3 CONFERENCE REGISTRATION Before completing your Conference Registration, please read the following information carefully. Your registration must be submitted on or before Friday, November 16, 2018 to meet airline deadlines. This form provides us with the necessary information to confirm your Conference reservations. Submit your registration by the deadline even if you re not sure you can attend simply follow the instructions outlined on the Conference Registration Form on the website. Legal Name Please note that the name which appears on your passport is recognized as your legal name. Your airline tickets must be booked in your legal name and must match the name on your documentation. Badge Name Your Badge Name may be different from your Legal Name. This is the name you want to be called during the Conference. This name will appear on your name badge and on any certificates or awards you may receive. 4 Guest Information You are allowed to bring one guest and CIBC requires that your guest be over the age of 16. As this is a business conference, children or additional guests are not allowed to attend during the Conference dates of January 13-19, Please note that we cannot make any exceptions. Your guest is invited to attend all listed functions, with the exception of the Achievers in Conversation which is for CIBC employees only. However, if your guest is a CIBC employee, he/she may attend. All Conference materials will be sent to your / business address. We do not require an address for your guest. Additional Forms In addition to the Conference Registration Form, there are three other forms related to the Conference: 1. Optional Extension Form, due on or before November 16, Incidental Funds Form, due on or before November 23, Excursion Form, due on or before November 23, 2018 These forms are accessible only from the registration website. Medical/Dietary Requirements Any medical or dietary needs can be accommodated. Please indicate your requirements, including any allergies, and provide details when registering, under the Medical/Dietary section of the form. 7 TRAVEL INFORMATION Travel Documents Airline E-Tickets Your travel documents will be sent to the address provided on your electronic registration form, no later than December 28, The attached documents will include airline e-tickets for you and your guest, baggage information and any applicable overnight/extension confirmations. If you do not receive the by Friday, December 28, 2018, please call the Achievers Desk at (in Toronto) or Toll Free (within North America). Note: Please print a copy of your e-ticket prior to travel. Always carry it with you while travelling. NEVER pack it in your checked luggage. Airport Check-In You are requested to check-in at your designated airport a minimum of 3 hours prior to your flight departure time to allow for check-in, baggage handling, and security and/or customs clearance. All flight times are subject to change with or without prior notice. We recommend you contact the airline before leaving for the airport to confirm your departure time. If you are having someone meet you at the airport upon your return, they should also call the airline to verify your arrival time. Please be aware that roundtrip transportation between your home city and the airport is your responsibility. In the event of unforeseen circumstances, we recommend that you include all essentials in your carry-on bag that may be required for the first few hours following arrival. Airport Staff Your CIBC Achievers Travel Team will be on location at major city airports to assist you with check-in. Please watch for the representatives holding Achievers signs in the check-in area. Baggage Information Airline baggage allowance policies & costs vary. Please refer to your specific airline(s) website(s) for the most up-to-date information on baggage allowance. You will receive baggage tags at your business address prior to travel. Please remove all other tags from your luggage. Complete the new baggage tags with your name and address and attach them to each piece of luggage, including carry-on. 8 Changes/Inquiries/Cancellations CIBC has negotiated special group rates with all airlines. That means it is NOT possible to offer any changes to the pre-arranged group flight schedules (different airlines, routings, etc.). All inquiries regarding flights or tickets MUST be directed to the CIBC Achievers Desk at (in Toronto) or Toll Free (within North America). This is a special group booking so your local travel agent cannot assist you with changes. Your airline tickets are non-transferable and 100% nonrefundable. You will be responsible for costs incurred for any changes made after tickets have been issued on November 19, Therefore, it is very important that you provide the correct legal name spelling for both you and your guest when you submit your Registration Form. Please notify the Achievers Desk immediately if you have any changes or corrections. If you have any questions or travel changes on the day of departure, the CIBC Achievers Desk will be open all day on January 13, CIBC Achievers Desk Your CIBC Achievers Team is available to answer questions or clarify information from now to the day of departure. Customs If you are travelling with valuables such as cameras, watches, expensive jewelry or electronics, we suggest that you register these items with Canada Border Services at your local airport prior to your departure from Canada. Without a Customs