1 Josephine County Public Health Department s-establishments Updated: 20, 2008 Includes only full service establishments A SCORE OF 70 TO 100 COMPLIES WITH HEALTH DIVISION STANDARDS A SCORE BELOW 70 FAILS TO COMPLY WITH HEALTH DIVISION STANDARDS scores are way of indicating overall management practices and have a direct bearing on the management of a facility. They are not a means of saying a facility is safe or unsafe. Facilities that consistently maintain high scores are indicators that the facility has a good management team, and that Health Division Standards are being followed. Facilities that fail to meet the minimum score of 70 fail to comply and are posted accordingly. If the facility fails to obtain a score of 70 or more during the reinspection the facility can be closed. Facilities that have failed to meet the minimum score and/or are temporarily closed are highlighted in yellow. CRITICAL VIOLATIONS INCLUDE: Personal hygiene is inadequate: hand washing, unprotected cuts, smoking, ill food handlers. Potentially hazardous foods held at improper temperatures. Raw or ready-to-eat foods are not properly protected from cross-contamination. Potentially hazardous food is improperly cooled or reheated. Raw animal foods are improperly cooked. Food is from unapproved sources or prepared at home. Presence of rodents or insects, facility not protected from the same. Food is unsafe, adulterated, or not honestly presented. Failure to clean and sanitize equipment, or utensils in contact with food. Equipment is not able to hold food at the proper temperature. Lack an approved water system including hot or cold water not being available. Toxic chemicals or cleansers not properly stored, used, or labeled. Re-inspections for critical violations are performed within 14 days for correction. Semi-annual inspection means an unannounced complete inspection conducted twice during the calendar year; one in each half of the year, but not less than 90 days or more than 270 days apart Quarterly inspections are conducted four times a year for facilities that have failed to maintain at least a score of 70 for three consecutive quarterly inspections.
