For this reason we feel Camp needs to be a place for campers to come have fun and be relaxed. We know that it is stressful for any dog to be away from their family no matter where they stay. This is because they innately want to be with their pack. Since 1996, we have gone above and beyond to provide the best atmosphere possible for campers and will always strive to improve for those furry friends of ours. S. I thought there had to be other people that felt just like me, so the idea of Camp was born. I took into consideration and developed a unique routine for this industry. From the beginning, I only thought about how important it was for pack animals to have this environment, even though it was totally against the flow of a traditional kennel. 1

2 Our routine is Apples to Oranges in comparison: Our track record has proven over the years that our routine addresses both the physical and psychological needs of dogs, resulting in happier, better adjusted campers. Dogs do not need to be confined 20 hours a day when away from home, as this idle time causes stress and high levels of separation anxiety. We are not a kennel we are a benchmark facility specializing in interactive care. There have been many new organizations attempting to mimic Camp; however, no others have duplicated our meticulous routine or detailed facility design. We understand the logistical and financial reasons they have not; but we believe that the benefits the camper receives with our specialized routine and environment far outweigh the costs. These are only a few examples of the traits that set Camp Canine apart, and therefore it cannot be compared to other kennels, as it would be comparing apples to oranges. We start each morning at 7am. Everyone goes out for potty, like at home. They also have the opportunity for pack greetings and a little social time with their pals with no extra charges for this service. We feed the campers their own diet at no extra charge and monitor their eating. We have a detailed routine if someone is not eating. We don t feed a kennel diet as this disrupts their digestive system. Morning meal time is after morning potty. We administer any medication as instructed with no extra charge. We allow 1 hour after eating and then a full day of playtime starts for everyone. Some places rotate dogs in playgroups for short periods of time, cutting down on actual socialization. Throughout the day, campers are all cared for individually even though we accommodate many campers. Special needs are met, and we pay close attention to the happiness and comfort of campers in playgroup. There is a minimum of 4-6 hours of play in a day; we do not leave dogs inside all day. We do not charge extra for playtime. Campers that are not as fond of large play groups are given the opportunity to hang out with inside staff. The campers are never put in their rooms and left for the day or put in a confined space. We feed their evening meal after a minimum of one hours rest after playtime and then allow them to rest and digest before going out again. Clock watching with food intake is not a normal procedure with kennels. This procedure helps prevent tummy problems and issues like bloat. We record every camper s food intake and have a protocol for anyone not eating that includes hand feeding or adding approved things like yogurt. We do whatever is needed to help them be comfortable and get their nutrition. Recording times and allowing staff to spend time making the dogs comfortable at mealtime is unique to Camp. Beginning at 8 pm we take everyone out again for potty. In this pack group they are allowed to play and socialize to once again alleviate alone time. We go above the standard practice by allowing evening time outside. We provide necessary medical care and extra attention. We provide extra time for meals, hand feeding, adding yogurt, cheese, peanut butter to give pills, all without extra charges. Some other places charge for administering medicine. We also uniquely absorb the costs of the items that are used to help campers when they need it. Our staff finishes up around 10pm, then locks up and leaves for the night. We have a caretaker that sleeps on the premises and is here for major emergencies or just in case a camper needs to be checked on late at night (this does happen). We are happy that we are able to provide the peace of mind of having a 24-hour caretaker. We started this way in 1996 and the staffing routine has not wavered seven days per week, 365 days per year and never will. Even holidays are fully staffed and follow our normal routine. Our staff is all certified in First Aid, Emergency Care & CPR, and are all trained in canine behavior. The training is required of each employee to ensure the health and safety of each and every camper. We maintain a high standard of employees and hire only full time staff. We dedicate a considerable amount of time and financial resources into training for all staff. Staff is hired with a career in mind, or a career step into the animal world. We do not hire seasonal help for this reason. Our staff keeps up with a mountain of paperwork and reports. Everything from potty breaks, feedings, medications, and behavior are all recorded with times, dates, and staff initials. This is above and beyond what is required by licensing. Our facility is state of the art. The facility was designed by Camp s owner, Suzanne Keeys, and was custom built in We have over 14,000 square feet indoors with no chain link or kennels, 11 heat and air units, emergency backup power, 24 hour monitored security, a 24 hour recording surveillance system with 15 cameras, surgical grade flooring on all interior floors, built in steam cleaning, and that s just a few inside amenities. 2

