Community Health Status Assessment Findings WePLAN 2020 WePLAN is the Cook County Department of Public Health s community health assessment and community health improvement plan. The following summary presents key findings from the Community Health Status Assessment (CHSA). The purpose of the CHSA is to identify priority community health and quality of life issues. Questions answered include: "How healthy are our residents?" and "What does the health status of our community look like?" The structure for this assessment and the selection of indicators was based on guidance from the National Association of County and City Health Officials CHSA At-A-Glance document and the County Health Rankings. Data were included from a wide variety of sources including vital records from the Illinois Department of Public Health, USA 2000 and 2010 Census, American Community Survey (ACS) 2009-2013, US, IL, Cook, and the SCC Behavioral Risk Factor (BRFSS) and Youth Risk Behavior (YRBS) surveys. Findings: There has been a significant net increase in the overall number of vulnerable populations, including children living in poverty, in Suburban Cook County (SCC). From 2000 to 2009-2013, the number of people living in poverty in SCC increased by 71% (from 156,249 to 267,274 persons). Poverty rates ranged from over 16% in the South district to 6% in the North district. The number of children living in poverty in SCC more than doubled from 2000 to 2009-2013. A deplorable one out of every 4 children in the South district lives in poverty compared to 1 out of 10 children in the North district. Although there was very little population growth in Suburban Cook County, the racial/ethnic composition changed drastically. The total minority population increased by over 30%, while the non-hispanic white population decreased by 14%. Hispanics had the largest rate of growth, 46% with the Non-Hispanic African American population increasing by 17%. In 2000, there were more African Americans than Hispanics (340,351 and 318,096 respectively), and by the 2010 Census the Hispanic population (465,897) exceeded the African American population (399,822). Chronic diseases continue to be the leading causes of death in SCC. Coronary Heart disease (CHD) is the second leading cause of death among SCC residents. However the CHD mortality rate is significantly decreasing in SCC and across all districts thanks largely to improved medical care. Cancer is the leading cause of death in SCC with significant disparities. The South district experienced the highest overall age-adjusted cancer mortality rate (213 deaths per 100,000) as well as Non-Hispanic African Americans (207 deaths per 100,000). The age adjusted mortality rate for colorectal cancer for Non-Hispanic African Americans was nearly 70% higher (26.0 per 100,000) than that of Whites (16.0) Similar disparities exist for breast cancer mortality rates among females as well. The age-adjusted female breast cancer mortality rate for the North district (19.1 per 100,000) was 40% smaller than rates for females in the South district (33.2 per 100,000). Stroke (cerebrovascular disease) is the 3 rd leading cause of death in SCC. Stroke mortality for the South district is significantly decreasing since 2008. The age-adjusted Diabetes-related mortality rate for Non-hispanic African Americans in SCC (82.0 per 100,000) is more than double that of Whites (37.9 per 100,000) Age-adjusted mortality rate for suicide among Whites (13.9 per 100,000) in SCC is more than double that of Nonhispanic African Americans (5.7 per 100,000). However, for homicides, the age-adjusted mortality rate for Nonhispanic African Americans is 20.4 per 100,000 compared to SCC whites at 1.6 per 100,000. Overall crude birth rates are decreasing in SCC with teen birth rates decreasing for all race/ethnic groups except Asians (with insufficient data). Infant mortality rates for African Americans in SCC (14.7 per 1,000 live births) is significantly higher than that for Whites (3.6 per 1,000) live births. Chronic diseases continue to represent the leading causes of deaths in SCC. Disparities among race/ethnic groups as well as geographical have remained persistent. Demographics Between the 2000 and 2010 censuses, there was very little population growth in Suburban Cook County (SCC). The racial/ethnic make up however changed drastically. The age ratios were very similar except over 85 there were more females than males, women out numbered men nearly 3 to 1. Although there was very little population growth in Suburban Cook County, the racial/ethnic composition changed drastically. The total minority population increased by over 30%, while the non Hispanic white population decreased by 14%. Poverty The nuber of persons in poverty in SCC increased by 71% from 2000 to 2009 2013. The North and Southwest districts experienced the highest increase (70%) in the number of people in poverty. Across SCC, and even larger increase (88%) occurred in the number of children living in poverty. The number of children living in poverty more than doubled for the North district and increased 88% for children in the South district. Socioeconomic Status Overall the unadjusted median income for SCC increased 12% from $52,482 in 2000 to $58,693 according to the 2009 2013 ACS estimate. The medican income increased for all districts except the South wich remained at approximately $48,000. The unemployment rate more than doubled in SCC from 4.8% in 2000 to 10.6% for 2009 2013. The unemployment rate doubled as well for all SCC districts The West district experienced the lowest increase (102%) in unemployment with the Southwest district experiencing the largest increase (137%). Overall, the South district had the largest unemployment rate at 16%. The total population of people over 25 without a high school diploma or the equivalent changed very little between the 2000 Census and the 2009 2013 ACS. In SCC, the percentage of persons over 25 with a high school diploma was around 26%. The highest percentage of people without high school diplomas were in the Southwest district, over 32% at both time periods.

