee and applicable state & local taxes; j. The Dallas Renaissance Hotel is a Smoke Free Facility. No Smoking is allowed in Guest Rooms, Courtyards, Pool, Public Areas or Restaurants. k. There is a $ cleaning charge for smoking in a non-smoking room; l. Trip Winner is responsible for a refundable $ hotel deposit (cash or credit card) applicable to any hotel services desired other than those included in the trip (i.e. internet, telephone, in-room movies, room service, restaurant charges, etc). m. Trip participants are responsible for all of their meals, beverages and spending cash.

7 n. Airport Shuttle Buses will run from 9:00 AM until 6:00 PM on Friday, April 17 th from Dallas Love Field (DAL) and Dallas/Fort Worth International Airport (DFW) to Dallas Renaissance Hotel; o. Airport Shuttle Buses will run from 6:00 AM until 6:00 PM on Monday, April 20 th from Dallas Renaissance Hotel to Dallas Love Field (DAL) and Dallas/Fort Worth International Airport (DFW). p. Trip Participants arriving or departing before or after scheduled Shuttle Bus hours are responsible for the method and payment of their transportation to and/or from Dallas Renaissance Hotel. q. Shuttles to Dallas West End will run from 10:00 AM until 5:00 PM and from 10:00 PM until 1:00 AM on Friday, April 17 th - from 10:00 PM until 1:00 AM on Saturday, April 18 th - from 9:00 AM until 3:00 PM and from 11:00 PM to 1:00 AM on Sunday, April 19 th and from 9:00 AM until 4:00 PM on Monday, April 20 th. r. Shuttles to Forth Worth Stockyards will run from 9:00 AM until 4:00 PM on Saturday, April 18 th and 9:00 AM to 2:00 PM on Sunday, April 19 th. s. Shuttles to ACM Party for a Cause will run from 11:00 AM until 12:30 AM on Friday, April 17 th and from 11:00 AM until 12:30 AM on Saturday, April 18 th. t. Shuttles to ACM Awards Show will depart Renaissance Hotel at 4:00 PM. Shuttles will return to Renaissance Hotel at approximately 10:30 to 11:00 PM. u. The Trip Check-In and Ticket Distribution Desk at Dallas Renaissance Hotel (Main Lobby) Hours are from 9:00 AM to 7:00 PM on Friday, April 17 th. v. The Trip Assistance & Hospitality Desk Hours at Dallas Renaissance Hotel (Main Lobby) are from 9:00 AM to 1:00 AM on Friday, April 17 th from 8:00 AM to 1:00 AM on Saturday, April 18 th from 8:00 AM to 1:00 AM on Sunday, April 19 th and from 6:00 AM to 6:00 PM on Monday, April 20 th. w. Only the Primary Trip Winner may pick up the event ticket package and laminates & lanyards at the Trip Check-In Desk. Photo Identification must be presented. x. All ACM Awards tickets will be in the Upper Section of AT&T Stadium; y. All ACM Party for a Cause tickets are General Admission. z. Only the Trip Winner may register for the Dallas Renaissance Hotel room. Photo Identification must be presented. aa. Scheduled trip events are optional. Attendance at events is not mandatory. bb. Any trip participant choosing not to participate in scheduled events shall be solely responsible for securing of and payment for their own transportation, admission, and other fees and expenses at the unscheduled event of their choosing. cc. Once distributed, misplaced, lost or stolen tickets to any scheduled event cannot be replaced. NO EXCEPTIONS. dd. Event admission tickets, lodging accommodations, ground transportation and laminates & lanyards are not transferable.

