About Us Avenues East is a subsidiary of the Avenues Group and an independent charity which has been making a difference to the lives of disabled people for over 30 years. We support people across the East of England living with disabilities, illness or injury to live the best lives possible. We work to provide people with the opportunities to play an active part in the local community and to help them recognise their equal right to take part in all aspects of everyday life. Our services include: Community Support Supported Living Specialist acquired brain Injury support Our services include: LEISURE WORK SUPPORT Avenues East, Community Support Services (CSS) supports children, young people and adults who have learning disabilities, physical disabilities, sensory impairments and complex health needs. Joined up support can make it easier for people with disabilities to live their life as they wish. CSS helps bring support closer together to provide the right support at the right time. We help provide support during everyday life, including support at home, out and about in the community and to access a variety of leisure activities and work based opportunities. 3
LEISURE CSS LEISURE encourages and enables people with disabilities to take part in activities which help to improve their health, wellbeing, social skills and independence. We do this by delivering a wide range of activities, social groups, short breaks and events throughout the year which provide fun and enjoyment for everyone. LEISURE Registration To access our leisure activities we require people to complete a registration and self assessment form and pay an annual subscription fee of 20 (only 5 per three-month period). 4
Social Groups Teaming Up supports and enables people with disabilities to access and take part in a variety of age appropriate leisure, recreational, social and well-being activities within their local community. All participants are provided with the opportunity to develop and maintain good friendships, whilst enjoying a social life. Our social group sessions operate on a pay-as-you-go basis and participants are invoiced at the end of every three-month period. 5
Social Groups (ages 10-15) Teaming Up (ages 10 to 15) takes place on Saturday mornings and enables children and young people with disabilities to have lots of fun and make new friends. Each session offers high quality activities which take place at a variety of leisure and entertainment venues for example bowling, cinema or just meeting up with friends. Activity cost included A programme of activities is sent to all Teaming Up participants. 6
Social Groups (ages 16-25) Teaming Up (ages 16 to 25) meet weekday evenings at a variety of leisure and entertainment venues. The sessions help young people develop money handling, social and communication skills, whilst hanging out with friends. A programme of activities are provided including bowling, cinema, meals at local restaurants, swimming, visiting places of interest and much more. Days Out during the school holidays Days Out & About run during school holidays to support disabled children and young people to take part in a variety of activities at leisure/entertainment venues and to visit local attractions via public transport. Our Days Out & About are run as part of Teaming Up. Details of the days and dates are detailed in the Teaming Up programme of activities. For all our days out & about full payment is due at the time of booking. 10
Daytime Activity Groups Daytime Activity Groups provide weekly daytime activities that enable people to have an enjoyable day, keep active, learn new skills and maintain social contacts. Stowmarket Every Monday Time: 9:30am - 2:30pm Cost: 25 per session Stowmarket Community Centre, Hillside, Stowmarket, IP14 2BD Mondays at our Stowmarket Daytime Activity Group are varied and include: arts & crafts, exercise to music, group activities and games. Every Thursday Time: 9:30am - 2:30pm Cost: 25 per session Out and about in the local community Thursdays at our Stowmarket Daytime Activity Group includes community based activities, volunteering and sporty games. 11
Daytime Activity Groups Needham Market Needham Community Centre, School Street, Needham Market, IP6 8BB Every Tuesday Time: 9:30am - 2:30pm Cost: 25 per session Our Needham Market Daytime Activity Group includes a wide variety of arts & crafts, games, music, light exercise and themed days. There s also an optional 2 course hot meal for only 4.00. Ipswich St Peter s by the Waterfront, College Street, Ipswich, IP1 1XF Every Monday & Friday Time: 9:30am - 3:30pm Cost: 30 per session Our Ipswich Daytime Activity Group consists of days packed full of community-based activities. Short Breaks Our Short Breaks provide supported day trips and holidays that enable people to have a break away, try out new things, have fun and make new friends. ITFC Match Assistance ITFC Match Assistance enables people with a disability to watch Ipswich Town home matches. We support people who require assistance to watch Ipswich Town matches at Portman Road who do not already have their own personal assistant or carer to accompany them to the game. Fixture information & prices will be sent on request. ITFC games must be booked and paid for in advance. 20
WORK CSS WORK provides first step support to access employment through voluntary placements and work experience opportunities. We support people with disabilities to access and participate in volunteering or work experience placements within their local community. 21
SUPPORT CSS SUPPORT provides a range of day-to-day assistance, both at home and in the community. Our aim is to enable people with a disability to be as independent as possible and to live their lives the way they choose. We provide support to: access the local community. take part in a range of activities, at home and in the community. successfully live at home independently. undertake any personal care duties, when required. to develop daily living skills, such as money handling and using public transport. manage household duties, such as bills, food shopping, cooking, cleaning and gardening. plan and go away on a short break. 22
SUPPORT Our one to one support is available from 4 hours to 24 hours a day, 7 days a week. Every person we support has a personalised support plan which relates directly to their individual' goals, wishes, needs and aspirations. 23
