Incident Management Major Impact Traffic Incidents and Mitigation 1/1/2015 (Thursday) Time: 8:16 pm 10:51 pm ***Fatal*** Location: St. Louis County Southbound I-55 before Bayless Avenue Event: Two-Vehicle Crash Estimated Initial Impact: Non-Rush Hour: 4 of 4 travel lanes affected equals (=) 100% Action: Motorist Assist responded along with MSHP, St Louis County Police, Lemay Fire emergency responders, Squires and AAA Towing. The TMC messaged for the event using roadside messaging and alerts advising motorists of the closure. The EOC, Community Relations and Motor carriers were notified. Result: Vehicle 1 was stopped from an earlier crash with the passenger standing outside of the vehicle. A second vehicle was unable to avoid striking vehicle 1 and the passenger. Passenger was transported to the hospital and pronounced after arrival. Crash occurred in two left lanes with all lanes closed for crash investigation. Traffic was diverted off I-55 to Weber Road. MSHP requested oil dry as part of cleanup before progressively re-opening roadway. Incident Management Major Impact Traffic Incidents and Mitigation 1/2/2015 (Friday) Time: 5:28 am 7:44 am ***Weather Related*** Location: St. Louis County Eastbound Route 364 before Maryland Heights Expressway Event: Multi-Vehicle Crash Estimated Initial Impact: Rush Hour 5 of 5 travel lanes affected, equals (=) 100%. Action: MoDOT Emergency Responders, MoDOT Maintenance Maryland Heights Police, EMS and Doc s Towing responded to the event. TMC posted alerts and advanced warning messages advising motorists of closure and to watch for ice on bridges and exits. Result: Light snow with icy conditions existed on overpass where multi-vehicle crash occurred. Maintenance was called-in to treat the roadway. Crash initially closed lane 5 and eventually all lanes were closed for treatment and vehicle removal. All 4 vehicle occupants were transported for injuries. Time of day, icy conditions and tow vehicle mechanical issues all contributed to lengthy clearance time. The crash caused a 1.5 mile queue at onset which grew to 3.5 miles during the rush hour. Lanes were progressively opened with a 40-45 minute queue when incident cleared. Incident Management Major Impact Traffic Incidents and Mitigation 1/8/2015 (Thursday) Time: 6:51 am 9:30 am ***Fatal*** Location: St. Louis City Westbound I-70 at West Florissant Avenue Event: Single-Vehicle Crash with Overturned Vehicle Estimated Initial Impact: Rush Hour 3 of 3 travel lanes affected, equals (=) 100%. Action: MoDOT Emergency Responders, St. Louis City Police, Fire, EMS, Medical Examiner and City Tow responded to the event. TMC posted advance warning messages advising motorists of 20 mph speed ahead and to use center lane. Result: Incident began with a vehicle traveling on West Florissant Avenue, attempting to enter Westbound I-70 striking the guardrail, overturning and coming to rest on Westbound I-70. Crash resulted in a fatality as the driver (not wearing a seatbelt) was ejected from the vehicle. Delay in clearing the incident was due to waiting on Medical Examiner arrival. City Police were in charge of the reconstruction. This incident was a major problem for mobility due to time of day (AM Rush). Traffic was being diverted from I-70 Westbound at West Florissant to Kingshighway Boulevard back to I-70 Westbound via Bircher. Traffic was queuing for approximately 6 miles with a 20-25 minute additional travel time. There was a 16 minute queue once all lanes were opened. Incident Management Major Impact Traffic Incidents and Mitigation 1/10/2015 (Saturday) Time: 2:40 am 6:46 am ***Fatal*** Location: St. Louis County Southbound I-170 at Delmar Boulevard Event: Two-Vehicle Crash Estimated Initial Impact: Non-Rush Hour 3 of 3 travel lanes affected, equals (=) 100%. Action: MoDOT Emergency Responders, University City Police, Fire and EMS, Olivette Police and Hartman tow responded to the event. TMC posted advance warning messages advising motorists of closure and to seek an alternate route. Result: This was a wrong way driver incident that resulted in a fatal fire. Wrong way driver entered Southbound I-170 at Delmar and traveled a total of 60 before crashing. Driver of the vehicle traveling Southbound on I-170 was pronounced on the scene by EMS. Vehicle was taken to firehouse for extrication. All lanes of Southbound I-170 were closed and traffic diverted off onto Olive. Both vehicles sustained heavy damage, spilling fluids, and spreading debris across all lanes of traffic including Northbound I-170 lanes. Incident