3 Community Room: We have designed an innovative program for severe separation anxiety dogs that was introduced to our inspector with the NCAD in 2008 as an in place program. We tested for 10 years the community overnight rooms. With many of our current member s permission we provided community overnight environments for certain campers. Many campers that had severe separation anxiety were so stressed once the social time was over that they could not settle down in their individual rooms. This stress was concerning to us as we know that stress can bring on physical problems due to the emotional upset. So we set up an environment that was free of personal items and large enough to accommodate a few campers together. Over time 96% of the campers we put in the community room calmed down and were able to get a good night s sleep, free from the stress of being alone. The few campers that this did not work for were never aggressive or destructive and we discussed other options with their parents. This program is not in any way open to all campers; however, we have helped many campers alleviate severe separation anxiety. The only way community rooms are available is after we have observed the behavior of each camper enough times to feel comfortable with them being together over night. It is also discussed with parents and permission is given first. This is not offered anywhere else because of the expertise that is involved. Due to the highly social and active nature of Camp, it is not always the right fit for each dog. If we feel that your camper will not benefit from the structure of Camp, we will always be honest with you. STAFF: Camp Canine is a family of dedicated animal lovers who have learned to respect and appreciate pack behavior. We do not hire people that want a job, only people that want a career with animals or are in need of experience for a career involved in the animal world. Several former employees that have gone on to veterinary school or other animal careers have come back to say that what they learned at Camp in behavior has not been duplicated in any other training they received. All employees get specific training to enhance their knowledge and ability to care for our campers.training areas for team members include: Behavior; Animal First Aid; Emergency Care; Canine CPR; Animal Standards of Care and Regulation. Each team member must complete a series of training, both in-class and on-the-job, before advancing to the next level of responsibilities. Junior members are paired with more seasoned members and are never left in charge of an active pack. We are also dedicated to ongoing learning and understanding of behavior. Our office staff can assist you with registrations, payments, and any questions or needs. Overall attitude and enthusiasm of our counselors keeps a happy environment for all. This makes a difference for the physiological wellbeing of our campers. Our personal commitments to the campers we come to know and love makes us feel like an extended part of their family. Getting to Know You and Your Dog We would like to know a little more about you and your dog. We do not want a cold business atmosphere but rather want our members to be an extended part of the family. It is helpful to know what you like and do with your dog to help us quickly identify a camper s personality and behavior. Although we need certain acceptable behaviors to socialize campers, we know that every camper is not perfect. People Accepting Canine Behavior is one of our foundations. Please add any additional information we have not ask, where you feel comfortable. How or where did you acquire your dog? Have you used a dog park, other day care, or any other social dog settings? Do you or anyone in your household work from home or is at home the majority of the day? How many people live in your home? What activities do you do with your camper? This is set in place to keep campers accustom to the staff, routine, and atmosphere. This will help lower the stress level for your camper and make their visits healthier both mentally and physically. This can be done with overnight, day camp, or babysitting for a few hours; this is strictly to keep your camper comfortable and happy with Camp. We want to make sure campers have good experiences with Camp and this is done with regular visits. Policies and Procedures The following information is a breakdown of ways we handle situations at Camp. It is a short summery or explanation of policies that we want you to be aware of. Please read these carefully, as initials to accept are required on each. Any comments you have will be discussed individually. ROUTINE Overnight campers: Our day starts at 7AM. All campers are taken out for morning bathroom needs. If they eat in the AM, breakfast is fed. After a minimum of an hour of rest, around 10AM, campers are grouped by size and/or temperament for playtimes. The groups are either outside in our large play areas or inside in the gym or community room. The weather will determine the areas of play. Everyone 4

