3 he All American Sports Plus Program e offer a wide variety of enjoyable activities for children of all ages.your child will have a minimum of 5 to 8 activities on UESDAY and URSDAY. All activities are taught by our experienced staff and average 30 minutes to one hour. Your child's schedule will depend on their group and special activities they sign up for. SPORS INSRUCION OERED IN... Baseball Basketball ootball risbee Golf Gymnastics Hockey Lacrosse Recreational Games Soccer Softball Swimming ennis rack & ield Volleyball GROUPING & SUPERVISION Each child is placed in small groups according to grade level, friendships and skill level to allow time for maximum selfexpression and individual attention.he group is supervised by EXPERIENCED counselors and has a ratio of ONE counselor for every SEVEN campers. (ajor ield trip ratio is 1 to 4) ROOKIE PROGRA (K and 1st Grade) Our youngest group of campers will have their own program designed to meet their needs.he ratio of ONE counselor to every IVE campers will help to assure a positive camp experience. Swimming and Swim Lessons uesday and hursday afternoons, campers will have the option of swimming under the supervision of certified lifeguards. All campers are tested for their swimming ability and placed accordingly. Swim lessons are available for the beginning swimmer. (No additional cost) 3 ONDAYS... he Camper s Choice ALL campers will have a morning of recreational games and sports. Extra time is devoted to the explanation of rules, expectations and the introduction of new campers to help create a positive and fun camp experience. After lunch ALL campers will have the option to go ice skating, miniature golf/arcade or stay at camp for special sports contests and activities. UESDAYS & URSDAYS... Sports Days In the morning ALL campers will learn the fundamental skills of sports and participate in non-competitive games in a positive environment. Afternoon choices include swimming, waterslide, camp games, individual sports clinics, arts n crafts and tons of fun. EDNESDAYS... Combo or Beach Day he campers stay at camp for a morning of sports and recreational games followed by an afternoon of bowling, roller skating or sports tournaments. Campers 4th grade and up may choose to go to the beach for a day full of activities; boogie boards, sand castles, volleyball, beach run, football and kite flying. Campers K thru 3rd grade will have the option of a special beach trip every other ednesday starting with the 2nd week of camp. RIDAYS... ield rips & Special Days hat a way to end a week! Choices! Choices! Choices! Campers 2nd grade and up have the option to attend our exciting major field trips (see page 5). ALL other campers will have a morning filled with sports, recreational games and fun events. In the afternoon campers will have the choice of going to a movie, bowling or staying at camp for exciting activities.

4 Surf Camp (4th Grade & up) June 12 - Aug. 9 uesday, ednesday, hursday rom beginner to advanced, come surf at California s premier surfing beaches. Our Surf Camp is designed to give campers of all abilities the opportunity to surf in a fun and safe environment. Our experienced staff is there to help the rookie surfer feel comfortable in the ocean and the more advanced surfer improve their skills. Surf trips will depart between 8:00-8:30 am and return between 3:30-4:00 pm depending on which camp site you call home.he cost of each trip is plus the surfer must be signed up for the day at camp. All equipment and wet suits will be provided. Surfers must bring their own lunch. Space is limited! Beach Sports Days (4th Grade & up) On uesdays and hursdays campers have the opportunity to play sports and activities that were made famous at Southern California s beaches. Daily morning highlights are listed below. he afternoons will feature follow up games, boogie boarding, and good old fashion chillin with friends at the beach.hese one day Beach Sports trips will depart camp at and return by 4:00 pm (No additional cost) ues. Campers must provide their own lunch/drinks OR bring money to buy lunch/drinks at the beach. hat others are saying about Sports Plus y boys have attended Sports Plus for the last 5 years and every year they are excited to go. hey know they will be playing sports and they love the special activities and field trips. he best part of Sports Plus is that they get to make so many choices of what they want to do!!! hey are already planning on becoming part of the Sports Plus Counselor in raining progarm. Paige Beck y 2 boys enjoy having choices of activities and really love the Sports Plus lunches.hey also like the fact that they know and meet kids from all different parts of their lives, be it from school, sports leagues, and afterschool and tutoring programs. Danette