declaration or sales receipt, the Customs official may assess duty on the item(s) upon your return. If you and your guest do not have the proper identification for entry into Mexico, or if you are refused entry for any reason, CIBC cannot be held responsible. Canadian citizens must carry a valid, undamaged* passport for travel to Mexico, which must be valid for a minimum of 6 months following the date of return to Canada (i.e. valid until at least July 20, 2019). For Canadian passport information, please visit english/passport. Canadians travelling on a Canadian passport do not require visas to enter Mexico if the purpose of their trip is tourism and the stay does not exceed 180 days. A Mexican Tourist Card is required and can be applied for on-line using the following link. mexicotouristcard.com/. Note: Mexican Tourist Cards have a validity of 180 days from date of issue. Your passport s date of expiry must not be before the date of expiry on your Mexican Tourist Card. Canadians who hold dual citizenship will only be allowed to fly into Canada with a valid Canadian passport. Temporary Canadian passports and Canadian emergency travel documents are also acceptable. Non-Canadian citizens must carry a valid, undamaged* passport and must also check with the Mexican Embassy or Consulate for entry requirements. We recommend you do this as soon as you receive your Invitation because, in some cases visas may be required, passports may need to be valid for a specific period of time after travel dates. Permanent Residents in Canada are required to have a valid, unexpired Permanent Resident Card, in addition to a valid, undamaged* passport, to return to Canada on a commercial aircraft. Please note that you will be required to provide your PR Card information on the Conference Registration Form. For further information, or an application, please contact or english/information/pr-card. We recommend that you take a colour photocopy of the detail pages of your passport that can be accessed while you are away should your passport be compromised. Please note that entry into another country may be refused even if the required information and travel documents are complete. You and your guest are solely responsible for complying with all government travel requirements and acknowledge that CIBC and SM+i are not liable or otherwise responsible for such compliance or for the actions of government authorities. Important: If your guest is 16 or 17 they are considered a minor by the Mexican government; you must have a notarized letter of consent from the other parent, or from both parents if you are not the parent. 10 Mexican Tourist Card All foreign passengers arriving in Mexico must fill out an Official Entry Immigration Form (Tourist Card) before arrival in Mexico. Many of the airlines, including Air Canada, are no longer distributing the Tourist Cards onboard their flights. We strongly suggest that you complete the entry form online in advance of travel. Detailed information and the form can be found at You will require access to the following information/items to complete the online form: Contact information Passport information Full Name of the Hotel or address in Mexico Arrival flight information address to send you the Tourist Card A printer to print the Tourist Card IMPORTANT NOTES: Each person travelling, regardless of age, must have their own tourist card Forms cannot be filled out online until you are within 30 days of your date of departure to Mexico Your passport must not expire before the Tourist Card expires which has a validity of 180 days from date of issue. Tourist Cards are issued in two parts. Part 1 is collected by the Mexican immigration official upon arrival in Mexico. Part 2 is stamped and must be retained and provided to the Mexican immigration official upon departure from Mexico. Please ensure that you keep the form in a safe place with your passport. Charges for lost cards may apply including fines of USD $60.00 You must be from an eligible country to complete the tourist card form. A list of eligible countries can be found at: Note: If you are not from an eligible country you must contact the Mexican Consulate nearest your place of residence to determine entry and visa requirements. Consulate information can be found at: notices/7266-doclegalization Zika Virus Zika Virus continues to pose an on-going risk to travellers. SM+i/Vision Travel and CIBC strongly advise any traveller visiting any of the affected countries, including areas of Florida, Caribbean, Mexico, Central America and South America, to educate themselves and follow any government advisories. For more information, please visit zika-virus to access up-to-date resources and information from the Government of Canada, Public Health Agency of Canada, World Health Organization and more. Flight Schedules Flights have been pre-booked to and from Cancun for travel on group dates. Extensions are permitted at your own personal expense and must be requested by completing the Extension Form available on the Achievers Conference website. This is a special group booking so your local travel agent cannot assist you with extensions or changes. All participants and guests who deviate from group travel dates will incur an automatic deviation fee of $150 + GST/ HST per person (Canadian funds). This fee is in addition to any extra costs associated with your trip extension (e.g., difference in airfare, hotel, rental cars, etc.). If it is necessary due to your flight routing to overnight at a major city, CIBC will arrange accommodation at an airport hotel. A detailed confirmation will be included with your travel documents, if applicable. Extensions If you are planning to extend your stay, extension requests must be received by Friday, November 16, We cannot make any exceptions. Flight itineraries must be booked in conjunction with your extension plans. 