5 gets playtime and playgroups every day no matter what the weather. These groups are monitored at all times. A short midday break allows the campers to rest and the counselors to take their lunch breaks. Then it is back to playtime. This allows all staff to be out for monitoring playtime. At 4PM campers go into their rooms to settle down before dinner. We give them a minimum of an hour to relax before evening feedings at 5PM. At 8PM, we begin the last outings for bathroom visits, which usually include a little more social time. All campers are in for the night around 9PM. The staff tops off all water bowls, checks on all campers, and tells everyone good night; usually finishing around 10 PM. This schedule is the same 365 days of the year. We also have extracurricular activities for even more one on one time. These activities are an additional cost because it takes additional staffing. We have found campers do enjoy these activities, but extras are certainly not necessary. Feeding takes a lot of extra time since we ask parents to bring the camper s own food. This is better for the camper s digestive system instead of feeding them a kennel food. Due to resting requirements before meals, midday feeding can cause your camper to lose several hours of playtime. Unless there is a medical need for feeding at noon, we suggest you limit to AM and PM feedings. Also large meals one time per day can increase the chance of bloat, therefore, we suggest feeding twice a day instead of once. If no please explain why Day Camp: This program is a slightly different routine from overnight, since campers are not eating at day camp. Our day starts at 7AM and goes until 7PM Monday-Friday or 8AM-9PM Saturday-Sunday. You are not required to have your camper here the entire time; these are just the maximum times during the day. You do not have to purchase day camp if staying overnight to get your camper in a social group, all overnights come with playtime. The day campers are also grouped by size and/or temperament. Campers are generally in playgroup most of the day. The majority of the campers will regulate themselves in playtime and take naps as they choose. Any given day you can see day campers taking naps in their groups. We do provide the midday rest time for the day campers as well. We closely monitor the campers and cater to their individual needs; if they need extra breaks inside we will provide for them. We do have highly social campers; however in some cases, due to personalities, we have had to ask parents to not come on certain days due to particular campers that do not get along. Staff will work with parents to find the right day for your camper. We cannot just put them in another group as this is not so easy if you want the best harmony for all campers! So, if campers are not getting along, the last one added to a day may be asked to switch days for future day care visits. This is one reason we do not allow drop in day care. At times when two campers like this may be staying overnight, we do our best to keep the campers out of sight and mind. If no please explain why SOCIALIZATION This profile and our yearly membership contain a contract. Each time you sign into Camp for overnight the check-in form is also a contract. In Home and In Camp Behavior: Behaviors can change over time. A camper that socialized well for years could act out with some unexpected form of mild aggression. A few reasons could be upset family atmospheres, such as divorces, moving, new roommates, new babies, or underlying medical conditions that have not surfaced. We will do our best to work with any emotional needs that may occur with your camper. We understand you may want personal issues to be kept private; however, it can be very important to your camper s care for Camp to be aware of these circumstances. Although we do not want details, we do need to know if they have additional stress at home. Please keep in mind that when things change at home, our fur babies are affected. Staff is always with campers in daily play groups; but campers can be very fast and before you can prevent it, a tooth can catch and someone can get a wound. Redirected aggression happens when campers get worked up for various reasons. We do all we can to keep this from happening. Redirection is usually a one-time, quick bite, where one dog bites another, usually the one closest to him. In most cases it ends there. Even in small groups, things happen so quickly that you cannot definitively determine which camper actually bit. These bites not full on aggression and these things happen very rarely. Some bites can even occur and not be detected right away if a camper s coat is medium to long. Counselors are trained to do body checks multiple times for this reason. It is always a little bit of a risk that an injury can occur when dogs play together; but dogs are pack animals and need socialization. No camper is going to be maimed or psychologically damaged from any injury that might occur; no camper ever has. You need to understand you are taking a risk when you socialize your fur baby. If you have a show dog, socializing them in this environment may not be your best choice. We will still take your camper if there is a physical limitation that arises that would 5