8 ee. Each trip participant must possess two (2) valid forms of personal identification, including one (1) picture I.D. ff. Due to the many physical activities encompassed by trip events, winners must be in at least such general good health as to be able to participate in scheduled trip events. gg. During the course of the Trip, if any Event is canceled or postponed by any occurrence not occasioned by the conduct of Haas Entertainment or the Academy of Country Music, whether such occurrence be by act of God or the result of war, riot, sovereign conduct, or the act or conduct of any person or entity not party to the Trip, then Haas Entertainment and the Academy of Country Music shall be excused from any and all obligations pertaining to the canceled or postponed trip event. hh. This trip is rated G for General Audiences. Disorderly behavior and/or illegal conduct may result in revocation of trip winner and their guests hotel accommodations, airport shuttles, event tickets and participation in scheduled trip events. ii. Each Trip Participant must each execute a release & waiver forms in Section Five (5) of the Trip Winner Kit before commencing on the trip, releasing Haas Entertainment and The Academy of Country Music and their owners, employees, trip personnel, representatives, agents, directors, officers, successors and/or assigns from any and all claims or liability including but not limited to such liability arising from any acts or omissions resulting in injury to body or property of contest winners while they are participating in the Trip including while in transit to and from the Dallas Metroplex area. jj. Haas Entertainment must be in possession of completed Trip Winner and Guest s Forms from Section 5 of the Trip Winner Kit before Trip Winner can check into the trip and pick up their event ticket package. kk. By their participation in this promotion all Trip Participants automatically authorize Haas Entertainment, Sponsoring Radio Station and the Academy of Country Music's use and/or other exploitation, including promotional and/or commercial, of each and every Trip Participant's name, image, voice and/or likeness in audio, video, website, print and/or other media. ll. Trip Winner is responsible for any and all applicable federal, state and local taxes mm. IRS 1099 Forms are to be provided to trip winner by sponsoring station. nn. All shuttle, events, artists, dates, venues, times and ground package guidelines are subject to change. AIRFARE PACKAGE GUIDELINES a. Sponsoring Radio Station, Haas Entertainment or Primary Trip Winner will reserve, book and secure roundtrip coach class airline transportation for the Trip Winner and their Guest utilizing the Travel Stipend provided by Academy

9 of Country Music. This Stipend is to help DEFRAY Airfare Costs and is not a guarantee to cover all Airline Costs; b. FINAL Trip Winner AND Guest names must be provided to Haas Entertainment by Thursday, May 1, 2014 at 12:00 noon Central Standard Time; c. Trip Participants are responsible for transportation to and from the airport they will be flying out of and back in to and if applicable any airport parking fees; d. Trip Participants will be flown into and out of Dallas Love Field (DAL) and/or Dallas/Fort Worth International Airport (DFW); e. Dallas Love Field (DAL) is a Local Airport located in the city of Dallas and is approximately 6 miles from the Renaissance Hotel. Dallas Love Field is serviced by Southwest, Delta and United Airlines; f. Dallas/Fort Worth International Airport (DFW) is an International Airport located between Dallas and Fort Worth and is approximately 21 miles from the Renaissance Hotel. Dallas/Fort Worth International is serviced by Alaska, American, American Eagle, Delta, Frontier, Jet Blue, Spirit, Sun Country, United, US Air and Virgin America. DFW IS NOT serviced by Southwest Airlines. g. All airlines EXCEPT SOUTHWEST are now selling seats for Friday, April 17 th through Monday, April 20 th. Southwest seats are projected to go on sale on Monday, October 27 th. h. Trip Participants may be required to fly out of and back into the closest MAJOR market airport to the city of the their Sponsoring Station; i. Trip Participants airline tickets are booked on the open market. Roundtrip coach class airfare will include base fare, taxes, passenger facility charges and security fees; j. Trip Participants are responsible for any baggage charges. Most airlines now charge $25.00 to check one (1) piece of luggage and $35.00 for a second (2) bag EACH WAY; k. Flights into Dallas Love Field (DAL) and/or Dallas Fort Worth International (DFW) will be ticketed for travel on Friday, April 17 th and will be scheduled to arrive between the hours of 9:00 AM and 6:00 PM; l. Flights out of Dallas Love Field (DAL) and/or Dallas Fort Worth International (DFW) will be ticketed for travel on Monday, April 20 th and will be scheduled to depart between the hours of 6:00 AM and 8:00 PM; m. Once Trip Participants airline tickets are purchased the passenger names can not be changed; n. Airline Tickets are not transferable; o. Trip Participants are required to fly on the same flights together. If any Trip Participant wishes to fly out of or back at a different time or date or from a different city, then the Trip Participant is responsible for any difference in airfare; p. Trip Participants may be required to fly on the flights that have been prereserved by and/or for their Sponsoring Station. If any Trip Participant wants a change in airlines, flight times and/or connecting city, then the Trip Participant