Social Determinants of Health The diversitydatakids.org project and the Kirwan Institute for the Study of Race and Ethnicity (the national leader in community opportunity mapping) have combined their knowledge and experience to construct a first of its kind, neighborhood level Child Opportunity Index for the 100 largest US metropolitan areas. The goal of this work is to create measures of neighborhood based opportunities that children need to develop and lead healthy lives. The Child Opportunity Index is a measure of relative opportunity across all neighborhoods (i.e., census tracts) in a metropolitan area. The index is constructed of 19 individual data indicators across three topical domains: Educational Opportunity, Health and Environmental Opportunity, Social and Economic Opportunity. Dark colors represent healthier areas with more opportunities at the neighborhood level for children whereas lighter colors represent those areas that are less healthy and do not offer many neighborhood based opportunities for children. Every community must prepare for and respond to hazardous events, whether a natural disaster like a tornado or disease outbreak, or a human made event like a harmful chemical spill. A number of factors may weaken a community s ability to prevent human suffering and financial loss in the event of disaster. These factors are known as social vulnerability. The Social Vulnerability Index (SVI) helps emergency response planners and public health officials identify and map the communities that will most likely need support before, during, and after a hazardous event. The SVI uses U.S. Census data to determine the social vulnerability of every Census tract. Census tracts are subdivisions of counties for which the Census collects statistical data.the SVI ranks each tract on 14 social factors, including poverty, lack of vehicle access, and crowded housing, and groups them into four r elated themes: Socioeconomic Status, Household composition, Race/Ethnicity/Language, and Housing/Transportation. Dark colors represent areas of higher vulnerability and lighter colors represent neighborhoods which are less vulnerable.

Risk Behaviors Cigarette smoking harms nearly every organ of the body, causes many diseases, and reduces the health of smokers in general. Nationwide, cigarette smoking is responsible for more than 480,000 deaths annually (including deaths from secondhand smoke). Over 1 in 5 adults in Illinois smoke compared to 18% of adults in Illinois and the U.S. However the prevalence of smoking among youth in SCC (12%) is lower than that of youth in Illinois (18%) and the U.S. (19.5%). Regular physical activity can produce long term health benefits. People of all ages, shapes, sizes, and abilities can benefit from being physically active. The more physical activity you do, the greater the health benefits. Over 27% of adults in SCC are physically inactive compared to 22% of adults in Illinois and the United States. As part of a healthy diet, fruit and vegetables can lower risk of stroke, heart disease and some cancers.they are often low in fat, helping you to maintain a healthy weight. Eighty five percent of adults and 77% of youth in SCC do not consume the recommended 5 servings of fruits and vegetables per day. Two out of three adults and one out of three youth in SCC are obese and/or overweight.