6 make it unsafe for them to socialize, we will work with you on an individual basis for this situation. The only exception to this is transmittable diseases or parasites. Add any comment here Corrective methods: We do not practice routines of training or discipline at Camp when campers are here for visits. We will not promote sits, no jumping, or general commands. The only corrective methods we use are very non-threatening minor voice commands if they are in play groups and are not playing well. In the case of jumping, if it is unsafe for staff or other campers, we will try to work with them. However, you may be required to do so outside of Camp as well. If they are consistently barking outdoors or playing rough, we will need to settle them down. We cannot let them bark uncontrollably, this can cause stress for other campers and redirected aggression. Again, we use voice commands, change their play groups, or may use a time out. Sometimes timeouts are inside and sometimes they are in an outside area. These timeouts are short times, never hours, in most cases they are less than 10 minutes. We never use any aggressive or terrifying methods on any camper. We will inform you if they don t respond to simple methods. We will work together to help make their Camp experience a good one. During their time here at Camp we will not use specific training or commands that you may use at home or be following with a trainer. We will work with you if a behavior is in need of some reconditioning, however, keep in mind we want to keep Camp as fun as possible for them since it is stressful for them to be away from you even in the best environment. If no please explain why Feeding and Medications: Our feedings are designed for the benefit of the campers in order to prevent upset stomachs. This does take a considerable amount of time and organization, but we do it for the campers. We do need your help so please read this carefully. We require you to bring the campers luggage as follows: 1. All bedding and personal items need the camper s name in permanent marker. 2. We do require that food be bagged in zip lock bags with their name. NO other types of bags are allowed. NO medications in the food bags. We will return the baggies to you for recycling. 3. Medications need be clearly identified. Medications should be in a zip lock bag with the camper s name, medicine s name, and the dosing written in permanent marker on the bag. We will go over these with you at check in, and we will only keep the amount of medication we need for their stay. In the case of injected medications they need to be in the syringe with the proper amount already drawn in each syringe. We have refrigerators if necessary. 4. Supplements such as vitamins may be in food bags, but we need to know what they are. 5. Limit toys to two. NO tennis balls or other toys that could be ingested during late night hours and get stuck in their throats! 6. NO rawhides or greenies or anything that may be ingested during late night hours and get stuck in their throats! We all know that our babies can do without a few toys or chews for a few days to take away the risk of choking. We have never had this happen, and we don t want it to! If no please explain why Holidays: During holidays our staff is very busy caring for the campers. We have certain days before and after holidays that we are closed and do not allow pickup and drop offs. The dates we close during the year are posted in January on our web site. We close to keep the concentration on the campers and not on the paperwork. Since holidays can be stressful for everyone we have found this works best. Do you understand this policy? 7 MEDICAL This profile and our yearly membership contain a contract. Each time you sign into Camp for overnight the check-in form is also a contract. Body checks: Overnight campers may be sent home with letters from camp. These are just notes about a specific issue that did not warrant a call to you on your vacation. These notes can be about anything from a bill to discussing something about your camper. All overnight campers go home with a report card. Counselors are available to discuss the visit if you have any questions. We also keep all records of medications administered, bathroom habits, eating, and behavior for one year after each visit. The report card lists general topics and summarizes your camper s visit. The report card also gives you information on their appetite, bathroom habits, baths, body checks, and any basic medical issues. Fur can hide things that are minor but with body checks the idea is that we can let you know before they go home. You are required to sign the report card and we suggest that if you have any reason to question anything do so prior to leaving camp. This report card is our release and is approved by the North Carolina Department of Agriculture, which is the issuing agency of our license. Therefore, according to the NCDA, we cannot release without an approved signature. For your information, an approved signature that quantifies release is the guardians listed on the profile, the emergency contacts, and/or designated pick up individuals only. If you want to add or change these names at any time, you may do so in writing. This is part of your contract. Do you understand this policy? Add any comment here De-worming policy: Our de-worming policy is in place to prevent the infestation of intestinal worms. All new campers are required to have a preventative de-worming done with either Drontal or Panacur prior to their first visit. There are other de-worming products on the market and any that cover hookworms, roundworms, and whipworms will be accepted. After that, the type of heartworm preventative currently taken will set the protocol of whether or not your campers will need quarterly de-worming. If the heartworm preventative used covers hookworms, roundworms, and whipworms, then the camper will not need to have a quarterly de-wormer. We require this due to our common play areas. Some