was of long duration due to accident reconstruction and the cleanup of the large amount of fluids spilled on the roadway. The time of day/day of the week helped mitigate impact on mobility. Incident Management Major Impact Traffic Incidents and Mitigation 1/10/2015 (Saturday) Time: 11:09 am 1:27 pm Location: St. Louis County Southbound I-270 past Dorsett Event: Two-Vehicle Crash Involving Tractor Trailer Estimated Initial Impact: Non-Rush Hour 2 of 5 travel lanes affected, equals (=) 40%. Action: MoDOT Emergency Responders, MSHP, Maryland Heights Police, Fire and tow responded to the event. TMC posted advance warning messages advising motorists of closure. Result: Crash occurred on Southbound I-270 as the driver of the Tractor Trailer suffered a medical condition, lost control and then left the roadway on the right side. Vehicle struck two road signs and the debris from the tractor trailer struck another vehicle. Fire and EMS responded to the scene a second time to render emergency medical attention to the Tractor Trailer driver. Awaiting tow caused a delay in clearing the incident. Action: MoDOT Emergency Responders, MSHP, St. Louis County Police, Mehlville Fire and EMS, and K&K and Squires Towing responded. The TMC posted web alert messages warning motorists of closure and to seek alternate route. TMC notified Area Engineer, EOC, Motor Carriers, Community Relations and MoDOT Incident Management Coordinator. Result: This crash caused a secondary crash in the southbound direction. Vehicle 1 was traveling Northbound on I-55 and struck the center median and knocked a large chunk of the median wall into the southbound lanes where vehicle 3 struck the debris and overturned. Vehicle 2 struck the rear of vehicle 1. Injuries occurred to all three drivers and two passengers of vehicle 1. All Northbound lanes were closed for 65 minutes then the right shoulder was opened for traffic. No detour was implemented as traffic was moving on shoulder. Action: MoDOT Emergency Responders, MSHP, St. Louis County Police and Medical Examiner, Mehlville Fire and EMS and Miners Towing responded. The TMC posted web alert messages warning motorists of closure and to seek alternate route. MoDOT Incident Management Coordinator was notified. Result: Crash involved a tractor trailer with a blown tire trying to move to the right shoulder when a car traveling at high speed with flashers on struck the tractor trailer. The car wedged under the rear of the tractor trailer resulting in fatal injuries to the driver of the car. Separating the vehicles and reconstruction took a great deal of closure time. No queues or additional travel times occurred during incident. Action: MoDOT Emergency Responders and Maintenance, St. Louis City Police, Fire and EMS, and Tow responded. The TMC posted web alert messages warning motorists of closure and to follow detour route. TMC notified EOC, Motor Carriers, Community Relations, MoDOT Incident Management Coordinator and City Traffic Engineer. Result: Crash involved a car that struck guardrail, barrier wall, overturned and burst into flames. Both occupants of the vehicle suffered fatal injuries with one extrication and one ejection. There was roughly 40 foot of guardrail and 5 posts damaged. Maintenance assisted with traffic control and police shut down traffic at Memorial Drive setting up a detour to keep traffic flowing. Freeway Management NEW for 2015! The focus area for calculating the average 24 hour volumes was revisited and consolidated to the more urban interstates of the St. Louis District. For example, data is generated from detectors on I-44 east of MO 141 (previously included all available in the District). All baseline data for 2014 was recalculated with the revised detectors on routes as shown above. 21

Data Key Work Zones Whenever additional travel time is reported, this additional time shall be evaluated using this chart and adequate mitigation measures should begin within the parameters of this chart. TMC and field personnel are partners in the mitigation of traffic and notification of additional travel times should trigger the appropriate response from both parties. Chart is designed to be an additional travel time reference to establish parameters for gauging the travel impact through work zones and to provide guidance for taking mitigation actions. Travel times can be collected or reported via Blue Toad technology, field personnel, call reports, ITS, TMC cameras, etc. Major and Moderate impacts trigger an IAR (Immediate Action Required) event.