Smith All three of my children have attended Sports Plus for the last 6 years. hey have always had fun, love the staff and ask to go back every year. heir caring and attentive staff set a great example of making camp an easy experience for us and a fun experience for our kids. Shawn and Peter Glassman 4 y 2 girls and son have 3 different interests and personalities and Sports Plus has been able to make them happy all these years. If they are happy, we as parents are very happy!!! Chris and Clare Eisenberg Sports Plus is the best way to spend a summer according to my 3 boys!!! y boys have loved camp since they were 5 years of age and 6 years later they love it even more!!! hey really mean it when they talk about the Sports Plus amily, because their staff treats everyone so well!! SAN DIEGO ANASY CAP-OU EEKEND riday July 27 - Sunday July 29 (4th Grade & Up) Campers will head South with Sports Plus for a weekend of cookouts, campfires, swimming, and fun in beautiful San Diego. e will sleep under the stars in a secure, private campground with all the finest facilities. Special events of this weekend include... a Padres Baseball game, Sea orld at night and Raging aters. Campers will be placed in small groups by grade level and ability to better insure their best possible experience. Our staff will consist of local high school and college coaches along with current players. Commitment and continuity are an important part of any quality program, therefore if your child drops a training camp after 7:30 am the morning the camp is to begin there will be a cancellation fee. All training camps will be held at our raining Camp Complex (Birmingham High School). ree daily round trip transportation is available from our Calabasas and Sherman Oaks Camps to the raining Camp Complex. No extended day care is available at our raining Camp Complex. Campers must attend onday thru hursday of a training camp week. 7 Registration Information Summer Program 2018 June 11 thru Aug.17 Scheduling: You may choose any combination of weeks and days, (to attend a riday ajor ield rip, you must attend 2 other days in that same week). If you cannot take advantage of all 5 days in a week, it is recommended that you send your child on consistent days of each week.e feel that this type of scheduling creates an environment which builds positive camper/camp-counselor relationships. Camp Days and Hours: onday-riday, to 4:00 pm (Please see page 5 for ajor ield rip departure and return times). ree Extended Day Care Available: 7:30 am to and 4:00 pm to ($1 per minute after ). Not available at our raining Camp Complex. here: Calabasas Camp, Sherman Oaks Camp, Surf Camp and raining Camp Complex. Rates and Discounts: Days paid by April 1st will be: $54 per day, $35 per half day Days paid by ay 15th will be: $59 per day, $37.50 per half day Days paid after ay 15th will be: $64 per day, $40 per half day A Non-refundable Registration ee of $10 will be charged to each camper who registers for our summer camp. Each additional child in the same family will receive a $5 a day discount. Round trip Door to Door bus service is available for an additional $12 per day. Express bus service or one way Door to Door bus service is available for an additional $7 per day. Please see page 5 for ajor ield rip fee, payable the morning of the trip. Payment: Camp fees are due upon registration or a deposit of 0 is due with registration. he balance is to be paid prior to the camper s start of camp, unless prior arrangements have been made with camp office. Refunds: Prior to June1st all fees paid except the $10 registration fee are fully refundable. After June 1st there are no refunds given. Absences and ake Up Days: Parents are financially responsible for all days that the camper has registered to attend camp. here are no refunds given for days missed. If your child will be unable to attend camp that day, please contact the camp office by 7:30 am. If you call the camp office by 7:30 am the day your child will be unable to attend camp, a make-up day will be given, based on availability. If you do not call the camp office when your child is absent from camp, no make up day will be given. Rates Include: Beverages and popsicles each day, a camp shirt, athletic equipment and all field trip admissions (except ajor riday ield rips). Lunch: A nutritious lunch is available for $5 per day or you may bring a sack lunch.e provide the drinks. Door o Door Bus Service Campers may take this bus in the morning only, afternoon only or round trip. If you have special transportation needs, just let us know and we will be glad to help. Pick-up imes he van driver will contact you the weekend before each camp week begins to give you specific pick-up and drop-off times. If you are using a pick-up spot, he/she will designate the exact location. 