11 Insurance Outlined below is the travel/medical insurance coverage provided to Canadian CIBC Achievers and their eligible dependents. As each person s requirements for travel insurance varies, it is your responsibility to ensure that appropriate coverage needs have been met for you and your guest. This may include, but is not limited to, medical and hospital benefits, baggage, personal effects, personal currency, etc. Please remember it is a good idea to consult your physician regarding your medical needs prior to travelling, and to pack all prescription medication in your carry-on luggage. Achievers outside of Canada (USA, Asia and Caribbean) are not covered under the same policy; please refer to your region s insurance policy for coverage information. Policy Information All Canadian CIBC Achievers and their eligible dependents (i.e., spouse/partner and or dependent children) will be covered by CIBC Travel Medical Protection and Travel Assistance benefits under CIBC s medical plan for the duration of the Conference, beginning January 13, 2019 and ending January 19, The administrator of CIBC s medical plan is Sun Life and the Policy Number is The travel medical protection and travel assistance benefit is provided by Allianz Global Assistance. Under this benefit, you also have access to 24-hour emergency assistance services, such as doctor and hospital referrals. If your guest is not eligible for this coverage, individual travel medical insurance coverage must be obtained prior to departure. 24-Hour Help Line If emergency assistance is needed, a 24-hour help line is available. Multilingual coordinators at Allianz Global Assistance have access to a worldwide network of professionals who offer help with medical, legal and other travel-related emergencies. The 24-hour help line can assist you and your eligible dependent if you have lost your passport or visa, if you need to find a local legal advisor, or if you require telephone interpretation services. You can also call the help line and leave important messages for family, friends or business associates; likewise, they can call the help line and leave messages for you while you travel. When calling the 24-hour help line, please be ready to state your provincial health plan number, your policy number and your employee ID number. HOW TO MAKE A CLAIM If emergency assistance is needed, call the 24-hour Allianz Global Assistance help line immediately. From within Canada and USA, call From anywhere else, call collect through an international operator. This number is also identified on your travel medical card. Allianz Global Assistance can advise you of coverage before you travel and will assist you at the time of the emergency. TO ENSURE PAYMENT OF YOUR EXPENSES 1. Call the 24-hour help line immediately. If you are physically unable to call the help line yourself, have a family member, traveling companion, or medical personnel call for you. Simply showing your travel assistance card to a doctor, nurse or hospital personnel will not ensure payment of these expenses and you may incur medical expenses not covered by your plan. 2. Allianz Global Assistance will verify your CIBC medical plan coverage and pre-authorize any invasive or investigative procedures so payments can be arranged on behalf of you or your eligible dependent. 3. You will be required to sign an authorization form allowing Allianz Global Assistance to recover any amounts payable by the provincial medical plan. If you receive any subsequent bills for these expenses, please forward them Allianz Global Assistance for processing. For specific details about this benefit, please call the Allianz Global Assistance help line at within Canada and USA. From anywhere else, call collect through an international operator. 12 DESTINATION INFORMATION Achievers Hospitality Desk Throughout your stay at Grand Velas Riviera Maya, the Achievers Hospitality Desk staff will be pleased to assist you with any questions or requirements that you may have. Climate Riviera Maya enjoys a tropical climate. In January, average daytime temperatures range from 23-27º C (74-81º F) and dip to a nighttime low of 19º C (67º F). Currency The local currency is the Mexican Pesos. However, US Dollars are widely accepted. HELPFUL HINTS: US money is readily accepted by vendors but your change will be in Pesos. When bargaining at the market, be sure to have Pesos available in small denominations. You can exchange your Canadian or US funds for Pesos at the hotel s Front Desk (ID required). Note: The hotel is only able to exchange small amounts of cash, up to $ at a time, and exchange fees apply. Bank machines dispense cash in Pesos. There is one ATM located in the "Grand Class" lobby area. Dress Code Pack as you would for any resort location. Clothing for daytime activities should be lightweight and casual. For daytime excursions be sure to bring a comfortable pair of shoes and a sun hat, as well as sunscreen and your camera! We recommend you bring a light sweater or jacket for the evenings. Resort elegant attire is required for all evening functions, except the Gala Awards evening, which is business attire. Dress codes for Conference events are available on the Week at a Glance document on the Achievers website. Please refer to the guidelines below for details: Casual attire: shorts, pants, t-shirts, polo shirts, skirts and blouses, sun-dresses, etc. Resort elegant attire: for men, collared shirts, long pants and closed-toe shoes are required (no sandals); for women, dresses, skirts and blouses and pant suits are appropriate. Please note, the hotel strictly enforces their dress code policy above in addition to no beachwear, no shorts, no t-shirts, no flip flops in restaurants during the dinner hours. 