Mortality Community Health Status Assessment

All Cause Mortality What is it? All cause mortality rate is the number of total deaths from any cause per 100,000 people in a population over a certain time period. Why is it important? All cause mortality can assist in monitoring and comparing the health of populations, serve as a proxy for life expectancyi and can be one of several factors used to calculate population size. All cause age adjusted mortality rates declined from 715.2 to 681.3 per 100,000 population between 2008 and 2012 for SCC. However, disparities remain with males, and Non hispanic African American populations experiencing the highest rates as well as among populations in the South district. Coronary Heart Disease Mortality What is it? Coronary heart disease (CHD) is a narrowing of the small blood vessels that supply blood and oxygen to the heart. What causes it? Factors that increase the risk of CHD include smoking, lack of exercise, obesity, diabetes, high blood pressure, and elevated total cholesterol levels CHD is the second leading cause of death in Suburban Cook County (SCC), responsible for over 3,500 deaths per year and results in large costs to the health care system and lower productivity. All Cancer Mortality What is it? Cancer is a term used for diseases in which abnormal cells grow and divide without control, spread to other tissues, and do not function normally. Lung, prostate, and colon cancer are the leading causes of cancer death for men; lung, breast, and colon cancer are the leading causes of cancer death for women. Why is it important: Cancer is the leading cause of death in the U.S. and Suburban Cook County (SCC). More than 5,000 SCC resdidents die of cancer each year. The age adjusted mortality rate for NH African Americans (207.7 per 100,000 population) was almost double the rate for Hispanics (109.1) in 2012. The rate for males from 2008 to 2012 was conistently higher than for females with the age adjusted mortality trend decreasing for females. Cerebrovascular Disease Mortality What is it? Cerebrovascular disease, also known as a stroke, occurs when an artery in the brain bursts or is clogged by a blood clot, resulting in brain damage. What causes it? Factors that increase the risk of stroke include atrial fibrillation, hypertension, high cholesterol, diabetes, artherosclerosis, tobacco use, advanced age (55+ years) and previous history of stroke. Why is it important: Stroke is the third leading cause of death in Suburban Cook County (SCC), responsible for over 1,000 deaths each year. For 2012, the South district had the highest age adjusted stroke mortality rate (48.2/100,000 population) compared to the other districts. Motor Vehicle Accident Mortality What is it? Motor vehicle accident mortality involves the death of a driver, passenger, or pedestrian (including bicyclists) as the result of a collision with a motor vehicle. Why is it important? While great efforts have been made to reduce injuries and fatalities in the last century, nationwide, motor vehicle accidents (MVA) are a leading cause of death for people 1 to 44 years old. Alcohol and age are important factors in MVA fatalities and many are preventable. While MVAs account for nearly 135 deaths in SCC each year, the age adjusted mortality rates for all age, major race/ethnic, and geographical groups were below the HP2020 goal of 12.3 per 100,000 population. Diabetes Related Mortality What is it? Diabetes mellitus is a group of diseases in which the body either does not properly utilize or does not produce insulin resulting in high blood glucose levels. What causes it? The causes of Type 1 diabetes are unknown. Type 2 diabetes is associated with obesity, physical inactivity, family history, and race/ethnicity. Why is it important: Over 1,300 people die each year in SCC due to diabetes related conditions. However, disparities exist. NH African Americans are the only major race/ethnic group remaining that have not achieved the HP2020 goal of 66.6 age adjusted deaths per 100,000 population. Suicide Mortality What is it? A suicide is a death resulting from the use of force against oneself. Why is it important? Nationally, suicide is the second leading cause of death for persons 25 34, and the third for persons 15 24. Suicide accounts for nearly 215 deaths each year in SCC. The age adjusted suicide rates for non hispanic African Americans (5.7/100,000) and females (3.6/100,000) in SCC was below the HP2020 goal of 10.2 deaths per 100,000 population. However, the age adjusted suicide rate for Whites (13.9 per 100,000) was more than double the rate for non hispanic African Americans and the rate for males (16.8/100,000) was more than four times the rate for females. Homicide Mortality What is it? Homicide is the intentional use of force against another resulting in death. Why is it important? Homicide is often associated with other crimes, notably robbery. Motivation is usually different between genders as well. Among female victims, homicide is often associated with intimate partner violence, whereas male victims are often related to drugs.from 2008 to 2012, homicide was the leading cause of death of young adults (15 24 years of age) in Suburban Cook County (SCC) accounting for 25% of all deaths among this age group. Firearm related Mortality What is it? Firearm fatalities include homicides and suicides where the weapon was a firearm, as well as firearm accidents. Why is it