7 Express Bus Service or those parents who are concerned about minimal travel time on the bus and for campers living in our outlying service areas the Sports Plus Express bus program will meet your needs. Assumption of Risk and Release of Liability: In consideration of the named camper being permitted to participate in Sports Plus Day Camp activities, we, the parents or legal guardians of the camper, on our own behalf and on behalf of the camper (hereinafter releasors ), hereby acknowledge and agree that we understand and fully appreciate the risk of injury involved to the camper in participating in camp activities.e understand that the camper will travel in a camp bus or van and engage in various activities.e further understand and acknowledge that such activities, whether or not they are listed in the camp s brochure, on this form or elsewhere, including but not limited to competitive sports, and surfing, have increased risk of injury associated with them, and that the camp cannot guarantee that the participants, equipment, premises and/or activities will be free of hazards.he Releasors nevertheless hereby release, waive, and discharge Sports Plus Day Camp, its officers, employees, agents, counselors, and representatives, (hereinafter Releasees ) from all liability to the releasors for any loss or damage, and any claims or demands on account of injury to the camper caused in any way while the camper is participating in any of the Sports Plus Day Camp activities.he Releasors hereby agree to indemnify and hold harmless the Releasees, and each of them, for any loss, liability, damage, or costs and expenses including attorneys fees they incur due to the campers participation. It is the express intention of the Releasors to exempt and relieve the Releasees from any liability for personal injury, property damage, or wrongful death related in any way to the participation of the camper in Sports Plus Day Camp s activities.he undersigned acknowledges that he or she is fully aware of the legal consequences of signing this application containing the express waiver and release of liability. I,E, the undersigned, parents of, a minor, do hereby authorize the SPORS PLUS DAY CAP and its delegated leaders as agents for the undersigned to consent to any x-ray examination, anesthetic, laboratory, pathology, deemed advisable by, and to be rendered under the supervision of any physician, dentist, or surgeon licensed under the provisions of the medical staff of any licensed hospital, whether such diagnosis or treatment is rendered at the office of said physician or at said hospital. It is understood that this authorization is given in advance of any specific diagnosis or treatment and that reasonable effort shall be made to contact the undersigned prior to rendering treatment, but then any of the above treatments shall not be withheld if the undersigned cannot be timely reached. his authorization shall remain in effect unless revoked in writing.he authorized person(s) further understands and agrees: 1.hat if medication is to be given to the registered minor SPORS PLUS DAY CAP and its leaders or directors are not legally nor financially liable for administering or results of administering medication. If medication is to be given, parent must make arrangements in writing with Camp Director. inors are not to have medication of any kind on their person or in their belongings at any time. 2.he camp is not responsible for lost articles. Parents are requested to mark the camper s first and last name on all items. 3.here are no cell phones, or other electronic devices allowed at camp. 4.here are no refunds given for days missed and parents are financially responsible for all days and activities that the camper has registered to attend camp. 5.All camp fees are due prior to the camper s start of camp. All balances not paid in full by your child s start of camp are subject to a 1% per month finance fee. 6. If a camper is dismissed from camp for disciplinary measures, there will be no refund for any unused days. 7.All campers are required to wear a Sports Plus Camp Shirt on ondays & ridays. Campers not wearing a camp shirt on these days will be given one and their parents will be charged. 8.hat in case of accident SPOR PLUS DAY CAP assumes no financial responsibility beyond the secondary insurance coverage. 9.hat the registered minor may be transported in SPORS PLUS DAY CAP leased vehicles and may participate in camp sponsored field trips. 10. By providing Sports Plus Day Camp with a credit card, you hereby agree that all charges incurred at camp, including additional days will automatically be billed to that credit card at the end of our summer session. 11.o all policies listed in the brochure and registration form.
