Assist Other Agency 10/09/2016 03:34:57 2016-00009557 GREEN BAY RD / BATH AND TENNIS CLUB RD Narrative: Officers responded to the area of Green Bay Road at West Center Avenue to assist Lake Forest Police with a vehicle pursuit. Lake Forest units terminated pursuit on Green Bay Road at Route 176. Vehicle continued northbound on Green Bay Road at a high rate of speed. Officers checked the area to the north limits and were unable to relocate the vehicle. Officers located the vehicle heading northbound on Route 43 at Route 176. Officers followed the vehicle and stopped on Clover Lane. Lake Forest officer arrived on scene and the driver was turned over to Lake Forest Police. Officer met with the driver, who stated a piece of the trailer kept falling. The driver secured the piece and left the scene. Animal Found 10/09/2016 10:53:47 2016-00009573 SUNRISE AVE Narrative: A complainant responded to the Public Safety Building to turn in a dog that she had found running at large in the 500 block of Sunrise Avenue. The dog was described at a black and white colored Pug type breed with a lite blue collar and a tag identifying the dog as "Dottie". The complainant called the telephone number on the tag and left a message. Officer transported the dog to the Knollwood Animal Hospital. Public Health and Safety 10/09/2016 13:40:26 2016-00009574 ROCKLAND RD / GREEN BAY RD Narrative: Officer responded to the intersection of Green Bay Rd and Rt. 176 for a log in the roadway. Officer arrived on scene with the Lake Bluff Fire Department and Lake Forest Ambulance 421. The ambulance crew evaluated the patient before transporting the patient to Lake Forest Hospital.

LBFD determined a worker hit the wrong button on the panel and set the alarm off in error. Suspicious Veh 10/10/2016 17:45:08 2016-00009621 ALBRECHT DR Narrative: Officer located a vehicle on the south side of a business on Albrecht Drive with two subjects inside. Officer made contact with the subjects who stated they were playing handheld games. Both subjects cleared LEADS/NCIC and were sent on their way. Alarm Residential 10/10/2016 20:41:07 2016-00009624 E PROSPECT AVE Narrative: Officers responded to the 200 block of E. Prospect Ave for a report of an activated burglar alarm. Officers arrived on scene and found all exterior doors secured and noted no irregularities. Suspicious Veh 10/10/2016 23:24:29 2016-00009625 WARRINGTON DR Narrative: Officers responded to the parking area on James Court for a report of a suspicious vehicle. Officers located a BMW sport utility vehicle with the headlights on occupied by one person. Officers met with the subject who was a resident of Tangley Oaks who stated she stopped there to make a phone call before going home. No irregularities were observed while speaking with the resident. Animal - Carcass 10/11/2016 00:25:29 2016-00009626 GREEN BAY RD / BELLE FORET DR Narrative: While on patrol, officer located a deer carcass in the roadway on Green Bay Road at Belle Foret Drive. Officer pulled the carcass out of the roadway and moved it to the curb line. Dispatch contacted IDOT and advised them of the carcass for removal. Ambulance Assist 10/11/2016 02:53:52 2016-00009633 NORTH SHORE DR Narrative: Officer responded to the 900 block of North Shore Drive for a report of an employee on site not feeling well. Lake Forest and Lake Bluff Fire Department responded. Lake Forest Ambulance A-421 transported the subject to Lake Forest Hospital for further evaluation. Assist Fire 10/11/2016 06:40:59 2016-00009636 ALBRECHT DR Narrative: Officer responded to the 100 block of Albrecht Dr. for an odor of natural gas inside the building. Lake Forest, Knollwood and Lake Bluff fire departments responded. The odor was determined to have been caused by a gas line fitting. The company made arrangements to have the fitting repaired. The situation did not pose a hazard and all units cleared. Animal Complaint 10/11/2016 08:52:33 2016-00009641 E WOODLAND RD Narrative: Officer responded to the 200 block of East Woodland Avenue for a report of a German Shepard running at large. Owner was able to retrieve the dog prior to officer s arrival. Parking Problem 10/11/2016 12:46:12 2016-00009652 SUNRISE AVE Narrative: Officer responded to the 400 block of Sunrise for a parking complaint, construction vehicles parked in no parking area. Officer spoke with the contractors and advised them to move the vehicles off of Sunrise.

