3 MIDDLESEX COUNTY MUNICIPAL JOINT INSURANCE FUND REGULAR MEETING June 25, 2014 A meeting of the was held on Wednesday, June 25, 2014 at 1 Jocama Blvd, Suite 2B Old Bridge, NJ. The meeting was called to order at 12 :20 p.m. by Chairman Michael Januszka. Notices were placed in the newspapers and posted in accordance with the Open Public Meetings Act. Roll call was taken by William Kurtz. Present: Michael Januszka, Milltown; Denise Jawidzik Jamesburg; Russ Marchetta, New Brunswick; Robert Sheehan, Plainsboro; Wayne Hamilton Monroe; Joseph Merritt, New Brunswick Parking Authority; Fredrick Carr & Joseph Zanga South River; Dawn McDonald, Spotswood; and Bernie Hvozdovic, South Brunswick; Joseph Belina Freehold; Kevin Boyle Pompton Lakes. Also Present: William M. Kurtz, Executive Director; Glen T. Kurtz, Assistant Executive Director; Edward Testino, Fund Counsel; Sean Kelly, Bordman Perlman; Jay McMannus and Robin Raccioppi (NAIMC); Craig Josephson (DMH Agency); Jim Gotay (Skyline Environmental, Inc.). Chairman Michael Januszka appointed Bernie Hvozdovic, to the Executive Committee to fill in for absent Commissioner. Approval of Minutes May 28, 2014 Approval of May 28, 2014 minutes, was moved by Denise Jawidzik, seconded by Bernie Hvozdovic with the exception of Wayne Hamilton, Joseph Bellina, and Kevin Boyle who abstained, was approved by an all aye vote from the Executive Committee. Yield an estimated annual interest of $135,234.

For the year 2014, the JIF had a total savings of $2,333,119 or 67% with a network penetration of 96%. Safety & Loss Control G. Kurtz The report of new claims for the month of April (see attached MCMJIF NEW CLAIM REPORTS ), the report of Closed Files ( MCMJIF CLOSED FILES ), the report on pending subrogation s (see attached MCMJIF CLAIMS SUBROGATIONS ), the report on actual subrogation (see attached MCMJIF CLAIMS RECOVERIES and MCMJIF Pyramid MC QC RECOVERIES ) revealing a total of $13,659, in recoveries. Glen Updated the members on the training program held at the JIF office on Flagger training. Skyline Environmental, Inc. Jim Gotay Jim discussed issues the state is have with the new reporting system with respect to compliance of the OSHA inventory compliance. Mr. Gotay, advised members to reach out to his office if contacted by OSHA with respect to the filing. Finance Report Joseph Monzo, absent, submitted his finance report for review by the commissioners (see attached) with Commissioners, showing a trial balance of $46,236, as of May 31, Claims Litigation Moved by Denise Jawidzik, seconded by Dawn McDonald and approved by an all aye vote from the Executive Committee to go into closed session to discuss claims litigation and potential litigation. Moved by Kevin Boyle, seconded by Denise Jawidzik and approved by an all aye vote from the Executive Committee to return to open session. Moved by Robert Sheehan, seconded by Wayne Hamilton and approved by an all aye vote from the Executive Committee to establish reserves for all claims authorized in closed session.