of the heartworm preventatives do not aid in the prevention of contagious intestinal worms and we need to require de-worming to maintain a healthy play environment for our campers. This is part of your contract. Do you understand this policy? Add any comment here Stress: We know that it is stressful for any dog to be away from their family pack no matter what the environment. We feel we have designed a daily routine to ease tension and stress. However, each camper is different and will react differently to being away from home. The staff is very attentive and will do all they can to help eliminate any stress. We may consult with you if we feel it is becoming a serious concern for your camper. Diarrhea is the most common sign of stress. It can be an excitement stress from the camper doing so much more than they are accustom to each day OR it can be a separation stress. This can show up immediately within the first 24 hours or sometimes after being here several days. It depends on the camper. We have a protocol approved for any camperby our veterinarian. If this becomes a serious case with pure liquid, blood, or doesn t clear up within 48 hours we will contact you and take them to see our veterinarian. Otherwise, we follow the protocol and inform you at pick up. Protocol: Withhold food for 24 hours this allows the intestinal flora to regain normality. Feed rice and yogurt only, for 2 feedings this allows the intestinal lining some mild food to retain. During the withhold period we watch water intake and may provide pedialite and or plain nonfat yogurt this is to provide electrolytes to prevent dehydration and some good bacteria for the intestinal flora. Return to normal food the stomach should be settled and accept regular food. In our protocol we would take into considerations any camper with a dairy issue or other allergies. If any dairy allergies we would leave out the yogurt. Are you ok with this process, if not please explain? This is part of your contract. Do you understand this policy? 7

8 Emergency care: Our customers are typically owners that truly see their dog as a part of the family. We feel that the best way to approach and evaluate medical needs is the same. In any situation we ask ourselves what would I do for my baby? All staff members are certified in first aid, emergency care, and CPR once they have reached a certain level of employment at Camp. Our staff is experienced in evaluating the need for emergency care. We would never wait or risk ANY major medical needs. In the case of emergency, WE WILL do anything we have to do to save and stabilize the life of your pet. This is part of your contract. Major medical decisions after that must be made by you and the veterinarian. We encourage you to inform us of all past, present, and future medical situations that arise, even ones you may feel are minor. This will give us a good history, if something should happen. Any and all major medical needs are either handled with our vets on call Preston Animal Hospital and The Animal Emergency Clinic of Cary. The hospital used would depend on several things such as the emergency, available doctors, and ETA s. These hospitals would consult your veterinarian if they feel they need more information. Initially in an emergency situation they will go on the information you have provided to us. This is why it is important for you to keep us informed! In the case of wounds, we are experienced in evaluating and cleaning minor cuts and wounds. If we feel it is safe to wait a short time period, such as overnight, to see our vet during regular hours, then in the meantime we will triage. We will also do this if we feel the wound can be triaged and wait to see your own vet. We know enough medically to make this decision and we would make the decision in good conscience, as we would for our own pups. Again, we would never risk any camper s health by waiting unless we are absolutely sure they are safe and stable. We simply do not want you to have an emergency bill for something that was not a true emergency as those bills can be three to four times the cost of a regular visit. This is part of your contract. Do you understand this policy? FINANCIAL This profile and our yearly membership contain a contract. Each time you sign into Camp for overnight the check-in form is also a contract. Medical bills: IF any campers are injured or become sick at Camp, any veterinary bills are the responsibility of the owners. This is a blanket policy that states any and all medical bills as being the responsibility of the owners. The credit card you use at check in will be charged for the veterinary bill in full. Add any comment here Maintaining Health Policies: You are expected to maintain your camper s vaccines and all medical and pharmaceutical requirements. Any time that we may request proof of this you will agree to provide it to the Camp office. For any yearly updates, you will be responsible for getting a copy of the updates to our office. Also any ongoing medical issues with your camper will be fully disclosed to us. If you have any questions, our requirements are available to you at any time by contacting the office or looking at our web site. We have found it much easier for all members to ask their vet s office to fax or a copy of any information to our office each time you visit. This way we are always in the loop and you will not get hung up at a check-in or get calls for updates. Add any comment here Evaluation Fee: Current pricing is on our website. The evaluation fee is for all dogs prior to overnight or day camp visits or membership commitment. If more than one dog comes on the same day you will receive a discount for the additional dogs. 8

