Avenues East Avenues East is a subsidiary of the Avenues Group and an independent charity which has been making a difference to the lives of disabled people for over 30 years. We support people across the East of England living with disabilities, illness or injury to live the best lives possible. We work to provide people with the opportunities to play an active part in the local community and to help them recognise their equal right to take part in all aspects of everyday life. Community Support Services Community Support Services (CSS) supports children, young people and adults who have learning disabilities, physical disabilities, sensory impairments and complex health needs. Our services include: LEISURE WORK SUPPORT Joined up support can make it easier for people with disabilities to live their life as they wish. CSS helps bring support closer together to provide the right support at the right time. We help provide support during everyday life, including support at home, out and about in the community and to access a variety of leisure activities and employment opportunities. 4

LEISURE Our LEISURE service encourages and enables people with disabilities to take part in activities which help to improve their health, wellbeing, social skills and independence. We do this by organising a range of activities, social groups, short breaks and events throughout the year which provides fun and enjoyment for everyone. Service Registration To access our wide range of leisure, sport and social activities we require people to complete a registration and self assessment form and pay an annual subscription fee of 18 (only 1.50 per month). 5

Social Groups Teaming Up supports and enables people with disabilities to access and take part in a variety of age appropriate leisure, recreational, social and well-being activities within their local community. All participants are provided with the opportunity to develop and maintain good friendships, whilst having a social life. Social group sessions operate on a pay-as-you-go basis and participants are invoiced at the end of each month. 6

Social Groups (ages 10-15) Teaming Up (ages 10 to 15) takes place on Saturday mornings and enables children and young people with disabilities to have lots of fun and make new friends. Activity cost included Each session offers high quality activities which take place at a variety of leisure and entertainment venues for example bowling, cinema or just meeting up with friends. A programme of activities will be issued once registered. 7

Social Groups (ages 16-25) Teaming Up (ages 16 to 25) meet weekday evenings at a variety of leisure and entertainment venues. The sessions help young people develop money handling, social and communication skills, whilst hanging out with friends. A programme of activities are provided including bowling, cinema, meals at local restaurants, swimming, visiting places of interest and much more. Daytime Activity Groups Daytime Activity Groups provide weekly daytime activities that enable people to have an enjoyable day, keep active, learn new skills and maintain social contacts. Ipswich St Peter s by the Waterfront, College Street, Ipswich, IP1 1XF Every Monday & Friday Time: 9:30am - 3:30pm Cost: 30 per session The day consists of accessing community based activities and facilities including; swimming, cinema, the seaside, cooking, cafés, parks or any outing/ activity chosen by the group. Every Thursday Time: 9:30am - 1:30pm (4hr session) 1:30pm - 3:30pm (afternoon) Cost: 4hr session only - 20 Afternoon session only - 10 Full session - 30 Between 9:30am - 1:30pm the group takes part in a variety of activities and outings in the community. From 1:30pm 3:30pm our weekly pool & social afternoon takes place. 13

Stowmarket Stowmarket Community Centre, Hillside, Stowmarket, IP14 2BD Every Monday Time: 9:30am - 2:30pm Cost: 25 per session Days are varied and include; arts & crafts, exercise to music, accessible group activities, games and outings in the community. Daytime Activity Group Sessions operate on a pay-as-you-go basis and participants are invoiced at the end of each month. 14

Supported Day Trips Supported Day Trips provide enjoyable days out to a variety of entertainment venues and visits to tourist attractions. Come and see all your favourite WWE stars live in action. WWE Live is sure to provide ultimate entertainment! Wednesday 10th May 2017 Price: 110 Our day trip package includes ticket, transport via private bus and a full day of group support, with return transport from Bury St Edmunds, Ipswich and Stowmarket. A fun day out full of rides, shows and attractions for you to enjoy. Wednesday 19th July 2017 Price: 65 Our day trip package includes park entrance, transport via coach and a full day of group support, with return transport from Bury St Edmunds, Ipswich and Stowmarket. 15

Supported Day Trips Enjoy the ultimate shopping experience all under one spectacular roof, including a fresh food court and plenty of leisure and entertainment facilities. Wednesday 29th November 2017 Price: 48 Our day trip to Westfield Shopping Centre includes a full day of group support from Bury St Edmunds, Ipswich and Stowmarket train stations (train ticket not included). Enjoy a day to London with a ticket to the multi-award winning musical The Lion King. Wednesday 28th February 2018 Price: 110 Our day trip package includes ticket, accessible transport via coach and a full day of group support, with return transport from Bury St Edmunds, Ipswich and Stowmarket. Additional information: Due to theatre restrictions there will be 2 fixed and 2 transferable wheelchair spaces available. Due to high demand bookings are taken on a first come, first served basis. Once your place is confirmed a non-refundable deposit of 20% of the total amount is required. 16

ITFC Match Assistance ITFC Match Assistance enables people with a disability to watch Ipswich Town home matches. We support people who require assistance to watch Ipswich Town matches at Portman Road who do not already have their own personal assistant or carer to accompany them to the game. Fixture information & prices will be sent on request. ITFC games must be booked and paid for in advance. 21

WORK Our WORK service provides first step support to access employment through voluntary placements and work experience opportunities. We ensure that people with disabilities are trained on the necessary vocational and life skills to access volunteer or work experience placements independently within their local community. On accessing work placements independently, ongoing support is provided to enable the individual to develop workplace skills as well as aspirations to be ready for employment and to progress towards paid work. 22

SUPPORT Our SUPPORT service provides day-to-day assistance that enables people with a disability to be as independent as possible. To enable individuals to live their lives the way they choose, the support we provide will vary between each person. Support is available from 4 hours a week to 12+ hours a day, 7 days a week. We provide support to: access the local community participate in leisure activities, education and volunteering plan and go away on day trips and holidays develop and maintain life skills, such as money handling, food shopping, cooking and household chores to develop independent living skills live at home Every person we support has a personalised support plan which relates directly to their individual' goals, wishes, needs and aspirations. 23
