1. PURPOSE OF THIS REPORT The report is a weekly summary of the daily reports submitted by the Indigenous Construction Monitors that are working on spread nine. The social and cultural findings have been tabulated with locations and mitigation measures below. A summary of the construction activities has been summarized for the weeks of September 17 th to September 22 nd, 2018. 2 Matrix Solutions Inc.

Additional Observations and Summary of Activities or Concerns On September 17 th, safety theme identified, congested areas, uncontrolled intersections, driving conditions, wild life, equipment on the ROW, communication, slips trips falls, wear all PPE required; Environment: proper disposal of Garbage, recycling, and food scraps, proper separation of garbage, recycling, and food scraps, smoke in designated areas, proper disposal of cigarette butts, tires, vehicles, and boots are cleaned, disinfected, and inspected, vehicles are equipped with spill kits and disinfectant; met as a team and discussed the days work scope for the Junior Monitors, as well had went over the ERP, Muster Points, and hazards and controls for the work day; the Juniors had set out to the SSKP'S that have been assigned to them, I had taken off to view the progress of the Horizontal Directional Drill set up at SSKP 1041 - Dead Horse Creek. So far they are still setting up there was a telehandler unloading trailers; made my way out to kick off to view the progress of the current construction happening, I was told it would be wise to stay off the ROW as it is really congested right now so I did and viewed from the road and had zig zagged through the back roads to find and observe the Ditcher in action; found the ditcher working at Shoe Fly 6, and had observed it digging up the ditch and creating spill piles to the north of the ROW; Work forecast: Will be heading to dead horse creek and connecting with the foreman for the HDD to find out when they will be drilling and letting him know that the Junior Monitors and I will be there for the drilling of dead horse creek, the juniors will be going to SSKPS assigned to them. On September 18 th, safety theme identified as congested areas, uncontrolled intersections, driving conditions, wild life, equipment on the ROW, communication, slips trips falls, wear all PPE required; Environment: proper disposal of Garbage, recycling, and food scraps, proper separation of garbage, recycling, and food scraps, smoke in designated areas, proper disposal of cigarette butts, tires, vehicles, and boots are cleaned, disinfected, and inspected, vehicles are equipped with spill kits and disinfectant; instructed the Jr. monitors to go to SSKP'S 1072, 1076, 1079, and 1085.; y after the daily work scope was discussed I had conducted and lead the safety meeting for the morning discussing the ERP, Muster Points, and the Hazards and Controls for the work day; set out to kick off area to view and observe the construction taking place, I had not went out on the ROW as it was congested and I would of only been a hazard, I found the ditcher at Shoe-Fly 10 it had seemed to be moving at a productive pace; SSKP 1026 was an interest of mine as I have not been there in a while, when I got there most of the pipe there had been welding and was just waiting for the tie ins, the pipe gang had made it down the bottom of the escarpment; went to Dead Horse Creek towards the end of the day, I had avoided going into site as it was still being set up and the drilling won t start until tomorrow; met up with Mick Morrison and we had taken a ride together down the ROW and viewed the progress of the Bending crews; Work forecast: Go to Dead Horse creek and observe the drill do its job, as well as view other SSKP s of interest. 4 Matrix Solutions Inc.

On September 20, 2018, safety theme identified, safety theme identified as congested areas, uncontrolled intersections, driving conditions, wild life, equipment on the ROW, communication, slips trips falls, wear all PPE required; Environment: proper disposal of Garbage, recycling, and food scraps, proper separation of garbage, recycling, and food scraps, smoke in designated areas, proper disposal of cigarette butts, tires, vehicles, and boots are cleaned, disinfected, and inspected, vehicles are equipped with spill kits and disinfectant; went to Dead Horse Creek first thing in the morning and had decided not to enter site as there was lots of movement with big equipment commencing, around SSKP 1043 the tackifier truck was working spraying the dirt piles I had observed the work that was being done; decided to zig zag through the back roads following the line to see and observe the construction progress, there was not much to see other than laying of the pipe and road bores; made my way back to the office and had started planning out the Indigenous Construction Senior Monitor job structure and format with guidance from Mick Morrison, we feel that we need to establish a strong front with the other Senior Monitors as well as the Liaisons from the other spreads so we are all on the same page and make this job position a successful one; Michals horizontal directional drill site at Dead Horse Creek to establish communication with Michel's, I had connected with HSE Lead Trent Seaward and we had planned for me do be orientated tomorrow and for the Juniors and I to be a part of the morning safety meeting. The Junior Monitors will be joining me at Michel's HDD site but only with a visitor pass, we feel it will be safer and wise to limit them with their access at this site as they are green to dangerous and fast paced heavy construction; Work forecast: Orientation at Michel's HDD site, attend the morning safety meeting for Michel's. Send the Junior Monitors to assigned SSKP'S of interest, they will also be joining me at Dead Horse Creek. On September 21, 2018, safety theme identified, safety theme identified as congested areas, uncontrolled intersections, driving conditions, wild life, equipment on the ROW, communication, slips trips falls, wear all PPE required; Environment: proper disposal of Garbage, recycling, and food scraps, proper separation of garbage, recycling, and food scraps, smoke in designated areas, proper disposal of cigarette butts, tires, vehicles, and boots are cleaned, disinfected, and inspected, vehicles are equipped with spill kits and disinfectant, conditions are wet and muddy - Avoid the ROW if possible, no need to create a mess; no SSKP'S assigned as is had rained heavy yesterday evening and the ROW is to muddy for us to be on today, so they Juniors had spent the morning and early afternoon practicing their computer skills and honing their reporting skills; went to the horizontal directional drill site at Dead Horse Creek and monitored the work being done there, the safety precautions and environment protocols in place were pristine and impressive; the HDD site, they were continuing to drill the underground bore for the 36" pipe to eventually be dragged through. I had stayed at a safe distance back while observing seeing as there is lots of potential hazards including overhead loads, congested areas, heavy equipment and noise hazards; spent time in the afternoon with the Junior ICM's helping with computer tasks as well as teaching them to enhance their computer skills and knowledge, as well finished my daily report and had our weekly round table with Matrix-Solutions Manager Greg Lewis; Work forecast: Monitor Dead Horse Creek, monitor sites of interest depending on the ROW condition. 5 Matrix Solutions Inc.

On September 22, 2018, safety theme identified, safety theme identified as congested areas, uncontrolled intersections, driving conditions, wild life, equipment on the ROW, communication, slips trips falls, wear all PPE required; Environment: proper disposal of Garbage, recycling, and food scraps, proper separation of garbage, recycling, and food scraps, smoke in designated areas, proper disposal of cigarette butts, tires, vehicles, and boots are cleaned, disinfected, and inspected, vehicles are equipped with spill kits and disinfectant; conducted the daily morning safety meeting discussing the Hazards and controls for the work day as well gave direction to the junior monitors of what to do during the work day; the Jr monitors were instructed to go a view the construction progress from the road as the ROW was too wet for them to enter on, I, Cory Monkman had not been out on the ROW today as I was giving permission to attend the MMF'S General assembly.