10 is responsible for any difference in airfare and (if applicable) itinerary change fees; q. If a Trip Winner is pregnant, travel within 4 weeks of the delivery date or within 7 days after delivery requires a medical certificate as well as clearance from the airline s special assistance coordinator; r. Airline Electronic Tickets will be ed directly to the trip winner; s. Trip Participants are responsible for calling the airline they will be traveling on to book their seat assignments, provide a telephone contact number and, if applicable, find out baggage charges and arrange for any special needs; t. If an Airline delay or cancellation occurs due to circumstances beyond an airlines control (i.e. weather, air traffic control etc.) and passengers can not make it to their destination on the day scheduled, most airlines policies state that it is up to the passengers to pay for any additional costs including but not limited to hotel accommodations, ground transportation and/or meal costs in the city the passengers are delayed in. Passengers should ask an airline agent for discount coupons; u. If an Airline delay or cancellation occurs due to circumstances within an Airlines control (i.e. aircraft swaps, crew delays, etc.) and the passengers can not make it to their destination on the day scheduled, most airlines policies state that the Airline usually will pay for any additional costs including but not limited to hotel accommodations, ground transportation and/or meal costs in the city the passengers are delayed in. Passengers should ask an airline agent for details; v. Individual Airline Rules will be included on each passengers Electronic Ticket; w. Individual Airline terms, conditions and restrictions also apply to airfare guidelines; x. If the primary Trip Winner lives with-in a 350 miles of Dallas then airline transportation may not be included; y. Airfare package guidelines are subject to change; TRIP WINNER TAX GUIDELINES IRS TAX OBLIGATIONS The primary trip winner needs to understand their IRS tax responsibilities AND obligations that go along with the acceptance of this prize. Please consult an accountant or call the IRS if you have any tax questions in regards to the requirements that go along with this prize. IRS DEFINITION OF TRIP WINNER S TAX LIABILITY If a person wins a prize by a lucky number drawing, television or radio quiz program, beauty contest or other event, they must include it in their income. For example, if a person wins a $50 prize in a photography contest, they must report this income on form 1040.

11 If a person refuses to accept a prize, they do not include its value in their income. Prizes and awards in goods or services must be included in their income at their Fair Market Value. (800) is a direct line to the Internal Revenue Service Specialty Tax Department. IRS DEFINITION OF FAIR MARKET VALUE What a willing seller can get from a willing buyer with no pressure to buy or sell by either party and with both parties knowing all the facts and circumstances. TRIP WINNER S GUEST TAX LIABILITY If a Trip Winner takes a guest on a trip they have won, only the trip winner is responsible for taxes. The only exception is if a prize is awarded in two names. ESTIMATED FAIR MARKET VALUE The ESTIMATED Fair Market Value of the Dream Trip of a Lifetime to the Academy of Country Music Awards Ground Package AND Airfare Package is to be between $2, and $4, (depending on airfare package costs). HAAS ENTERTAINMENT Haas Entertainment does not provide IRS 1099 tax forms to the Trip Winner as Haas Entertainment did not directly gift the prize to the Trip Winner. Haas Entertainment produces the "Dream Trip" for the Sponsoring Radio Station FORMS Sponsoring station must provide the Primary Trip Winner completed IRS 1099 Forms by January 31, TAX RETURN RESPONSIBILITIES Even if a Trip Winner does not receive a 1099 form from their Sponsoring Station, the Trip Winner is still obligated to declare the Ground Package AND Airfare Package prize Fair Market Value as income on their 2015 tax return. TRIP EVENTS Airport Greetings We will be picking up trip participants at BOTH Dallas Love Field (DAL) and Dallas/Fort Worth International Airport (DFW). Specific Airport Meeting Locations and Shuttle Times will be posted in your Trip Winner Information Kit.

12 Haas Entertainment Trip Staff telephone numbers will be posted in the Trip Winner Kit, if you are having challenges locating our trip staff and/or shuttle bus pick-up locations. Airport Shuttles to Renaissance Hotel On Friday, April 17 th the first shuttles will depart Dallas Love Field (DAL) and Dallas/Fort Worth International Airport (DFW) at 9:00 AM and the last shuttles will depart the airports at 6:00 PM. A trip staff member will be on every shuttle to answer questions and assist trip participants with any travel challenges. Dallas Renaissance Hotel Trip Participants will be staying at the Dallas Renaissance Hotel featuring magnificent facilities, amenities and restaurants. The Renaissance Hotel is located at 222 North Stemmons Freeway, Dallas, Texas Their telephone number is (214) The Renaissance Hotel will not have a record of you as a guest until right before the trip. Your reservation is secured in our Group Block. If you have any special rooming needs please let us know. Renaissance Hotel Check-In The Dallas Renaissance Hotel check-in time is 3:00 PM. If you arrive before 3:00 PM or if your room is not yet available, please check your luggage at the Bell Desk and feel free to take a shuttle to the Dallas West End Entertainment & Shopping District. Additional Nights at Renaissance Hotel Any Trip Winner needing to secure additional lodging accommodations (before Friday, April 17 th or after Sunday, April 20 th ) at the Dallas Renaissance Hotel needs to call the Marriott/Renaissance reservations line directly at (888) to arrange the reservations and payment; Trip Check-In & Ticket Distribution We will have a special counter in the Renaissance Hotel Main Lobby to check participants on to the trip, distribute event admission tickets, laminates & lanyards and provide general

