Can the Trust confirm if xxx case is one of the twenty adverse incidents picked up in the look back exercise. If not, can the Trust confirm that it has been added to the list of SAI incidents under review. It would also be helpful if the Trust could confirm if the review of 35,000 X rays taken at the Causeway hospital include CT scans and if so, is xxx case one of those being reviewed.