9 Enrollment: If you are accepted and enroll with Camp Canine Inc. you will be expected to follow all financial requirements. There will be a registration fee at the time of commitment; this is a one-time set fee per family. You will also fill out the camper profile and anytime information on your camper changes you will update any needed forms for our office. All cancellation policies, early pick-ups, late fees, deposits, policies on medical bills, and camper charges were explained in the Membership Agreement and Contract of Care. Submitting these forms constitutes your acceptance of our policies and that you had read and understood all fees and charges. Membership: Our acceptance of members is on a limited basis. Our membership dues are due in January each year. By January 31st you will be expected to have signed the Membership Agreement and have paid membership dues. Each year we include dues in the club options that allow members to pick programs that best fit their needs. This membership helps us to maintain the policy of not accepting the general public into Camp. Any new service added to Camp during the year will be available to you as a member. General Bob s Animal Rescue Kids (BARK): Camp Canine has always been involved in the rescue of dogs in one manner or another. We have responded to national disasters as well as working with local shelters. BARK is a non-profit organization that is legally set up to help abandoned animals. It is a self-sustained 501c3. No officers of Camp Canine are on the board. Camp Canine does not profit from any funds donated to BARK. As well, any dogs that are taken into BARK and accepted at Camp Canine are carefully medically evaluated. All rescues follow the same medical policies and behavioral policies for socializing. Once the rescue dogs are evaluated, they are allowed to socialize at Camp. We never take chances with a member s dog in this process. Member s dogs are perfectly safe with any rescue taken in both medially and behaviorally. We allow members and specific volunteers to help with rescue dogs at Camp. Anyone that does help is restricted to assisting only with rescues and cannot assist a member s dog. Members also may foster or take rescues on visits. Members that adopt from BARK are allowed some discounted or free services to assist in the adoption process. Our counselors can let you know if a particular rescue plays well with your camper. Do you understand and agree with this relationship with Camp Canine? Revocation: If any camper becomes aggressive in any way at any time, Camp Canine, Inc. has the right to cancel their enrollment and no fees will be refunded. If we feel that at any time you become unreasonable with our staff or are not abiding with our policies, Camp Canine, Inc. has the right to cancel your enrollment and no fees will be refunded. If accepted you will be expected to pay any fees for enrollment. We will not charge your registration fee until after your dog s evaluation. You will be responsible for the dog s half day evaluation fee whether they pass or fail. The registration fee will only be charged after you are accepted, your dog passes evaluation, and only if you want to register with Camp Canine, Inc. These fees are not refundable or transferable if your enrollment is cancelled for any reason. 9

10 If accepted as a Camp Member the following is a copy of the current Contract OF Care for 2016 that will be the contractual agreement with CCI. Please read and make sure any questions are addressed at your interview. Guardians (owner of registered pet, person signing agreements for the current membership year) are responsible for reading and knowing all policies, costs, and this entire contract of care prior to any reservation or event. Camp Canine, Inc. hereinafter call CCI CONTRACT OF CARE (contract of care is an agreement with Camp Canine, Inc. in 2016 any and all members; guardians will adhere to this contract). This is a contract between Camp Canine, Inc. hereinafter call CCI, in unification with any Veterinarian or Veterinary Group CCI indentures to care for said pet, and the pet s guardian or guardians or representative whose signature appears hereinafter called the guardian. Any representative of the guardian will be acting as such and the guardian will be responsible for this entire contract by authorizing the representative to register the pet. Guardian certifies the accuracy of all information given about said pet on the contract and camper profile. Guardian agrees to become a member of CCI and understands the membership only facility position CCI holds and shall agree to subsequent policies related to membership. Guardian agrees to the terms of membership and understands the requirements to retain membership. CCI will exercise reasonable attempts to contact the guardian at any time medical attention is required if contact is unattainable all medical decisions shall be designated to a manager of CCI s sole discretion. If the pet becomes ill or if the state of the animal s health otherwise requires professional attention CCI, in its sole discretion, may engage the services of a Veterinarian. The guardian agrees to release the pet s medical decisions to a manager of CCI s discretion for decisions of health and care to be made in the guardian s absence. This shall include any