13 trip assistance & guidance. This desk will be staffed on Friday, April 17 th from 9:00 AM to 7:00 PM. After 7:00 PM Tickets will be distributed at our Hospitality & Assistance Counter in the Renaissance Hotel Main Lobby. Suggested Dress Code Dress casual and comfortable for most scheduled events and activities. Be ready for extremes from outdoor heat to indoor air conditioning. For ACM Party for a Cause: 50 Years of Music & Memories - Concert/informal clothing is recommended; shorts, polo shirts, t-shirts, western wear, etc. For ACM Awards - "Country Chic" clothing is recommended; a wide range from Western Wear to Business Casual to Formal Wear. No T-Shirts, Baseball Caps, Shorts or Flip-Flops are permitted. ACM Party for a Cause PARTY FOR A CAUSE: 50 Years of Music & Memories will be held at Globe Life Park (Home of the Texas Rangers Baseball Team) in Arlington. This outdoor music festival will include LIVE country music performances, interactive experiences, a family-friendly country marketplace with food trucks and vendor booths and more. PARTY FOR A CAUSE: 50 Years of Music & Memories: Friday AND Saturday - Noon to Midnight. ACM Awards The 50 th Annual ACM Awards will be held at AT&T Stadium (Home of the Dallas Cowboys) from 7:00 PM to 10:00 PM. All seating will be in the upper levels. Shuttle Times to Dallas West End Our shuttle buses will loop from the Dallas Renaissance Hotel to the Dallas Historic West End Entertainment & Shopping District on Friday, April 17 th from Noon to 5:00 PM and from 10:00 PM until 1:00 AM. Saturday, April 18 th from 10:00 PM until 1:00 AM; Sunday April 19 th from 9:00 AM until 3:00 PM (there will be no shuttles during the ACM Awards) and from 11:00 PM until 1:00 AM and Monday, April 20 th from 9:00 AM until 5:00 PM.

Shuttle Times to Fort Worth Stockyards Our shuttle buses will loop from the Dallas Renaissance Hotel to the Fort Worth Stockyards on Saturday, April 18 th from 9:00 AM until 4:00 PM and from 9:00 AM until 2:00 PM on Sunday, April 19 th. Shuttle Times to ACM Party for a Cause Shuttle Buses will run from the Dallas Renaissance Hotel to Globe Life Park on Friday and Saturday between the hours of 11:00 AM and 12:30 AM. Buses will depart from the Renaissance Hotel every hour at the Top of the Hour (i.e. 11AM, Noon, 1PM, etc). Buses will depart Globe Life Stadium every hour at the Bottom of the Hour (i.e. 11:30 AM, 12:30 PM, 1:30 PM, etc.). Final shuttle times will be posted in the Trip Winner Kit. Shuttle Times to ACM Awards The 50 th Annual ACM Awards Show will start at EXACTLY 7:00 PM Central Standard Time. Buses will be departing the Dallas Renaissance Hotel at 4:00 PM. Buses to ACM Awards will not loop - they will remain parked at AT&T Stadium until the show is over.

15 Please allow up to 90 minutes from the time you depart the Dallas Renaissance Hotel until you arrive at AT&T Stadium. The venue is located approximately 20 miles from the Renaissance Hotel. Upon arrival at the stadium there is an approximately 20 minute walk from the Venue Parking Lot to the Admission Gates, then approximately 20 minutes to get to your seats. Buses will be departing AT&T Stadium immediately following the ACM Awards. Buses will shuttle to the Renaissance Hotel, then start looping to the Dallas West End Entertainment & Shopping District. Shuttles Back to Airports On Monday, April 20 th our shuttle buses will loop from the Dallas Renaissance Hotel to Dallas Love Field (DAL) and Dallas/Fort Worth International Airport (DFW), with the first shuttle starting at 6:00 AM and the last shuttle leaving at 6:00 PM. Please plan on arriving at the airport at least 90 minutes before your flight is scheduled to depart.
