Post-cruise flight arrangements include a city tour, hotel day-room use and transfer to the airport for an overnight flight with arrival back in Los Angeles on April 29. Air deviations can be requested. There is a $100 per person charge along with any fare difference that may apply. Transfers and hotel day-room use are not valid on the dates deviated. 5 Continuing Education Recognition Awards The Professional Education Society Seminar fee is $495 for each physician, dentist, nurse or health related professional and $295 for each non-professional or family member attending the seminars. Seminars will be held in Raiatea, Bora Bora and Moorea. Physicians: This program has been reviewed and is acceptable for up to 15 prescribed hours by the American Academy of Family Physicians. Dentists: The Professional Education Society, as provider, is authorized to confer up to 15 hours of California Continuing Education credit units and 15 FAGD/MAGD credit hours with the Academy of General Dentistry. The Professional Education Society is an ADA CERP Recognized Provider. ADA CERP is a service of the American Dental Association to assist dental professionals in identifying quality providers of continuing dental education. ADA CERP does not approve or endorse individual courses or instructors, nor does it imply acceptance of credit hours by boards of dentistry. Professional Education Society designates this activity for 15 continuing education credits. Tax Deduction Nurses: The provider is approved by the California Board of Registered Nurses, Provider Number CEP 5428 and is authorized to confer 15 contact hours for this program. Although the California Board of Registered Nurses does not approve individual continuing education courses, providers are required by law to be approved by the Board. Reciprocity for continuing education hours for nurses is available in most states. Please check with your regulation board regarding requirements. This travel seminar in French Polynesia has been designed to comply with current IRS convention rules and deductibility requirements under Code Section 274(h)(1) as directly related to the conduct of the attendee s profession and meeting the reasonableness standard of the foreign seminar rules. The formal meetings will be scheduled with lectures and discussions. Seminars in conference rooms, clinics or hospitals will be conducted by foreign speakers as well as distinguished members of the Professional Education Society. Documentation verifying the scheduling dates, locations, hours of attendance, certificate of participation, and appropriate certificate of continuing medical, dental or nursing education will be provided at the completion of the conference. Consult your tax advisor for detailed information concerning deductibility of expenses related to your particular situation. Terms and Conditions Passports are required for travel. Book early to ensure your preferred choice of stateroom or suite! Deposits: A deposit of 20% is required at time of booking. Seminar deposit is $200 per person. Final Payment: Final payment is due by by January 13, Trip cancellation insurance is highly recommended; information will be sent with your deposit confirmation. Single Travelers: Single occupancy is available on a limited basis and, subject to availability at the time of reservation. Single fares are capacity controlled and may be withdrawn at any time. Third Berth: A limited number of suites are available and designated for triple occupancy. Third passenger sails free, however, the FREE air from Los Angeles promotion does not apply. Not Included: Any increases in air and airport taxes and security or fuel surcharges, travel insurance and all items of a strictly personal nature. Cruise Cancellation: All cancellations must be made in writing to the Professional Education Society and are subject to a $200 per person cancellation fee. Additional Paul Gauguin Cruises per person cancellation charges policy: From time of deposit to Dec. 22, 2011: $100 per person; From Dec. 23, 2011 to Jan. 21, 2012: 10% of fare; From Jan. 22, 2012 to Feb. 20, 2012: 20% of fare; From Feb. 21, 2012 to March 21, 2012: 50% of fare; After March 22, 2012: 100% of fare.
