Narratives Subject: Overdose Type Date Time Author Approving Officer Initial Report 03/27/2017 1648 SUSANA, TRACEY E BRAUN, RICHARD K 954 Recovered: 4-Trazondone pills, 2-Ambien pills-zippered case inside vehicle-sgt. T. Susana A call of an overdose was received advising a female in a blue Jeep was overdosing in the parking lot of the main entrance to FRMC. Upon arrival, hospital security had already pulled her from the vehicle and had wheeled her to the hospital doors to await SFD. SFD then transported her around to the ER entrance. An unidentified person told hospital security that two people took off running from the vehicle after pulling into the parking spot. The female was identified as Shelby Taraschke and she was the registered owner of the vehicle. The vehicle was checked for any signs of drug activity with numerous prescriptions being located in the console with Shelby's name on them. The listed pills were found in a small zip lock back inside a zippered pouch laying in the back of the vehicle along with naloxon that had been provided by Huron County "Project Dawn". No signs of illegal narcotics use was located inside the vehicle other than a cap from a syringe. The pills were tagged for disposal. Shelby was talked with further in the ER with her stating she normally snorts methamphetamines but shot up heroin 4 days ago and then again today. She was noted to have a needle mark and bruising on her right arm. She indicated she had been clean for about five months but relapsed last month. When this officer tried explaining to Shelby what had occurred, she stated she drove to Sandusky from Bellevue. She could not comprehend that someone else had driven her car to the hospital with her and another occupant inside. Shelby was told the two other people had left her in the car and walked away from the area. She then mentioned being with a friend named Stephanie and her boyfriend, Lee. Stephanie is a w/f and Lee is a b/m but she did not know any additional information on them. Shelby was asked where the heroin came from, she stated Stephanie and Lee had the drugs. Shelby was told her vehicle was parked in a handicapped spot in the main lot and there were no keys to secure it. She was told she needed to get the vehicle moved or it could be ticketed and towed. Shelby still seemed confused and just kept crying. She was left in the care of FRMC staff. Video was obtained of the parking lot as well as the main entrance. The vehicle can be seen pulling in the lot from Hayes Ave. where it circles around and parks in a handicapped spot. A subject with light colored pants is observed to exit the driver seat and then another exits the left rear passenger door. They both walked westbound towards the main entrance but one turns around and begins walked eastbound towards the car. This subject does not stop at the vehicle
but walks eastbound out of the view of the camera towards Hayes Ave. The other subject continues into the main entrance where it is believed he alerts someone to Shelby's condition. This subject is clearly observed to be a bald black male wearing gray track pants and a black sweatshirt with a white box on the front outlining "NIKE". He exits a short time later and goes back towards the Jeep. He lingers near the vehicle but then walks away eastbound out of view when hospital security approaches. The disc containing this footage was obtained, tagged and placed into evidence. SGT. T. SUSANA