Two Operation Kids Child Safety Seat Checklists were signed and the seats were installed. Suspicious Veh 10/11/2016 15:25:02 2016-00009655 INDIAN RD Narrative: Officers responded to the 100 block of Indian Rd for a delayed suspicious vehicle. The caller stated a white van with no windows in the back but two windows up front and a white box on top of the roof circled the cul de sac three times today. The occupants were two older male whites. The caller also stated the vehicle had a white license plate with black lettering. Officers checked the area and could not locate the vehicle. Premise Check 10/11/2016 16:40:11 2016-00009656 HAMILTON CT Narrative: Officers responded to the 100 block of Hamilton Ct for a premise check. The complainant stated that he had returned home and observed the front door of the residence standing open. Officers arrived on scene and checked the interior of the residence noting no irregularities. The residence is currently listed for sale and the realtor lock box was left open and the key was in the door. Officers were able to re secure the residence and secured the key back in the realtor lock box. Suspicious Incident 10/11/2016 20:45:50 2016-00009658 E WITCHWOOD LN / SHERIDAN RD Narrative: Officers responded to Sheridan Rd and E Witchwood Ln for a report of an unknown person going through people s garbage with a flashlight. The refused caller then stated the vehicle, a red sedan, was last seen heading north on Sheridan Rd. Officers checked the area and also Sheridan Rd up to the Navy train depot. Officers were unable to locate the vehicle. Alarm Residential 10/12/2016 12:01:20 2016-00009679 N ENVIRON CIR Narrative: Officer responded to the 29700 block of N. Environ Circle for an activated burglar alarm. Dispatch advised that proper code was given. Officer continued in to verify. Officer arrived on scene and checked the exterior of the residence. No answer at the door and the alarm company was trying to get ahold of a keyholder. Officer observed no irregularities and cleared. 911 Call Investigation 10/12/2016 14:28:50 2016-00009683 MAWMAN AVE Narrative: Officers responded to the 700 block of Mawman Ave for a 911 verify. Officers met with the homeowner who stated one of her children dialed 911 on accident. Officers observed no irregularities. Alarm Residential 10/12/2016 15:16:01 2016-00009684 SHORE ACRES DR Narrative: Officers responded to the 1700 block of Shore Acres Rd for an activated burglar alarm. Officers arrived on scene and checked the exterior of the residence finding all doors secured and noted no irregularities. Officers cleared. Suspicious Veh 10/12/2016 20:52:29 2016-00009686 S WAUKEGAN RD Narrative: Officers responded to the 300 block of S Waukegan Rd for a suspicious vehicle report. The refused caller stated two males were sitting in a green Dodge van. Officers met with the occupants who stated they were picking up a friend from work at a local business. Officers observed no irregularities and the occupants cleared LEADS/NCIC.