important? Firearm fatalities are preventable. Of all the firearm fatalities in the U.S., 54.6 percent were suicide and 41 percent were homicide.firearm injuries have a high case fatality rate, approximately 40%. Overall, the firearm related age adjusted mortality rate for SCC (8.0 per 100,000) in 2012 met the HP2020 goal of 10.6 per 100,000 population. While the age adjusted mortality rates for Whites (6.1/100,000) and Hispanics (4.1/100,000) for SCC in 2012 were similar. the non hispanic African American rate was 20.2 per 100,000 population, almost double the HP 2020 goal. Crude Birth Rate What is it? The crude birth rate (CBR) is the number of live births for a specified geographic area divided by the total population for that area and multiplied by 1,000. Why is it important? The CBR in the U.S. has remained relatively stable for the past 20 years. Much of the stabilization can be attributed to family planning and contraceptives and is listed as one of the CDC s Ten Great Public Health Achievements. The CBR for SCC is dcreasing significantly from 13.0/1,000 live births in 2008 to 11.7 in 2012. The CBR is decreasing for all major race/ethnic groups except for Whites whose rates have remained stable. Infant Mortality Rate What is it? Infant mortality is the number of deaths to infants under one year of age per 1,000 live births in a given year. Why is it important? The infant mortality rate is often used as a basic tool to measure the health of a population and for comparison between populations. Whilte the overall infant mortality rate for SCC in 2012 (6.5/1,000 live births) was close to the HP2020 goal of 6.0/1,000 live births, the infant mortality rate for non hispanic African American mothers (14.7/1,000 live births) was four times greater than that for White mothers (3.6/1,000 live births) in SCC. Furthermore, the infant mortality rates is decreasing significantly for Whites, but has remained the same for non hispanic African Americans. Preterm Birth Rate What is it? Preterm birth is the birth of an infant before 37 weeks of pregnancy. Why is it important? Preterm birth is the greatest contributor to infant death, with most preterm related deaths occurring among babies who were born very preterm (before 32 weeks). The overall preterm birth rates were highest in the South district (11.1 per 1,000 live births) and lowest in the North district (9.1 per 1,000 live births). The overall premature birth rate (9.5 per 1,000 live births) was similar for Asians, Hispanics, and Whites. However, the premature birth rate for non hispanic African Americans was 13.2 in 2012 and did not meet the HP2020 goal of 11.4. Inadequate Prenatal Care What is it? Inadequate prenatal care (IPNC) is the percentage of births to women beginning prenatal care after the 3rd month of pregnancy or receiving no prenatal care. Why is it important:? Beginning prenatal care in the first trimester of pregnancy is important because it allows the health of the fetus to be monitored. Women with IPNC are more likely to have poor outcomes such as preterm birth and low birth weight babies. The overall percentage of women who received IPNC has remained around 12% from 2008 to 2012. Although non Hispanic African Americans (14.0 %) and Hispanics (15.1%) have the highest rates of IPNC, trends have significantly decreased for both groups between 2008 and 2012. Singleton Low Birth Weight What is it? Singleton Low birth weight (LBW) refers to infants weighing less than 5 pounds 8 ounces (2,500 grams). Why is it important: LBW is the greatest factor for neonatal mortality. LBW babies are also more likely to have disabilities and suffer greater morbidity later in life. LBW has remained stable in SCC (approximately 6.1 % of births) and across all districts from 2008 to 2012. With 5% of live births resulting in LBW, the Norh had the lowest rate and the South, with 8.1% of births had the highest rate when compared to other SCC districts. The percentage of LBW births is significantly trending downwards for Asians and Whites, and increasing upwards for Hispanics from 2008 2012. Teen Birth Rate What is it? The teen birth rate (TBR) is measured as the number of births to teens ages 15 19 per 1,000 females in the population 15 19. Why is it important? Teen mothers are also less likely to be married or have social support. They are also more likely to smoke and less likely to initiate prenatal care in the first trimester, which can lead to poor birth outcomes. The teen birth rate has decreased in SCC and across all districts and major race/ethnic groups. The non hispanic African American teen birth rate has decreased from 57.1 per 1,000 females in 2008 to 30.7 per 1,000 females in 2012. Gonorrhea Incidence (15 44 yrs) What is it? Infection with the bacterium Neisseria gonorrhoeae (GC) causes gonorrheal infections, the second most commonly reported communicable disease in Suburban Cook County, Illinois and in the United States. Gonorrheal infections are easily treated with appropriate antibiotics, but drug resistant strains are on the rise. Why is it important? Untreated gonorrhea can cause serious and permanent health problems in both women and men. In women, gonorrhea can spread and cause pelvic inflammatory disease (PID). In men, gonorrhea can cause a painful condition called epididymitis in the tubes attached to the testicles. If not treated, gonorrhea can spread to the blood or joints. This condition can be lifethreatening.