13 Electricity The electrical current is 120V (same as in Canada). Wi-Fi Service Standard wireless internet is included in the guest suites at the Grand Velas Riviera Maya. Upgraded high speed internet is available upon request. Extra charges apply for upgraded internet service and are at your own expense. Excursions Please refer to the online Optional Excursions Brochure for excursion descriptions, details, cancellation policy and US Dollar costs. All excursions must be booked online with a valid credit card number by Friday, November 23, Optional excursions are subject to availability and minimum/maximum participation; you are encouraged to make your reservations early. Optional excursion details will be noted in your personal itinerary. A welcome package, including your personal itinerary, will be available for pick-up upon arrival at Grand Velas. You will also need to stop by the Achievers Hospitality Desk to sign your Optional Excursion billing receipt and pick-up your excursion tickets. Note: We respectfully ask that you do not book personal activities (such as spa) until you are onsite at the hotel in order to avoid any scheduling conflicts with Conference events or optional excursions. Gratuities All gratuities for planned meals, group functions, airport porters, bellmen, room maids and waiters (at included meal functions only) have been prepaid. Gratuities for personal activities, such as spa and salon treatments, are not included in the program. All gratuities related to personal charges should be taken care of by each individual at the time the service is rendered. Suggested gratuity amount is 15% 20%. Incidental Expenses Grand Velas will maintain an incidental account to cover your personal expenses such as phone calls, laundry, spa treatments, etc., which are not included in the program. Please make sure you settle this account prior to checking out of the hotel on January 19. 14 Safety Guidelines As with any destination it is important to be aware of your surroundings and take steps to ensure your safety. BEFORE YOU LEAVE: In case of an emergency abroad or a personal emergency at home it is recommended that you register your trip with the Canadian Government s Department of Foreign Affairs and International Trade. Non-Canadian residents can register with their respective governments web-site. When travelling, carry your travel documents, medication and valuables with you. Do not pack them in your checked luggage. WHILE AT THE RESORT: Remain on the resort property and avoid non-approved excursions this includes personal outings to Cancun and Cozumel. Upon arrival at the resort, when at the pool, beach or on an excursion, secure your passport, travel documents and valuables in the safe provided in your guest room. Mexican weather (heat) can represent extremes from your normal conditions. Use an appropriate sunblock, remain hydrated and avoid excessive alcohol Strong currents and undertows are common on many of Mexico s beaches. Take posted warnings about swimming conditions seriously and familiarize yourself with the beach flagging system. When in doubt about water conditions, consult hotel staff, lifeguards or local authorities WHEN OFF RESORT: Avoid wearing valuable jewelry/ accessories while traveling or participating in off-resort activities. Withdraw or exchange money at ATMs, exchange bureaus, reputable financial institutions, hotels and malls. Avoid street-side withdrawals and exchanges. Always cover the keypad when entering your personal identification number (PIN) Keep your credit card in sight when paying for goods and services Language Mexico s official language is Spanish but many people speak English in tourist areas. Time Zone The east coast of Mexico is on Central Standard Time. 16 TAXATION NOTICE Employee Taxable Benefit for Guest Attendance Canada Revenue Agency (CRA) guidelines and rulings indicate that while guest attendance (i.e., in most cases an employee s spouse/partner) at an annual conference may be encouraged, it is not essential to the employee s participation. Therefore, the market value of the incremental cost is an employee taxable benefit. As a result of this requirement, the incremental cost CIBC pays for a guest to attend the Conference, i.e., airfare and meals, will be reported as an employee taxable benefit. Cost of guest accommodation is not applicable as it is shared with the employee. FOR EACH EMPLOYEE WHO BRINGS A GUEST TO THE CONFERENCE, THIS MEANS: Extra tax i.e., income tax, CPP/QPP, and EI (where applicable) will be assessed on the $1,800 amount. The incremental taxes to be paid by the employee will be in the range of $600 $900 (depending on the employee s total pay amount and marginal tax rate), and will reduce the employee s net pay amount for the February 22, 2019 pay.