Accident PI 10/12/2016 23:54:55 2016-00009687 SKOKIE HWY Narrative: Officers responded to N/B Route 41 near a dealership for a report of a single vehicle accident, car vs tree, with possible injuries. The Lake Bluff Fire Department and Lake Forest Ambulance 421 responded to the scene. The driver and passenger both signed a medical release. The vehicle was towed and officer completed a crash report and cleared. Ambulance Assist 10/13/2016 13:03:35 2016-00009705 S WAUKEGAN RD Narrative: Officer responded to the 100 block of S Waukegan Rd. for a subject with a nose bleed. Lake Forest, Knollwood and Lake Bluff fire departments responded. Ambulance A-421 transported the subject to Lake Forest Hospital. Alarm Residential 10/13/2016 13:19:23 2016-00009706 OAK RIDGE CT Narrative: Officer responded to the 400 block of Oak Ridge Court for an activated burglar alarm. Dispatch advised the alarm was set off by accident and the homeowner is on scene. Officer arrived on scene and met with the home owner to confirm no irregularities were observed. Sex Offender Registration 10/13/2016 15:01:24 2016-00009708 E CENTER AVE Narrative: Officer responded to the PSB to conduct an annual sex offender registration for a resident. Theft Under $300 10/13/2016 15:49:06 2016-00009711 ROCKLAND RD Narrative: Officer responded to a business in the 900 block of Rockland Rd for a delayed theft of alcohol. The suspects had fled in a vehicle. Formal report on file. Child Seat 10/13/2016 16:53:27 2016-00009712 E CENTER AVE Narrative: Subject came in to have her child safety seat checked. An Operation Kids Child Safety Seat Checklist was signed by subject and the seat was installed. Abandoned Veh 10/14/2016 00:51:22 2016-00009715 SHERIDAN RD / CRAB TREE LN Narrative: While on patrol, officer observed a black Nissan truck parked without lights on the east shoulder of Sheridan Road north of Crab Tree Lane. Upon checking the vehicle, officer determined that the vehicle was unoccupied and the engine was cold. Officer conducted a check of the area for the driver with negative results. An impound warning notice was left on vehicle advising removal within the next 12 hours and officer cleared. Suspicious Veh 10/14/2016 05:06:47 2016-00009727 W SHERIDAN PL Narrative: Officer responded to the 200 block of West Sheridan Place for a report of an occupied suspicious vehicle. Officer met with the driver who stated he is running an estate sale at the location later this morning and was reviewing the inventory list. The subject cleared LEADS/NCIC and officer observed no irregularities. Traffic Stop 10/14/2016 07:41:01 2016-00009731 S WAUKEGAN RD / ROCKLAND RD Narrative: Officer conducted a traffic stop on Route 43 at Route 176. The driver was arrested for expired vehicle registration, driving on a suspended driver's license, and failure to provide insurance.

Found Property 10/14/2016 09:36:00 2016-00009733 E SCRANTON AVE / E CENTER AVE Narrative: A subject turned over a debit card to the Records Department. Records contacted the possible owner and left a message. A found property form was completed and the item was secured. The owner later responded to the PSB to claim his property. Lock Out 10/14/2016 13:34:21 2016-00009739 S WAUKEGAN RD Narrative: Officer responded to the 40 block of S Waukegan Road for an auto lockout. Officer arrived on scene and spoke with the complainant who stated that they accidently locked his keys in the car. A waiver release form was completed by the complainant and entry was gained. Fingerprinting 10/14/2016 15:10:02 2016-00009742 E CENTER AVE Narrative: Officer responded to the PSB to fingerprint a resident. Citizen Assist 10/14/2016 15:59:31 2016-00009745 SKOKIE HWY Narrative: Officer responded to a dealership for a VIN inspection. VIN was verified and paperwork completed. Fireworks 10/14/2016 20:59:02 2016-00009750 LAKELAND DR Narrative: Officer responded to the area of Sunrise Ave for the report of fireworks. Officer was unable to hear or locate any fireworks in the area. Driving Complaint 10/14/2016 23:28:15 2016-00009752 ROCKLAND RD / SKOKIE HWY Narrative: Officer responded to the area of Route 176 at Route 41 for a refused complaint of a subject operating a black Toyota Prius without activated lights. Officer checked the area but was unable to locate the vehicle. Citizen Assist 10/15/2016 09:03:42 2016-00009764 SKOKIE HWY Narrative: Officer responded to a dealership for a Vin verification. Officer determined a previous verification had been done on the vehicle, but the paperwork was lost. Narrative: Officer responded to the PSB and met with a resident regarding political signs in his front yard. He stated political signs were removed from his and his neighbor's front yards. He stated they were close to the roadway and believes the Village may have removed them due to an ordinance violation. Officer advised him to check with the Village Hall during normal business hours and they can verify if they removed the items. Officer directed the subject to speak with the church and the subject stated that no one was available at the church. Officer advised the subject that we could not provide financial assistance without permission from the church and asked if there was any other assistance this department could provide. The subject stated no further assistance was needed at this time and was allowed to use the lobby of the PSB to charge her cell phone until her train arrived. The driver was issued citations for expired registration, driving on a suspended driver's license and no insurance.