treatment necessary including medications, testing, sedation or surgeries. CCI has the explicit authority to engage any Veterinarian or Veterinary Practice or Veterinary Specialist or Veterinary Specialty Care Hospital or Veterinary Specialty Clinic necessary for appropriate medical attention. Guardian will not hold CCI responsible for medical treatments made by any Veterinarian. The guardian authorizes the financial commitment any medical attention the care may involve to the attending Veterinarian. The credit card on file with CCI will be charged the appropriate amount for all inclusive veterinarian care or prescribed medications. The Veterinarian engaged for services has the authority to charge the guardian s credit card for services either by CCI management in person or under verbal authority by phone. In the event the guardian s credit card is invalid CCI will pay any expenses. The guardian agrees to reimburse CCI for all monies within 15 days that are paid to the Veterinarian for services, medication or emergency care. CCI shall exercise reasonable care for the pet while lodging at CCI. The guardian further agrees to be solely responsible and indemnify and hold CCI harmless from any liability for and all acts of behavior of said pet while it is in the care of CCI including but not limited to damage or injury to persons or other animals caused by the pet. Guardian agrees to the socialization of the pet with other pets while in the care of CCI. Guardian agrees to the containment decisions CCI deems necessary for the care of the pet, this may include group containment where CCI feels the physiological or physical health of the pet is in jeopardy. Guardian agrees to hold CCI harmless for any acts of behavior or injury while socializing pets and the guardian agrees to pay any medical attention necessary for their pet solely and will not hold any other guardian or CCI financially responsible for any medical attention. Injuries that occur to persons by the pet will be paid solely by the guardian directly to the person. CCI will not be held financially responsible. Guardian agrees to all outside and social activities for the pet with other pets and agrees to hold CCI harmless for any acts of nature or injury caused by other pets of wild animals. Guardian agrees to hold CCI and any other guardian harmless if any pregnancy occurs with a non-altered adolescent pet. It is expressly agreed by the guardian that CCI s will not be held financially responsible for death of the pet while in care of CCI. Guardian consents to pet(s) being the subject of photographs or video during their stay at CCI and acknowledge and agree that CCI shall be the owner of such photos and footage and all copyright and other intellectual property rights related thereto. Guardian shall irrevocably grant CCI the royalty-free right to cause the photographs or other likeness of pet(s) to be exhibited, broadcast, distributed, transmitted, published, copied, or otherwise used or sold, either in whole or in part, as still photographs, transparencies, motion pictures, television, video, digital or other medium now known or later discovered, for any purpose desirable or by profit or to CCI. This contract contains the entire agreement between the parties and shall be governed by NC law. All terms and conditions to this contract shall be binding on the heirs, administrators and personal representatives of the guardian and CCI. This agreement is for all encompassing services both in liability and financial responsibility and holds resolute here forth for the existence of the relationship between CCI and the guardian. CCI retains the right to deny reservations if the contract of care of policies are not adhered to for any reason. CCI retains the right to change or add terms to this contract at any time. This includes any addendums, membership club policies or special event rules and policies. Membership Contracts, Check in forms, and Applications all include this Contract of Care and are a legally binding agreement for PAYMENT TERMS By signing CCIs registration form, by using Day Camp and other services and by purchasing a CCI membership you are authorizing any charges to your card under our terms and policies. authorization will be accepted and held legally binding. Deposits and cancellations as well as all costs and charges for services requested and all veterinary costs for the pet during the time the pet is in the care of CCI shall be the sole responsibility of the guardian. All customers are required to provide payment upon request for services. CCI accepts Master Card, Visa, Discover, personal checks and cash. The credit card used at check in will be used if other charges are incurred such as any Veterinary service, medications or emergency unless other methods are agreed upon prior to services verbally or by . The credit card may also be charged for any additional food, additional nights, day camp, and donations or for services provided by Camp Canine after check in if authorized by the guardian verbally or by . After hour s services, this is available by appointment and the fee is in addition to daily rates Cancellation, ANY person that does not call to cancel a regular or holiday reservation and does not check in on the scheduled day will be charged any current designated fees. Holiday Reservations, ALL customers will be required to sign a confirmation form confirming dates for ANY holiday reservations. CCI retains the right to not allow future reservations for non-payment.
